1.Carcinosarcoma in Recurrent Lesion of Serous Cystadenocarcinoma of the Ovary: A case report.

Neoplasms showing both carcinomatous and sarcomatous features are well established, and have been reported in practically every organ where carcinoma can occur. But the difference in terminology and difficulties in histopathologic interpretation have hampered adequate understanding of these neoplasms. We report a case of carcinosarcoma in the recurrent form of serous cystadenocarcinoma of the ovary. The patient was a 64-year-old female who underwent total hysterectomy and bilateral salpingoophorectomy three years ago, due to perforated papillary serous cysta denocarcinoma of the right ovary. Recurrent pelvic masses contained both carcinomatous and sarcomatous components. Morphological transition between carcinomatous and sarcomatous components, and epithelial characteristics in the sarcomatous component by immunohistochemistry were recognized. We postulate the histopathogenetic mechanism based on the phenotypic conversion of carcinoma into sarcoma in the carcinosarcoma.

2.Ovarian Mucinous Cystadenocarcinoma with Mural Nodules of Anaplastic Carcinoma: A case report.

A case of an ovarian mucinous cystadenocarcinoma with mural nodules of anaplastic carcinoma is reported. The patient was a 46-year-old female with a right ovarian cystic mass and underwent a total abdominal hysterectomy with bilateral salpingo-oophorectomy. The patient died of the disease in five months. Macroscopically, the right ovarian cyst wall contained numerous well-demarcated mural nodules. Histologically, the cysts were mucinous cystadenocarcinoma, and the nodules were composed of sarcoma-like spindle and polygonal cells with atypia and numerous mitoses. Spindle cells in the mural nodule showed epithelial characteristics by electron microscopy and immunohistochemistry. This case supports an epithelial origin of the mural nodule of anaplastic carcinoma in ovarian mucinous tumors.

3.A Case of Adenoid Squamous Cell Carcinoma.

We report a case of adenoid squarnous cell carcinoma occurred in a 64-year-old male. The patient had a single, centrally ulcerated nodule with an indurated border on the superior helical rim of the left ear. The histopathologic finding showed a squamous cell carcinoma infiltrating der mis as multiple gland-like structures. The wall of gland-like structure was composed of one to several cell layers and the lumen was filled with dyskeratotic, acantholytic cells.

4.Relationship between HPV Infection and bcl-2 Protein Expression and Apoptosis in Invasive and In Situ Squamous Cell Carcinoma of the Uterine Cervix.

Human papillomavirus (HPV) 16/18 is a causative agent of uterine cervical carcinoma. HPV 16/18 can alter cell cycle regulation through apoptosis. Bcl-2 is an important regulatory gene of apoptosis. A study was done to evaluate the relation between HPV 16/18 and bcl-2 and apoptosis in 21 cases of carcinoma in-situ (CIS), 5 cases of microinvasive carcinoma and 23 cases of invasive squamous cell carcinoma. HPV 16/18 was detected by hybrid capture system (HCS), bcl-2 protein by immunohistochemical method and apoptosis by using the hematoxylin-eosin stained slide. The results were as follows: Expression of the bcl-2 protein was 43% (9/21) in CIS and 26% (6/23) in invasive carcinoma. Expression of the bcl-2 protein was 42% (5/12) in CIS with HPV 16/18 infection, 44% in CIS without HPV 16/18 infection, 20% (2/10) in invasive carcinoma with HPV 16/18 infection and 31% (4/13) in invasive carcinoma without HPV 16/18 infection. Mean apoptotic index (mAI) was 3.36 in CIS, 5.23 in microinvasive and 6.25 in invasive carcinoma. mAI was 3.66 in CIS with HPV 16/18 infection, 2.86 in CIS without HPV 16/18 infection, 6.18 in invasive carcinoma with HPV 16/18 infection and 6.30 in invasive carcinoma without HPV 16/18 infection. Based on these results, we conclude that there are no correlation between HPV infection and bcl-2, and between HPV infection and apoptosis in invasive and in situ carcinoma of the uterine cervix, and apoptosis is increased according to tumor progression.

6.THE EFFECT OF APPLICATION OF n-PTFE ON MANDIBULAR BONE DEFECTS OF RABBITS.

