2.Alcohol Problems and Related Service Needs in Urban Korean Community.

OBJECTIVES: The purpose of this study is to investigate the characteristics of drinking patterns and the prevalence of hazardous drinking and alcohol use disorder with the assessment of alcohol policy needs in community population of urban city in Korea. METHODS: Data was collected from 1,000 residents in a single urban city in Korea, from June 1 to July 31, 2011 by a questionnaire. As research tools, socio-demographic characteristics composed of variables based on study purpose, alcohol problems and alcohol use disorder state scales composed with Alcohol Use Disorder Identification Test(AUDIT) and Cut down, Annoyed, Guilty, Eye-opener(CAGE), and the assessment of alcohol policy needs in community population questions were used. RESULTS: The lifetime experience of alcohol drinking was 97.4%. For the frequency of drinking, 21.9% of population responded 2 to 4 times per weekly. 24.4% of male had one time or more high risk drinking, defined as having four or more drinks in a sitting, per week. 51.6% experienced the first time drinking of alcohol in adolescence. The lifetime prevalence of hazardous drinking and alcohol use disorder were 14.9% and 3.3%, respectively using AUDIT. Alcohol use disorder were 9.6% with the CAGE scale. Problem drinking were 8.4% in this study. They requested the program such as prevention education(57.2%), construction of liaison system to treatment facilities( 50.1%), alcohol counseling and intervention(41.7%), and rehabilitation program(39.8%). CONCLUSIONS: These results demonstrate that alcohol problem and alcohol use disorder were much prevalent, and these problem started in the earlier age. It is necessary to develop an effective alcohol prevention program or policy with considering the needs in community population.

5.Anterior Cervical Interbody Fusion Using a Plate Cage System in Degenerative Cervical Disease.

OBJECTIVE: The plate cage system is an intradiscal cage combining with an integrated plate. To evaluate its suitability for an clinical application, the authors present a retrospective analysis of outcome assessment of our series. METHODS: From March 1998 to November 2001, fifty three patients with degenerative cervical disease underwent anterior cervical interbody fusion with the PCB(R)(PCB(R) instrument, SCIENT'X, Paris, France). Single level fusion was accomplished in 31 patients, two levels in 19 patients, and three levels in two patients. RESULTS: All patients were improved without intraoperative complications ; excellent in 28(52.9%), good in 15(28.3%), and satisfactory in 10(18.8%). Four cases of screw loosening were identified, however, there was no cage backout, worsening of symptom or reoperation due to screw loosening. The bony fusion was confirmed in all patients by cervical flexion and extension X-ray and computed tomography during follow-up. CONCLUSION: The design of this plate cage system appears to prevent bone-graft recipient site and donor site complications, provides immediate stability, and restores height and lordosis.

6.Carpal Tunnel Syndrome in Chronic Renal Failure Patients Undergoing Continuous Ambulatory Peritoneal Dialysis(CAPD).

BACKGROUND: Carpal tunnel syndrome(CTS) is a long-term complication in dialysis patients which results from compression of the median nerve within the carpal tunnel. It has been reported to occur with increased frequency in the hemodialysis population, but, there are few reports concering long-term complications in CAPD because of the relatively shorter duration of dialysis in most CAPD patients. The aim of this study was to determine the incidence of CTS in patients undergoing CAPD. METHODS: We analyzed 21 chronic renal failure (CRF) patients(males 14, females 7; age range 19-79 yr) undergoing CAPD. The patients were evaluated by questionnare, physical examination, and nerve conduction test(NCT). Age, gender, duration of CRF before CAPD, duration of CAPD, diabetic history were determined. RESULTS: Among the total 21 patients undergoing CAPD, only 5 patients(24%) were CTS, diagnosed by NCT. Two of these patients were symptomatic CTS, three patients were non-symptomtic CTS. So, there were no relationship between the incidence of CTS and clinical symptoms. Among the diabetic dialysis patients, the incidence of CTS was 28.57%. Also there was no increase in the number of diatetic patients with CTS. CONCLUSION: It is concluded that the incidence of CTS in CAPD patients was similar with the previous reported incidence(2-31%) of hemodialysis patients. Since CTS is treatable, annual or even semiannual nerve conduction tests is indicated all CRF patients on chronic dialysis.

7.The new approach to maxillary and mandibular anterior dental arch forms: The prediction to maxillary and mandibular anterior occlusal relationship by computer program.

When we deal with maxillary and mandibular anterior dental arches showing problems in occlusal relation and aesthetics caused by malformations of teeth and congenital missing, et al during the orthodontic treatment, we could not often decide the functional occlusion by only relying on the orthodontic treatment. If orthodontists can predict what kinds of treatments are needed for functional occlusion in maxillary and mandibular anterior dental arches, they can not only effectively treat patients but also facilitate the cooperation with other field during the treatment. Our previous research showed the correlation among intercanine width, segment depth and arch perimeter by using the Korean normal occlusion model. At this time, we produced the computer application program by taking advantage of this correlation. And then, we applied this program to setting up the treatment plans for 2 patients with the damaged maxillary and mandibular dentures. With the help of this program, we could not only easily acquire the information about the change of variables required by treatment plans but also understand the change of the anterior dental occlusion corresponding to the change of each variables such as the intercanine width, segment depth and arch perimeter. Later, if we can have the information about the relationship between the change of the angle of incisors depending on facial types and arch forms and, in addition, can acquire the appropriate intercanine width, we can have the ability to produce the 3 dimensional occlusogram for the anterior dental arch forms.

