1.Postoperative Results and Complications of Arthroscopic Anterior Cruciate Ligament Reconstruction using Bone-Patellar tendon-Bone Autograft.

We reviewed the post-reconstructive results and complications of arthroscopic anterior cruciate ligament reconsbuction using the patellar bone-tendon-bone autograft in 91 patients who underwent the index surgery at least one year ago. The changes of Lysholm knee score, limitation of motion, instability of the reconstructed ACL, musc)e strength, presence of anterior knee pain and other complications were evaluated at final fol- low-up(mean, 2.6 year). The results were as follows. 1. The mean Lysholm knee score was 65.5+-15.5 preoperatively and improved to 88.5+-7.4 postoperatively(p<0.05), 2. Of all, nine(9.9%) complained anterior knee pain. 3. Three patients(3.3%) showed limited range of motion. 4. Five(3.5%) showed instability of reconstructed ACL during the followup. Two were caused by trauma. Ln other three, no definite causes were found. 5. In 23 patients, cybex isokinetic thigh muscle strength test was perfornied preoperatively and at post-up. One year. The strength of quadriceps of the operated side at the angular velocity of 60 degrees per second showed mean 41.7% deficit of the normal side at postoperative one year. 6. Other complications were one case of chronic click sound in motion, and one case of the fracture which occurred at the autograft donor site of the proximal tibia.

2.Measurement of Ophthalmic-arterial Blood-flow Velocity Using Transcranial Doppler Ultrasonography in Patients with Central Serous Chorioretinopathy.

PURPOSE: To measure and compare the ocular blood-flow velocity of ophthalmic artery in patients with central serous chorioretinopathy (CSC); affected eyes, asymptomatic fellow eyes, and CSC resolved eyes using transcranial Doppler ultrasonography. METHODS: A total of 31 patients (age 20-52 years) with CSC were evaluated using power motion mode Doppler 150 digital transcranial Doppler ultrasonography. The peak systolic velocity (PSV), peak diastolic velocity (PDV), and resistance index (RI) were measured in the ophthalmic artery of both affected and asymptomatic fellow eyes. After 2 months, 23 patients with resolved serous retinal detachment underwent repeated measurement of the above hemodynamic indices. RESULTS: The PSV and PDV of the ophthalmic artery were 30.05 ± 5.34 cm/sec and 14.07 ± 2.90 cm/sec in affected eyes and 33.03 ± 2.00 cm/sec and 17.17 ± 2.76 cm/sec in asymptomatic fellow eyes, respectively. Both indices were significantly lower in affected eyes (p = 0.004, p < 0.001, respectively). The RI was 0.52 ± 0.08 in affected eyes and 0.43 ± 0.04 in fellow eyes, a significant difference (p < 0.001). After 2 months, in 23 eyes with resolved CSC, the PSV and PDV were 32.94 ± 6.24 cm/sec and 15.54 ± 2.88 cm/sec, respectively. Both indices were significantly higher than at baseline (p = 0.031, p = 0.045, respectively). However, RI was 0.48 ± 0.04 and was not significantly different (p = 0.073). CONCLUSIONS: The CSC-affected eyes showed lower ocular blood-flow velocity and higher RI than the asymptomatic fellow eyes. The ocular blood-flow velocity was increased as CSC was resolved. These findings provide insights into the ophthalmic arterial vascular factors related to the pathogenesis of CSC.

3.A Clinical Study of Amosulalol Hydrochloride(YM-09538) on the Antihypertensive Effects in Essential Hypertension.

BACKGROUND: Newly developed alpha-, beta-receptor blocker, Amosulalol HCI(YM-09538) was evaluated for its hypotensive efficacy, safety and usefulness in patients with mild to moderate essential hypertension. METHODS: Thirty patients of essential hypertension(male 8, female 22), mean age 55 years were included for the study. Amosulalol HCI was administered 20-60mg daily for 10 weeks and initial starting dose was 10mg twice daily and dose was increased 40mg and 60mg daily in cases of insatisfactory hypotensive effect ever 2 weeks interval. Patients were evaluated every two weeks on blood pressure, pulse rate, improvement of subjective symptoms and side effects. Laboratory examinations were carried out routinely in principle two times before trial and after completion of study. RESULTS: Blood pressure began to fall significantly after tow weeks of administration and changed from initial 172.4mmHg of systolic pressure to final 149.3mmHg and from 104.7mmHg of diastolic pressure to final 92.5mmHg. The response rates were marked fall in 30%, satisfactory fall in 40%, thus overall hypotensive effect was observed in 70%. Pulse rate decreased slightiy but significantly from 4 weeks of administration. Improvement of subjective symptoms were observed in 7 cases out of 12 cases and no significant side effects were observedd except of two mild transitory cases of polyuria and indigestion. Laboratory examination also did not show any significant changes before and after medication. CONCLUSION: The daily administration fo 20-60mg of Amosulalol HCI to moderate essectial hypertension seems to be effective and safe with clinical usefulness.

4.A Study on Distribution of Heavy Metals in Normal Korean Tissues.

