4.Cystic Neoplasms of the Pancreas.

BACKGROUND: Cystic neoplasms of the pancreas are rare tumors that constitute about 10% of all cystic lesions of the pancreas and less than 1% of all pancreatic neoplasms. This study was conducted to investigate the diagnostic accuracy and postoperative results for these rare tumors. METHODS: The authors reviewed retrospectively 7 patients with cystic neoplasms of the pancreas who had been treated surgically in the period from 1986 through 1998 at the Department of Surgery, Paik Hospital, Seoul. RESULTS: There were 3 serous cystadenomas, 3 mucinous cystadenomas, and 1 papillary and cystic neoplasm. All 7 patients were women. The mean age of the patients was 36.7 years, 45 years for the serous tumors and 37 for the mucinous tumors. Two patient had diabetes mellitus. Major symptoms were abdominal pain (71%) and an abdominal mass (29%). One patient with a mucinous cystadenoma presented with bleeding from gastric varix which developed as a consequence of a splenic vein obstruction by the tumor. The positive rate for CEA was 33.3%, and that for CA 19-9 was 50.0%. No patient showed an abnormally elevated serum amylase level. Computed tomography was helpful in differentiating cystic tumors from pseudocysts or adenocarcinomas of the pancreas. The sites of the lesions were the tail in 3 cases, the body in 2cases, and the head in 2 cases. A distal pancreatectomy was performed in 5 cases, a pylorus-preserving pancreatoduodenectomy in 1 case, and enucleation of the cyst in 1 case. All the patients were alive at 3 to 145 months postoperatively and showed no evidence of disease. CONCLUSION: We conclude that the prognosis for resected cystic neoplasms of the pancreae is good. The correct preoperative differential diagnosis of pancreatic cystic lesions is important for planning the appropriate management, even though the clinical and the radiological features of these lesions are not always reliable.

5.The Effect of a 5-0 Prolene Ligation Around the Ahmed Glaucoma Valve Tube on the Postoperative Hypotony.

PURPOSE: To evaluate the efficacy and safety of 5-0 prolene ligation around the Ahmed glaucoma valve tube to prevent the postoperative hypotony and flat anterior chamber. METHODS: Medical records of twenty one eyes of 20 patients those had got Ahmed valve implant surgery with temporary ligation of the tube with 5-0 prolene were retrospectively evaluated for efficacy of the pressure control and complications before and after the remove of the ligation suture. RESULTS: Preoperative intraocular pressure(IOP) was 46.1+/-9.1 mmHg. The ligation suture was removed at 2.8+/-1.5 postoperative day. Postoperative IOP before the removal of the suture was 24.8+/-15.4 mmHg at the day of removal. After removal of the ligation suture, the IOP decreased to 15.6+/-5.9 mmHg and 12.2+/-4.2 mmHg at one day and 12 weeks after the removal respectively. Complications before the removal of the ligation suture were mild hyphema and corneal edema, developed in one eye each. After the removal, shallow anterior chamber and choroidal detachment was developed in one eye. CONCLUSIONS: Although the efficacy of the temporary ligation of the tube in Ahmed valve implant surgery in controlling the IOP was similar to that of conventional Ahmed valve implant surgery without ligation, the early postoperative complications associated with the overfiltration appeared to be less frequent than conventional Ahmed valve implant surgery.

6.Clinical Effects of Propafenone and Disopyramide on Ventricular Premature Complexes: Double-Blind, Placebo-Controlled, Randomized Cross-Over Study.

