1.An Assessment on the Human Body Composition of Females with Magnetic Resonance Image.

We have tried this experiments about measure of Human Body Fat from transverse body scans with Magnetic Resonance Image (0.5, Tesla). Images were created with a spin echo sequence using a repetition time 500msec, echo time 20msec, and 1cm length between 10mm cross sectional slices, and gained through a whole body. In vivo quantification of body fat with MRI was measured by two healthy Females Volunteers, each cut obtained with MRI was analyzed, traced papers on the view finder, and then digitized, at last calculated for the areas of Human Body Fat. The results of this study can be summarized as follows : Through the analyses of the Ratio of Human Body % Fat with MRI and Densitiometry, in Sub.1, Sub.2, MRI is estimated higher than Densitiometry, that is, Keys & Brozek's Method (1960) has the most remarkable score gaps, 6.94% (Sub.1), 6.21%(Sub.2). Chinn & Alleys Method (1960) has showed the score getting closest to MRI, 1.67% (Sub.1), 1.36%(Sub.2). And Siri's (1956) Brozek et al's (1963), which have been used as the most popular methods, make the difference of 4% approximately. As a result of this study, such as preceding studies about it Ratio of Human Body Composition with MRI has considered to be validated and trusted. Therefore, if we estimate for Ratio of Human Body Fat with much more subjects than this experiments we can suggest that the method with MRI is possible to develope low data adaptable in every field.

2.Development of Endocrine Cells of Human Pancreases: A Morphologic and Immunohistochemical Study.

Pancreases from 123 human embryos and fetuses ranging from 3 weeks to 40 weeks of gestation age were studied. Thirty four cases were examined by immunohistochemical preparations to investigate the temporal development of the pancreatic islet particularly with reference of its origin, time of appearance and interrelation of various types of islet cells. Following results were made. 1) The endocrine cells and the acinar cells both originated from the primitive pancreatic duct cells. 2) Alpha, beta and delta cells began to appear almost simultaneously at 10 weeks og gestation. A, B and D cells, in decreasing order of frequency, were all found in the entire areas of the pancreas. 3) Four types of fetal islets i.e., preimtive, "homologous", bipolar and mantle, could be recognized during development. The latter 3 types first appeared simultaneously at 17 weeks of gestation. They continued to increase in sized and number throughout the fetal life and constituted permanent islets. 4) Two types of islet cells could be distinguished with routine H&E staining after the second trimester. 5) The size of fetal pancreatic islets were rapidly increased between early (64 micrometer) and mid-term (113 micrometer) (p<0.05). 6) The primitive islets were composed of 37.3% of alpha cells, 41.3% of beta cells and 13.4% of delta cells. At mid-term the mantle islets were composed of 36.4% of alpha cell, 42.7% of beta cell and 25.6% of delta cell. At term beta cell was higher value (52.6%) than alpha cell (29.3%) and delta cell (17.6%) (p<0.05).

3.Clinicopathological Analysis on the 104 Cases of Malignant Melanoma.

The cliniopathological analysis was done on the 104 cases of malignant melanoma diagnosed at the Seoul National University Hospital (SNUH) from 1984 to 1993. The basic clinical data and the pathological items were based on the New Mexico Melanoma Registry Worksheet. The results were as follows. The male to female ratio was 1 : 0.79. Primary cutaneous melanoma was more common in the male (M : F=1 : 0.56) but primary extracutaneous melanoma with slight female dominancy (M : F=1 : 1.25). The peak age was the 6th decade in both cutaneous and extracutaneous malignant melanoma. In 66% (35 cases) of primary cutaneous malignant melanoma, the primary site was located in the acral area (including cases of acral lentiginous and nodular type), of which 63% (41% of total cutaneous melanoma) was acral lentiginous type. Major components of tumor cells were epithelioid. Clark's level of tumor was III or more at the time of the first visit in the majority of the cases (85%). The incidence rate of extracutaneous melanoma was 34.6% (36 cases) among the primary melanoma, and the eyeball (17.3%) was the most prevalent organ. All these features suggest that the racial difference between the Korean and the Caucasian is evident and also that etiologic role of sun damage is not quite marked in the Korean. We also suggest that an early detection program is very important to cure this malignant tumor.

4.Studies on the bfp Gene, Adherence to HEp-2 Cells and Serotyping of Escherichia coli Isolated from Urine.

