4.The Evaluation of Liver Cytosol Extracts Affecting Liver Regeneration after Partial Hepatectomy in Rat.

It was known that the regenerating liver cytosol accelerated liver regeneration, whereas normal liver cytosol inhibited it during first 28 hours, but promoted it later by the thymidine incorporation test. In experiments, Sprague-Dawley male rats were divided into 3 experimental groups (A, B and C) which had 10 rats in each. Also, each group was equally subdivided into 2 subgroups such as A-8 and A-28 in the A group, B-8 and B-28 in the B group and C-8 and C-28 in the C group. All groups were partially hepatectomized equally. The remnant livers were procured at 8 hours in the 3 subgroups A-8, B-8 and C-8 and at 28 hours in the other 3 subgroups A-28, B-28 and C-28. Another experiment was performed in 10 rats under the same partial hepatectomy to get the average weight of the remnant liver. These data could be taken as the initial weight of the remnant liver in the above experimental animals. The normal saline, normal liver cytosol extract and regenerating liver cytosol extract were injected intraperitoneally after the partial hepatectomies, 4 ml of in the A, B, and C groups, respectively. Since specific structural changes might precede demonstrable functional alterations, morphometric measurements were done in the histological specimens of the 3 subgroups A-8, B-8, and C-8. The remnant livers, obtained in all six subgroups, were weighed to calculate the regeneration rates. The overall results are as follows : 1) The liver regeneration rates in the six subgroups were 4.1% in A-8, 3.8% in B-8, 4.3% in C-8, 28.4% in A-28, 16.7% in B-28, and 32.8% in C-28. 2) The morphometric study showed that the nucleus/cytoplasm ratios in the three subgroups were 0.1389 in A-8, 0.2048 in B-8, and 0.3705 in C-8. These results mean that regenerating liver cytosol extract promotes liver regeneration and that normal liver cytosol extract inhibits it during first 28 hours after a partial hepatectomy, but promotes cell division for subsequent regeneration.

5.Status of intestinal parasitic infections in a remote island, Yondo, Jeonranam-do(province).

This study was performed to observe the present status of intestinal parasitic infections in a remote island, Yondo, located in southern part of Jeonranam-do(province), Korea. In February and May 1988, total 1,011 individual stool samples were collected and examined for intestinal helminths and protozoa using formalin-ether centrifugal sedimentation technique. The results are summarized as follows: Of 1,011 ingabitants examined, 398(39.4%) were positive for intestinal parasites. Helminth positives were 372(36.8%), and protozoan cyst positives were 56(5.5%). Ten species of parasites were found. Trichuris trichiura revealed the highest infection rate of 27.5%, Ascaris lumbricoides 17.4%, Taenia sp. 5.8%, Entamoeba coli 3.3%, Giardia lamblia 1.5%, Endolimax nana 0.8%, Hymenolepis nana 0.4%, hookworm 0.2%, Trichostrongylus orientalis 0.2%, and Entamoeba histolytica 0.2%, respectively. The female group showed higher positive rate (44.0%) than males (34.7%). Also, higher positive rates were observed among adults as compared with the group younger than 10 years old. Average value of E.P.G. was 1,876(range 200-17,800) in A. lumbricoides positives, and 327(range 200-1,600) in T. trichiura positive cases. In helminth egg positive cases, single infection was 63.4%, double infection 34.7%, and triple infection 1.9%, respectively. Among protozoan cyst positives, single infection was 94.6%, and double infection was 5.4%. The present study revealed that the prevalence of intestinal parasites among inhabitants in Yondo island is still so high that special control measures should be performed.

Cystic dilatation of bile duct, so called choledochal cysts are a well documented abnormalities. Although choledochal cyst is a relatively rare disease, the number of reports have been increasing especially in tht Orient. It may be easily overlooked by the conventional diagnostic methods, such as upper gastrointestinal series, intravenous cholangiography and abdominal ultrasonogrsphy. Recent advances on diagnostic methods, such as endoscopic retrograde cholangiopancreatography and percutaneous transhepatic cholangiography had been helpful in demonstrating choledochal cysts. 29 patients with choledochal cysts who had been admitted in Hanyang Universty Hospital between September l981 and August 1991 were reviewed and the clinical endoseopic and rediographic findings were discussed comparing with that from foreign literatures. 7.A fatal case of acute arsenic poisoning.

Arsenic trioxide is the most prevalent natural inorganic form of arsenic, which is a widely used heavy metal and is ubiquitously distributed in the environment. A 71-year-old man ingested about 10 g of arsenic trioxide powder in an attempted suicide. He developed severe vomiting, watery diarrhea and abdominal pain, and he presented to the emergency room 15 hours after the ingestion. On admission he was slightly drowsy, but shocked. He showed metabolic acidosis, acute renal failure and abnormal electrocardiogram suspecting myocardial infarction. The chest and abdominal X-rays revealed radiopaque materials in the stomach and small bowel. The stomach was irrigated via a nasogastric tube and activated charcoal was given to bind unabsorbed arsenic. The whole bowel irrigation was tried to remove the remained arsenic in the gastrointestinal tract and BAL (British anti-lewisite) was repeatedly administrated. Despite of intensive supporting management with supplemental oxygen, aggressive volume expansion, correction of metabolic disturbances and administration of cardiotonic agents, he developed progressive hypotension and died 42 hours after the ingestion. Random urine arsenic concentration, which was collected at the time of admission, but confirmed after death, was 3,564 microgram/L.

The varicella during pregnancy is rare. The incidence of varicella among pregnant women is about 5 in 10,000 pregnancies. In 1947, Laforet and Lynch were the first to describe an association between maternal varicella infection in early pregnancy and congenital anomalies in the offspring. the existence of a syndrome was first postulated by Srabstein et al(1974), Brunell(1983), Kotchmar(1984), and recently Higa et al.(1987) described most of the malfomations associated with maternal varicella infection. Varicella-Zoster viurs(VZV) infections acquired during pregnancy may result in serio us disease for both the mother and the fetus. The effects on maternal health can range from a mild chikenpox exanthem to servere life-theatenig illness often complicated by viral pneumonitis. The fetus may remain unaffected, but VZV infection during early pregnancy has been associated with a congeital syndrome including muliplie defects of the skin, limb, eyes, and brain. We reports our experience with a varicella during pregnancy with a brief literature review.

OBJECTIVE: To describe the thin-section CT findings of arc-welders' pneumoco-niosis. MATERIALS AND METHODS: Eighty-five arc-welders with a three to 30 (mean, 15)-year history of exposure underwent thin-section CT scanning. The extent of abnormalities detected was correlated with the severity of dyspnea and pulmonary function tests. For comparison, images of 43 smoking males (mean 25 pack-year) who underwent thin-section CT for other reasons (smokers' group) were also analyzed. RESULTS: Fifty-four welders (63.5%) and six smokers (14.0%) showed positive findings. Predominant thin-section CT findings were poorly-defined centrilobular micronodules (30/54, 55.6%), branching linear structure (18/54, 33.3%), and ground-glass attenuation (6/54, 11.1%). In the smokers' group, poorly-defined micronodules were found in four patients, branching linear structures in one, and ground-glass attenuation in one. In welders, the extent of abnormalities seen on thin-section CT showed no significant correlation with the severity of dyspnea or the results of pulmonary funotion test. CONCLUSION: Poorly-defined centrilobular micronodules and branching linear structures were the thin-section CT findings most frequently seen in patients with arc-welders' pneumoconiosis. Less commonly, extensive ground-glass attenua-tion was also seen
