5.Clinical Features and Outcomes of Renal Mycetoma Complicated by Candida Sepsis in Newborn Infants.

PURPOSE: The kidney is a common site of end organ involvement in patients with candidemia. This study was undertaken to investigate clinical features and outcomes of renal mycetoma complicated by Candida sepsis in neonates. METHODS: A retrospective analysis of the medical records and radiologic images was conducted in a neonatal intensive care unit over a 9-year period (2002-2010). RESULTS: During the study period, there were 69 episodes of Candida sepsis. C. albicans was the most common pathogen (49.2%), and C. parapsilosis was the second (42%), but emerging. Of the 42 patients who underwent ultrasound examination in early phase of infection, renal mycetoma was observed in 8 cases (19.1%). Renal mycetoma was more frequently developed in patients with C. albicans sepsis than those with non-albicans candidemia, but the result was not significant. The demographic characteristics of the two groups of patients who had Candida sepsis (with renal mycetoma vs. without renal mycetoma) were similar. Candiduria was more common in the mycetoma group (P<0.01). In addition, the duration of antifungal medication was longer in patients with mycetoma (P<0.001). On follow-up ultrasounds, improvement and resolution of renal echogenic lesions took a median time of 2 and 5 months respectively for the mycetoma group. CONCLUSION: Candiduria may be an initial sign of renal mycetoma complicated by Candida sepsis in newborn infants. Echogenic foci on the renal ultrasound are persistent for a considerable period after antifungal therapy, and therefore follow-up evaluations are important.

6.Feasibility of Single-Incision Laparoscopic Appendectomy in Pregnant Women.

OBJECTIVE: Owing to advances in laparoscopic technique and equipment, a single-port laparoscopic surgery has been applied in various fields of surgery. However, the application of a single-port laparoscopic surgery for pregnant women has been rarely reported. The aim of this study was to emphasize the feasibility of single-port laparoscopic appendectomy in pregnant women. METHODS: In seven pregnant women who underwent abdominal ultrasonography and physical examination, surgery was performed under the diagnosis of acute appendicitis. In all the patients, single-port laparoscopic appendectomy was performed in the same way. The laparoscopic surgical devices used during surgery were same as those in conventional laparoscopic appendectomy. RESULTS: Age ranged from 25 to 33 years (mean, 29.7 years), gestational age ranged from 7 weeks 3 days to 17 weeks 6 days (mean gestational age, 14 weeks 3 days). The mean operation time was 19 minutes (range, 16–22 minutes). No surgical related complications and any issues occurred. CONCLUSION: Single-port laparoscopic appendectomy for pregnant women is considered one of the treatment methods.

7.Reproducibility of Gated Myocardial Perfusion SPECT for the Assessment of Myocardial Function : Comparison with Thallium-201 and Technetium-99m-MIBI.

PURPOSE: We compared the reproducibility of 201Tl and 99mTc-sestamibi (MIBI) gated SPECT measurement of myocardial function using the Germano algorithm. MATERIALS AND METHODS: Gated SPECT acquisition was repeated in the same position in 30 patients who received 201Tl and in 26 who received 99mTc-MIBI. The quantification of end-diastolic volume (EDV), end-systolic volume (ESV), and ejection fraction (EF) on 201Tl and 99mTc-MIBI gated SPECT was processed independently using Cedars quantitative gated SPECT software. The reproducibility of the assessment of myocardial function on 201Tl gated SPECT was compared with that of 99mTc-MIBI gated SPECT. RESULTS: Correlation between the two measurements for volumes and EF was excellent by the repeated gated SPECT studies of 201Tl (r=0.928 to 0.986; p<0.05) and 99mTc-MIBI (r=0.979 to 0.997; p<0.05). However, Bland Altman analysis revealed the 95% limits of agreement (2 SD) for volumes and EF were tighter by repeated 99mTc-MIBI gated SPECT (EDV: 14.1 ml, ESV: 9.4 ml and EF: 5.5%) than by repeated 201Tl gated SPECT (EDV: 24.1 ml, ESV: 18.6 ml and EF: 10.3%). The root mean square (RMS) values of the coefficient of variation (CV) for volumes and EFs were smaller by repeated 99mTc-MIBI gated SPECT (EDV: 2.1 ml, ESV: 2.7 ml and EF: 2.3%) than by repeated 201Tl gated SPECT (EDV: 3.2 ml, ESV: 3.5 ml and EF: 5.2%). CONCLUSION: 99mTc-MIBI provides more reproducible volumes and EF than 201Tl on repeated acquisition gated SPECT. 99mTc-MIBI gated SPECT is the preferable method for the clinical monitoring of myocardial function.

8.A Case of Traumatic Cerebral Infarction due to Injury of the Petrosal Internal Carotid Artery Resulting from Head Trauma.

Deteriorated mental status and new neurological deficits after head trauma signify the formation of intracranial hematoma or brain edema. In a case of head trauma accompanied by basal skull fracture, even if without intracranial hematoma formation or edema, we should be alert to the possibility of injury of the petrosal carotid artery which needs prompt diagnostic evaluation and management. We report a case of traumatic cerebral infarction due to injury of the petrosal internal carotid artery and review the literatures.

9.A Case of Neuromyelitis Optica(Devic's Disease).

An 8 year-old boy was presented with typical symptoms of neuromyelitis optica. Hematologic data revealed leucocytosis and elevated erythrocyte sedimentation rate(ESR). CSF analysis showed elevated protein and lymphocyte dominant leucocytosis. Treatment with high dose steroid was administered. He recovered fully within a few weeks after the onset or paraparesis. Neuromyelitis optica is a clinical syndrome. Pathophysiologically, it is a autoimmune process involving the optic nerve and the spinal cord. It is characterized by rapid progressive paraparesis and bilateral visual disturbance. There are no definitive laboratory clues for diagnosis. Treatments including the use of steroid, ACTH, antiviral agent and immunosuppresant, all result in variable effect. Prognosis is very variable ranging from full recovery to death. The most common cause of death is respiratory failure with the cord lesions. Neuromyelitis optica is very rare in Korea. We report the case with review of the literature.

10.Effective treatment of a digital ulcer and pain in a Raynaud's syndrome patient with using a spinal cord stimulator : A case report.

Raynaud's phenomenon associated with connective tissue disease (Raynaud`s syndrome) may be difficult to manage with conservative therapy, and no gold standard therapy currently exists. Raynaud's syndrome can cause digital ulcer and necrosis, digital amputation is inevitable in some patients. Spinal cord stimulation seems to be an effective treatment for ischemic vascular disease, although little is known about the mechanisms of this effect. We present here a case for which cervical spinal cord stimulation was used to treat a digital ulcer and the intractable digital pain in a 60-year-old male patient who had severe Raynaud's syndrome. The patient had Raynaud's phenomenon associated with mixed connective tissue disease for 10 years. Three years before visiting our clinic, he underwent amputation at the second and third distal phalanges of his right hand because of digital necrosis. Two months after spinal cord stimulator implantation, his pain significantly improved from 100/100 mm VAS to 0/100 mm and the function of his hands noticeably improved.