For the purpose of investigation to identify the quantities of heavy metals contained in the tissues of the Korean people, a series of analyses was conducted with atomic absorption spectrophotometry to measure the amount of lead, cadmium and copper distributed in various tissues, such as brain, liver, kidney, heart, lung, spleen, large intestine, hair, muscle, fat, stomach, costal cartilage, blood and urine, obtained from 30 cadaverous bodies who were believed not exposed to the said heavy metals during their life time either occupationally or therapeutically. 1. Lead content: Inter-individual difference was noted in lead contents in each tissue, vis., the average content of lead in hair was the highest with 14.90+/-9.74 ppm. The next was in costal cartilage that contained 5.06+/-3.86 ppm. The average contents of lead in liver, kidney and muscle were the lowest in value, showing 1.11+/-0.92 ppm, 0.73+/-0.48 ppm and 0.06+/-0.06 ppm, respectively. The lead contents in tissues of children under the age of 10 were significantly lower than those of adults, and the higher values were shown in finales than in females in general. The lead contents in most of the tissues such as hair, costal cartilage, kidney, lung, fat, stomach, large intestine, heart, muscle and urine were well correlated with age. 2. Cadmium content: The average content of cadmium in kidney appeared to be the highest, of ether tissues showing 20.7+/-29.82 ppm, and liver came nest with the value of 1.17+/-0.99 ppm. It was estimated that 83.9% of the total cadmium absorbed into the body was stored in kidney, 4.7% in liver, and the remaining 11.4% was distributed in the rest of the tissues. Cadmium contents in tissues showed difference between both sexes showing higher values in the females than in the males, which was quite contrary to the lead content. Cadmium contents in tissues steadily increased in amount with age, showing a significant correlation with age in all tissues. 3. Copper content: The average content of copper in hair was the highest with 10.36+/-2.21 ppm, and liver came next with 6.31+/-1.24 ppm. The copper that was absorbed into the body w4s distributed in each tissue: 29.9.% in hair, 18.2% in liver, 5~11% in brain, heart and kidney, and 3.0~3.5% in stomach, blood, and lung. The copper contents in tissues of children under the age of 10 showed significantly higher values in liver, kidney, heart and large intestine than those of the adults. The copper contents in brain, costal cartilage and fat were well correlated with age showing the highest correlation coefficient of 0.870 (p<0.01) in brain tissue. There was no difference in copper contents in tissues between both sexes, and the values were, in general, lower than those for Japanese.

Infectious conditions increase the incidence of autoimmune hemolytic anemia (AIHA), but reports of AIHA occurring after abdominal trauma surgery are rare. Therefore, we report a case of sepsis due to fasciitis and AIHA after abdominal trauma surgery which was successfully managed following the appropriate treatment of both conditions.

7.A Case of Subclinical Hypothyroidism Associated with Turner's Syndrome

Recently it is known that Turner's syndrome is frequently associated with hypothyroidism. We report a case of Turner's syndrome associated with subclinical hypothyroidism. A 23-year-old female was admitted to the hospital with complaints of amenorrhea and short stature. She had a mosaicism of 45, X0/46, Xi(X_q) in the cell, cultured from the peripheral blood. The plasma thyroxine and triiodothyronine were normal and there was no clinical symptom of hypothyroidism. But the thyroid-stimulating hormone(TSH) concentration was unusually higher(184 uU/L). She has been treated with the cyclic therapy of conjugated estrogen and medroxyprogesterone, in addition to the thyroxine replacement therapy. After 2 months, the menstruation was restored and TSH was normalized.

8.A Case of Kaposi's Sarcoma in a Systemic Lupus Erythematosus Patient after Long Term Immune Suppression.

A 41-year-old woman was admitted because of dyspnea on exertion for one month. She was diagnosed to have systemic lupus erythematosus 4 years ago and has taken prednisolone and azathioprine. One month prior to admission she visited dental clinic for painful gingival swelling and gingival biopsy was performed. Physical examination showed multiple round elevated purpuric rashes in thoracic wall. Gingiva and skin biopsy showed Kaposi's sarcoma. Computed tomography of abdomen and chest revealed ill-defined nodules in both lung fields and multiple small para-aortic lymphadenopathies. Chemotherapy with paclitaxel was given for Kaposi's sarcoma. Prednisolone was tapered. After the 7th course of chemotherapy, the lesions show marked improvement in size and number.

9.Clinical Observation on the Cryptorchidism.

A clinical observation was made on 116 patients of cryptorchidism who had been admitted to the Department of Urology, Gil Genersl Hospital during 34 months from March, 1992 to January l995. The results were as follows: 1. The age distribution varies from to 1 to 33 years and the most frequent group was l to5 years old(62%). 2. Bilateral cryptorchidism was found in 22 cases(19%), and in the case of unilatera1, right in 52 cases(44.8%) and left in 42 cases(36.2%). 3. Of the 138 testes, most frequent location was in the inguinal canal in 123 testes(89.1%). 4. Orchiopexy was perrormed on 130 testes(94.2%) by subdartos pouch technique and 7 testes were performed the orchiectomy 5. Among the associated anomalies, the most common anomaly was inguinal hemia in 131 cases and others were contralateral hydrocele, hypospadias, congenital heart disese and syndactyly.

10.Staurosporine Induces ROS-Mediated Process Formation in Human Gingival Fibroblasts and Rat Cortical Astrocytes.

In the present study, we investigated the effect of staurosporine on the formation of cellular processes in human gingival fibroblasts and rat astrocytes. Staurosporine caused a rapid induction of process formation in human gingival fibroblasts and rat astrocytes in a concentration dependent manner. The process formation of human gingival fibroblasts and rat astrocytes was prevented by the pretreatment with N-acetylcysteine, suggesting that staurosporine-induced ROS production was responsible for the process formation. Colchicine, a microtubule depolymerizing agent, inhibited the staurosporine-induced process formation, whereas cytochalasin D, an actin filament breakdown agent, failed to suppress the formation of cellular processes. This result indicated that polymerization of microtubule, and not actin filament, was responsible for the formation of cellular processes induced by staurosporine. In support of this hypothesis, Western blot analysis was conducted using anti-tubulin antibody, and the results showed that the amount of polymerized microtubule was increased by the treatment with staurosporine while that of depolymerized beta-tubulin in soluble fraction was decreased. These results indicate that staurosporine induces ROS-mediated, microtubule-dependent formation of cellular processes in human gingival fibroblasts and rat astrocytes.
