2.Radiological manifestations of tuberculosis of the spine

The classical Roentgenographic features of tuberculosis of the spine such as narrowing of the intervertebraldisc, collapse of the vertebral bodies, with or without the shadow of a cold abscess, present a specific pictureof the actural pathological process. However, the diagnosis of tuberculosis is not confirmed unless proven bybiopsy, or the finding of the tubercle bacillus. 120 cases of proven spinal tuberculosis, at Kwangju ChristianHospital during the period form Jan 1973 through Aug. 1980, were studied and analysed. The results were asfollows; 1. The ratio of male to female was 1:1.1. The age preference was under 30 years old. 2. The most frequentsite of involvement was the lumbar spine(44.1%) and next the thoracic(39.2%0. 3. The incidence of lytic type was50.8%, mixed type 36.7%, and sclerotic type 12.5%. 4. Associated pulmonary tuberculous lesions were observed in 94patients (78.3%). 5. The central type, with wedging or collapse of the vertebral body, was more common in theyounger age group and the intrevertebral articular type in older patients. 6. The incidences of typical radiologicfindings were : Collapse of vertebral body(90%), cold abscess (78.3%) and narrowing of intervertebral space(70%).7. Associated kyphosis was observed in 37 cases (30.8%). Among these 37 cases the range of angulation was between21 to 40 degrees in 40%.

Dermatomyositis is a severe, idiopathic systemic disorder with predominant involvement of skin and skeletal muscle. The association of adult dermatomyositis and malignancy in particular has generated much attention. The reported prevalence of cancers in this disease varies from 6.7% to 52.2%. We experienced a concurrent case of transverse colon cancer and dermatomyositis on 67 year old female patient and report with review of literatures.

5.Evaluating Scapular Notching after Reverse Total Shoulder Arthroplasty.

BACKGROUND: Scapular notching can happen at diverse location depending on implant design or operative technique, therefore, it is easily misdiagnosed. Thus, this study purposed to suggest a method helpful to assess scapular notching. METHODS: The subjects were 73 cases of reverse shoulder arthroplasty (RSA) for cuff tear arthropathy during the period from May 2009 to April 2014 and followed-up for over a year. There was medialized RSA in 22 cases, bone increased offset RSA (BIO-RSA) in 36 cases, and metal increased offset RSA (metal-RSA) in 15 cases. Scapular notching was not determined by bone defect at the inferior of glenosphere as Sirveaux's classification, but scapular notching at the site where the rotational route of the polyethylene of humeral implant met the scapular neck were examined. The results were compared with conventional method. RESULTS: By conventional method, scapular notching was observed in 10 cases (45.5%) in medialized RSA, 12 cases (33.3%) in BIO-RSA, and none in metal-RSA. By new method, it was observed in 9 cases (40.9%) in medialized RSA, 10 cases (27.8%) in BIO-RSA, and none of metal-RSA. The site of scapular notching was apart from glenoshpere in 18 cases, and at inferior of glenosphere in 1 case. Absorption of bone graft was observed in 4 (11.1%) out of 36 cases of BIO-RSA. CONCLUSIONS: It is hard to distinguish scapular notching from absorption of bone graft in BIO-RSA, and bone absorption at the lateral lower end of glenoid in medialized RSA. Thus, it is considered useful to assess scapular notching at the site where the rotational route of the polyethylene insert meets scapular neck.

6.The Results of the Ultrasonographic Evaluation for the Contralateral Achilles Tendon in Patients with Acute Achilles Tendon Rupture

Purpose:
This study was performed to evaluate changes in Achilles tendon (AT) characteristics of asymptomatic tendons in patients with acute AT ruptures on the contralateral side by using ultrasonography.
Methods:
From January 2016 to December 2018, 31 patients were enrolled. The contralateral asymptomatic ATs were assessed: (1) fluid collection of paratenon, (2) echogenicity, and (3) thickness. The ATs were divided into the distal, middle, and proximal thirds for evaluation and were assessed before the surgery, as well as at 6 weeks, 3 months, 6 months, and 12 months after the surgery. At each time, pain in the tendons was recorded.
Results:
In all cases, it was observed that there was a hypoechoic lesion or fluid collection of the paratenon, which did not show a significant change over time. There was no significant difference in tendon thickness according to the period in the distal and proximal areas, and significant differences were observed only in the middle area (p< 0.05). A new pain around the tendon occurred most often at 6 weeks after surgery (eight cases of 31 cases, 25.8%). Eight cases of pain (47.1%) remained at 12 months after surgery.
Conclusion
In patients with acute AT rupture, ultrasonographic hypoechoic lesions or fluid collections of the paratenon were initially observed on the contralateral tendon, but this did not show significant changes over time. A significant change in tendon thickness was observed in the middle area, but this did not show a close association with pain.
7.A Case of Malignant Lymphoma with Cardiac Involvement at Initial Presentation.

