1.A Case of Effusive-Constrictive Pericarditis in and Infant Treated by Pericardiectomy.

Effusive-constrictive pericarditis is a very rare disease in infants but has high motality rates when not treated. There were some reports of pericardial constriction associated with intrapericardial abscess that led to pericardiectomy. The patient was admitted due to fever, cyanosis, and abdominal distension. We treated the patient with antibiotics and pericardiostomy but the symtoms did not improved, therefore, pericardiectomy was perfomed immediately. The patient with effusive-constrictive pericarditis was immediately relief on the symptoms and the treatment was successful.

5.A Case of Enema - Induced Colitis.

Enemas are sometimes used in hospital and domicilliary practice and occasionally by patients themselves without a valid indication. The present repart is one in which enernas were done for 3 months by a patient himself to treat his hemorrhoid and resulted in severe inflamimatory change with large-siaed illdefined shallow ulceration in rectum and sigmoid colon. This patient, a 53 year old male, entered the hospital with the complaint of diarrhea with passage of bloody muct.. Colonoscopic examination revealed severe inflammatory change, friable mucosa and several relatively large-sized ill-defined shallow ulceration in rectum and sigmoid colon. A biopsy was performed and microscapic examination revealed intense infiltrates and plasma cells and crypt abscess with decreased goblet cells in the lamiina propria. The submucosa showed edema with infiltrates of chronic inflammatory cells and neutrophils. Conservative treatment with discontinue of enemas and fluid supply was performed. And his symptoms were improved. After second week, follow-up colonoscopy revealed inflammatory changes were improved.

PURPOSE: To evaluate the effect of acupuncture on regional cerebral blood flow (rCBF) at acupoints suggested by oriental medicine to be related to the treatment of cerebrovascular diseases. MATERIALS AND METHODS: Rest/acupuncture-stimulation Tc-99m ECD brain SPECT using a same-dose subtraction method was performed on 54 normal volunteers (34 males, 20 females, age range from 18 to 62 years) using six paradigms: acupuncture at acupoints GV. 20, GV. 26, LI. 4, ST. 36 and SP. 6. In the control study, needle location was chosen on a non-meridian focus 1 cm posterior to the right fibular head. All images were spatially normalized, and the differences between rest and acupuncture stimulation were statistically analyzed using SPM for Windows . RESULTS: Acupuncture applied at acupoint GV. 20 increased rCBF in both the anterior frontal lobes, the right frontotemporal lobes, and the left anterior temporal lobe and the left cerebellar hemisphere. Acupuncture at GV. 26 increased rCBF in the left prefrontal cortex. Acupuncture at LI. 4 increased rCBF in the left prefrontal and both the inferior frontal lobes, and the left anterior temporal lobe and the left cerebellar hemisphere. Acupuncture at ST. 36 increased rCBF in the left anterior temporal lobe, the right inferior frontal lobes, and the left cerebellum. Acupuncture at SP. 6 increased rCBF in the left inferior frontal and anterior temporal lobes. In the control stimulation, no significant rCBF increase was observed. CONCLUSION: The results demonstrated a correlation between stimulation at each acupoint with increase in rCBF to the corresponding brain areas.

8.Autologous Transfusion in Pregnant Women with Significant Risk for Hemorrhage.

Autologous Transfusion, storage of one's own blood for subsequent infusion if needed, is safe and effective in a variety of scheduled operative procedures. Obstetric involvement in such programs in very limited, however, because of concern over the possibility of inducing premature labor or causing fetal distress by blood volume change or vasovagal reactions. We describe our experience with pregnant women in this program. The incidence of vagovagal reactions of autologous donation was 9.5% (2.21). After entry into this program, 17pastients received a total 37pints, which consist of 19 Autologous and 18 Homologous. Homologous transfusion was avoided in 30% of patients receiving blood. The values of the mean hematocrits before and after hpebotomy were 34.1% and 31.8% respectively. It was stastically significant (p<0.01). We recommended that autologous blood donation by pregnant women in third trimester is safe for mothers or infants and it should be strongly encouraged for patient with placenta previa and repeated cesarean section.

9.Report of Free Operations for the Blind to Celebrate the Bicentennial of Korean Catholic Church.

Year of 1984 is the bicentennial of the birth of Korean Catholic Church. unlike the Churches in most other countries, the Korean Catholic Church was founded by the laity without priest. despite Severe oppression, the Korean Catholic Church sustained its vitality and blossomed out splendid flower of catholicism for the seeds of the blood of many martyrs. By reason of this, the bicentennial(1784-1984) has special meaning for Korean Catholic Church. So, to celebrate the bicentennial and to put into action the love which is basic vocation as christians, the Korean Catholic Church decided to perform free eye operations for the poor blind and assigned 11 Catholic Hospitals including of Kangnam St. Mary's Hospital in Seoul for this project. During the last 2 years and 4 months from 5th of May 1983 to 30th of September, 1985, 1,136 persons (1,350 eyes) underwent operations such as cataract extraction, glaucoma surgery and keratoplasty in these hospitals and the final outcome showed satisfactory result with the good vision improvement.

10.Shunt Overdrainage Caused by Displacement of the Pressure Control Cam after Pressure Adjustment.

Although the Codman-Hakim programmable valve is one of most popular shunt systems used in the clinical practice for the treatment of hydrocephalus, malfunctions related with this system have been also reported which lead to underdrainage or overdrainage of the cerebrospinal fluid. While obstruction of the ventricular catheter by tissue materials or hematoma and catheter disconnection are relatively common, the malfunction of the valve itself is rare. Herein, we report on a rare case of shunt overdrainage caused by displacement of the pressure control cam after pressure adjustment. A 57-year-old female, who underwent a ventriculoperitoneal shunt eight years ago, experienced aggravating symptoms of shunt overdrainage after pressure adjustment. Displacement of the pressure control cam was revealed on the X-ray, and a shunt revision was performed. The purpose of this report is to provide a working knowledge of the valve structure and to enhance the ability to interpret the valve setting on an X-ray for diagnosis of valve malfunction.
