3.Total Elbow Arthroplasty for the Ankylotic or Painful Elbow

We evaluated the results of twelve total elbow arthroplasties that had been performed from Feb. 1986 to Mar. 1993 in ten patients. The duration of follow-up averaged three years two months(range, one year to seven years one month). There were six females and four males. Two females had bilateral procedures. The average age at the time of the operation was 35.3 years(range, 22 to 53). Five patients had severe rheumatoid arthritis, four patients had fracture sequelae around elbows and one patient had sequele of tuberculous arthritis. Total elbow arthroplasty had been performed for improving the range of motion and relief of pain. Total elbow arthroplasties had been performed in six cases of total ankylotic elbows and in six cases who complained severe painful limitation of motion of the elbow joints. Preoperatively, ankylotic group were fixed by 30 degrees of flexion on an average. In painful elbow group, the average total range of motion was 68 degrees. The prosthesis which were used in total elbow arthroplasties were semiconstrained type in nine cases and noncostrained in three cases. Most of the patients had relief of pain. Postoperative complications were permanent ulnar nerve palsy in one case, superficial infection in one case and dislocation in one case. Total gain of range of motion(ROM) was 17 degrees in painful elbow group and 102 degrees in ankylotic elbows. Total elbow arthroplasty was effective method for improvement of range of motion in ankylotic elbows and relief of pain in elbows which had painful limitation of motion.

6.Solitary Senescent Osteochondroma of the Sacrum Producing Sciatica: A Case Report.

Benign solitary osteochondroma is uncommon in the vertebra (2%). Vertebral ostoechondroma arises predominantly in the lumbar and cervical regions, and rarely in the sacrum. We describe a case of a sacral solitary senescent osteochondroma compressing the sciatic nerve, producing sciatica. The tumor was removed by posterior paramedian incision. The excised mass was cylindrical, measuring 3.5x1x1 cm in size and consisting of lamellar bone with Haversian system similar to the architecture of normal cortical bone and trabecular bone.

7.Histopathologic Correlation of Magnetic Resonance Image Findings in Ischemic Necrosis of the Femoral Head (INFH)

Magnetic resonance image of INFH were correlated with histologic sections. Seventeen patients withe eighteen hips were included in this study. reoperative radiographs and MRI were taken for the patients. Three hips were in stage II, nine hips were in stage III, and remaining six were in stage IV respectively(Ficat and Alert). These hips were replaced with artificial joint and resected heads were examined. The specimens were bisected along the imaging plane, and studied histologically and matched with respective MR images of T1 and T2. Specimen MRI was performed on three femoral head immediately after femoral head removal. Necrotic portion of the femoral head in earlier stage showed higher signal intensity in T1-weighted image. Subchondral void, necrotic bone and saponified fat were responsible for low signal intensity in necrotic portion. Low signal band adjacent to the necrotic foci represented inner fibrous tissue and outer reactive sclerotic bone. Outside the fibrous band, the signal intensity diminished compared with normal fatty marrow. these findings were attributed by cellular infiltration and trabecular bony proliferation. MRI patterns were variable in various stages, but corresponded well with histologic findings.

8.Successful Intraoperative Management to Minimize Blood Loss Using the Combination of Tranexamic Acid and Hemocoagulase for Jehovah’s Witnesses Patient with Liver Cirrhosis

A 78-year-old Jehovah’s Witnesses women with hepatitis B viral liver cirrhosis scored as Child-Pugh A was scheduled for debulking operation of peritoneal carcinomatosis due to right ovarian cancer. Despite the need for preoperative strategies for correction of anemia and coagulopathy due to the risk of significant intraoperative hemorrhage, preoperative management was missing. In addition, she exhibited preoperative hemoglobin, hematocrit levels, and platelet count of 10.5 g/dL, 33.6%, and 85,000/μL, respectively. To minimize intraoperative blood loss, we used the combination of tranexamic acid and hemocoagulase, intraoperative blood loss counted about 700 mL, and the immediate postoperative hemoglobin was 9.0 g/dL. She recovered well without any anemic complications or thromboembolic events.

10.An Open-Label Study of the Improvements in Clinical Symptoms and Neurocognitive Functions in Korean Children and Adolescents with Attention-Deficit Hyperactivity Disorder after Treatment with Metadate CD.

OBJECTIVES: This study aimed to investigate the efficacy and safety of Metadate CD (MCD) when given to Korean children and adolescents with attention-deficit hyperactivity disorder (ADHD). We also explored the effects of the drug on diverse neurocognitive functions. METHODS: Ninety-one subjects with ADHD (mean age 8.6+/-2.2 years) were recruited at 6 outpatient clinics in Seoul, Korea. We used the ADHD Rating Scale (ARS), Clinical Global Impression (CGI), and comprehensive attention test (CAT) to measure the drug's effects. RESULTS: After 0.92+/-0.32mg/kg/day of MCD were administered for 57.4+/-7.6 days, there was a 48.5% reduction in the mean total ARS scores (p<.001). Fifty-seven subjects (64.8%) showed either much improved or very much improved outcomes on the CGI-Improvement scale. The CGI-Severity scale also decreased from an average of 4.7 to an average of 2.9 (p<.001). Errors and response time standard deviations of the CAT, sustained attention test-to-response tasks, the flanker test, and divided attention test scores decreased after treatment (p<.05). The forward memory span of the spatial working memory test scores increased (p<.05). Thirty-five patients (39.8%) experienced side effects, of which the most common were headache (14.8%), nausea (12.5%), and anorexia (9.1%). CONCLUSION: This open-label study suggests that MCD is effective and safe in improving the symptoms and neurocognitive functions of Korean children and adolescents with ADHD.
