Male Breast cancer is an uncommon disease with an incidence of I per cent of all breast cancers. Male breast cancer usually appears as a small mass with well-defined contour which is eccentrically located in relation to the nipple on mammogram. We report a case of breast cancer in a 51-year-old man with mammographic appearance of large hyperdense mass with nipple inversion and axillary lymphadenopathy, gray-scale sonographic finding of homogeneous solid mass and mu Itiple tumor vessels with in the mass on color Doppler ultrasound.

Lipoma is one of the unusual benign breast neoplasms and usually manifests at fatty breast of women at the age of 40 to 60. We experienced a case of large breast lipoma nearly replacing the whole left breast parenchymal tissue with mammographic finding of well-defined radiolucent mass, sonographic finding of hyperechoic mass with disorganized echopattern and computerized tomographic finding of very low attenuation mass, characteristic to adipose tissue, in a young woman of her dense breast.

5.Mini-open Treatment Using Plate of Clavicle Mid-shaft Fractures.

BACKGROUND: Increased frequency of comminuted clavicle mid-shaft fractures and importance of functional satisfaction through early joint exercise has resulted in higher emphasis on surgical treatments. This study aimed to evaluate the clinical radiological results of treatment of clavicle mid-shaft fractures by open reduction and internal fixation using a plate with a small incision. METHODS: The subjects of this study were 80 clavicle mid-shaft fracture cases treated with internal fixation using a plate from October 2010 to July 2014. Clavicle mid-shaft fractures were internally fixated using anatomical plates or locking compression plates. Achievement of bone union, union period, and clavicle length shortening were evaluated radiologically, and clinical assessment was done by using Constant and University of California at Los Angeles (UCLA) scores. RESULTS: All 80 cases were confirmed to have achieved bone union through radiographs with an average union period of 10.9 weeks (range: 7–18 weeks). The average clavicle length of shortening in the affected side was 1.8 mm (range: 0–17 mm). The average UCLA score and Constant score were 33.6 (range: 25–35) and 92.5 (range: 65–100), respectively. Regarding complications, four cases reported skin irritation by metal plates, and one case reported a screw insertion site fracture due to minor trauma history. CONCLUSIONS: We were able to induce successful bone union and obtain clinically satisfactory results in displaced mid-shaft fractures of the clavicle without major complications such as nonunion through treatment of internal fixation using a plate.

6.Prenatal imaging of thanatophoric dysplasia: a case report.

Thanatophoric dysplasia is the most common lethal congenital chondrodysplasia with characteristic features of narrow thorax, short rib, severe platyspondyly, short bowed limbs and skull deformity, etc. It is not a hereditary disorder and there is usually no family history of dysplasia. We experienced a case of thanatophoric dysplasia at 38 weeks of gestation with antenatal sonographic and abdominal radiographic findings of small thorax, short bowed extremities with surrounding thickened soft tissues and marked platyspondyly. Soon atter delivery, the baby died and post-mortem radiographs showed the characteristic findings of thanatophoric dysplasia.

8.Effect on Nerve Regeneration after End-to-Side Neurorrhaphy with Epineurium Removal in Rats.

End-to-side neurorrhaphy had been firstly reported at the beginning of this century, but no further clinical reports have been released. In some cases of end-to-end neurorrhaphy, donor nerve incision or even sacrification is inevitably necessary. In doing end-to-side neurorrhaphy, has not been widely accepted because previous results were not satisfactory. The purpose of this study is to evaluate the applicability of end-to-side neurorrhaphy in rats. Twenty female Sprague-Dawley rats, weighing approximately 200-250 g(232.50+/-12.6g) were used. In 10 rats, their right and left peroneal nerves were cut and the distal end of peroneal nerves were end-to-side neurorrhaphied to the tibial nerve. On the right side, the epineurium was left intact and on the left side, it was removed. The proximal end of peroneal nerves were imbedded in the thigh muscle. After 6 months, eletrophysiologic tests, including nerve conduction velocity, the amplitude and the areas of action potential of the tibial cranial muscle were done and then all rats were sacrificed for histologic analysis. The other 10 offspring rats were raised and used for control group. The mean weight of the tibial cranial muscle was 1.74(+/-0.30)g on the right side, 1.54(+/-0.31)g on the left side and 1.97(+/-0.28)g in the control group, which was statistically not significant. The number of muscle fibers in x 100 magnification was 106.10(+/-4.28) on the right side, 109.67(+/-4.69) on the left side and 107.80(+/-4.64) in the control group, which was also not significant. The amplitude and width of muscle action potential were not statistically different among the right side, the left side and in the control group. However, the nerve conduction velocity was bilaterally delayed if compared to control group. In histologic analysis, the presence of epineurium did not impact the results and end-to-side neurorrhaphy were successfully regenerated in all rats. End-to-side neurorrhaphy seem to be a functionally and histologically tolerable alternative when end-to-end neurorrhaphy is not available.

9.Neutrophil to Lymphocyte Ratio at Admission: Prognostic Factor in Patients With Acute Ischemic Stroke.

BACKGROUND: Inflammatory mechanisms play an important role in acute brain ischemia, and they contribute to the functional outcome. The neutrophil-to-lymphocyte ratio (NLR) has recently been described as a predictor of clinical outcomes in patients with acute coronary syndrome. This study assessed the clinical significance of NLR as a new predictor of the outcome in patients with acute ischemic stroke. METHODS: The study included 371 patients diagnosed as acute ischemic stroke within 48 hours after the onset. All subjects were divided into three groups according to tertiles of the NLR in the initial blood test. Functional outcomes were divided into two groups at 3 months and 1 year after stroke onset: favorable (mRS 0~2) or unfavorable (mRS 3~6). RESULTS: The proportion of patients with mRS 0~2 (favorable outcome) decreased as the NLR increased from the first tertile to the third tertile at discharge (74%, 62.9 and 49.2, p<0.001) and at 3 months (74%, 66.9 and 42.7, p<0.001) and 1 year (67.5%, 64.5 and 41.1, p<0.001) after stroke. NLR was higher in patients with an unfavorable outcome than in those with a favorable outcome (3.88 vs. 2.27 at 3 months, p<0.01; 3.67 vs. 2.31 at 1 year, p<0.001). Multivariate analysis revealed that the significant predictors of an unfavorable outcome at 3 months were NLR in the highest tertile (OR 2.28, 95% CI 1.13~4.60), age (OR 1.04, 95% CI 1.01~1.07), and NIHSS score (OR 1.27, 95% CI 1.18~1.36) at admission. CONCLUSIONS: NLR at admission can be used as a predictor of functional outcome at 3 months after ischemic stroke.