BACKGROUND: Propafenone is a new class Ic antiarrhythmic compound.This study was performed to evaluate the clinical efficacy and safety of propafenone by double-blind, placebo-controlled, randomized cross-over comparison of propafenone and disopyramide in patients with stable ventricular ectopy. METHODS: All patients were included in the study if they had an average of at least 30 PVC/hr on a 24-hour Holter recordings. During the 1st 7 days, two placebo tablets(identical in apperance to the propafenone and the disopyramide tablets) were administrated in a double-blind manner(run-in period). Then 1st treatment period lasting 1 week with one verum and the other placebo, wish-out period of 3 day,2nd treatment period lasting 1 week with cross-over drugs were followed. RESULTS: Twenty patients were enrolled. During the run-in period, VPCs were reduced to 18%, compared to the baseline data before the administration of placebo.During the treatment period,propafenone 600mg/day reduced VPCs by 43% and disopyramide 400mg/day reduced VPCs by -10% Propafenone was effective(80% or greater reduction of VPCs) in 7 of 20 patients. Disopyramide was not effective in all patients. Propafenone and disopyramide produced no significant change of paired VPCs and VT events. Propafenone had no effect on heart rate. It increased the PR interval(7.9%;p<0.01) and QRS interval(5.2%;p<0.01). The drug did not change QTc interval(-1.1%) significantly. There were no cardiovascular side effects. Propafenone produced nausea in one patient. Disopyramide produced dysuria in 2 patients. CONCLUSIONS: Propafenone was more effective in controlling VPC than disopyramide, and there was no major limiting side effects.

8.Clinical course of infants with congenital heart disease who developed thyroid dysfunction within 100 days.

PURPOSE: We investigated the clinical course of infants with congenital heart disease (CHD) who experienced thyroid dysfunction within 100 days of birth. METHODS: We performed retrospective medical reviews of 54 CHD patients (24 male patients) who underwent a thyroid function test (TFT) between January 2007 and July 2016. Data were collected on birth history, diagnosis of CHD, underlying chromosomal or genetic abnormalities, medication history, surgery, ventilator care, and exposure to iodine contrast media (ICM). Results of neonatal screening tests (NSTs) and TFTs were reviewed. RESULTS: A total of 36 patients (29 transient, 7 permanent) showed thyroid dysfunction. Among the seven patients with permanent hypothyroidism, three had an underlying syndrome, three showed abnormal NST results, and one was admitted to the intensive care unit for macroglossia and feeding cyanosis. We found that infants with transient thyroid dysfunction had a lower birth weight and were more commonly exposed to thyroid disrupting medication and/or ICM. However, these risk factors were not significant. A total of 8 patients with a history of ICM exposure showed thyroid dysfunction. Excluding 3 patients with elevated thyroid stimulating hormone before ICM exposure, 5 patients recovered from transient thyroid dysfunction. CONCLUSIONS: We observed thyroid dysfunction in two-thirds of CHD infants (53.7% transient, 13.0% permanent) who had risk factors and received TFT screening within 100 days, despite normal NSTs. Further studies with larger sample sizes are required to revise the criteria for TFT screening in CHD infants.

10.Novel Vaccine Development against Escherichia coli O157: Immunologic Properties of O-Specific Polysaccharide-Shiga-like Toxin Conjugate Vaccines .

Escherichia coli (E. coli) O157:H7 is an important cause of hemorrhagic colitis (HC) and hemolytic uremic syndrome (HUS). LPS-based vaccines need improvement since the anti-LPS antibodies raised by the vaccines are bactericidal and release toxin that may precipitate the development of HUS. Despite huge efforts, the treatment of infection with E. coli O157 has been difficult because antibiotics do not change the course of the enteritis caused by E. coli O157 and may increase the incidence of HUS through the release of Shiga-like toxin (Stx)-I. For this aim we tried the conjugate of E. coli O157 OSP bound to the nontoxic B subunit of Stx-I B as a vaccine that can induce both serum IgG anti-LPS antibody with bactericidal activity and neutralizing antibody to Stx-I. Mice were immunized s.c. with OSP-Stx-I B conjugate. Anti-sera were analyzed for the anti-LPS antibody, anti-Stx-I B antibody, complement-dependent bactericidal activity, and Stx-I neutralization activity. Mice injected with the bivalent conjugate elicited both bactericidal antibodies to E. coli O157 and neutralization antibodies to Stx-1. We also analyzed the distributional, functional changes of T lymphocytes in vitro and in vivo. After the injection of Stx-I, splenocytes showed a decrease in proliferation when stimulated with phytohemagglutinin (PHA) or LPS, and the number of CD4+ and CD8+ T cells also decreased. Interleukin (IL)-2, IL-4 and IFN-gamma productions by CD3+ T cells were slightly increased in the Stx-I injected mice.
