1.Association between depression and clinical outcomes in elderly patients undergoing percutaneous coronary intervention

Objective To investigate the association between depression and clinical outcomes in elderly patients undergoing percutaneous coronary intervention (PCI).Methods Four hundred and fifty-one patients aged ≥ 60 years undergoing PCI in cardiovascular department of Anyang People's Hospital from January 2012 to June 2014 were prospectively analyzed in this study.Patients were divided into PCI without (SDS<50) and with (SDS≥50) depressiongroup.They were tested by the self-rating depression scale (SDS) at discharge,and followed up for 1 year.And the 1-year mortality was compared between two groups.Multivariable Logistic regression analysis was used to confirm the independent risk factors for the 1-year mortality.Results The 1-year mortality was higher in PCI with depression group than in PCI without depression group (8.9oo vs.3.9%,P=0.035).As compared with PCI patients without depression,PCI patients with depression showed the increased odds ratio (2.55,95% CI:1.05 ~ 6.17,P=0.039) for 1-year mortality by multivariable Logistic regression analysis.Conclusions Depression is an independent predictor for mortality in elderly patients undergoing PCI.
2.hUC-MSCs promote proliferation and migration of osteosarcoma cells by secreting IL-6

AIM: To investigate the effects of human umbilical cord-derived mesenchymal stem cells ( hUC-MSCs) on the proliferation and migration of osteosarcoma cells ( Saos-2 ) and the underlying molecular mechanism. METHODS:hUC-MSCs were isolated and cultured by tissue explants adherent method.The cell surface markers on hUC-MSCs were identified by flow cytometry.The effects of conditioned medium ( CM) from hUC-MSCs ( hUC-MSCs-CM) , re-combinant human interleukin-6 (rhIL-6) and IL-6 neutralizing antibody on the proliferation of Saos-2 cells were detected by CCK-8 assay and cell counting.IL-6 secretion of hUC-MSCs was assayed by ELISA.RT-PCR was used to assess the tran-scription level of proliferation-related genes proliferating cell nuclear antigen ( PCNA) , cyclin D1 and survivin.The migra-tion potential of hUC-MSCs and Saos-2 cells was measured by Transwell assay.RESULTS:hUC-MSCs migrated to Saos-2 cells.hUC-MSCs-CM contained a high concentration of IL-6, up to (1 835.5 ±134.1) ng/L.hUC-MSCs-CM and rhIL-6 promoted the proliferation and migration of Saos-2 cells.Addition of neutralizing antibody against IL-6 in the hUC-MSCs-CM impaired this proliferation and migration of Saos-2 cells.The mRNA expression of PCNA, cyclin D1 and survivin was up-regulated by hUC-MSCs-CM and rhIL-6, while this effect was dramatically attenuated by treatment with IL-6 neutralizing antibody.CONCLUSION:hUC-MSCs migrate to osteosarcoma cells and promote the proliferation and migration of osteo-sarcoma cells through secreting IL-6 in vitro.
3.Anti-tumor effects of chemotherapeutic drugs on human gastric cancer cells

Objective To evaluate the anti-tumor effects of chemotherapeutic drugs on human gastric cancer cells. Methods From April 2006 to February 2007, 84 patients with gastric cancer underwent surgical resection at General Hospital of Jinan Military Command. The single-cell suspension of these gastric tumors was prepared. The gastric cancer cells were cultured with hydroxycamptothecin, cisplatin, adriamycin, 5-fluorouracil and mitomycin for 48 hours, and changes in activity of the gastric cancer cells were studied via MTT assay. The expression of survivin and PTEN was detected by immunohistochemistry. Data were analyzed by chi-square test, rank sum test and Fisher exact test. Results The anti-tumor effects of different chemotherapeutic drugs were different, and the poorly-differentiated gastric cancer cells were more sensitive to the cytotoxic effects of chemotherapeutic drugs than the well-differentiated gastric cancer cells. The expression levels of survivin in the signet ring cell carcinoma, mucinous adenocarcinoma and other poorly-differentiated adenocarcinoma were significantly higher than those in papillary carcinoma and tubular carcinoma (χ~2 = 10.625, P <0.05), while the expression of PTEN was inverse to that of survivin (χ~2 = 6.060, P < 0.05). The expression of survivin was related to the resistance of the gastric cancer cells to 5-fluorouracil and adriamycin (χ~2 = 6.609, 6.350, P < 0.05). Conclusions In vitro chemosensitivity assay is helpful in selecting the chemotherapeutic regimen for specific types of gastric cancer. Survivin may contribute to the chemotherapy-resistance of certain types of gastric cancer cells, and its expression is related to that of PTEN.
4.A comparative study for diagnostic value of normal fetal cardiac by transvaginal ultrasonography as compared with transabdominal approach

