1.Retrieval of Articles in Personal Computer.

PURPOSE: Although many useful articles appear in the journals published in Korea, they are not always cited by researchers mainly due to absence of efficient searching system. The authors made a program with 6 predefined filtering forms to detect published articles rapidly and accurately. MATERIALS: The program was coded using database management system CA-Clipper Version 5.2 (Computer Associates International, Inc.) through preliminary work for 1 year. We used 486 DX II (8 Mbyte RAM, VGA, 200 MByte Hard Disk), Ink-jet Printer (Hewlett Packard Company), and MS-DOS Version 5.0 (Microsoft Co.). We inputed total of 1986 articles published in the Journal of the Korean Radiological Society from 1981 to 1993. METHODS AND RESULTS: The searching time was 10 to 15 seconds for each use. We had very flexible user interfaces and simplified searching methods, but more complicated filterings could also be performed. CONCLUSION: Although the previous version have had some bugs, this upgrade version resolved the problems and fitted in searching articles. The program would be valuable for radiologist in searching articles published not only in the Journal of the Korean Radiological Society, but also in the Journal of the Korean Society of Medical Ultrasound and the Korean Journal of Nuclear Medicine.

2.Inflammatory Pseudotumor of the Liver Treated by Hepatic Resection: A Case Report.

Inflammatory pseudotumor (IPT) of the liver is rare benign tumor. When the diagnosis of IPT is established with biopsy, simple observation or conservative therapy is preferred because of the possibility of regression. But IPT is unresponsive to the conservative treatment, surgical resection should be considered. We experienced a 63-year-old male, who was suspected hepatocellular carcinoma in abdominal computed tomography (CT) and magnetic resonance image (MRI) scan, presented with 2-month history of intermittent fever and weight loss. Percutaneous ultrasound guided core biopsy confirmed IPT of the liver. Non-steroidal anti-inflammatory drugs and antibiotics were administered for 8 and 4 weeks, respectively, but fever continued. So, extended right hepatectomy was performed for IPT of the liver and then fever subsided. The patient remains well during a follow-up period of 12 months.

An inflammatory pseudotumor of the liver is a rare benign tumor, which is frequently misdiagnosed as malignant. A 63 year-old man presented with a 4-month history of weight loss and a 2-month history of intermittent fever and chills. A computed tomographic (CT) scan and magnetic resonance imaging of the abdomen demonstrated a large tumor in segment 8 of the liver. An ultrasound-guided core biopsy confirmed an inflammatory pseudotumor of the liver. His fever persisted for 3 months, despite the administration of Naproxen and antibiotics. A right trisegmentectomy was carried out, after which the fever subsided. The patient has remaineds well for the last 7 months.

4.Treatment of Esophagojejunostomy Leakage with Expanding Stent.

It would be possible to manage the intestinal anastomotic failure with intraluminal stenting, but its reports are very rare. We experienced a effective and dramatic improvement of esophago-jejunal anastomotic leak in a esophageal and gastric double cancer patient with intraluminal stenting. The intraluminal stenting was tried at the 28th postoperative day and the anastomotic leak and inflammatory signs were disappeared about 3 weeks later. Postoperative 11th months now, the stent was moved about 1 cm downward but not changed further, and he enjoys regular diet without any problems. And we think the stenting would be helful with some limitations in the intestinal anastomotic leak patient.

5.Malignant Extrarenal Rhabdoid Tumor of the Pelvic Paravertebral Region: Case Report.

Malignant rhabdoid tumor (MRT) is a rare but distinctive neoplasm of unknown histogenesis, occurring primarily in children. It has a characteristic histologic pattern and aggressive clinical behavior, and was originally thought to be a malignant sarcomatous variant of Wilms tumor; numerous cases of MRT arising from extrarenal sites have, however, been reported. We describe the radiologic findings of two cases of malignant extrarenal rhabdoid tumor that arose in the pelvic paravertebral region of two children. Both were confirmed by surgical excision and pathologic examination.

6.The Characteristics of Schizophrenia to Commit Homicide.

The authors analyzed the mental evaluation reports for 5 years from 1995 to 1999, done in the Forensic Psychiatric Hospital for the schizophrenic patients who were prosecuted for homicide. The survey was done for the schizophrenics' sex, age, marital status, job, the first or second offense, a residential status, offense place, weapons used, chief motive for homicide, whether he or she was drunken at the time of commission of homicide, the victims' characteristics, the age of the onset of disease, previous psychiatric treatment history, previous criminal history, the duration of schizophrenic disease. In the age of onset of disease, 20-29 years old were the most, 30 cases of 67%. In the psychiatric treatment history for the chronic cases, there's statistical significance. For the chronics, it can be said that the experiences of psychiatric treatment were statistically significant. For the chief motive of homicide, delusion and hallucination were of 44%, but angers were 18 cases(40%) and excitements were 3 cases. What is specially noteworthy is the angers and excitements, consisting of 46.7% of all. And for the chronic cases, delusion and hallucination were the major motive with statistical significance.

