1.Glycosides from Machilus wangchiana.

Ten glycosidic compounds were isolated from an ethanol extract of Machilus wangchiana by a combination of various chromatographic techniques including column chromatography over silica gel and Sephadex LH-20 and reversed-phase flash chromatography and HPLC. Their structures were identified by spectroscopic data analysis (IR, MS, and NMR) as icariside B1 (1), boscialin-3-O-β-D-glucopyranoside (2), pisumionoside (3), isolariciresinol-9'-O-β-D-xylopyranoside (4), 5'-methoxyisolariciresinol-9'-O-β-D-xylopyranoside (5), lyoniresinol-9'-O-β-D-xylopyranoside (6), (E) -4-hydroxyphenylprop-7-ene 4-O-β-D-glucopyranoside (7), (E) - 4-hydroxy-3-methoxyphenylprop-7-ene 4-O-α-L-rhamnopyranosyl-(1 --> 6) -β-D-glucopyranoside (8), 4-hydroxy-3-methoxyphenylprop-8-ene 4-O-β-D-xylopyraosyl-(1 --> 6) -β-D-glucopyranoside (9), and 4-hydroxy-3,5-dimethoxyphenylprop-8-ene 4-O-α-L-rhamnpyranosyl-(1 --> 6)-β-D- glucopyranoside (10), respectively.

2.Hepatitis B virus X protein induces epithelial-mesenchymal transition through c-Src activation in liver cancer cells

Objective To examine the role of c-Src activation in hepatitis B virus X (HBx) protein induced epithelial-mesenchymal transition (EMT) in liver cancer.Methods SMMC-7721 liver cancer cells were transfected with HBx gene to induce EMT and the activated c-Src expression was evaluated by Western blot.Both the morphological changes and the epithelial and mesenchymal markers expression (real-time PCR,western blot and immunocytochemistry) of HBx-transfected SMMC-7721 cell treated by c-Src kinase inhibitor PP2 and negative control PP3 were observed and compared,respectively.Results The activated c-Src expression in HBx gene transfected SMMC-7721 cells was significantly increased compared to that in mock transfected cells,c-Src kinase inhibitor PP2 could enable the HBx-transfected SMMC-7721 cells to transmit from spindle-like shape to original epithelial morphology.Western blot and immunocytochemistry confirmed that the expression of epithelial markers and mesenchymal markers almost returned to the levels of parental cells,indicating the mesenchymal-epithelial transition.Conclusions c-Src activation plays a key role in the process of EMT induced by HBx protein in SMMC-7721 cells.
3.CT and MRI findings in patients with autoimmune pancreatitis.

OBJECTIVETo evaluate computed tomography (CT) and magnetic resonance imaging (MRI) findings in patients with autoimmune pancreatitis (AIP).
METHODSThe imaging findings of pancreas and extra-pancreas in 24 patients with AIP were retrospectively reviewed. Among them, CT scan was performed in 18 patients, MRI in 11, and bGth CT and MRI in 10.
RESULTSThe pancreas showed diffuse enlargement (25%, 6/24), focal enlargement (37. 5%, 9/24), combined enlargement (25%, 6/24) ,and no enlargement (12. 5%, 9/24). Unenhanced CT showed hypoattenuation in AIP area (n = 2) . After intravenous injection of contrast medium, 17 patients showed abnormal contrast enhancement in the affected pancreatic parenchyma, including hypoattenuation during the arterial phase (50%, 9/18) and hyper attenuation during the delayed phase (94. 4%, 17/18). Precontrast MRI showed abnormal signal intense (n =9), including hypointense on T1-weight images (T1 WI) (n = 7), hyperintense (n = 7) and hypointense (n = 2) on T2-weight images (TIWI). Enhanced MRI demonstrated abnormal contrast enhancement within lesions (n = 11), including hypoattenuation during the arterial phase (81. 8%, 9/11) and good enhancement during the delayed phase (100%, 11111). A capsule-like rim was seen around pancreas (37. 5%, 9/24), among which CT detected in 6 out of 18 patients and MRI found in 7 out of 11 patients.The main pancreatic duct lumen within lesions has no visualization (100%, 24/24) and upstream dilation of the main pancreatic duct (n = 8) , ranging from 2. 2 to 4. 5 mm(mean 3. 1 0. 47 mm) in diameter. Narrowing of the common bile duct was shown in 14 patients. Miscellaneous findings were: infiltration of extrapancreatic vein (n = 9) and artery (n = 1); mild fluid collection around pancreas (n = 2); pseudocysts (n = 3). Fourteen patients also presented one or more of the following extrapancreatic imaging findings: narrowing of the intra-hepatic bile duct or hilar duct (n = 5); thickening of gallbladder wall (n = 5); fibrosis in mesenteric (n = 2), in retroperitoneal (n = 2) and in ligamentum teres hepatis (n = 1); renal involvement (n = 3); peri-pancreatic or para-aortic lymphadenopathy (n = 10); and ulcerative colitis (n = 3).
CONCLUSIONAIP display some characteristic CT and MRI imaging features: sausage-like change of the pancreas; capsule-like rims around lesions; delayed contrast enhancement in the affected pancreatic parenchyma; segment or diffuse pancreatic duct stenosis but mild upstream dilation and extrapancreatic organs involvement. CT and MRI findings combining with serological tests and pancreas biopsy can assist physicians to make accurate and timely diagnosis.

