1.The Comparison of Therapeutic Effectiveness Between Lesional and Whole Body Exposure on Oral PUVA for Generalized Vitiligo.

BACKGROUND: For the treatment of generalized vitiligo patients with oral PUVA, we can use two different methods; one is to treat the lesions while the whole body is exposed. Another one is to treat the lesions while only the lesions are exposed. PURPOSE: This study was performed to determine whether lesional and whole body exposure in oral PUVA for generalized vitiligo show any therapeutic differences in effectiveness. METHODS: The vitiligo lesions were distributed over the whole body skin of the subjects and the lesion area was less than 6% of the whole skin area. PUVA was done to the subjects more than 20 times after oral administration of psoralen. The patients were classified into two different groups. One is the lesional exposure group in which the patient exposed only the vitiligo lesion. The other is the whole body exposure group in which the patient exposed almost their whole body. RESULTS: Our results show that there is no statistical difference of the therapeutic effectiveness between the two methods. CONCLUSIONS: We recommend lesional treatment rather than whole body treatment to prevent the oral PUVA side effects.

3.Cystic Neoplasms of the Pancreas.

BACKGROUND: Cystic neoplasms of the pancreas are rare tumors that constitute about 10% of all cystic lesions of the pancreas and less than 1% of all pancreatic neoplasms. This study was conducted to investigate the diagnostic accuracy and postoperative results for these rare tumors. METHODS: The authors reviewed retrospectively 7 patients with cystic neoplasms of the pancreas who had been treated surgically in the period from 1986 through 1998 at the Department of Surgery, Paik Hospital, Seoul. RESULTS: There were 3 serous cystadenomas, 3 mucinous cystadenomas, and 1 papillary and cystic neoplasm. All 7 patients were women. The mean age of the patients was 36.7 years, 45 years for the serous tumors and 37 for the mucinous tumors. Two patient had diabetes mellitus. Major symptoms were abdominal pain (71%) and an abdominal mass (29%). One patient with a mucinous cystadenoma presented with bleeding from gastric varix which developed as a consequence of a splenic vein obstruction by the tumor. The positive rate for CEA was 33.3%, and that for CA 19-9 was 50.0%. No patient showed an abnormally elevated serum amylase level. Computed tomography was helpful in differentiating cystic tumors from pseudocysts or adenocarcinomas of the pancreas. The sites of the lesions were the tail in 3 cases, the body in 2cases, and the head in 2 cases. A distal pancreatectomy was performed in 5 cases, a pylorus-preserving pancreatoduodenectomy in 1 case, and enucleation of the cyst in 1 case. All the patients were alive at 3 to 145 months postoperatively and showed no evidence of disease. CONCLUSION: We conclude that the prognosis for resected cystic neoplasms of the pancreae is good. The correct preoperative differential diagnosis of pancreatic cystic lesions is important for planning the appropriate management, even though the clinical and the radiological features of these lesions are not always reliable.

4.A Case of Solitary Gastric Neurofibroma Presenting as Hematemesis and Melena.

Gastric neurofibroma is rare and its exact incidence is unknown. We experienced a case of solitary gastric neurofibroma. A 49-year-old woman was admitted because of hematemesis and melena. Neither cafe-au-lait spots in skin nor superficial tumor were found. Gastrofibroscopic examination and UGI series showed a 3 * 4 cm sized luminal protruding mass with adjacent bridging mucosal fold and cental ulceration on the anterior wall of lower body. Mass excision was performed. Grossly, the mass was 4.5 * 3.0 * 2.0 cm sized, well circumscribed submucosal tumor with homogenous cut surface, Microscopically, each of the tumor cells had oval shaped nucleus and spindle shaped cytoplasm. Nuclear atypism and frequent mitosis were not observed. We report a rare case of solitary gastric neurofibroma with review of the literatures.

5.A Case of Congenital Self-Healing Reticulohistocytosis with Skull Involvement.

Congenital self-healing reticulohistiocytosis(CSHRH) consists of multiple disseminated papulonodular skin lesions of varying sizes and regress spontaneously in several weeks to months with little or no scarring, and until recently it has been regarded as a benign cutaneous disease without systemic involvement. However, involvement of organ systems other than the skin has been described occasionally and recurrence of disease at sites distant from the skin has been documented. We present a case of CSHRH in a full termed, normal spontaneous vaginal delivered male neonate. He had reddish colored nodules on the whole body without systemic symptoms. Four weeks later, the skin lesions cleared concurrently with bony involvement of the skull. Because multiple organ systems can be involved and recurrences are possible, long-term follow-up of these patients is indicated.

6.A Case of Syringocystadenoma Papilliferum, Sebaceous Epithelioma, and Basal Cell Carcinoma Arising from Nevus Sebaceus.

