2.A Case of Lyme Disease Complicated with Atrioventricular Block in a 13-year-old Boy

Lyme disease is a common vector-borne disease caused by Borrelia burgdorferi. Erythema migrans represents the most common manifestation during the early phase of this disease;however, systemic manifestations involving the nervous system, joints, or heart are known to occur. We report a case of Lyme disease accompanied by cardiac complications in a 13-year-old Korean male adolescent. The patient developed annular erythematous lesions on his lower extremities after a field trip during his visit to Connecticut, USA, for a boarding school camp, and his skin lesions were consistent with erythema migrans. Indirect immunofluorescence assay and Western blot analysis for Lyme immunoglobulin M showed positive results. Electrocardiography revealed a first-degree atrioventricular block, and he was diagnosed with Lyme carditis and received a 4-week course of oral doxycycline. Follow-up electrocardiography performed a week later revealed normal findings, and the patient showed an uneventful recovery. Lyme carditis often presents as an asymptomatic heart block that can progress to an advanced heart block; however, this condition is reversible with appropriate antibiotic treatment.

4.The MRI Findings in the Patients of Diffuse Brain Injury: Review of the Distribution and Clinical Course.

In the last decade Computed Tomography(CT) has played a critical role in the diagnostic evaluation of the patients with focal brain injury. But it is apparent from pathologic studies that CT underestimates the severity of the many forms of cerebral injury such as primary brain stem injury, non-hemorrhagic cortical contusion and diffuse axonal injury(DAI). Magnetic Resonance Imaging(MRI), however, has been shown to be highly sensitive in detecting diffuse brain injury(DBI). Among the consecutive 13 cases of DBI patients in this series for 10 months, twelve patients were verified as MR evidence of injury in prospective studies. The anatomical distribution of the injuries were 11 cases of corpus callosal lesion, 6 cases of lobar white matter lesion, 1 case of primary brain stem lesion. The sensitivities of MR imaging in detecting the primary lesion were 76.9%(10/13) in T1WI and 92.3%(12/13) in T2WI. In DBI, patients with callosal injuries had higher incidence(8/12) than lobar white matter and primary brain stem lesion, the corpus callosal atrophy by midsaggital MR imaging and behavioral seguellae in survivous of severe head injury implicate the corpus callosal injury and degeneration. More accurate detection and delineation of traumatic lesions with MR should permit more accurate prediction of neurologic and cognitive recovery and assist in optimizing form of treatment.

The purpose of this study was to determine the effects of ankle dorsiflexion, neutral, and plantarflexion on lower extremity muscle activity and break point angle (BPA) during Nordic hamstring exercises.
Methods:
Twenty-four members of a college soccer club (age, 21.68±2.39 years; height, 175.63±4.76 cm; weight, 71.88±6.29 kg) were recruited to participate in the experiment, and all subjects were measured three times in triplicate for Nordic hamstrings at three different ankle angles with all subjects in one group. Surface electromyography equipment (miniDTS, Noraxon Inc.) was used to acquire 3 seconds of muscle activity data at the starting point (90°) during exercise, and motion analysis software (Kinovea version 0.9.5, Kinovea) was used to collect kinematic data at the point where knee strike angular velocity exceeded 30°/sec for BPA data acquisition.
Results:
The results of this study, lower extremity muscle activity was not significantly different in the three variants of Nordic hamstring exercises, but BPA was significantly lower in the dorsiflexion position (60.28°±6.35°) compared to the neutral position (65.32°±6.35°) and plantarflexion position (63.82°±7.01°) (p< 0.001).
Conclusion
These results suggest that the dorsi flexion position of the ankle during Nordic hamstring exercises allows the body to maintain the position for a longer period of time against eccentric forces in situations where the body is moving forward. This suggests that the ankle dorsi flexion position can be used as a position for effective Nordic hamstring exercises in amateur soccer players.

10.The Long-term Clinical Outcome after Corneal Collagen Cross-linking in Korean Patients with Progressive Keratoconus.

PURPOSE: To evaluate the long-term clinical effectiveness and safety of corneal collagen cross-linking (CXL) in progressive keratoconus compared with untreated contralateral eyes. METHODS: In this retrospective study, nine eyes of nine patients with progressive keratoconus who received CXL (treatment group) and nine untreated contralateral eyes with keratoconus (control group) were included. All patients were followed for at least 5 years and assessed with best-corrected visual acuity, maximum keratometry, mean keratometry, corneal astigmatism, and corneal thickness. Clinical data were collected preoperatively and at 1, 3, 6, 12, 24, 36, 48, and 60 months, postoperatively. RESULTS: Mean best-corrected visual acuity improved significantly from 0.58 ± 0.37 logarithm of minimum angle of resolution preoperatively to 0.39 ± 0.29 logarithm of minimum angle of resolution at 5 years after corneal CXL (p = 0.012). There was significant flattening of the maximum keratometry and mean keratometry from preoperative values of 63.39 ± 10.89 and 50.87 ± 6.27 diopter (D) to postoperative values of 60.89 ± 11.29 and 49.54 ± 7.23 D, respectively (p = 0.038, 0.021). Corneal astigmatism decreased significantly from 7.20 ± 1.83 D preoperatively to 5.41 ± 1.79 D postoperatively (p = 0.021). The thinnest corneal thickness decreased from 434.00 ± 54.13 to 365.78 ± 71.58 µm during 1 month after treatment, then increased to 402.67 ± 52.55 µm at 5 years, which showed a statistically significant decrease compared to the baseline (p = 0.020). In the untreated contralateral eyes, mean keratometry increased significantly at 2 years compared with the baseline (p = 0.043). CONCLUSIONS: CXL seems to be an effective and safe treatment for halting the progression of keratoconus over a long-term follow-up period of up to 5 years in progressive keratoconus.
