1.Pulmonary artery sling: case report.

Aberrant left-sided pulmonary artery(pulmonary artery sling) is an uncommon anomaly, which may cause significant respiratory abnormality. We report a case of pulmonary artery sling which is combined with persistent left superior vena cava and dextrocardia. This case was identified by esophagogram and CT and confirmed by MRI and angiography. We consider that MRI is a valuable new method for the diagnosis of aberrant left-sided pulmonary artery.

2.AN IMMUNOHISTOCHEMICAL STUDY ON THE EXPRESS1ON OF TGF-beta IN THE AMELOBLASTOMA AND DEVELOPING TOOTH GERM OF HUMAN EMBRYO AND FETUSES.

Dysregulation of TGF-beta that is a modulator of cell growth and differentiation can result in uncontrolled growth and tumor formation. Monitoring these pattern of genes and protein expression during tumor development will provide a basis for understanding normal growth and distortion of osteochondrogenesis. Our comparative studies on the experssion of TGF-beta protein indicate that TGF-beta may primarily be a regulator of epithelial differentiation during tooth development (between 4 weeks and 40 gestational weeks) and tumorigenesis of odontogenic neoplasm (ameloblastoma). In early human tooth germ (cap/early bell stage) TGF-beta protein was found in the epithelial dentallamina and enamel orgen. TGF-beta experessions were seen in inner and outer dental epithelium during bell stage. During enamel and cementum appositional stage, TGF-beta expression shifted from the ameloblast to the odontoblast. In eruption stage, TGF-beta expressions look like moderate positive pattern in odontoblast and rare pattern in enamel, dentin and cementum. TGF-beta expressions of follicular and plexiform amelobalstoma are moderate positive reaction in ectodermal tumor components and mild positive in vessels of stroma tissue. In acanthomatous type, TGF-beta expression was shown severely positive finding in stellate reticulum cell. TGF-beta expressions were noted moderate reaction in margin of tumor epithelium and stellate reticulum cell of granular ameloblastoma. In unicystic ameloblastoma, TGF-beta expression was rare feature in cystic luminal epithelium. With these result, I should be suggested that TGF-beta may play an important role in epithelial-mesenchymal interactions in human tooth morphogenesis and development of odontogenic epithelial tumors.

4.Observations of Arrhythmias by 24 Hour Ambulatory ECG Monitoring in Early Convalescent Phase of Acute Myocardial Infarction.

In order to observe the arrhythmia which occured in early convalescent phase of acute myocardial infarction, a 24 hour ambulartory ECG monitiring was performed in 15 patients who is admitted at dept of Int Medicine, Ewha Womans University hospital from Aug. 1985 to Sept. 1986. during 7.5+/-3.4 days after admission. In all cases one or more of the following arrhythmias were observed;atrial premature beat, ventricular premature beat(VPB), transient paroxysmal ventricular thchycardia, atrial fibrillation, sinus arrest and junctional beat. Of 15 cases, 11(77.3%) had VPB's:Simple VPB's in 4 cases(26.7%) and complex VPB's(6.52+/-2.52)(P<0.05). But there were no difference of premature and abberancy index between complex VPB's and simple VPB's(P>0.1). In addition, we compaired the arrhythmias in CCU with those of early convalescent phase of acute myocardial infarction. But there was no correlation between them.

7.Treatment of Tachycardia by Radiofrequency Catheter Ablation in Children and Adolescents.

