1.Acute Pancreatitis Induced by Azathioprine and 6-mercaptopurine Proven by Single and Low Dose Challenge Testing in a Child with Crohn Disease.

We report here a case of drug-induced acute pancreatitis proved by elimination and single, low dose challenge test in a child with Crohn disease. A 14-year-old boy with moderate/severe Crohn disease was admitted due to high fever and severe epigastric pain during administration of mesalazine and azathioprine. Blood test and abdominal ultrasonography revealed acute pancreatitis. After discontinuance of the medication and supportive care, the symptoms and laboratory findings improved. A single, low dose challenge test was done to confirm the relationship of the adverse drug reaction and acute pancreatitis, and to discriminate the responsible drug. Azathioprine and 6-mercaptopurine showed positive responses, and mesalazine showed a negative response. We introduce the method of single, low dose challenge test and its interpretation for drug-induced pancreatitis.

2.Gd-DTPA Enhancement of Lumbar Discs on MRI: Frequency and Pattern.

PURPOSE: To assess the frequency and pattern of disc enhancement of lumbar spine on enhanced MR images and to determine whether enhanced MR images are useful for locating internal disc lesions. MATERIALS AND METHODS: MR images of a total of 225 discs from 45 patients were reviewed. Fifteen discs from 15 patients who had previously undergone an operation were excluded and 210 discs were therefore studied. T1-, T2-, and gadolinium-enhanced T1-weighted images were obtained after intravenous injection of Gd-DTPA(0.2 ml/kg). Findings were analyzed retrospectively. RESULTS: Sixty-nine(32.6%) of 210 discs showed enhancement. The pattern of enhancement was intradiscal(linear or nodular) or extradiscal. Of 165 morphologically normal discs, 34(20.6%) discs were enhanced; 32 showed linear and two showed nodular enhancement. All of six ruptured discs were enhanced. Six patients were operated on after MRI study ; one case of intradiscal linear enhancement revealed a bulging disc, three of five patients were confirmed on surgery as having ruptured discs in view of the extradiscal enhancement and the other two patients were confirmed having protruded discs. CONCLUSION: The pattern of disc enhancement is classifiedinto intradiscal and extradiscal. The former is further classified into linear and nodular. Gd-DTPA enhanced MRimages may be helpful in diagnosing ruptured discs and in detecting pathologic conditions in those patients whose discs appear to be morphologically normal before enhancement.

3.A survey of the medical practitioners' offices in Korea 2011.

Because of various current problems and issues which are low health insurance medical fee of National Health Insurance Corporation, extreme competition with very large general hospitals, new supply of more than of 3,000 new doctors, many medical practitioner's offices who are suffering management difficulties are increasing. In order to investigate their current overall management situations, we made this study through statistical extraction procedures with the sample population of the medical practitioners which are registered in Korean Medical Association and subsequently we made the study with 1,031 selected cases. The average number of patients is 53.6 patients per day. 44.4% of the medical practitioners' offices are working at night and legal holidays and 8.5% are working at Sunday. Average working hours is 50.1 hours per week, which greatly exceeds 40 hours per week that is regulated in the Labor Standards Act. According to the management performance analysis of those medical clinics through profit and loss statements, average total annual sales revenue in 2010 fiscal year was 444,167,867 KRW, the expenses were 314,217,081 KRW and the earnings before taxes was 129,940,786 KRW. The average net profit (earnings before taxes) of the director of the medical practitioners' offices was 122,337,868 KRW per year and 10,194,822 KRW per month. According to the study results, we have found that we need to increase doctor's bill for outpatient, and establishment and its improvement of medical service delivery systems and classification standards of medical services for first and succeeding outpatients. Considering overall results of the study, readjustment of outpatients' treatment fees and reestablishment of more efficient medical service delivery systems which require the first medical service is to be provided properly should be realized in order to improve the management performance of the medical practitioners' offices.

4.The Analysis of Methylenetetrahydrofolate Reductase Mutation in Recurrent Spontaneous Abortion.

OBJECTIVE: To analyze the methylenetetrahydrofolate reductase (MTHFR) mutation in patients with recurrent spontaneous abortion. MATERIAL AND METHOD: The blood samples of patients with recurrent spontaneous abortion were tested by PCR-RFLP method. RESULTS: Of 51 cases of study group, 14 (27.5%) were normal, 25 (49.0%) were heterozygosity, and 12 (23.5%) were homozygosity. Of 58 cases of control group, 20 (34.5%) were normal, 30 (51.7%) were heterozygosity, and 8 (13.8%) were homozygosity. But the difference between two groups was not significant (p=0.190). CONCLUSION: Hyperhomocysteinemia due to MTHFR mutation is a cause of recurrent spontaneous abortion. Therefore, the study for MTHFR mutation should be included in the workup of recurrent spontaneous abortion.

5.Financial state of primary care physicians under the Korean insurance system.

Greater than its influence on the medical practitioner's individual ability is the National Health Insurance System's influence on the management of medical practitioners' offices in Korea. However, despite the important effect health insurance exerts on the income of medical clinics, recently, the financial difficulties of medical clinics have often become an issue, and financial difficulty has been aggravated as much as a solution has been sought. The current state of the overall management of medical clinics was investigated to understand the factors influencing the sales and expenses in their management. A questionnaire was completed by 1,009 physicians registered in the Korean Medical Association who were participating in a statistical extraction course. As a result of the study, the factors influencing the total revenue and total expenditures of medical clinics, such as increases in the total number of doctors, increasing numbers of outpatients, the size of the medical office, medical disputes, and clinical specialties (based on the first medical treatment) showed statistical significance. In conclusion, in order to improve medical clinic management, a health insurance medical fee should be more reasonably fixed, a medical transfer system should be reestablished, and a cooperative strategy should be created for medical clinics and general hospitals in order to attract patients. As a result, low cost and highly efficient medical services could be provided and the satisfaction of patients improved.

