2.The Experience After Operation Using Tension-free Vaginal Tape for Stress Urinary Incontinence.

PURPOSE: The object was to study prospectively the results of a modified intravaginal sling operation for the surgical treatment of female stress urinary incontinence using tension-free vaginal tape (TVT) as a new continence procedure. MATERIALS AND METHODS: 55 women with demonstrable stress urinary incontinence underwent a nonrandomized, prospective study using the TVT sling operation. Preoperatively the patients were evaluated with Stamey grade and urodynamic testing. After having TVT sling operation, we observed the postoperative results throughout the 1-year follow-up. RESULTS: The mean age of the patients was 52.8 years and mean operation time was 26.6 minutes. Mean duration of postoperative catheterization was 1.1 days. 35 patients (63.6%) were completely cured throughout the 1-year follow-up period, and 18 patients (32.7%) were significantly improved. They did not loose urine apart from an occasional leakage during severe cold. CONCLUSIONS: TVT sling operation is a safe, simple and effective procedure for the treatment of stress incontinence.

3.Mucous Gland Adenoma of the Bronchus: A case report.

Mucous gland adenoma, one type of bronchial adenoma, is first introduced by Rosenblum and Klein2) in 1935 and very rare. Only twenty-one cases had been reported in english literature. This tumor must be differentiated from other types of bronchial adenoma because of completely benign clinical course. A case of mucous gland adenoma is presented which showed marked secondary calcification.

7.Unilateral Parotitis and Kawasaki Disease in a Child.

Kawasaki disease is generally diagnosed base on its clinical features. Sometimes unusual or atypical presentations make the diagnosis of Kawasaki disease difficult. We experienced an unusual case of Kawasaki disease presented with unilateral parotitis in a 23-month old girl. Despite of intravenous antibiotics treatment, fever and unilateral parotid swelling persisted. Skin rashes, conjunctival injections, and coronary abnormalities showed up on the 8th day of fever. After the intravenous immunoglobulin and salicylates treatment, all symptoms disappeared. Although unilateral parotitis is very unusual presentation of Kawasaki disease, in case of no response to antibiotics, Kawasaki disease should be included in the differential diagnosis.

8.Papillary Eccrine Adenoma.

We report a case of papillary eccrine adenoma occurring in a 39-year-old male who had a walnut sized, firm nodule on the right knee without subjective symptoms. Histopathologic exammination revealed a well circumscribed, non-capsulated dermal nodule consisting of multiple ducts with various sizes. These ducts were composed of double layered epithelial cells in most areas, but in some areas severe intraluminal papillation snd pseudostratification into the lumen were seen and the lumina were filled with a homogenous eosinophilic substance and partially with calcium deposition. Immunohistochemical studies with S-100 antigen and csrcinoembryonic sntigen (CEA) were also performed. Positivity for these sntigens strongly supports the hypothesis that this unusual neoplasm differentiates toward the eccrine secretory coil.

10.Hemodynamic Changes during Heas-Down and Up Tilt after Ethanol Ingestion.

BACKGROUND: The acute effect, of ethanol (EOH) include lowered blood pressure through peripheral vasodilation and decreased circulating blood volume. This study was aimed at examing the effects of EOH on the hemodynamic response to up and down head tilts. METHODS: Ten 21 to 23 year old male adults served as subjects. Each subject participated in both control and EOH experiments. In the EOH experiment, 3 mls of 25% EOH per liter of total body water was administered orally, 35 min were allowed for the blood EOH level to reach maximum before the tilt protocol was initiated. The tilt protocol consisted of 5 stages, each stage was 3-min in duration : supine (0 degrees)-head down tilt (HDT, 15 degrees)-supine (0 degrees)-head up tilt (HUT, 25 degrees)-supine (0 degrees). Hemodynamic parameters were measured with an impedance cardiograph (NCCOM3-R7, BoMed) with two electrodes placed around the neck and two around the thorax. Blood pressure (BP) was measured with an automatic sphygmomanometer (DATEX). Data was collected during every second half-minute throughout the duration of the protocol. RESULTS: In the control, HDT produced an increase in the end-diastolic index (EDI), the stroke index (SI), the cardiac index (CI), and the peak flow index (PFI) ; there were no significant changes in heart rate (HR), the systemic vascular resistance index (SVRI), and BP. In contrast, HUT resulted in a decrease in EDI, SI, EF, CI, and PFI and an increase in HR, SVRI, and BP ; the latter changes sugges a sympathetic overactivation. In the EOH experiment, the basal EDI, SI, and systolic BP were lower and HR was higher than in control. HDT and HUT caused similar changes as in control experiments. CONCLUSIONS: There results indicate that EOH cause volume depletion to result in reduced central blood volume and compensatory tachycardia. These EOH-induced changes were not altered by 15 degreeshead-down and 25 degreeshead-up tilts.
