1.Risk factors of stroke-associated infection and their correlation with immunodepression

Objective To investigate the risk factors of stroke-associated infection (SAI)and their correlation with immunodepression.Methods One hundred fourteen consecutive patients with acute ischemic stroke admitted with 24 hours after symptom onset were enrolled.They were divided into infection (n =45) and non-infection (n =69) groups according to whether they had infection or not within 7 days after admission.Automatic blood analyzer and flow cytometry were used to measure the peripheral blood lymphocyte count and T lymphocyte subsets on the day of admission.Univariate analysis and multivariate logistic regression analysis were used to analyze the related risk factors for SAI.Results There were statistical significances in vomiting,disturbance of consciousness,dysphagia,indwelling gastric tube,indwelling catheter on admission,and the National Institutes of Health Stroke Scale (NIHSS) score 0-3 days after admission between the infection and non-infection groups.Further more,the numbers of peripheral blood lymphocytes,CD3 + and CD4 + T lymphocytes in the infection group were significantly lower than those in the non-infection group.Multivariate logistic regression analysis showed that dysphagia,the increased NIHSS score 3 days after admission,and the decreased numbers of CD4 + T lymphocytes were the independent risk factors for SAI.Conclusions The T lymphocyte immune function decreased in patients with SAI.It was the result of the combined action of various factors.Targeted prevention and treatment should be taken to reduce the occurrence of SAI.

Objective To study the changes of endothelin-1(ET-1) and tumor necrosis factor alpha(TNF?) in lung homogenate and plasm when acute respiratory failure(ARF) animal models are treated by bilateral femoral arteriovenous extracorporeal membrane oxygenation(bAV-ECMO).Methods Ten ARF dog models were treated by the bAV-ECMO.Right atrium blood and lung tissues were collected at different time points,and ET-1 and TNF? were assayed by radioimmunological method.Results ET-1 content in plasm decreased at first,and at 1h it was elevated and at 2h reached the peak.ET-1 content in lung homogenate reached the peak at 2h and then started to decrease.The analysis of variance showed notable differences(P

Objective To investigate the role of HMGA2 expression and epithelial to mesenchymal cell transformation in the regulation of TGF -β1 in human gastric cancer cells .Methods Human gastric cancer cell SGC-7901 was treated by TGF-β1 at different time points(0,3,6,24,48,72 h).We observed the changes in cell morphology of SGC-7901 through the microscope;HMGA2 assay,E-cadherin,Vimentin protein expression wereperformedbyWesternblot;cellinvasioncapabilitywasanalyzedbytranswellchamberassay.Results (1) With the prolonged duration of action of TGF -β1,SGC-7901 changes in cell morphology ,cells gradually be-came longer and larger.(2)In the role of TGF-β1,gastric cancer cells SGC -7901 are varying degrees of HM-GA2 expression and E -cadherin、Vimentin.HMGA2 protein begun to express from 0h to 6hrs.and HMGA2 pro-tein became the strongest at 6hrs and decreased slightly from 24hrs.E-cad protein expression at 0h begun at 3hrs highest expression ,and the reduction started from 3hrs.Vimentin protein was the highest at 48hrs then gradu-ally decreased.Conclusion In the TGF-β1 induced the process of epithelial to mesenchymal cell transforma-tion in gastric cancer cells SGC -7901 ,HMGA2 protein play an important role .HMGA2 can be used as snail pro-moter,combined with Smads ,promotes the process of epithelial -mesenchymal transformation .HMGA2 may also be regulated by Wnt /β-Catenin signaling pathway on growth and apoptosis of tumor cells .

Tumor metastasis and invasion of cancer are the leading causes of death in patients .The processes of tumor metastasis and invasion involves a variety of factors in which epithelial -to-myofibroblast transition( EMT) plays an important role .Transcription factor Snail as EMT of tumor cell key regulator has played an important role .Through the study of Snail regulating EMT of tumor cell signaling pathways will help us under -stand the mechanisms of tumor invasion and metastasis better and provide a basis for finding effective targets .

Bruton tyrosine kinase (BTK) is a non-receptor tyrosine kinase which belongs to the Tec kinase family and plays an important role in B cell receptor signaling.Nowadays,BTK has been a nove] target for treating some B cell malignancies.Recently,some studies have confirmed that BTK inhibitor,PCI-32765 (ibrutinib),can effectively treat chronic lymphocytic leukemia,mantle cell lymphoma and so on.This review will discuss the preclinical and clinical development of this BTK inhibitor in B cell malignancies.
6.Recognition of walking stance phase and swing phase based on moving window.

Wearing transfemoral prosthesis is the only way to complete daily physical activity for amputees. Motion pattern recognition is important for the control of prosthesis, especially in the recognizing swing phase and stance phase. In this paper, it is reported that surface electromyography (sEMG) signal is used in swing and stance phase recognition. sEMG signal of related muscles was sampled by Infiniti of a Canadian company. The sEMG signal was then filtered by weighted filtering window and analyzed by height permitted window. The starting time of stance phase and swing phase is determined through analyzing special muscles. The sEMG signal of rectus femoris was used in stance phase recognition and sEMG signal of tibialis anterior is used in swing phase recognition. In a certain tolerating range, the double windows theory, including weighted filtering window and height permitted window, can reach a high accuracy rate. Through experiments, the real walking consciousness of the people was reflected by sEMG signal of related muscles. Using related muscles to recognize swing and stance phase is reachable. The theory used in this paper is useful for analyzing sEMG signal and actual prosthesis control.