Objective To assess the value of ultrasound examination in transvaginal diagnosis of normal fetal heart with 11-14 weeks of gestation.Methods Totally 158 cases of normal fetal heart with high risk pregnancy and nuchal translucency thickness were examined by two ultrasoud approaches:transvaginal(TVS) and tranabdomina(TAS) in 11-14 weeks of gestation.Results The group of TVS was obviously clearer than TAS by displaying the normal fetal cardiac structural in 12 week of gestation and 13-14 weeks of gestation with significant difference between the two groups (P<0.05),respectively.No significant difference between the two groups in 11 week of gestation was observed.Conclusion The transvaginal echocardiogram is of clinical value in the high risk gravida during the late first and the early second trimester.
5.A large sample and multi-center survey of the depression and anxiety status of patients with refractory irritable bowel syndrome in Guangdong province

Objective To explore the depression and anxiety status of patients with refractory irritable bowel syndrome (IBS)and its subtypes.Methods In this large sample,multi-center,prospective survey(Registration number in Chinese Clinical Trial Registry:ChiCTR-TRC-12001969),the patients who conformed to the Rome Ⅲ criteria for IBS were recruited from the department of gastroenterology of three hospitals in Guangdong province from June 2012 to September 2012.And 100 healthy subjects from the examination center of hospitals were recruited at the same time.The 17-item Hamilton Depression Rating Scale(HAMD) and the 14-item Hamilton Anxiety Rating Scale(HAMA) were used to analyze the depression and anxiety,respectively.Results 9802 patients were collected from department of gastroenterology,and a total of 1128 (11.5 %) patients were diagnosed as IBS.601 of questionnaires were collected,including 142 (23.6%) of refractory IBS and 459 (76.4%) of non-refractory IBS.The incidences of depression,anxiety,depression and anxiety and the HAMD,HAMA scores were higher in refractory IBS group than those in the non-refractory IBS and control group,((66.2% vs 23.3% vs 9.0%),P<0.05 ;(65.5% vs 28.5% vs 10.0%),P<0.05;(58.5% vs 17.9% vs 5.0%),P<0.05;(9.77 ±5.16)vs(5.48 ±4.03)vs(1.66±2.50),P<0.05;(9.75 ±5.08)vs(5.91 ±3.80)vs(2.26 ±2.68),P<0.05).And the incidences of moderate-severe depression and moderate-severe anxiety were higher in refractory IBS group than those in the non-refractory IBS and control group ((8.5% vs 1.3% vs 0),P<0.05; (15.5% vs 3.7% vs 0),P<0.05).The incidences of depression,anxiety,depression and anxiety and the HAMD scores were not obviously different among IBS-D,IBS-C,IBS-M and IBS-U subtypes of refractory IBS (P > 0.05).However,the HAMA scores of IBS-D and IBS-M were higher than that of IBS-U((10.10 ± 5.03) vs(7.55 ± 3.22),P < 0.05 ; (12.08 ±6.89) vs (7.55 ± 3.22),P < 0.05).Conclusion The high incidences of depression,anxiety and depression and anxiety,and the serious degrees of depression and anxiety existed in the patients with refractory IBS.And these are the important causes for refractoriness in these patients.
6.Analysis of the eating behaviors associated with refractory functional dyspepsia