Nevus sebaceus is a congenital cutaneous hamartoma which has a potential to develop both benign and malignant neoplasm in late life. The most common benign tumor developed within the nevus is syringocystadenoma papilliferum and the most common malignant tumor basal cell epithelioma. Although two different tumors often develop simultaneously on the lesion, it is rare to see more than two different tumors developing on the lesion of nevus sebaceus. In Korean literatures only three such cases were reported. We report an additional case of nevus sebaceus developing three different tumors, syringocystadenoma papilliferum, sebaceous epithelioma, and basal cell carcinoma, in a 67-year-old female patient.

7.The Effect of Leukemia Inhibitory Factor on Embryos to the Blastocyst Formation.

OBJECTIVE: To determine the effects of leukemia inhibitory factor (LIF) on embryonal development in in vitro culture. METHODS: This is designed in vitro model using eggs from mouse. The eggs from mouse were assigned 29 for control group, 53 for 20 ng/ml of LIF, 88 for 40 ng/ml of LIF, 68 for 80 ng/ml of LIF respectively for in vitro fertilization. And 26 fertilized eggs at 2 cell stage from mouse also were assigned. The mouse embryos of all groups were cultured in medium supplemented with LIF in different concentrations, whereas the eggs in control group was cultured in medium without supplement of LIF. RESULTS: At 72 hours culture of eggs from in vitro fertilization, there was a slight increas in rate of embryonal development to morula in both LIF-20 and LIF-40 as results of 64.15% and 75% respectively, while 42.65% in inferior rate of LIF-80, compare with 51.72% in control group. But the difference between these each groups were not significant in statistically (p< or =0.05). And after 96 hours culture of eggs, the rates blastocyst formation was significantly higher in both LIF-20 and LIF-40 as 56.6% and 63.63% than those in control and LIF-80 as 44.83% and 35.29% respectively. On culturing eggs from in vivo fertilization, the rates of blastocyst formation was significantly not only higher as 85% and 81.81% respectively in medium supplemented with LIF-40 and LIF-80 than 42.3% in LIF-20 but also embryonal cell viability were remakedly improved at 96 hours after culture. CONCLUSION: The LIF in low dose is embryotrophic, but LIF in high dose is embryotoxic on eggs from in vitro fertilization. Whereas on culturing eggs from in vivo fertilization, LIF is more beneficial with dose dependent in high concentration.

8.Primary Myoepithelioma of the Testis: A Case Report.

Myoepitheliomas are well-established to occur in the salivary glands, but they have also been described in the breast, upper aerodigestive tract, skin, and soft tissues. We report here on a unique case of primary myoepithelioma that occurred in the right testis of a 28-year-old man. The tumor was entirely confined to the testis and it was clearly separated from the epididymis. Histopathology revealed mixed architectural patterns in which the reticular areas merged into the chondromyxoid stroma. The tumor cells, which were focally immunoreactive to pancytokeratin and S-100 protein, were round to ovoid and spindly arranged in cords, strands, and fascicles. They showed mild nuclear pleomorphism, sparse mitotic figures and a low Ki-67 proliferative index. There was no ductal differentiation in the tumor. To the best of our knowledge, there has been only one case report of a primary testicular myoepithelioma in the English medical literature.

9.Effect of Anti-inflammatory Mediator on the Proliferation of Human Corneal Keratocyte.

PURPOSE: The purpose of this study was to evaluate the effect of anti-inflammatory mediators like dexamethasone, nordihyroguaratic acid (NDGA), and diclofenac sodium on proliferation of human corneal keratocytes, and to investigate the cellular morphology of keratocyte. METHODS: Human corneal keratocytes were exposed to 0.05, 0.1, 0.3, 0.8, and 1.0 mM concentration of each drug for period of 24, 48, and 72 hours. MTT based colorimetric assay was performed to assess the metabolic activity and inhibition of cellular proliferation. Cellular morphology was evaluated by inverted phase contrast micrograph and electron microscopy. RESULTS: The higher the concentration of inoculated each drugs was, the more the inhibitory effect of human keratocyte proliferation was found (P<0.05). NDGA, over 0.3 mM and diclofenac, more than 0.1 mM had significant more inhibitory effect on keratocyte proliferation compared with dexamethasone within 48 hours of exposure to each drug. With the concentration and exposure time of each drug, human corneal keratocytes were visible more rounded and swollen rather than spindle shape, and detached from the bottom of the dish. The damaged keratocytes had degenerative changes like cellular membrane disruption, microvilli disappearance, enlarged rough surfaced endoplasmic reticulum and mitochondria, vacuole formation and nuclear membrane damage by TEM. CONCLUSIONS: On basis of this study, the anti-inflammatory mediators such as NDGA and diclofenac sodium have less side effects and stronger inhibitory effects of human keratocyte proliferation than dexamethasone.