7.Investigation of status quo of self-perceived burden of young and middle-aged patients with first-attack of ischemic stroke and analysis of its influencing factors

Objective To know the self-perceived burden (SPB) of young and middle-aged patients with first-attack of ischemic stroke and to analyze the influential factors of SPB as well as its relevance to the quality of life.Methods The patients' general information questionnaire,SPB Self-evaluation Scale,QOL-BREF were adopted to investigate young and middle-aged patients with first-attack of ischemic stroke.Results 100 patients were included,66 were male,34 were female.The Minimum age was 19.4 years old.The maximum age was 59 years old.The mean age was (50.80 ± 7.98) years old.The average SPB score was (24.21 ± 7.39) points,which was in the medium level.31 cases (31%) didn't have obvious SPB; 65 cases (65%) suffered mild-to-moderate SPB; 4 cases (4%) claimed they had severe SPB.There was negative correlation between the SPB and their quality of life.The multi-factor analysis showed that gender,the patients' understanding of disease,and caregivers' health situation were the influential factors of SPB.Conclusions Since self-perceived burden is common in young and middle-aged patients with ischemic stroke,clinical nurses should assess patients' self-perceived burden so as to provide targeted nursing intervention and health education,thereby to ease the burden on patients' self feeling,eventually to improve the level of their mental and physical health.
8.The express of angiotensin Ⅱ in lungs during bilateral femoral arteriovenous shunt for extracorporeal membrane oxygenation

Objective To study the change of angiotensin Ⅱ in the applications of pumpless ECMO, and its effect on prognosis of the acute respiratory failure. Methods The study was performed in ten dogs [ weight 18 - 35 kg, mean weight ( 23.4 ± 4.7 ) kg].A respiratory failure animal model was established end then was treated by bilateral femoral artery-venous ECMO. Collection right atriurn blood end constitution of lung at different time (before ECMO, during ECMO 1h, 2h, 3h, 4h). Angiotensin Ⅱ content in blood and lung homogenate was detected by radio-immunity mothod. Angiotensin Ⅱ expression loci in lung were detected by immunohistochemistry mothod. Results Angiotensin Ⅱ content in plasma was decreased at completion of the model, it was back up at 1 hour and reached the peak at 3 hours, then it slowly declined. Angiotensin Ⅱ content in lung homogenate increased at the beginning, peaked by 2 hours, end then it decreased. Loci angiotensin Ⅱ in lung by immunohistochemistry were expressed in most of epithelial cells cytoplast of bronchiole dissepimont, smooth muscle cell cytoplast of small blood vessel around it end a few macrophage cytoplasts during the model time..Conclusion It is valuable to measure engiotensin Ⅱ in blood through the bilateral femoral artery-venous ECMO, because it can reflect angiotensin Ⅱ in lung and the resume of lung in certain degree.

Anorectal malformations (ARM)comprise a broad spectrum of congenital disorders that account for 25 % of gastrointestinal malformations.Despite numerous technical advances for treatment of ARM,complications such as fecal incontinence and constipation still occur and can greatly deteriorate patients' quality of life.It is recognized that lumbosacral spinal cord anomalies in ARM has been an important factor affecting the fecal function after procedure.Researchers have found that lumbosacral myelodysplasia is the common seen complication of ARM and neural cells decreased in lumbosacral spinal cord by the study on animals and human.Due to numerous factos affecting nerve innervation on annrectus and pelvic floor muscle,this review summarizes the nervous system complications and abnormal intervention of human and rats ARMs.The developments of study on ARM complications and intervention are detailed.Then the new direction of the research about the anorectal malformation nervous system is put forward,and the new strategy of improving the prognosis of anorectal malformation surgery is explored.
10.Diagnosis judgment values of trimester ultrasound screening for simple single umbilical artery fetal malformation

Objective To investigate the diagnosis judgment values of trimester ultrasound screening for simple single umbilical artery fetal malformation.Methods From January 2011 to January 2015,selected the 15-28 weeks pregnant singleton pregnancies in Shunyi Hospital of Beijing Chinese Medicine Hospital for system fetal ultrasound,in which the single umbilical artery group of 50 patients,50 cases of healthy pregnant women (control group),the two groups were carried ultrasound screening with single umbilical artery flow spectrum analysis,and observed the prognosis.Results The single umbilical artery fetal bladder after abdominal oblique horizontal cross-section of the umbilical artery bypass saw one side of the bladder,and there were 24 patients the malformations with the incidence rate was 48.0%.In the two groups can be clearly demonstrated umbilical artery condition,but the umbilical artery pulsation index(PI),arterial resistance index (RI) and peak valley ratio of umbilical artery blood flow velocity (S/D) value in the single umbilical artery group were 1.02±0.17,0.66±0.06 and 3.08 ±0.57,significantly higher than those of the control group (0.75 ± 0.21,0.53 ± 0.08,2.11±0.27;t=7.298,11.874,15.983,P< 0.05).In the single umbilical artery group,there were 32 patients of abortion,10 patients of birth were,8 patients ogpregnancy.Perinatal death in 8 cases,the mortality rate was 16.0%,the single umbilical artery fetal malformations of perinatal mortality was significantly higher than that of children with simple single fetal umbilical artery (x2 =9.333,P < 0.05).Conclusion Trimester ultrasound screening for single umbilical artery combined with simple two-dimensional ultrasound and umbilical arteryblood flow velocity conditions can improve fetal malformations value judgment,provide a reliable detection method to reduce oerinatal mortality.