7.Valve Replacement in an Anuric Patient with Chronic Renal Failure: 1 Case Report.

It is now generalized to perform cardiac surgery in the patients with end-stage renal disease. The growing number of patients with chronic renal failure wake us up to the need to prepare for proper management. There are not only the prevalence of coronary artery disease, but also a great amount of valve dysfunction exist in this group. Peritoneal dialysis may be one of the obstacles for cardiopulmonary bypass but it is not a great hindrance in cardiac surgery with careful preparation and well organized perioperative care. The author has performed mitral valve replacement in a 33-year-old anuric female patient with chronic renal failure and severe mitral insufficiency. Preoperatively, the patient was kept in adequate fluid and electrolyte balance using peritoneal dialysis. Peritoneal dialysis continued and regulated according to the laboratory data in this patient during and after the surgery. She recovered well showing an uneventful course and was discharged on postoperative 1 th day.

8.The Effect of Cold Air Application for the Intraarticular and Skin Temperature Changes of Knees.

The purposes of this study are to investigate the effect of the cold air application in the skin and intraarticular temperature changes and to observe the rebound temperature changes after cooling. We recorded the changes of the skin surface and intraarticular temperatures of knees during and after the cold air application. The intraarticular temperature was measured by a temperature probe inserted into the knee joint cavity and the skin temperature by the infrared system. Eighteen healthy subjects were examined. The knee was cooled by a 5-minutes application of CRAis (Kyung-won Century, Korea) machine and the intraarticular and skin temperatures of knees were measured at every 0.5-minute during and after the cold therpy, then at every minute for 5 minutes, and every 5-minute for the next 110 minutes. We also evaluated the variables that might affect the skin and intraarticular temperature changes. Results showed that the mean skin temperature dropped from 31.8oC to 10.5oC immediately after the cold air application for 5-minutes. The mean intraarticular temperature dropped from 33.9oC to 30.0oC after the cold air application for 5-minutes. Two hours after the initiation of treatment with cold air, the mean intraarticular temperatures did not recover to the baseline values(p<0.01). No significant correlations were found between the body mass index with the intraarticular and surface temperatures of knees. A highly significant correlation was noted between the baseline skin surface and intraarticular temperatures(p<0.01). In conclusion, the reduction of the joint temperature by the cold air application using CRAis machine can be a useful treatment method for the synovitis of knees.

10.Changes of Liver Volume and Functions after Hepatic Resection according to the Fibrosis (Stage of Chronic Hepatitis) of Liver Parenchyma.

PURPOSE: The aim of this study was to assess the regeneration rate and functional recovery of the liver due to the stage of parenchymal fibrosis following a hepatectomy. METHODS: We reviewed 41 patients that had undergone a hepatectomy for living liver donation and 35 for an HCC. The HCC patients were classified into three groups according to their histopathological status. The liver volume was serially estimated using CT scans before the operation, and on postoperative days (POD) 7 and 30. The liver function tests for AST, ALT and bilirubin were serially checked before the operation, immediately postoperative, and on POD 1, 3, 5, 7 and 30. RESULTS: There were 61 and 15 males and females, respectively, with a mean age of 40 years. Patients were divided into four subgroups: parenchymal fibrosis, none (normal), periportal, septal and cirrhotic fibrosis. Within all the patients 55, 13 and 8 right lobectomy (RL), left lobectomy (LL) and left lateral segmentectomy (LLS), respectively, were performed. In the RL cases, the regeneration rate (RR) was significantly higher than those in the cases of LL and LLS. The RR of the fibrotic livers was significantly lower than that of normal livers, and became lower as the degree fibrosis increased. The RR at POD #7 was lowest in cirrhotic livers. The levels of Serum AST and ALT rapidly increased on the immediate postoperative day, and increased maximally on POD #1, but thereafter decreased gradually, and recovered to normal on POD #30. The serum bilirubin level reached a maximal value on POD #2, and returned to normal on POD #7. CONCLUSION: The regenerative activity in chronic hepatitis patients is poor, especially in cases of severe fibrosis - cirrhosis. Therefore, in cases of major resection in HCC patients with chronic hepatitis, it is advisable to decide the extent of resection after confirmation of the stage of fibrosis.
