1.A Case of Effusive-Constrictive Pericarditis in and Infant Treated by Pericardiectomy.

Effusive-constrictive pericarditis is a very rare disease in infants but has high motality rates when not treated. There were some reports of pericardial constriction associated with intrapericardial abscess that led to pericardiectomy. The patient was admitted due to fever, cyanosis, and abdominal distension. We treated the patient with antibiotics and pericardiostomy but the symtoms did not improved, therefore, pericardiectomy was perfomed immediately. The patient with effusive-constrictive pericarditis was immediately relief on the symptoms and the treatment was successful.

5.Is Carotid Artery Ultrasound Still Useful Method for Evaluation of Atherosclerosis?.

Carotid ultrasound is an imaging modality that allows non-invasive assessment of vascular anatomy and function. Carotid intima-media thickness (IMT) has been shown to predict cardiovascular (CV) risk in multiple large studies. However, in 2013, American College of Cardiology/American Heart Association guidelines designated that the carotid IMT as class III evidence level was not recommended for use in clinical practice as a routine measurement of risk assessment for a first atherosclerotic CV event. Following the announcement of this guideline, combined common carotid IMT and plaque, including plaque tissue characterization and plaque burden, using 3D ultrasound was reported to be better than either measurement alone in a variety of studies. Moreover, changes in the intima thickness were related to aging and early atherosclerosis, and remodeling of the media thickness was associated with hypertension. Separate measurement is useful for evaluating the effects of different atherosclerotic risk factors on the arterial wall; however, a more detailed and elaborate technique needs to be developed. If so, separate measurement will play an important role in the assessment of atherosclerosis and arterial wall change according to a variety of risk factors, such as metabolic syndrome. In addition, although carotid blood flow velocity is a useful tool for risk classification and prediction in clinical practice, further clinical research is needed. The value of carotid IMT by ultrasound examination for risk stratification remains controversial, and groups developing future guidelines should consider the roles of plaque presence and burden and hemodynamic parameters in additional risk stratification beyond carotid IMT in clinical practice.

9.A photoelastic study of the stress distribution on the multiloop edgewise arch wire.

This study was designed to investigate the stress distribution, intensity and force mechanism derived from the MEAW by photoelastic stress analysis of the artificial teeth and surrounding bone composed of photoelastic(material/ PL-3 ). The findings of this study were as follows: 1. In case of no elastic on the MEAW with tip back, the moderate stress was observed on the molar and canine area, and the light stress was observed on the other area. 2. In case of the vertical elastic on the plain A.W. and the MEAW without tip back, the great stress was observed on the lateral incisor area, but on the MEAW with tip back, the moderate stress was observed on the anterior area and molar area. 3. In case of the C II elastic on plain A.W., the stress was concentrated on the anterior area hanged by elastic but on the MEAW without tip back, the stress was transmitted equally from the anterior area to the posterior teeth area, On the MEAW with tip back, the great stress was observed on the anterior and molar area. 4. In case of the C III elastic on the plain A.W., the stress was concentrated on the posterior area hanged by elastic but on the MEAW without tip back, the stress was transmitted equally from the posterior area to the anterior area. On the MEAW with tip back, the great stress was observed on the posterior area and the moderate stress was observed on the anterior area.

10.Efficacy and Safety of Balloon Kyphoplasty in the Treatment of Osteoporotic Vertebral Body Compression Fractures: Compared with Vertebroplasty.

OBJECTIVE: Kyphoplasty and vertebroplasty are two minimally invasive procedures for osteoporotic vertebral compression fractures. The purpose of this retrospective study was to compare the radiological findings and clinical outcomes between two procedures. METHODS: Osteoporotic vertebral fractures were treated in 76 vertebrae, using kyphoplasty (n=35 vertebrae) and using vertebroplasty (n=41 vertebrae). Fractured vertebral bodies were diagnosed by correlating the clinical symptoms with radiologic study. The responses of pain symptoms were measured by a self-reported Visual Analog Scale (VAS) score. Plain X-rays were checked preoperatively and postoperatively at admission and 6 months. The vertebral body height and kyphotic angle were measured to assess the reduction of the sagittal alignment. RESULTS: The mean pain scores were decreased significantly for both procedures postoperatively, but there were no significant differences between two groups. Kyphoplasty led to a significant reduction of the vertebral body height and improvement of kyphotic angle. There were no neurological deficits after kyphoplasty, but one patient experienced paraparesis after vertebroplasty. During the 6 months follow-up both procedures provided stabilization of the sagittal alignment. CONCLUSION: Kyphoplasty and vertebroplasty are considered effective minimally invasive techniques for the stabilization of osteoporotic vertebral body fractures, leading to a statistically significant reduction in pain. Kyphoplasty significantly restore sagittal alignment. Also, complications and the incidence of bone cement leakage are significantly lesser than vertebroplasty. Therefore, kyphoplasty seems to be reasonable procedure for osteoporotic vertebral body compression fractures when medical treatment fail.
