1.A Case of Erdheim-Chester Disease with Laryngeal Involvement.

Erdheim-Chester disease (ECD) is a rare form of non-Langerhans cell histiocytosis with multi-organ involvement. Bones, cardiovascular system, central nervous system, kidney, skin and many different organs can be involved but laryngeal involvement has not been reported in literatures. The diagnosis of ECD was based on clinical manifestations and immunohistochemical findings including CD68(+), CD1a(-) and S100(-). Currently, Interferon-a is the most extensively studied agent in the treatment of ECD and serves as the 1st line of treatment. Surgical resection of involved lesions can be tried but it leads to temporary improvement. A 60-year-old man visited with respiratory symptoms because of ECD with laryngeal involvement. We resected the obstructive lesion to relieve the symptoms. Tissue biopsy gave a diagnosis of ECD. As it is rare to encounter ECD involving the larynx, we report this case with a review of literatures.

2.Changes in characteristics of drug intoxication after COVID-19 pandemics: a retrospective study in a tertiary university-affiliated hospital

Objective:
The coronavirus disease 2019 (COVID-19) virus has adversely affected people in socioeconomic fields as well as mental health, thereby increasing suicidal attempts. This study analyzes the altered characteristics of drug-poisoned patients visiting the emergency department (ED) during the COVID-19 pandemic era in South Korea.
Methods:
This was a retrospective observational study using medical records of adults who visited the ED for intoxication. Data obtained were compared before and after the COVID-19 pandemic era.
Results:
In all, data from 1,102 drug-poisoned patients (586/516 during the pre-/post-COVID-19 periods, respectively) were included in the study. Despite a decrease in total ED visits, the pandemic period saw an increase in the proportion of poisoned patients visiting the ED. Young, female, and psychiatric patients accounted for a significant portion. Months, holidays, time zones, and urbanization showed no difference in the incidence of intoxication. Financial difficulty and couple and family troubles were determined to be frequent causes of intoxication during the COVID-19 pandemic. Comparing the drugs intoxicated, pesticide ingestion was decreased, while the ingestion of other less-fatal drugs was comparatively increased. Although there was an increase in the admission rate of poisoned patients, the severity during admission and outcomes at discharge were not worse after the COVID-19 pandemic era.
Conclusion
The COVID-19 crisis has seriously impacted people, especially female, young-aged, and psychiatric patients.
3.Clinical Analysis of Subdural Hygroma.

We have analysed 111 cases of subdural hygroma that were development after various neurosurgical conditions including head injuries. The results were summarized as follows. 1) The peak incidence was the age of 50's and occured most frequently in male. 2) Causes were head injuries(91 cases), traumatic intracranial hematoma removal(10 cases), operation for intracranial aneurysms(4 cases), hypertensive intracerebral hemorrhage evacuation(2 cases), ventriculoperitoneal shunt(2 cases), brian tumor removal(1 cases), arachnoid cyst excision(1 cases), and cerebral infarction(1 cases). 3) Acute subdural hematoma was most often associated with complex subdural hygrom. 4) Clinical manifestations were headache, altered mental state, disorientation, nausea in order. 5) Operation were underwent in 27 cases and surgical complications were reaccumulation and pneumocephalus in order. 6) Mortality rate was 6.3%. 7) The prognois of the simple hygroma was very good, but the complex was not.

4.A Case of Pulmonary Air Embolism during Endoscopic third Ventriculostomy: A Case Report.

Several cases of pulmonary air embolisms during surgery have been reported. However, such incidences are very rare during endoscopic surgeries. A 5-year-old boy with third ventricular arachnoid cyst and hydrocephalus underwent endoscopic third ventriculostomy under the general anesthesia, during which continuous intraventricular irrigation was maintained with normal saline. During the procedure, arterial and transcutaneous oxygen tension and end-tidal carbon dioxide tension became suddenly decreased while arterial carbon dioxide tension increased. Within 3 minutes after the inspired gas mixture was changed to 100% oxygen, the patient's respiratory variables returned to near base line. The second attack occured about 10 minutes later and decreased arterial oxygen tension with increased arterial carbon dioxide tension continued for about 5 minutes. After waking up from anesthesia, the patient suffered a generalized seizure attacks that was managed with anticonvusant therapy. We believe that we are the first to report an attack of pulmonary air emblism during brain endoscopic procedure. It was reported with the review of literatures.

5.Clinical Result of Arthroscopic Capsular Release and Repair for SLAP II Lesion with Stiffness.

