HIV-1 p24 was cloned into multiple cloning site of pMV261, extrachromosomal expression vectors carrying BCG replication origin and BCG-specific heat-shock promoter, and then introduced into BCG and E. coli. Western blot experiments showed that the p24 efficiently expressed in recombinant BCG (rBCG), but not in E. coli. Recombinant p24 expression induced by a single heat-shock of rBCG was maintained longer than 3 weeks. Immunoblot experiments with intact rBCG did not show any distinctive positive signal, suggesting that the recombinant protein was not secreted or exposed at the surface of BCG. The guinea pigs immunized with live rBCG showed delayed type hypersensitivity (DTH) by the systemic area as well as an effective humoral immunity, suggesting that tbis rBCG is believed to elicit eKcient immune responses against p24, even though the expression is restricted only in the cytoplasm as reported previously with other antigen. These results demonstrate that BCG can be developed as a live recombinant vaccine vector against a broad spectrum of infectious disease.

Bartter's syndrome is a rare tubular disorder characterized by hypokalemic, hypochloremic metabolic alkalosis, hyperreninemic hyperaldosteronism, hyporesponsiveness to pressor agents, and juxtaglomerular apparatus hyperplasia. We report here a case of Bartter's syndrome in a 5 month-old male infant who improved with potassium supplements. In addition to a case report, brief review of related literatures was done.

V. parahaemolyticus was isolated from blood culture of a 34-year old female patient with HCV viral hepatitis and liver cirrhosis. V. parahaemolyticus is one of the frequent causative agents of gastrointestinal infection, but rarely causes septicemia. This case is thought to be the 3rd report of V. parahaemolyticus septicemia in Korea.

PURPOSE: To examine how SCUBA diving activities in high-pressure underwater environment affect their intraocular pressure (IOP), visual field (VF), retinal nerve fiber layer (RNFL), and the shape of optic disc. METHODS: We performed visual acuity and refractory test, IOP test, VF test, RNFL photography, optical coherent tomography, and 3D optic disc photography for a group of 32 people and a control group of 32 non-divers, and analyzed the differences between the two groups. For the diver group, we conducted a questionnaire survey on the patterns of diving and diving experience, and analyzed their correlation with results of test. RESULTS: Compared to the Control, the diver group showed significant difference in the mean IOP (diver group: 15.71 +/- 2.54 mmHg, control group: 14.23 +/-2.15 mmHg, p = 0.019), and abnormal visual field (diver group: 7 eyes (11.3%), control group: 0 eye (0%), p = 0.006). 85.7% of abnormal visual field belonged to early defect. The diver group did not showed significant difference in the shape of optic disc (p = 0.546), but the optic nerve atrophy in shape of optic disc test field (diver group: 8 eyes (12.9%), control group: 2 eyes (3.1%), p = 0.042) was significantly different. CONCLUSIONS: Divers who did SCUBA diving activities need to have a glaucoma test regularly. Additional research and large cross or longitudinal study are needed to evaluate causes that scuba diving activities affect.

We report herein an unusual case of tick bite. A 56 year old woman complained of erythematous nodular skin lesion on the left upper chest. An excision biopsy specimen of the lesion showed darkish-red nodule, in which there was a tick. It was identified as an adult female of Ixodes nipponensis by the scanning electron microscopic examination.

u-Health is a good alternative in providing health care service under special situations where access to health care is limited. With the expansion of life space and the increase of travel, people are being put in danger across various situations on the sea, in the sky, disasters, and so on. It is not easy to provide health care in those situations, while people may still have high expectation to heath care. This gap can be narrowed by using u-Health, which is based on advanced information and communication technologies. The special situation for u-Health is where normal health care cannot be performed because of physical limitation and danger, which can be further broken down to five situations. The first is a situation on means of transportation such as ship and airplane. The second situation is when medical facilities are far away from life space, in places like backwoods. The third situation occurs on the place where it is difficult for patients to receive treatment due to restrained freedom, for example, a prison. The forth is the situation where the medical team is unable to approach easily, such as war zone and disaster area. The last special situation is the extreme environments like polar region. In order to make the u-health system more helpful for patients under special situations, there is a need for a research development and investment on sensors to measure accurate bio-signals, the network to transmit the data, and the technologies to analyze the data and to provide feedback. Therefore, institutional supports for technology development are required for further development of u-Health for people in great needs.

PURPOSE: The purpose of this study was to evaluate the appropriateness of prehospital emergency care by 119 rescue services in Seoul. METHODS: We enrolled all patients who used 119 rescue services in Seoul between January and Sepember 2006 and analyzed the 119 ambulance patient care record databases. Major emergencies were defined and categorized into four groups: cardiopulmonary complaint (CP), neurologic complaint (NR), cardiorespiratory arrest (CA) and major trauma (MT). Interventions provided by emergency medical technicians were scored as 2:essential, 1:optional and 0:unnecessary in each category of major emergency. We summed scores of provided interventions per patient and graded them into four levels for conducting the appropriateness review. Multivariate logistic regression analysis was performed to determine the relationship of risk factors to the appropriateness of intervention. RESULTS: Total number of cases was 171,112 of which 36,065 were major emergencies. The proportion of "very low" level appropriate was 40.1% for CP (N=11,522). 49.3% for NR (N=15,239), 14.5% for CA (N=1,591), and 32.7% for MT (N=7,713). The proportion of "high" level was less than 3% except for CA(11.3%). Variables affecting the "moderate" or "high" level of appropriateness of prehospital emergency care, as determined by multivariate logistic regression analysis, were age > or =65(OR: 1.425, 95% CI: 1.337-1.520), female gender (OR: 0.880, 95% CI: 0.828-0.935), and type of complaint in the order CP (OR: 0.181, 95% CI: 0.161-0.204), NR (OR: 0.184, 95% CI: 0.164-0.207), and MT (OR: 0.202, 95% CI: 0.178-0.229). CONCLUSION: The prehospital emergency care of 119 rescue services in Seoul was frequently inappropriate and was influenced by patient's age, gender and chief complaints.

BACKGROUND: The microRNA (miRNA) pathway has emerged as one of the biologic pathways implicated in stem cell regulation. miRNA is a noncoding, single-stranded RNA consisting of 20-25 nucleotides that inhibits the protein production at the step of translation. The molecular effects of lidocaine and procaine on adipose stem cells were investigated by examining RNA expression array. METHODS: Adipose stem cells were isolated from a prior abdominal liposuction procedure. The human adipose stem cells were cultured and then added to a mixture of 1 ml of culture medium plus 1 ml of 2% lidocaine or 2% procaine for the duration of 30 minutes. The expression levels of miRNAs were estimated by using peptide nucleic acid (PNA)-miRNA array analysis throughout the denaturation and hybridization processes after the isolation of miRNA. The miRNAs detected by microarray that either decreased by half fold or increased by 1.5 fold from the control level were interpreted as significant. RESULTS: According to microarray analysis there were 61 miRNAs in total, and no miRNA had decreased expression levels. The stem cells treatment with lidocaine showed 4 alteration of expression with miR-9a* (1.53 fold), miR-29a (1.64 fold), miR-296-5p (1.64 fold) and miR-373 (1.94 fold). The stem cells treated with procaine showed 32 miRNAs that were significantly up-regulated with a range of 1.5 to 2.06 fold. They were stem cell differentiation-related miRNAs, apoptosis and cell cycle-associated miRNAs, immunity-associated miRNAs and hormonal response-related miRNAs. CONCLUSIONS: Lidocaine and procaine affect the miRNA expression on adipose stem cells and the effect of procaine is more marked than that of lidocaine.