6.Myasthenia Gravis Aggravated by Steroid-Induced Isolated Mediastinal Tuberculous Lymphadenitis.

BACKGROUND: Myasthenia gravis (MG) is occasionally aggravated by chronic infection, of which there are many kinds. CASE REPORT: We report herein the case of a 56-year-old woman with MG aggravated by the activation of isolated mediastinal tuberculous lymphadenitis (MTL) during corticosteroid administration. The possibility of MTL had been disregarded in the differential diagnosis of aggravation of MG weakness, because MTL without pulmonary manifestations is uncommon even in areas where tuberculosis is endemic. CONCLUSIONS: This case suggests that chronic infections such as tuberculosis should be considered in myasthenic patients with progressive exacerbation if definite evidence for aggravating factors of MG is not obtained.

7.Importance of a Diversity Committee in Advancing the Korean Society of Gastroenterology: A Survey Analysis

BACKGROUND/AIMS: The numbers of women, young doctors, and foreigners in the medical field have increased continuously. On the other hand, the environment for these minority groups has not improved, particularly in Eastern countries. The authors aimed to increase the awareness of the importance of a Diversity Committee in the Korean Society of Gastroenterology (KSG) by an analysis of a survey. METHODS: From January to February in 2019, a survey was conducted on physicians and a few medical students by googling. The questionnaire consisted of the target doctors of the Diversity Committee, purpose, specific activities, and expected effects of Diversity Committee to the KSG. The participants requested to respond with yes/no or a 5-point scale. RESULTS: A total of 202 participants completed the questionnaire, and 93.5% (189/202) were medical specialists. The proportion of males was 61.9% (125/202), and 39.6% (80/202) and 36.1% (73/202) participants were in their 30s and 40s, respectively. A total of 174 participants (86.1%) agreed with the necessity of a Diversity Committee, and 180 participants (89.1%) answered this committee would help advance the KSG with significant differences between males and females (80.8% vs. 94.8%, p=0.006; 84.8% vs. 96.1%, p=0.011). Similarly, there were significant differences in the responses according to sex in most questions. CONCLUSIONS: Most participants of the survey expected a contribution of the Diversity Committee to the advancement of the KSG. On the other hand, in most of the priorities of the target, purpose, specific activities, and expected effects of the Diversity Committee, there was a difference in the perceptions between males and females. Therefore, continuous efforts are needed to reduce the differences within the KSG.

8.Emphysematous Pyelonephritis Caused by Citrobacter freundii in a Patient with Type 2 Diabetes and Neurogenic Bladder.

Emphysematous pyelonephritis (EPN) is a rare, life-threatening complication of upper urinary tract infections that is characterized by the presence of gas in the renal parenchyma and perirenal space. It commonly occurs in diabetic patients. Escherichia coli are the most common causative organisms, with few reports implicating Citrobacter freundii as the etiologic agent in EPN. A 57-year-old woman with diabetes and neurogenic bladder visited at our department with confused mentality, myalgia, and general weakness. Further investigation revealed that the patient suffered from unilateral EPN with sepsis caused by C. freundii. The patient's condition was improved considerably with percutaneous drainage and use of intravenous antibiotics for several weeks. However, renal function eventually deteriorated to permanent renal failure, which required hemodialysis. In conclusion, C. freundii may be the causative pathogen of EPN in a patient with type 2 diabetes and neurogenic bladder.

10.Comparison of Serum Interleukin-8 and Regulated on Activation in Normal T-Cell Expressed and Secreted Levels in Atopic and Non-Atopic Children with Lower Respiratory Tract Infections.

PURPOSE: Several studies have shown that viral respiratory infections induce more severe respiratory symptoms in atopic patients than in normal subjects. We attempted to investigate if there is any difference in the viral etiology, clinical manifestations, production of interleukin (IL)-8, and regulated on activation in normal T-cell expressed and secreted (RANTES) between atopic and non-atopic subjects with lower respiratory infections. METHODS: Sera and nasopharyngeal aspirates (NPA) were collected from 97 children with lower respiratory infections who were admitted to the pediatric ward. Seventy-one children were classified as atopic subjects. We detected respiratory viruses with multiplex reverse transcriptase-polymerase chain reaction in NPA and measured total immunoglobulin E (IgE) and specific IgE in sera. IL-8 and RANTES levels measured in sera by enzyme-linked immunosorbent assay, etiology, and clinical manifestations were compared between atopic and non-atopic subjects. Atopic patients were defined as having elevated specific IgE to more than one allergen or age-matched, high serum total IgE levels. RESULTS: Both serum IL-8 and RANTES levels were significantly higher in atopic than in non-atopic patients. There was no significant difference in viral etiology and clinical diagnosis between the two groups. The frequency of wheezing was higher in atopic than in non-atopic patients. CONCLUSION: Our study showed that both serum IL-8 and RANTES levels and the frequency of wheezing were significantly higher in atopic than in non-atopic patients. That suggests that chemokine responses to viral respiratory infection may be different between atopic and non-atopic patients and may be associated with a difference in clinical manifestation, such as wheezing, between the two groups. However, further prospective large-scaled studies are required to clarify our conclusion.
