Institute for International Medical Education(IIME) develops the concept of global minimum essential requirements,in which the knowledge and skills of"group health"are ranked as the important parts of training the qualified medical students. But in China, the medi-cal colleges almost run the schools according to the traditional teaching system. Whether teach-ing the knowledge of group health is consistent with the current personnel training requirements deserves our attention. Based on the investigation of teaching status about the medical students "group health"knowledge, the author finds the problems in the field of education with regard to the knowledge and skills and puts forward specific measures to improve it.

Objective To demonstrate the manifestations on positron emission tomographycomputed tomography (PET-CT) at different stages in anti-N-methyl-D-aspartate receptor (NMDAR) encephalitis. Methods PET-CT was performed in 10 patients with anti-NMDAR encephalitis at different clinical stages, and the images were analyzed to investigate the relation of metabolic patterns in the images with clinical presentations. Results Except for normal PET-CT images in 2 patients, images in 8 patients at early stage of the disease showed generally increased 2-[18F] fluoro-2-Deoxy-D-glucose(18F-FDG) uptake at frontal and temporal lobe, basal ganglion and cerebellum, indicating hyper-metabolism in these areas, while 2 of them also had mixture of hyper-and hypo-metabolism in parietal-occipital region. In longitudinal analysis of PET-CT images in these 8 cases, starting at basal ganglion,18 F-FDG uptake gradually decreased bilaterally, prominently at left dominant hemisphere and right cerebellum. Conclusions During the course of anti-NMDAR encephalitis,18F-FDG metabolism markedly increases at early stage and then gradually declines at late stage,at frontal, temporal, parietal and occipital lobes, basal ganglion and cerebellum, predominantly at left dominant hemisphere and right cerebellum. However,in relapsing anti-NMDAR encephalitis,18 F-FDG metabolism in brain does not show these characteristics.

Objective To compare the advantages and disadvantages of uniportal video-assisted thoracic surgery(uniportal-VATS)and three portal VATS in treatment of benign pulmonary diseases. Methods The clinical data of 66 patients with benign pulmonary disease treated by VAST from June 2015 to October 2016 were retrospec-tively analyzed. The patients were divided into two groups according to the specific operation. There were 32 patients (18 males and 14 females)in uniportal-VATS group. There were 34 patients(18 males and 16 females)in three portal VATS group. The operative time ,intraoperative blood loss ,thoracic drainage volume at 24 h after opera-tion,incision length, and the time of postoperative drainage of thoracic cavity, postoperative third day pain score and complication rate were compared between the two groups. Results The patients in the experimental and the control groups were successfully operated according to the scheduled protocol. No thoracotomy was performed. There was no statistical difference in the volume of blood loss, the volume of pleural drainage after 24 hours, the time of postoperative drainage of thoracic cavity ,the length of hospital stays and postoperative complications in uni-portal-VATS group and three portal VATS group(P>0.05). The pain score and postoperative third day pain score of the uniportal-VATS group was better than that of the three portal VATS group(P<0.05),but the operation time of the uniportal-VATS group was longer than that of the three portal VATS group(P<0.05). Conclusion Uniportal-VATS is safe and feasible for the treatment of benign lung diseases. It is more minimally invasive and beautiful than traditional three-hole thoracoscopic surgery. It is worthy of promotion and has broad prospects.

Objective To develop a model that could copy the pathological development of psoriasis, the triple-transgenic mice that harboring Plasminogen activator, urokinase ( PLAU) ,PLAU receptor ( PLAUR) and signal transducer and activator of transcription 3 ( STAT3 ) were generated. They are the important genes involved in the pathological development of psoriasis. Methods The transgenic plasmid was constructed by insertion of the PLAU, PLAUR and STAT3 into the downstream bovine keratin 5 promoter respectively. The transgenic mouse was produced by microinjection and the genotyping was detected by PCR.The expression level of the transgenic gene was determined by Western blotting. The pathological changes were observed by HE staining. Results One mouse line was selected with over expression of the PLAU, PLAUR and STAT3 in the tissue of skin. The transgenic mice showed decreased dermal layer, a hyperkeratinized cuticular layer and increased stratum spinosum. The number of hair follicle was reduced and developed abnormally in the transgenic mice. The Munro abscess in the dermal layer and the increased inflammatory cell infiltrates in dermal layer were also observed in the transgenic mice. Conclusions A transgenic mouse line was produced and passage stably, which expressed the PLAU, PLAUR and STAT3 in the tissue of skin and developed the psoriasis progressively. All of our results suggested that the transgenic mice were a useful animal model for psoriasis.

Objective To investigate the most sensitive methods for diagnosing spontaneous internal carotid artery dissection (sICAD) and spontaneous vertebral artery dissection (sVAD) respectively, for the sake of earlier and more accurate diagnosis. Methods Consecutive patients with sICAD and sVAD who visited the Department of Neurology and Radiology,Huashan Hospital Affiliated to Fudan University during 2008-2013 were retrospectively reviewed and the sensitivity of CT angiography (CTA),magnetic resonance T1-weighted fat-suppressed images (MR T1-FS) and digital subtraction angiography (DSA) for the diagnosis of sICAD and sVAD was compared. Results Eighty patients (62 male,18 female; mean age (45.7 ± 11.9) years) were included in the study. There were 99 arterial dissections in total,45 cases of sICAD,52 cases of sVAD and 2 cases of spontaneous middle cerebral artery dissections. The sensitivity of CTA,DSA and MR T1-FS for diagnosing sICAD was 97.5% (39/40),90.0% (36/40) and 69.6% (16/23) respectively, while for sVAD was 89.8% (44/49),84.6% (44/52) and 100.0% (27/27) respectively. Conclusions sICAD and sVAD have significant differences in many aspects including diagnostic strategies.CTA and MR T1-FS seem to be the most sensitive methods for the diagnosis of sICAD and sVAD respectively.Although DSA has been considered as the gold standard for the diagnosis of artery dissection, this imaging technique does not allow analysis of artery wall thickness, thus also has limitations. It is likely that the diagnostic sensitivity will be improved by combining CTA and MR T1-FS.

AIM: The study explored the significance of the imbalance of Th1/Th2 function after acute myocardial infarction (AMI). METHODS: Peripheral blood mononuclear cells from 33 AMI patients, 22 unstable angina (UA) patients and splenocytes from 35 AMI rats were collected. Cytokine-producing Th cells were monitored by 3-color flow cytometry after stimulated with phorbol myristate acetate (PMA) and ionomycin. IFN-? and IL-4 mRNA in the rat myocardium and chemokine receptors CCR3, CCR5 and CXCR3 mRNA on the surface of rat T lymphocytes after AMI were measured by RT-PCR. RESULTS: Th1 associated cytokine IFN-? significantly increased in patients with AMI and UA within 24 hours after the onset of symptom. the high ratio of IFN-?-producing T cells lasted short in patients with UA and recovered 1 week after the onset. In AMI patients, the high ratio of IFN-?-producing T cells could be examined 1 week and even 1 month after the onset. There was no significant difference on the frequencies of IL-4-producing peripheral T cells between each group. 1 week, 2 weeks and 1 month after AMI, IFN-? mRNA increased in the myocardium of rats, but there was no significant change on cytokine-producing Th cells and chemokine receptors on the surface of rat T lymphocytes. CONCLUSIONS: The Th1/Th2 functional imbalance and up-regulation of Th1 cell-functions exist after AMI and perhaps participate in the onset of AMI. Th1/Th2 functional imbalance may participate in the immune-mediated myocardial injury and ventricular remodeling after AMI as one of the pathogenesises of autoimmune disease.
