2.Analysis of 400 forensic cases of mental disorders due to brain damage

Objective To explore the clinical characteristics and related factors of mental disorder caused by brain injury in the judicial identiifcation of mental illness. Methods Forensic case data of brain injury ware collected and analized. Brain injury sverity was classied into 3 groups, namely severe group(group A); moderate group (group B) and mild group(group C) according to neurosurgery GCS scores; clinical characteristics were analyzed, diagnosis were made against the Chinese Classiifcation of Mental Disorders (the 3rd ed.) (CCMD-3). Relationship between the clinical characteristics and brain damage severity was analyzed. Results Organic neurosis-like syndromes are the main manifestation of mental disorders caused by mild and moderate brain injuries. Organic intellectual deifciency is the main manifestation of mental disorders due to severe brain injury. The difference between the three groups was significant (P ≤ 0.01). Conclusion Different clinical features are related to different brain injury severities in the forensic evaluation of mental disability.
3.Application of sensitive sulphur electrode assay to measure and analyze cystathionine-?-lyase/hydrogen sulfide pathway of cardiovascular tissues, cells and plasma in rats

Objective:To constract a method of measurement microamount hydrogen sulfide (H_2S) using sensitive sulphur electrode. Methods: According to the physical and chemical characters of H_2S, H_2S, which in the fluids by mean of physical dissolve and chemical shape, is turned to sulphur ion (S 2-) by chemical responses. After the microamount of S 2- was measured by sensitive sulphur electrode, and the concentration of H_2S was converted, a method was constructed to measure the H_2S. It was used to analyze the concentrations of H_2S of plasma in rats and humans, the endogenous concentration of H_2S of cardiovascular tissue in rats, and CSE activity of cardiovascular tissues and cells. Results: The exponential regression of S 2- in the extent including 1 to 80 ?mol/L was found using sensitive sulphur electrod. The H_2S levels of plasma in male and female rats were 40?4 and 41?5 ?mol/L, respectively, and significant difference was not found; those in venous blood plasma of men and women were 33?4 ?mol/L and 35?5 ?mol/L respectively, without significant difference. There were not significant differences in the aortic endogenous levels of H_2S (24?6 and 25?5 nmol/mg pro) and myocardial levels (19?4 and 19?6 nmol/mg protein) between female and male rats. There were no different results of CSE activity in aortal tissue using sensitive sulphur electrode or traditional methods, however, the CSE activity of vascular smooth muscle cells could be accurately measured using sensitive sulphur electrode, which was difficult in using traditional method. Conclusion: The sensitive sulphur electrode assay was fit for the analysis of CSE/H_2S pathway in cardiovascular research.

Integrin is an important cellular adhesion molecule,which mediates many biologic actions such as cell-cell adhesion or cell-extracellular matrix adhesion.The relationship between integrin and tumor metastasis,as well as the important role of integrin during tumor progress,is reviewed in this article,which gives us the theoretical base for new pathways of cancer treatment.
5.Restraint stress down-regulates L-Arg/NOS/NO pathway of platelet and aortic intima in rats

Objective: To investigate alteration and cross link of the aortic intima and platelet endogenous L-Arg/NOS/NO pathway induced by water immersion restraint (WIR) stress. Methods: After 7 h of WIR stress, the aortic intima was isolated and prepared the platelet, then NO_2- production released from aortic intima and platelet was measured with Greiss regent, NOS activity and L-arginine transport activity were detected by isotope tracer method. The standardized resident training is an important part of continuing medical education in China,which plays a vital role in the cultivation and development of doctors.At present,the standardized resident training system in our country is at an exploratory stage and different from those advanced countries.This paper contrastively analyzed the domestic and foreign standardized resident training modes from the perspectives of education patterns,training years,training organizations,training contents,assessments,and financial sources.By learning from the advanced experience of those foreign countries,it provided the reference for the standardized resident training system in China.
7.Volar locking compression plate fixation for dorsally displaced fractures of distal radius