Eighty-two strains of Escherichia coli isolated from urine specimens in Pusan University Hospital, were serotyped and analyzed for plasmid DNA profiles, PFGE profiles, MRHA of human blood cells, HEp-2 cell adherence ability and reactivity to bfpA, LT, STh and STp DNA probes. The following results were obtained. Fifty-three of the eighty-two strains belonged to thirteen different 0 serotypes, twenty-nine strains could not be typed with the antisera used. Thirty strains (43.9%) were hemolysin producer. MRHA is present on twenty-nine strains (35.37%) of eighty-two strains. MRHA positive strains carry a plasmid of 60MDa, a putative factor involved in adherence. This plasmid might be specific for MRHA positive strains. MRHA positive strains were observed in serotype 01, 018, 055, 086a, 0119, 0126, and 0142. Twenty-six strains of E. coli showed three patterns of adherence to HEp-2 cells namely, localized, diffuse, and aggregative adhesion. Twenty-two strains hybridized with the bfpA probe, while all eighty-two strains did not hybridize with the probes, LT, STh, STp. The restriction fragment patterns of chromosomal DNA digested with AotI analysed by PFGE of hemolysin-producing E. coli ten strains were compared with eight different types. Three of E. coli serotype 01, 08 and 0126 showed the same chromosomal DNA fragment patterns.

5.Role of Toe - Flexors in Ankle Plantar Flexion during Normal and Rapid Walking - 3 Dimensional Kinetic Study.

It is known that the toe-flexors exert some power generation in ankle plantar flexion. However, there has been no paper published in which the power generation was quantified. The purpose of this paper, therefore, is to quantify the amount of contribution of the toe-flexors to the ankle plantar-flexion in normal and rapid walking using the kinetic data of three dimensional gait analysis system. In order to restrict the action of the toe-flexors, we designed special braces which can be applied to the forefeet of the examinee during walking. We performed the gait analysis in ten normal adult volunteers with and without braces, and evaluated the moment and power of toe-flexors during terminal stance and pre-swing phase of gait cycle. Gait analysis was done with the VICON 3-dimensional motion analysis system (VICON, Oxford Metrics, Oxford, England) and 2 force plates (AMTI, Advanced Mechanical Technoiogy, Newton, MA, U.S.A,). The kinetic results are as follows: l. Average speeds of normal and rapid walking were 1.12m/sec and 1.41m/sec respectiveIy. 2. In normal walking, peak ankle plantar-tlexion moment decreased 5.5% with braces, and sum of ankle plantar-flexion moment decreased 12.3% with braces. Both of the results were not significant statistically (p>0.05). Peak ankle power generation decreased 11.0% with braces, and sum of ankle power generation decreased 10.4% with braces. These decreases were also insignificant statistically (p>0.05). 3. In rapid walking, peak ankle plantarflexion moment decreased 26.7% with braces. The decrease was horderline significant statistically (p=0.062). The sum of ankle plantar-tlexion moment decreased 26.6% with braces, but the decrease was not significant statistically (p>0.05). Peak ankle power generation decreased 40.2% with braces, and sum of ankle power generation decreased 37.9% with braces. These decreases showed borderline significance statistically (p=0.062). In conclusion, toe-flexors may contribute about 10% of the total ankle plantar-flexion power generation, and the contribution will be increased with increase of walking velocity. We must be very careful to sacrifice the toe-flexors in cases with weak triceps power.

6.Improvement of Cosmesis in the Surgical Treatment of Cleft Hand

Being a rare disease entity, there have been few references about the cleft hand in Korea. We evaluated the postoperative cosmetic results of 17 cleft hands in 14 patients, who were treated surgically at the Department of Orthopedic Surgery of Seoul National University Hospital, from 1982 to 1993. Among 14 patients, 8 were males. Bilateral cleft hands were in five patients and unilateral in 9. Three of five patients who had bilateral cleft hands, were treated surgically on both hands. Remaining two patients had an operation on one hand which affected more severely. Average age at the time of operation was 2 years and 5 months(5 months-10years). Average duration of follow-up was 34 months(1 year 1 month-7years 5 months). By Lange's classification, typical patterns were 16 hands and atypical pattern one. By Flatt's classification, there were 8 hands of I b, 7 I c, 1 I a, 1 II. One patient had family history whose father had both cleft hands and feet. Congenital anomalies other than cleft hand were associated in 10 of 14 patients. Two patients had 3 associated anomalies, 4 patients 2, and 4 patients one. Syndactyly was the most common associated anomaly of affected cleft hand. Central cleft was closed by Snow-Littler technique in 4 hands(24%), Barsky method in 12(71%) and others in 1(6%). Metacarpal bone was treated by intermetacarpal ligament reconstruction in 1 hand(6%), excision only 7(41%), transfer of 2(rd) metacarpal to 3(rd) metacarpal base after excision 5(29%), and 2(rd) metacarpal corrective osteotomy 3(18%). Complications were in 2 hands; one wound dehiesence of cleft, and one partial necrosis of skin. The cosmetic results of surgical treatment were graded into good, fair or poor by the parents-Good ; when parents were satisfied with the result, Fair; improved cosmesis but not satisfied, and Poor; no improvement after surgery. Good was in 16 hands and fair in one.

