1.A case of reticulate acropigmentation of Kitamura with hyperpigmented macular lesions on the scrotum.

An 18-year-old male patient presented with brownish, sliglitly atrophic, reticulated macular lesions on the dorsal parts of hands and feet sinc childhood, which were characteristic manifestation of ret.iculate acropigmentation of Kitariura. At about thirteen years old age, he noted multiple deep brownish, freckle-like macules on the scrotum. Histopatholigic findings of a scrotal lesion were compatible with those of Dowling-Degos disease, such as epidermal acanthosis, elorga ion of rete ridges and hyperpigmentation of the prickle and basal cell layer.

2.A Case of Transient Acantholytic Dermatosis Improved with Tetracycline Hydrochloride.

We report a case of transient acantholytic dermatosis which occurred on the neck and clavicular areas of eighteen-year-old female and was improved with systemic administration of tetracycline hydrochloride. She had mild pruritic and crusted erythematous papules. Family history was not contributory. Histopathologic findings were acantholytic dyskeratotic pattern with circumscribed suprabasal clefts, corps ronds, and grains in the epidermis of papular lesion. She obtained incidental improvement of lesions on the neck and clavicles during medication with tetracycline hydrochloride which was prescribed for acne vulgaris on the face.

3.Two Cases of Midline Granuloma of the Face.

We report two cases of midline granuloma of the face which occurred in 60-year-old man and 28-year old man. The former who wa.s suffered from nasal stuffness on the right nasal cavity for 10 months had yellow-brownish colored crusted erythema tous plaque on the right naris. The latter had painful erythematous swelling on the left lower eyelid and ulceration on the hard palate. Both shows polyrnorphous infiltration of atypical mononuclear cells with an admixture of normal inflammatory cells histopathologically.

4.A Case of Staphylococcal Septicemia with Skin Lesions Characteristic of Septic Emboli.

We report a case of staphylococcal septicemia with skin lesions characteristic of septic emboli in a 55-year-old man. Examination of the skin revealed small pustules surrounded by wide ring of erythema on the trunk and extremities. Histopathologic examination of a pustule showed subcorneal pustule and Gram positive septic emboli within blood vessels. There were neutrophilic infiltration and numerous extravasated erythrocytes within dermis. Staphylococcus aureus was isolated on the bacterial culture of blood and small pustules, He was treated with cloxacillin, but died due to respiratory distress syndrome.

5.A Case of Hunter's Syndrome.

We report a case of Hunter's syndrome in an 8-year-old boy, who presented with ivory-white colored papules and ridges on the left chest area, which were regarded as pathognomonic cutaneous markers for Hunter's syndrome. He also showed growth retardation, dear corneas, hepatosplenomegaly and fair intellect. The histopathological findings of papular lesions revealed loosely arranged collagen fibers with massive mutinous material which stained positively with alcian blue at both pH 2.0 and 0.5. On quantitation of glycosaminoglycans by hexuronic add assay in 24-hour urine, excessive excretion of creatinine was noted. To the best of our knowledge, this is the first case in Korea.

6.A Case of Spina Bifida Occulta with Faun - tail Nevus.

This 22-year-old female patient has had a lozenge shaped hair tuft with terminal hair on the lumbosacral area since birth. A limping gait due to discrepancy of the length of the lower extremities has also been noted since 5 year age. Subsequently multiple, recurrent trophic ulceration developed on the left foot because of sensory loss. Simple cervico-lumbar spine x-ray showed spina bifida occulta of L-2 to sacrum. Lumbar myelography revealed widened vertebral canal without other spinal anomaliea.

7.A Web-Based Bulletin Board System for Treatment of Skin Disease.

BACKGROUND: Electronic bulletin board systems have been used for education, information exchange, and medical consultation for a number of years. However, most of these systems have been text-based. The field of dermatology has never had a system that allowed a quick exchange of visual information. OBJECTIVE: Dermatology is a field of medicine which especially requires a visual image for an accurate diagnosis, and subsequent effective treatment. We have developed a web-based bulletin board system which can be used by any clinician with access to the internet to view clinical images of skin diseases with accompanying clinical findings. METHODS: A web-based bulletin board system was developed for the internet. The system is designed to operate independently of client hardware and software using hyper text markup language so participants using different platforms can access and use the site effectively. RESULTS: Any participant can easily upload a diagnostic image to the board along with relevant clinical information. Other participants can make comments regarding the presented case which may prove useful to the presenter in diagnosis and development of an effective treatment modality. CONCLUSION: Our web-based bulletin board system for discussion of skin diseases has the potential to help in the diagnosis and treatment of various skin diseases. Dissemination of information relating to clinical cases and participation of a widespread audience of experienced clinicians will allow better practice of dermatology and aid in further training of inexperienced clinicians.

8.A Case of Lightning Burn with Flame Burn.

Lightning injury is uncommon, as evidenced by the fact that most physicians never see or treat such a victim. To our knowledge, there were no reports of lightning burn with flame burn. We report a case of lightning burn with flame burn. On a rainy day, a 64-year-old woman with an umbrella in her left hand was walking along a road. When lightning struck the tip of her umbrella, her clothes were flamed. She threw herself into the water in a nearby field. The fire was extinguished. Then she lost consciousness, but she regained consciousness immediately. She was transported to our hospital. On arrival at our emergency center, her physical examination was remarkable. The patient had an entrance wound of the left palm and an exit wound of the left foot as well as 25 % total body surface burns, which were present in the flexor surface of the left hand, left arm, left leg and left flank. Histopathology showed epidermal and dermal necrosis. Three months later the burns were healed spontaneously, with some remaining scars.