8.A Study on Clinical and Phonological Characteristics of Patients with Specific Language Impairment.

OBJECTIVE: The purpose of this study is to determine clinical and phonological characteristics of the patients with specific language impairment (SLI). METHOD: Subjects were 24 SLI patients without known mental retardation, cerebral palsy, epilepsy, hearing loss or structural brain lesion and 23 normal children. Developmental history, oropharyngeal abnormality, and brainstem auditory evoked potentials were obtained in SLI patients. Receptive language age and quotient were assessed by Peabody picture vocabulary test. Phonetic characteristics of subjects were analysed using Visi-Pitch(R) and computerized speech laboratory. RESULTS: In perceptual evaluation, 32.3% of SLI patients showed incomplete articulation pattern. The patterns of incomplete articulation were substitution, distortion, nasalization, and addition. The receptive language of SLI patients (0.89 +/- 0.28) was significantly lower than control group (1.16 +/- 0.18). In Visi-Pitch(R) analysis, diadochokinetic rate and maximal phonation time were decreased in SLI patients. The total duration of three syllables in SLI patients were significantly prolonged, especially for bilabial heavily asperated and glottalized consonants and alveolar and velar slightly asperated, heavily asperated, and glottalized consonants. CONCLUSION: The patients with SLI showed characteristic defect in articulation as well as expressive and receptive language delayment. This articulatory defect may be resulted from disturbance of central programming and coordination of articulation. The objective and quantitative analysis of Phonological characteristics using computerized speech system in SLI patients can contribute to diagnose and evaluate the treatment outcome for the patients.

9.The effect of labial inclination on intrusion of the upper and lower incisors by three-dimensional finite element analysis.

This study was designed to investigate the position of anteroposterior center of resistance for genuine intrusion and the mode of change of the minimum distal force for simultanous intrusion and retraction of the upper and lower incisors according to the increase of labial inclination. For this purpose, we used the three-piece intrusion arch appliance and three-dimensional finite element models of upper and lower incisors. 1. Positions of the center of resistance in upper incisors according to the increase of the labial inclination were as follows; 1) In normal inclination situation, the center of resistance was located in 6mm behind the distal surface of the lateral incisor bracket. 2) In 10degrees increase of the labial inclination situation, the center of resistance was located in 9mm behind the distal surface of the lateral incisor bracket. 3) In 20degrees increase of the labial inclination situation, the center of resistance was located in 12m behind the distal surface of the lateral incisor bracket. 4) In 30degrees increase of the labial inclination situation, the center of resistance was located in 16m behind the distal surface of the lateral incisor bracket. 2. Positions of the center of resistance in lower incisors according to the increase of the labial inclination were as follows; 1) In normal inclination situation, the center of resistance was located in 10mm behind the distal surface of the lateral incisor bracket. 2) In 10degrees increase of the labial inclination situation, the center of resistance was located in 13mm behind the distal surface of the lateral incisor bracket. 3) In 20degrees increase of the labial inclination situation, the center of resistance was located in 15m behind the distal surface of the lateral incisor bracket. 4) In 30degrees increase of the labial inclination situation, the center of resistance was located in 18m behind the distal surface of the lateral incisor bracket. 3. The patterns of stress distribution were as follows; 1) There were even compressive stresses in and periodontal ligament when intrusion force was applied through determined center of resistance. 2) There were gradual increase of complexity in compressive stress distribution pattern with increase of the labial inclination when intrusion and retraction force were applied simultaneously.

10.Correlation of Apoptosis with HPV, p53 Expression, and PCNA Expression in Laryngeal Squamous Cell Carcinoma.

BACKGROUND AND OBJECTIVES: Programmed cell death (apoptosis) is distinctive form of cell death manifested by characteristic chromatin condensation and DNA fragmentation, whose function is the deletion of cells in normal development, organogenesis, immune function, and tissue growth, but which can also be induced by pathologic stimuli. The purpose of this study was to analyze the relationship between apoptotic index and HPV, p53 and PCNA expression, and clinicopathological findings in laryngeal squamous cell carcinomas. MATERIALS AND METHODS: Fourty-one cases of laryngeal squamous cell carcinomas were analyzed for the detection of HPV DNA by in situ hybridization, the expression of p53 and PCNA by immunohistochemical technique, and detection of apoptotic bodies by in situ hybridization. RESULTS: HPV DNA was detected in 8 (19.51%), p53 was expressed in 26 (63.41%) out of 41 cases, and the PCNA expression rate was 48.39+/-21.06%. The average apoptotic index was 9.38+/-2.89, there was no relationship between apoptotic index and HPV and p53 expression (p>0.05). The apoptotic indices were 11.26+/-1.86, 8.17+/-2.55, and 4.32+/-2.41 in well differentiatied, moderately differentiatied and poorly differentiatied carcinoma, respectively. The histopathological differences were statistically significant (p<0.05). Also there was an inverse proportion between apoptotic index and PCNA expression. CONCLUSION: The apoptotic index was related to cellular differentiation regardless of HPV and p53 expression. Also these results suggest that the numbers of apoptotic bodies in the tumor tissues can make it possible to presume indirectly the malignant potentiality of the tumor and will help us to understand the biologic behavior of head and neck cancer.