7.Neurilemoma of Trunk and Extremities

Neurilemoma is benign never-sheath tumor which has been described as painless mass and is usually discovered incidentally. It is the most common tumor of peripheral nerve origin and may arise in any nerve where Schwann cells occur. There are few domestic reports which were clinically evaluated in tens of cases of neurilemoma so far. We evaluated on the 56 cases of neurilemoma confined to the trunk and extremities, in the aspect of clinical characteristics, radiology, pathology, EMG, surgical treatment and postoperative results, who were surgically treated at the Department of Orthopedic Surgery of Seoul National University from 1984 to 1994. Among 56 patients, male were 29 and female were 27, and there was no sexual difference. The average age at surgical intervention was 46.4 years (16–83yrs) and 46.4% of patients were in the age of fourties and fifties. The anatomical locations of the tumors were as followed: upper extremities in 22(39%) cases, neck and supraclavicular area in 15(27%) cases, sacrum and lower extremities in 14(25%) cases and trunk in 5(9%) cases. Symptoms were palpable mass in 55 cases, local tenderness in 14, radiating pain in 11, pain in 10, paresthesia in 11, motor weakness in 2 cases. The average follow up period was 19.1 months(13–56 month). Median nerve was involved most frequently(14%), then ulnar nerve(11%). There was no cases which occurred in multiplicity or associated with neurofibromatosis. Marginal excision was done in 51 cases(91%), incisional biopsy only in 4 cases(7%), wide excision in 1 case. The size of the tumors in the longest axis was smaller than 2cm in 9 cases, between 2 and 4cm in 31 cases, between 4 and 6cm in 13 cases and more than 6cm in 3 cases. Malignant change or recurrence was not found in all cases.

8.Detection of the c-m c Oncogene Amplification in Ovarian Carcinomas by Differential Polymerase Chain Reaction.

The amplification of c-myc oncogene was evaluated in 42 cases of ovarian carcinomas to correlate with clinical parameters. Using oligonucleotide primers, sequences from the c-myc exon-3 gene and from a control gene, tissue plasminogen activator (tPA), were amplified simultaneously by polymerase chain reaction (PCR). After the products of differential PCR (d-PCR) were electrophoresed, slot blot hybridization was performed, and hybridized with P32 dATP-labeled myc and tPA oligonucleotide probes and then autoradiographed. The signal intensities of the two products were quantitated by densitometry and the ratios of two products (c-myc/tPA) were measured. The ovarian carcinomas showed significantly increased amplification of c-myc oncogene Oligonucleoti compared to normal control group (p<0.05). 15 of 42 cases (35.7%) showed various degrees of the MYC gene amplification up to 27 folds in various histologic types of ovarian carcinomas. No significant differences of the MYC gene amplification according to histologic subtypes, tumor action) grades and clinical stages of ovarian carcinomas were present.

9.Strategic serial extractions and immediate implantation for interdental papilla preservation: A case report.

Maintaining the blood supply of the interdental alveolar bone is crucial for preserving the interdental papilla. Rebuilding the interimplant papilla between adjacent implants is more difficult than rebuilding the interdental papilla between the natural tooth and implant. Therefore, preserving the interimplant tissue is necessary when adjacent implants are closely placed. In this case report, three effective methods for maintaining the surrounding tissue, namely strategic serial extraction, immediate implantation, and provisionalization of adjacent maxillary central incisors, were performed. The marginal gingiva and interimplant papilla were well maintained for 24 months.

10.Comparison of the 20-Gauge Conventional Vitrectomy Technique with the 23-Gauge Releasable Suture Vitrectomy Technique.

PURPOSE: To compare the efficacy of the transconjunctival releasable suture technique for pars plana vitrectomy using 23-gauge (23G) instruments versus the conventional 20-gauge (20G) technique. METHODS: A retrospective and interventional case series was consecutively performed for 199 eyes of the 192 patients that were a part of this study. Clinical data were reviewed retrospectively regarding the operation time, preoperative and postoperative intraocular pressure, visual acuity and astigmatism for 54 consecutive patients who received a 23G releasable suture vitrectomy and for 98 consecutive patients who received a 20G conventional vitrectomy during the period between April 2007 and September 2010. RESULTS: Mean operation time based on the operation record was 88.5 +/- 20.1 minutes in the 23G releasable suture vitrectomy group and 102.1 +/- 23.1 minutes in the 20G conventional vitrectomy group, respectively (p = 0.01). The last best-corrected visual acuity (BCVA) was significantly better than the preoperative BCVA in both patient groups (p = 0.01, p = 0.01). The 23G releasable suture group showed less surgically induced astigmatism than the 20G conventional vitrectomy group. Vitreous bleeding was observed to be in 6 eyes (5.9%) in the 23G group, and in 8 eyes (8.2%) in the 20G group. In addition, ocular hypertension was noted to be in 3 eyes (3.0%) in the 23G group, and 6 eyes (6.1%) in the 20G group. No serious complications such as postoperative hypotony or endophthalmitis were observed in either group. CONCLUSIONS: The 23G releasable suture technique is as effective as the 20G conventional technique and offers several advantages.