PURPOSE: We assessed the clinical results after the operative treatment of type 2 SLAP lesion with stiffness. MATERIALS AND METHODS: 13 patients who had SLAP lesion with stiffness were treated with arthroscopic capsular release, SLAP repair and treatment of the associated lesion. The average follow-up period was above 12 months. RESULTS: The postoperative mean VAS was scored 1.5 and the postoperative ROWE score was 92.3, which showed a significant improvement after the operation (P<0.001). The mean range of motion was a significantly improved after the operation (P<0.001). The ROWE score was excellent for all the cases. CONCLUSION: Arthroscopic capsular release and SLAP repair and treatment of the associated lesion in patients with type 2 SLAP lesion with stiffness are effective treatments for the increasing the range of motion and decreasing the pain.

6.The Effect of Derotational Closing Wedge Akin Osteotomy for the Treatment of Hallux Valgus with the Pronation of Great Toe.

PURPOSE: We tried to understand the effects of the derotational closing wedge Akin osteotomy during the operation for the hallux valgus with pronation of great toe. MATERIALS AND METHODS: Eighty five patients who had undergone Akin osteotomy among the eighty seven patients who had been treated with Scarf osteotomy with hallux valgus were included in this study. Derotational supination was added on the medial closing wedge Akin osteotomy at the base of proximal phalanx and it was secured with K-wire, headless screw or staple. We measured and analyzed pre- and post-operative hallux primus valgus angle and hallux pronational rotatory angle. RESULTS: The hallux primus valgus angle improved an average of 14+/-2.98 degrees to -1+/-1.68 degrees with the hallux pronational rotatory angle respectively from 24.8+/-7.64 degrees to 4.7+/-4.22 degrees. CONCLUSIONS: After the metatarsal osteotomy for the treatment of the hallux valgus with the pronation of great toe, derotational closing wedge Akin osteotomy can give us a belief that it can correct the hallux primus valgus angle and hallux pronational rotatory angle also and it can be a helpful method for minimizing the recurrence rate of the hallux valgus deformity.

Kohler's disease is an uncommon idiopathic osteochondrosis of the tarsal navicular. The diagnosis is clinical and the natural history is benign. The treatment is symptomatic, including shoe supports when the symptoms are mild and initial cast immobilization for at least 8 weeks when the symptoms are more intensive. The final clinical outcome is always favorable. Although it has been fully described both clinically and radiologically, There have been a few long-term follow-up reports in worldwide. This case report presents a Kohler's disease of the tarsal navicular with long-term follow-up of 12 years.

9.The Modified Phemister Operation with the Suture Anchor Added for the Augmentation of Conoid Ligament in Acute Acromioclavicular Dislocation.

PURPOSE: The purpose of this study was to present methods and results for the modified Phemister operation, with a suture anchor added for augmentation of the conoid ligament in cases of acute dislocation of the acromioclavicular joint. MATERIALS AND METHODS: We evaluated 14 cases of acute dislocation of the acromioclavicular joint. This included 11 cases of Rockwood type 3, and 3 cases of type 5. The mean age of patients was 45.2 years. We operated on them using an anchor for augmentation of the conoid ligament in the modified Phemister operation. The average follow-up period was 14 months and post-operative clinical analysis was conducted using the Weitzman classification, VAS Score, Constant Score and KSS Score. RESULTS: According to Weitzman scores, 13 cases were evaluated as excellent, and one case was good. They had mean joint ranges of forward elevation of 170.7degrees, lateral elevation of 166.4, external rotation of 68.2, and internal rotation to the level of T7. The mean VAS Score was 1.9, mean Constant Score 90.8, and the mean KSS Score 91. Radiologic analysis indicated that all cases had a good result. CONCLUSION: The modified Phemister operation with a suture anchor added for augmentation of the conoid ligament is very effective clinically in acute dislocations of the acromioclavicular joint.

10.Unilateral Menisco-Meniscal Ligament.

Menisco-meniscal ligaments in knee joint are known as four variants, anterior and posterior transverse meniscal ligament, medial and lateral oblique menisco-meniscal ligament. The ligament which originates from the anterior horn of the meniscus and attached to the posterior horn of the same meniscus, so-called unilateral menisco-meniscal ligament is extremely rare in English literature. The authors experienced a case of medial unilateral menisco-meniscal ligament with posterior horn tear of the medial meniscus in a 49-year-old man. We report this case with a review of literature.