Objective To explore the eating behaviors associated with refractory functional dyspepsia (RFD).Methods In this multicenter,prospective trial,1341 new outpatients with functional dyspepsia (FD) from three Guangdong hospitals who had been diagnosed according to the Rome Ⅲ criteria were enrolled from May to September in 2012.One hundred healthy volunteers were also enrolled as controls.A questionnaire was used to obtain data,and logistic regression analysis was used for analysis.Results ①RFD was diagnosed in 24.4% of the FD patients.②Unhealthy eating behaviors were significantly greater in patients with RFD and non-RFD than in the normal controls.Patients with RFD skipped meals more often,ate extra meals,and preferred sweets and gasproducing foods (P < 0.05).③A comparison among the non-RFD subtypes,showed that those with epigastric pain syndrome had a greater preference for spicy foods (47.5 % vs 35.7 %,P < 0.05),and those with postprandial distress syndrome had a greater preference for sweets(50.0% vs 36.4%,P < 0.05) and gas-producing foods (14.9% vs 7.1%,P<0.05).Those with both subtypes skipped more meals (30.1% vs 17.0%,P < 0.05),and ate extra meals (15.0% vs 6.5%,P<0.05).④Logistic regression analysis showed that meal skipping(95% CI,1.177 ~2.272; P=0.003),eating extra meals (95% CI,1.015 ~2.604; P =0.043),and a preference for sweets (95 % CI,1.040 ~ 1.757 ; P =0.024) and gas-producing foods (95 % CI,1.022 ~ 2.306 ; P =0.039) were risk factors for RFD.Conclusion Unhealthy eating behaviors,especially,meal skipping,eating extra meals,preferring sweets and gas-producing foods correlate with RFD and these behaviors may be the key reasons for the refractory characteristic of RFD.
7.Single utility port or single port complete video-assisted thoracoscopic surgery for pulmonary lobectomy

Objective To explore clinical application of single utility port or single port video-assisted thoracoscopic lobectomy.Methods Conducting a prospective study to 191 patients with lung disease who underwent complete video-assisted thoracoscopic surgery (VATS) lobectomy in Department of Thoracic Surgery of The People's Hospital of Wuhan University from June 2013 to Dec 2014.Results Of the 191 patients, 35 underwent left upper lobectomy, 42 underwent left lower lobectomy, 43 underwent right upper lobectomy, 11 underwent right middle lobectomy, 47 underwent right lower lobectomy, and 13 underwent bilobectomy.Operations were successful in all patients with 3 patients transferred for open thoracotomy for severe adhesion or bleeding.A total of 7 of 78 upper lobectomy and 8 of 113 middle or lower lobectomy was done with adding another incision for severe adhesion.The mean operative time was 50 ~ 190 (80.3 ±43.2) min.The mean blood losing was 90 ~ 350 (145.4 ± 56.2) ml.Thirty nine patients underwent upper lobectomy, who were placed two chest tubes, respectively.The upper chest drainage duration was (1.5 ±0.8) d, and the lower chest drainage duration was (4.2 ± 1.3) d.Forty eight patients underwent lower lobectomy, middle lobectomy, or bilobectomy, who were placed one chest tube, respectively.The chest drainage duration was(4.0 ± 1.7)d.The mean recovery time after operation was (6.1 ± 2.5) d.Seventy six patients were diagnosed with lung cancer, and the average number of dissected lymph nodes from each patient was (14.7 ±6.9).The lung cancer was classified as tumor node metastasis (TNM) stage Ⅰ A, Ⅰ B,ⅡA, ⅡB and ⅢA in 67, 61, 34, 22 and 7 cases, respectively.No serious complications, such as bronchopleural fistula or death, occurred in perioperation.Conclusions In consideration of placing two chest tubes after upper lobectomy and placing one chest tube after lower lobectomy, middle lobectomy, or middle and lower lobectomy, we think single utility port-VATS (2-port) for upper lobectomy and single port-VATS lobectomy for lower lobectomy, middle lobectomy, or middle and lower lobectomy are technically safe and have the advantages of drainage, lessening pain, rapid postoperative recovery, and have no significant difference in operation time, the incidence of complications and the number of removed lymph nodes, compared to traditional 3-port-VATS.
9.Development of a visualization system for minimally invasive surgical abortion.

This article introduces the principle, structure and components of a visualization system for carrying out minimally invasive surgical abortion. Without altering the current surgical approach or increasing the surgical difficulty, the surgical system integrated a mini-CMOS image sensor and LED light and a visual device to allow fixed-point removal of the fetus or embryo in the minimally invasive surgery.

10.Application of endovascular stent graft in treatment of traumatic carotid cavernous fistula

Objective To evaluate the effect of Jostent coronary stent-graft in endovascular treatment of traumatic carotid cavernous fistula. Methods Eight patients with traumatic carotid-cavernous fistula were treated by Jostent coronary steat graft from June 2001 to May 2007. Results The stent graft was successfully implanted in the target artery in all patients. The fistula in all patients was removed and the parent arteries kept unblocked. The clinical outcome was favorable, with no operation-related complications occurred. The ang4ogram showed normal patency of the parent arteries, without recanalization of the fistula six months after the stent graft implantation in six patients. Conclusions Stent graft is a useful tool for endovascular treatment of carotid cavernous fistula in selected patients. Further research is needed to optimize the stent graft for further use in cerebrovascular system.