4.Effect of surgical treatment for thoracolumbar spinal tuberculosis by anterior radical debridement with bone graft fusion and posterior pedicle screw-rods system fixation.

OBJECTIVETo study the clinical effects of one stage surgical treatment of thoracolumbar spinal tuberculosis by anterior radical debridement with bone graft fusion and posterior pedicle screw-rods system fixation.
METHODSFrom February 2002 to March 2007, 21 patients who were 45.3 years old on average with thoracolumbar spinal tuberculosis, including 13 males and 8 females were underwent surgical treatment of posterior pedicle screw-rods system fixation, correction of deformities, anterior radical debridement, bone graft fusion. Among them, 3 patients had 2 involved vertebras, 17 patients had 3 involved vertebras, and 1 patient had 4 involved vertebras. The patients were all complicated with kyphosis deformity with an average Cobb angle of (28.0+/-9.7) degrees. According to the Frankel neurological function grade system, 3 cases in grade B, 5 in grade C, 1 in grade D, 12 in grade E. All the patients received antiphthisic treatment with isonicotinylhydrazide (INH), rifampicin (RFP), Ethambutol (EMB), pyrazinamide (PZA), and support therapy in preoperation. The chemotherapy was continued 9 to 12 months after operation. Neurological functions of all cases were investigated, and radiography was analyzed pre-operation and post-operation to determine the state of bony fusion and the correction of spinal kyphosis. Statistical analysis was made according to the preoperative and post-operative Cobb angle.
RESULTSThe follow-up period ranged from 1 to 6 years with 2.5 years on average. All the patients had primary healing of the incisions. The Cobb angle was (9.8+/3.3) degrees in post-operation. The kyphosis deformity was corrected 17.2 degrees in thoracolumbar on average (t=-13.48, P<0.01). According to the Frankel neurological functional grade system, 1 case was in grade C, 3 in grade D, and 17 in grade E.
CONCLUSIONIt is effective for patients with thoracolumbar spinal tuberculosis in correction of spinal kyphosis, reconstruction of the spinal stability and bony fusion by anterior radical debridement with bone graft fusion and posterior pedicle screw-rods system fixation.

5.CT findings and dynamic changes of COVID-19 in the patients younger than 18 years old infected with SARS-CoV-2 Omicron variant