[Objective]To evaluate a preliminary outcome of volar surgical treatment of dorsally displaced fractures of distal radius with open reduction and internal fixation with T shape locking compression plate(TLCP).[Method]From Sept.2003 to Nov.2005,9 cases with dorsally unstable distal radius fractures were treated with open reduction and internal fixation with T-LCP.This study involved 3 males and 6 femals with an average age of 63.5 years(ranged from 52 to 74 years old).According to AO classification: 2 cases of types B2;1 case of type B3;2 cases of type C1;3 cases of type C2;1 case of type C3,all of them were closed fractures of distal radius.The fractures of 9 cases were fixated with T-LCP by volar approach and dorsal soft tissues were not dissected during the operation.The Osteosets and the artificial bone substitute were implanted into bone defects if they were large enough.[Result]Nine cases were followed up for 6 to 17 months and the average time was 10.7 months.The X-ray pictures showed that unions have been achieved in all patients and a mean healing time was 7 weeks.The Osteosets were implanted into large bone defects in one case.No complication was found as infection,non-union,loosing of nails,syndrome of wrist,and medium neuritis.The functional recovery was achieved from 6 to 29 weeks with an average time of 12.5 weeks after operation.Passive wrist motion,active finger motion and forearm rotation were encouraged immediately after surgery.Active wrist motion was suggested in 7 days postoperatively.The clinical outcomes were evaluated according to modified Mcbide grading system.There were 7 excellent,1 good and 1 fair,the satisfactory rate was 88.9%.[Conclusion]The volar fixation of T-LCP for dorsally displaced fractures of distal radius has a good clinical outcome.It allows the patients to begin early exercise.
8.Investigation on Health Impact of Noise Created by House Decoration in Residential Quarters

Objective To provide scientific basis for the investigation, prevention,control and administration of house decoration noise Methods The equivalent noise levels (Leq),and the levels of noise dose (D L) in decorated houses,next door houses and outdoor environment were measured during different periods such as morning,noon,afternoon and evening in one day, as well as the health effects of noise in 3 residential quarters were investigated Results The Leqs levels during per 10 minutes were 79 6~104 7 dB(A) in decorated house The individuals exposure D L of decoration workers were 90%~180% The Leqs during per 20 minutes were 70 9~76 4 dB(A) in next door houses and 63 4~76 5dB(A) in the neiboaring residential building and office building 6~45 meters far from the decoration noise sources The annoyance rate among 149 residents was high and reached 89 8% The mental labourers revealed significantly higher annoyance rate compared with that of physical labourers Conclusion The health effects of decoration noise on occupational individuals and residents shouldnt be ignored
9.Perioperative Stroke

Stroke is one of the severe perioperative complications.The incidence of perioperative stroke has great difference because of the surgical site,surgical complexity,and different basic diseases of patients.The induced anesthesia and the intraoperative management also have an important influence on the onset of stroke.The mortality of perioperative stroke is as high as 26%.Neturologists must guide the assessment of the potential risks for perioperative stroke,and carefully balance the risk-benefit ratio when making each decision.This article reviews the advances in research on the risk factors for perioperative stroke,prevention,and treatment.
10.Effects of adrenomedullin and proadrenomedullin N-terminal 20 peptide, alone or in combination, on the rat hearts in vitro

Objective: To study the effects of adrenomedullin (ADM) and proadrenomedullin N terminal 20 peptide (PAMP) alone or in combinations on the isolated rat hearts as well as the possible signaling pathways involved in their actions. Methods: In isolated rat hearts, the left ventricular pressure (LVP), LVP?dp/dtmax, coronary fluid (CF) and heart rate(HR) of the hearts infused at different concentrations of ADM and/or PAMP were determined by a 4 cannal physiological recorder, then the cAMP contents were assayed in myocardium. Results: After being infused with ADM from 10 -11 to 10 -8 mol?L -1 or PAMP from 10 -11 to 10 -8 mol?L -1 , the LVP and LVP?dp/dtmax of the isolated hearts decreased gradually in a concentration dependent manner, and at the same concentration, the effects of PAMP were more potent than those of the ADM. When ADM and PAMP were co administrated with both concentrations as low as from 10 -11 to 10 -10 mol?L -1 , the cardiac parameters were decreased more than either ADM or PAMP administrated alone. However, the inhibitory effects of ADM and PAMP were attenuated when they were in combination at higher concentrations as from 10 -9 to 10 -8 mol?L -1 . When the rat hearts were infused with ADM, PAMP,and ADM plus PAMP, the CF were always higher than those of the controls and decreased when co administrated with L NAME, an inhibitor of NOS, but the decreaseddegree of LVP and LVP?dp/dtmax were attenuated by L NAME.The cAMP contents in the left cardiac ventricle were increased significantly by ADM infusions but not changed obviously by PAMP, and were of no statistical difference in rat hearts with ADM administrated alone from those combinated with ADM and PAMP. Conclusion: These results showed that ADM and PAMP infused alone or in combinations inhibited the function of rat hearts in vitro, which might be partly involved with the NOS/NO pathway.