We observed the effect of application of n-PTFE on mandibular bone defects in the rabbit. GTR is based on the hypothesis that during wound healing, non-osteogenic cells are mechanically blocked from entering a bone defect. This allows the slower-migrating mesenchymal cells from the surrounding bone and marrow, having osteogenic potential, to move the defect site. Bilateral through & through defects, 8mmx4mm in size, involving the inferior border of mandible, were created in the mandible angle of 15 rats. The experimental side was applied with high-density n-PTFE membrane, with the opposite side serving as a control. In the first week of the early experimental period, the bone defects of experimental groups were filled with highly vascularized fibrous connective tissue showing prominent osteoblastic activity with osteoid formation, whereas the defects of control were replaced by dense fibro-muscular tissue without osteoblastlf activity. After 3 weeks, the experimental group revealed well formed bone trabeculae and fibro-vascular marrow within surrounding membrane. With time the amount of new bone was decreased with increase of hematopoietic marrow, and the cortical plate composed of dense mature bone was more thicker. In the whole experimental periods, the inflammation and foreign body reaction were not found around the inserted n-PTFE membrane. After removal of membrane there was marked remodeling of newly formed bone by active osteoclasts in periosteal soft tissue, which decreased the bone volume apparently. These findings suggest that n-PTFE has biocompatibility, flexibility, and rigidity and offer rapid bone regeneration within limited area by the membrane. On the other hand, the early removal of membrane caused reduction of bone volume by the osteoclastic resorption. Thus, the time of removal may be more important factor for maintenance of regenerated bone volume.

7.Associations between Self-Efficacy, Social Capital and Self-Rated Health Status in Healthy Individuals.

BACKGROUND: Discrepancies exist in the existing researches regarding the association between social capital and self-efficacy, and most of these researches have been done in developed countries. The aims of this study were to assess the independent association between individual social capital and self-efficacy and to assess the associations between individual social capital, self-efficacy and self-rated health. METHODS: Data were obtained from the Gyeongnam health survey (6,500 persons) conducted in 2008. A self-administered questionnaire was additionally administered to gather information on demographic variables, health behaviors, chronic diseases, self-efficacy, social capital, and self-rated health status. Total study population was 3,843 as those with chronic diseases were excluded. RESULTS: In males, trust was positively associated with confidence and preference to task difficulty, and participation was negatively associated with confidence and positively associated with regulation and preference to task difficulty. In females, trust was positively associated with confidence and preference to task difficulty and negatively associated with regulation. Participation was positively associated with preference to task difficulty. In males, the odds for self-rated health status was better for persons with confidence (95% confidence interval, CI 1.088-1.705), trust (95% CI 1.172-1.821) and participation (95% CI 1.268-2.117). In females, the odds for self-rated health status was better for those with confidence (95% CI 1.250-1.803), preference of task difficulty (95% CI 1.019-1.683) and participation (95% CI 1.024-1.555). CONCLUSIONS: High social capital measured at an individual level may promote self-efficacy and health status.

8.Multiple Primary Malignant Neoplasm Combined with Musculoskeletal Malignancy

From Jan. of 1990 to Dec. of 1992, we have experienced three cases of multiple primary malignant neoplasms originated from different histogenesis, in the Department of Orthopedic Surgery of Korea Cancer Center Hospital. Liposarcomas, malignant fibrous histiocytoma were combined with renal cell carcinoma, mucinous cystadenocarcinoma of ovary or prostatic cancer. With Moertel's classification, synchronous tumors were found in 2 cases and metachronous in 1 case. Synchronous tumors were 2 liposarcomas-renal cell carcinoma or mucinous cystadenocarcinoma of ovary. Metachronous tumors were malignant fibrous histiocytoma-prostate cancer. We couldn t find any family history among them. The causes of these multiple malignancies were considered as the individual predilrection to tumors.

9.Early Detection of Viability of the Femoral Head by 99mTc-MDP Bone Scan in Femoral Neck Fracture

After femur neck fracture, many techniques have been known for early detection of the viability of the femoral head which is very important to decide method of treatment. At the present time, bone scan with 99mTc-MDP is the best available radioistope for use in scanning, because it is very sensitive, non-invasive and simple, minimal radiation dose, easily reproducible. Bone scanning was carried out in 18 cases of femur neck fractures from July, 1981 to October, 1982 in Kyung Hee University Hospital. The results were as follows: l. In twelve cases in which radiological confirmation of viability of the femoral head was difficult to make, we could confirm the presence or absence of vascularity of femoral head using bone scan. 2. Six cases which were considered to have vascular impairment to the femoral head radiologically, were identified to have vascular impairment to the femoral head by bone scan and these were evidenced by operative findings and by histology of biopsy specimen. 3. In fresh fractures, we identified the vascular impairment by the bone scan minimally 5 days after injury. 4. In old fractures, we could confirm the viability of the femoral heads by bone scan. 5. Bone scan is considered to be the excellent technique for early detection of the vascular impairment to the femoral head after femur neck fractures.