navirus disease 2019 (COVID-19) in the patients younger than 18 years old infected with severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) Omicron variant, and to provide a basis for determining the chest CT changes and efficacy of COVID-19 caused by Omicron virus variant in patients younger than 18 years old. Methods The clinical and imaging data of 30 cases of patients younger than 18 years old infected with COVID-19 Omicron variant, who admitted to the Third People's Hospital of Shenzhen from February 11 to March 26, 2022 were collected and retrospectively analyzed. The clinical manifestations, imaging features and dynamic changes of lesions were summarized. Results A total of 41 intrapulmonary lesions in 30 patients with COVID-19 caused by SARS-CoV-2 Omicron variant. The main manifestations were patchy or nodular ground-glass opacities and/or consolidation, with focal subpleural distribution, lesions mainly occur in the right lung (70.73%, 29/41). There were 42 lesion morphologies, with 22 (52.38%) striped shadows and 16 (38.10%) nodular shadows, with small lamellar and patchy shadows predominating. There were 36 lesion density variations, with ground glass shadows being the most common, with a total of 24 ground glass shadows (66.66%) in each lobe of the lung, and also 6 consolidation lesions (16.67%) and 6 mixed ground glass opacity and consolidation lesions (16.67%). With the progression of the disease, lesions gradually enlarged, appeared on the 2nd day (312.93 mm3), peaked on the 9th day (1 837.18 mm3). The average absorption time of the lesions was (16±3) days, and there was no significant difference between the absorption time of patchy and nodular lesions (ground glass and/or consolidation) (t=0.853, P>0.05). The lesions showed focal ground-glass opacity in the early stage, 77.78% lesions were absorbed after treatment in the late stage. Inflammatory nodules were absorbed slowly (9-19 days), without residual fibrotic changes. Conclusions The imaging manifestations of COVID-19 in patients younger than 18 years old infected with SARS-CoV-2 Omicron variant have certain characteristics, showed patchy or nodular ground glass opacities and/or consolidation, mainly distributed in the subpleural area, with small and few lesions and slow change, didn't remain fibrosis. Being familiar with its clinical and imaging manifestations can assist in early diagnosis, but confirming the diagnosis requires a combination of epidemiological history, clinical symptoms, SARS-CoV-2 nucleic acid and radiological manifestations.
6.Association between dyslipidemia and chronic kidney disease: a cross-sectional study in the middle-aged and elderly Chinese population

Background Dyslipidemia,a well-known risk factor for cardiovascular disease,is common in patients with kidney disease.Recent studies discerned that dyslipidemias play a critical role in renal damage progression in renal diseases,but the association between dyslipidemias and chronic kidney disease (CKD) in the general population remains unknown.Thus,we assessed whether the growing prevalence of dyslipidemia could increase the risk of CKD.Methods A total of 4779 middle-aged and elderly participants participated in this study.Dyslipidemias were defined by the 2007 Guidelines in Chinese Adults.Incident CKD was defined as albuminuria and/or reduced estimated glomerular filtration rate (eGFR,<60 ml.min-1.1.73 m-2).Regression analysis was used to evaluate the association between dyslipidemia and albuminuria/reduced eGFR.Results Participants with hypercholesterolemia exhibited a greater prevalence of albuminuria and reduced eGFR (10.0% vs.6.1%,P=-0.001; 4.0% vs.2.4%,P=-0.028,respectively).Both hypercholesterolemia and low high density lipoprotein cholesterol (HDL-C) were independently associated with albuminuria (odds ratio (OR) 1.49; 95% confidence interval (CI) 1.08-2.07 and OR 1.53; 95% CI 1.13-2.09,respectively).The multivariable adjusted OR of reduced eGFR in participants with hypercholesterolemia was 1.65 (95% CI 1.03-2.65).As the number of dyslipidemia components increased,so did the OR of CKD:0.87 (95% CI 0.65-1.15),1.29 (95% CI,0.83-2.01),and 7.87 (95% CI,3.75-16.50) for albuminuria,and 0.38 (95% CI 0.21-0.69),1.92 (95% CI 1.14-3.25),and 5.85 (95% CI 2.36-14.51)for reduced eGFR,respectively.Conclusions Our findings indicate that dyslipidemias increase the risk of CKD in the middle-aged and elderly Chinese population.Hypercholesterolemia plays an important role in reducing total eGFR.Both low HDL-C and hypercholesterolemia are associated with an increased risk for albuminuria.
7.Clinical application of the free superficial peroneal artery perforator flap.