PURPOSE: RFCA has been proven to be an effective and safe tool for treating different kinds of tachycardia in adults. This study was designed to analyze the efficacy of this method in children and adolescents. METHODS: Seventy-eight patients referred to Gachon Medical School, Gil Hospital for ablation of supraventricular(SVT) and ventricular tachycardia(VT) between January 1997 and February 1999 were included in this study. An electrophysiologic study was performed in the same session to assure the diagnosis, and meet the appropriate criteria for ablation. All patients had regular follow-up at our center. RESULTS: Mean age of the patients was 9.9 years. Thirty-seven patients had Wolff-Parkinson- White syndrome, atrioventricular nodal reentrant tachycardiain in 12 patients, atrial flutter in 11 patients, atrial tachycardia in 9 patients, Mahaim tachycardia in 2 patients, nodoventricular tachycardia in one patients and VT in 6 patients was found. Seventy-seven patients were treated successfully(98.7%), 75 patients were treated in a single procedure, but 2 patients needed a second attempt. RFCA treatment failed in only one patient with nodoventricular tachycardia. In three of the 77 successfully treated patients(3.9%), tachycardia recurred after 1-3 months but could be treated by a further intervention. The duration of follow up after RFCA was from 2 to 27 months. CONCLUSION: RFCA is a highly effective method in the treatment of SVT and VT in children and adolescents. The major benefit of this technique is its potential as a cure for a chronic disease. RFCA can be used as the primary treatment for SVT and VT.

Extramammary Pagets disease(EMPD) of the vulva is an uncommon neoplasm which accounting for 2.5% of all vulvar malignancies and 0.02% of all female cancer. Several hundred cases have been reported worldwide since the first description by sir James Paget in 1874, It generally affects postrnenopausal age poup and presents high rate of recurrence but the precise incidence remines unclear, Although Paget's disease of the breast and the vulva are histologically similar, their histogenesis and biological behavior are considered to be significantly different and EMPD has gcnerally perceived rnore benign compared to that of the breast. Four histologic forms of vulvar EMPD have been recognized and treated vulvar EMPD in according to the 4 histologic classification. The high rate of recurtence disease remains a challenge for optimal management. Recently we experienced a case of Paget's disease of the vulva, diagnosed as an intraepithelial Pagets disease preoperatively and underwent radical vulvectomy and superiomedial thigh(SMT) flap, and present with a brief review of literature.

9.The Effects of Nitric Oxide and Sphingosine 1-phosphate on the Pulmonary Microstructure in a Rat Model of Acute Lung Injury: An Intravital Videomicroscopic Approach.

BACKGROUND: To evaluate the effects of inhaled nitric oxide (NO) and sphingosine 1-phosphate (S1P) as potential therapeutic agents of acute lung injury, we analyzed the morphology in vivo of the pulmonary microstructure using intravital videomicroscopy in a rat model of acute lung injury. MATERIAL AND METHOD: Sprague Dawley rats were divided into five groups: a control group that underwent normal saline aspiration, an acute lung injury (ALI) group that underwent hydrochloric acid aspiration, and three treatment groups that underwent hydrochloric acid aspiration and were administered therapeutic agents- the S1P group, the NO group, and the S1P+NO group (n=7 per group). To quantify alveolar compliance and interstitial edema, the diameters of all measurable alveoli and interalveolar septa were averaged at one and two hours after aspiration. Alveolar compliance was determined according to diameter changes during the respiratory cycle and the change in tidal volume. RESULT: At two hours after aspiration, the mean alveolar compliance (% change) in the ALI group decreased significantly versus the control group of rats (respiratory cycle: 1.9% for the ALI group vs 6.5% for the control group, p=0.03; tidal volume: 3.2% for the ALI group vs 9.1% for the control group, p=0.003) and versus the NO group (tidal volume: 3.2% for the ALI group vs 16.9% for the NO group, p=0.001). At two hours after aspiration, the mean interalveolar septal thickness in the NO group tended to be smaller as compared to that in the ALI group (15.2micrometer for the ALI group vs 12.3micrometer for the NO group, p=0.06). S1P did not exert a significant effect on the pulmonary microstructure of the injured rat lung. CONCLUSION: Improved alveolar compliance and reduced interstitial edema, observed by intravital videomicroscopy, suggest that inhaled NO ameliorates lung injury.

Pulmonary embolism is a serious complication, which is well known in patients undergoing total hip or total knee arthroplasty or lower extremity fracture surgery. But, there are few literatures concerning pulmonary embolism after upper extremity surgery. Pulmonary embolism after minor upper extremity fracture surgery is extremely rare. We report a case of 66-year-old female patient that developed pulmonary embolism after percutaneous cannulated screw fixation for a greater tubercle fracture of the proximal humerus with literature review.