Lymphomatous involvement of the heart, occurring at initial diagnosis and presentation, is extremely rare. We report here a case of 58 year old man who presented with generalized edema, pericardial effusion, and a large right atrial mass detected by transesophageal echocardiography. There is no other evidence of disseminated lymphoma in this patient. Tumor removal and pulmonary embolectomy was done. Pathologically, the mass was malignant lymphoma, diffuse large cell type. Unfortunately, we have no chance to perform the intensive chemotherapy. The patient discharged in moribund state.

8.Effect on Nerve Regeneration after End-to-Side Neurorrhaphy with Epineurium Removal in Rats.

End-to-side neurorrhaphy had been firstly reported at the beginning of this century, but no further clinical reports have been released. In some cases of end-to-end neurorrhaphy, donor nerve incision or even sacrification is inevitably necessary. In doing end-to-side neurorrhaphy, has not been widely accepted because previous results were not satisfactory. The purpose of this study is to evaluate the applicability of end-to-side neurorrhaphy in rats. Twenty female Sprague-Dawley rats, weighing approximately 200-250 g(232.50+/-12.6g) were used. In 10 rats, their right and left peroneal nerves were cut and the distal end of peroneal nerves were end-to-side neurorrhaphied to the tibial nerve. On the right side, the epineurium was left intact and on the left side, it was removed. The proximal end of peroneal nerves were imbedded in the thigh muscle. After 6 months, eletrophysiologic tests, including nerve conduction velocity, the amplitude and the areas of action potential of the tibial cranial muscle were done and then all rats were sacrificed for histologic analysis. The other 10 offspring rats were raised and used for control group. The mean weight of the tibial cranial muscle was 1.74(+/-0.30)g on the right side, 1.54(+/-0.31)g on the left side and 1.97(+/-0.28)g in the control group, which was statistically not significant. The number of muscle fibers in x 100 magnification was 106.10(+/-4.28) on the right side, 109.67(+/-4.69) on the left side and 107.80(+/-4.64) in the control group, which was also not significant. The amplitude and width of muscle action potential were not statistically different among the right side, the left side and in the control group. However, the nerve conduction velocity was bilaterally delayed if compared to control group. In histologic analysis, the presence of epineurium did not impact the results and end-to-side neurorrhaphy were successfully regenerated in all rats. End-to-side neurorrhaphy seem to be a functionally and histologically tolerable alternative when end-to-end neurorrhaphy is not available.

9.1, 000 Cases of Laparoscopic Cholecystectomy by a Single Surgeon.

PURPOSE: A Laparoscopic cholecystectomy has been the treatment of choice for acute and chronic disease of the gallbladder. We have performed about 1, 500 cases of laparoscopic cholecystectomy since 1991. The purpose of this study is to analyse 1, 000 cases of laparoscopic cholecystectomy performed by a single surgeon and observe the effectiveness of this form of treatment of for gallbladder disease. METHODS: From March 1993 to December 2001, 1, 000 patients underwent a laparoscopic cholecystectomy at our department by a single surgeon. They were divided into 2 groups; group 1 being those of the first half of the period and group 2 being those of second half. We reviewed the patient's charts, age, sex, associated medical diseases, final diagnosis, operation time, occurrence of conversion from laparoscopic to open cholecystectomy, surgical complication and hospital stay. RESULTS: The mean age was 51.2+/-2.37 years and the male-to-female ratio was 1: 1.3. There was no difference of age or sex between two groups. Nineteen percent from group 1 and 16.6% from group 2 had a history of previous abdominal surgery. 22.2% from group 1 and 28.6% from group 2 had a associated disease. Chronic cholecystitis was the most common final diagnosis in both groups. But while the incidence of acute cholecystitis of group 1 was 7%, that of group 2 was 20.6%, which significantly increased (P<0.05). The mean operation time was 69.4 and 45.2 minutes, the conversion rate was 3.8% and 1.2% (P<0.05). The surgical complication rate was 4.4% and 2.4%, and the length of hospital stay was 4.7 and 3.9 days in groups 1 and 2, respectively Conclusion: Laparoscopic cholecystectomy is a safe and effective treatment for gallbladder diseases. As surgical experience incerased with a rising number of cases, the conversion rate, complication rate and hospital stay decreased. However, the rate of acute cholecystitis increased. For this result, we think that an excellent skill and significant experience of surgeon is essential for this results.