OBJECTIVETo investigate the applied anatomy of the superficial peroneal artery perforator flap and report the clinical results of repairing the soft tissue defects with free perforator flaps.
METHODS15 fresh cadavers were injected with a modified lead oxide-gelatin mixture for three-dimensional visualization reconstruction using a 16-slice spiral computed tomography scanner and specialized software (Materiaise's interactive medical image control system, MIMICS). The origin, course and distribution of the superficial peroneal artery perforator in the anterolateral leg region were observed. Clinically 6 cases with hand defects and 6 cases with feet defects were treated with free superficial peroneal artery perforator flap transplantation. The defect size ranged from 3.0 cm x 4.5 cm to 5.0 cm x 11.0 cm.
RESULTSThe diameter of the superficial peroneal artery is (1.2 +/- 0.3) mm at its origin from the anterior tibial artery 5 cm below the fibula head. It is (5.6 +/- 1.8) cm in length. This artery is truly anastomosed with other perforators to form the chain of superficial peroneal nerve accessory artery. The superficial peroneal artery perforators [outer diameter (0.7 +/- 0.2) mm] with a vein are in the anterolateral leg region, supplying the skin in proximal-middle region. All the 12 cases were treated successfully. The clinical results were satisfactory after 3-12 months of following-up.
CONCLUSIONSThe superficial peroneal artery perforator flap has constantly, reliable blood supply, and good texture. It is a good option for repairing soft-tissue defect with free transfer.

8.Study on radiation sensitivity of colorectal cancer cells using gene expression profile.

OBJECTIVETo screen the radiosensitivity-related genes of colorectal cancer cells.
METHODSGene expression profiles of two different radiosensitivity cells(colorectal cancer cell line Lovo and SW480) were obtained by cDNA array and the differences of gene expression profiles between the two cells were analyzed.
RESULTSGenes of more than 2-fold expressive differentiation were screened. In Lovo cells, 908 up-regulated genes were found, including higher expression genes CEACAM5, THBS1, SERPINE2, ARL7, HPGD, while 1312 genes were down-regulated. In SW480 cells, higher expression genes were SCD, NQ01, LYZ, KRT20 and ATP1B1.
CONCLUSIONGene profiles can reflect the radiosensitivity of colorectal cancer cells, which will provide the choice for the further study of radiosensitivity in colorectal cancer.

9.Rule of Chinese medicine syndrome differentiation in the patients with thoracic diseases at perioperation stage.

OBJECTIVETo study the rule of syndrome differentiation in the patients with thoracic diseases at perioperation stage.
METHODSA standard was created referring to the related literature, and it was applied to differentiate the Chinese medicine syndrome in 150 patients before and three days after thoracic operation.
RESULTSBefore operation, Chinese medicine syndromes were as differentiated as phlegm type in 45.3%, blood stasis type in 17.3%, and qi-stagnancy type in 16.0%. The patients with asthenia syndrome markedly increased after operation, accounting for 34.0% (51 patients, including qi-, yin-, and blood-deficiency syndromes). The most frequently seen intermixed syndromes were qi-deficiency with phlegm-stasis syndrome and Pi ()-deficiency with phlegm-dampness syndrome. The intermixed syndromes revealed in 37.5% and 42.0% of the patients before and after operation, respectively.
CONCLUSIONSThe syndrome in the patients with thoracic disease before operation was mostly the excessive syndrome, mainly the phlegm syndrome type; at postoperation stage, Chinese medicine syndrome in patients become asthenia in essence with excessive superficiality, which is mostly revealed as Pi-deficiency with phlegm-dampness.

10.Nine cases of laparoscopic retroperitoneal lymph node dissection.

OBJECTIVETo assess the effects of laparoscopic retroperitoneal lymph node dissection in the treatment of stage I nonseminomatous testicular cancer.
METHODSFrom January 2001 to May 2002, laparoscopic retroperitoneal lymph node dissection was performed on 9 patients with stage I nonseminomatous testicular cancer.
RESULTSThe procedure was successful in all patients. The mean operation time was 260 minutes. None of the patients required blood transfusion and had major complications intraoperatively or postoperatively. The average period of hospitalization after the operation was 5.5 days. With a mean following-up of 9 months, retroperitoneal recurrence was not seen.
CONCLUSIONSLaparoscopic retroperitoneal lymph node dissection is feasible for stage I nonseminomatous testicular cancer and its procedure is safe, effective and minimally invasive.
