1. Aiming at tumors and their microenvironments: Targeted strategies of mesenchymal stem cell-mediated genetic therapy

The early detection of some cancer types combined with the advent of specific drugs has increased median survival of numerous cancer patients. However, many drugs with poor pharmacokinetics rapidly lose activity when delivered systemically and may even not reach the tumors. Based on tropism for tumors, genetic-modified mesenchymal stem cells provide a promising cellular vehicle for drug deliveries. During rapid progression of cancer disease, an activated stromal microenvironment is produced to facilitate tumor development. As a result, it appears a potential chance to modify the cancer therapy in a way, aiming at both the tumor cells and their microenvironments. Copyright © 2013 by TUMOR.
2. In vivo pharmacokinetics of strychnos alkaloids in rats after ig administration

Objective: To evaluate the in vivo pharmacokinetics of strychnos alkaloids [brucine, total alkaloids in Strychni Semen (TASS), and optimal total alkaloids in Strychni Semen (OTASS)] by ig administration to rats. Methods: The rats were ig administered with strychnos alkaloids and their blood samples were taken. Huperzine A was used as an internal standard. The brucine in plasma of rats was detected by HPLC method. The compartment model was fitted and the pharmacokinetic parameters were calculated by DAS1.0 program. Results: The pharmacokinetic analysis showed that brucine behaved as a two-compartment model in the three solutions after ig administration in rats. Compared with brucine monomer administration, the solution of TASS and OTASS could obviously increase the absorption of brucine in vivo, but other pharmacokinetic parameters had no significant difference. Conclusion: After ig administration with strychnos alkaloids, the other alkaloids in Strychni Semen could promote the absorption of brucine.

4.Topical anesthetic effects of compound lidocaine cream coated endotracheal tube to prevent cough and agitation during extubation in thyroidectomy under general anesthesia

Objective To evaluate the topical anesthetic effects of compound lidocaine cream ( CLC) coated endo-tracheal tube on the prevention of cough/agitation during extubation in thyroidectomy under general anesthesia . Methods 42 patients scheduled for thyroidectomy in Peking Union Medical College Hospital were continuously en -rolled in this prospective double-blinded randomized controlled trial .Patients were randomized into two groups . Group L were intubated with endotracheal tube coated with CLC , while the control group , group C with liquid par-affin .The end point of study was spontaneous or induced cough /agitation during extubation .Hemodynamic parame-ters and the consumption of opioids were recorded .Patients were followed for major complications during their stay in hospital .Results Spontaneous cough/agitation rate was lower in group L than group C ( 15% vs 65%, P<0.01 );so was induced cough/agitation (35%vs 90%, P<0.001 );the value of SBP ×HR was lower in group L at 1 min after extubation ( P<0.05 ) .Conclusions The topical anesthetic effects of CLC coated endotracheal tube could prevent cough and agitation during extubation in thyroidectomy under general anesthesia .
5.Preliminarily study of a research evaluation system in a level-two general hospital

Object To establish a proper research evaluation system for level-two general hospital.Method By using Delphi method,experts were invited by letter to review research evaluation indices for three times.Result Totally 62 indices were weighted and evaluated,including two primary-degree indices,14 secondary-degree indices,and 46 third-degree indices. A research evaluation system was preliminarily established in a level-two general hospital.Conclusion The evaluation system is simple and practicable,and can reflect the main aspects of research in the leveltwo general hospitals.
6.Serum Bile Acid Profile in Patients with Inflammatory Bowel Disease

Background: The incidence of inflammatory bowel disease (IBD) is increasing in recent years, and the etiology and pathogenesis of IBD remain unclear.Studies showed that disorder of bile acid metabolism plays an important role in the pathological process of experimental colitis.However, serum bile acid profile in IBD patients has not been reported.Aims: To investigate the changes of serum bile acid profile in patients with IBD.Methods: Seven healthy controls, 15 patients with ulcerative colitis (UC) and 16 patients with Crohn's disease (CD) at Shanghai Xin Hua Hospital were enrolled.High-performance liquid chromatography-mass spectrometry was used to determine serum bile acid profile.Results: No significant differences in serum concentrations of primary bile acid cholic acid (CA), chenodeoxycholic acid (CDCA), glycocholic acid (GCA), taurocholic acid (TCA), glycochenodeoxycholic acid (GCDCA) were found between UC or CD and controls (P>0.05).Compared with controls, serum concentration of secondary bile acid deoxycholic acid (DCA) in UC patients was significantly decreased (P<0.05), glycodeoxycholic acid (GDCA), taurodeoxycholic acid (TDCA) in CD patients were significantly decreased (P<0.05), serum concentration of lithocholic acid (LCA) in UC and CD patients was significantly decreased (P<0.05).Conclusions: The serum bile acid profile in IBD patients is significantly changed, which suggests that it may be involved in the pathological process of IBD.
7.A Novel Method to Detect a New Biopolymer Material—Poly-?-Glutamic Acid

A turbid solution was formed through the reaction between ?-PGA and cetylpyridinium chloride(CPC) and the turibidity was measured by using ultraviolet-visible spectroscopy at 680nm.The linearity between the concentration of ?-PGA and its absorbance,the stability,repeatability and recovery of the method were studied.Through the reaction of ?-PGA with CPC,a homogeneous turbid solution was formed.The turbid solution was stable in 3h under proper pH value and ion strength,the absorbance of the solution at 680nm had a good linear relationshi Pwith the concentration of ?-PGA in the range 12.5~50?g/ml(R2=0.9939).The recovery was within the range of 86%~99.75%(n=5).The relative standard division of the method for determining ?-PGA at the concentrations of 5,10 and 40?g/ml were 0.14%,0.23%和0.025% repeatability.The turbidmetric method has advantages of convenience,simplicity and good repeatability and can be used to control the quality of ?-PGA and its products.

9.Studies on flavonoids of Andrographis paniculata.

OBJECTIVETo study the flavonoids of the aerial parts of Andrographis paniculata.
METHODTwelve flavonoids from the 85% EtOH extract were isolated by the silica gel column, Sephadex LH-20 column, ODS column chromatography and HPLC, and their structures were identified by the spectal analyses and chemical evidences.
RESULTThe 12 compounds were obtained and identified as 7-O-methylwogonin (1), 7-O-methyldihydrowogonin (2), 5-hydroxy-7, 8, 2', 5'-tetramethoxy-flavone (3), skullcapflavone-2'-methoxylether (4), 5-hydroxy-7, 8, 2', 3'-tetramethoxyflavone (5), 5, 4'-dihydroxy-7, 8, 2', 3'-tetramethoxyflavone (6), dihydroskullcapflavone (7), 5, 7, 8-trimethoxydihydroflavone (8), 5, 2'-dihydroxy-7, 8-dimethoxyflvone (9), andrographidine C (10), 5, 7, 4'-trihydroxyflavone (11), 5, 7, 3', 4'-tetrahydroxyflavone (12).
CONCLUSIONCompound 6 is a new naturally occurring product isolated from A. paniculata, and compound 8, 11 and 12 are obtained for the first time from the Andrographis species.

10.Study of ultrasonography and contrast-enhanced computer tomography in the diagnosis of thyroid carcinoma and lymph node metastasis

Objective To evaluate the diagnostic value of ultrasonography (USG) and contrast-enhanced computer tomography (CT) for thyroid carcinoma.MethodsThe results of the review of the preoperative CT and those of the original US reports were compared with the histopathologic results in 73 cases of thyroid carcinoma who underwent radical operation and central compartment dissection from November 2009 to November 2011.ResultsThe final diagnostic rate of thyroid carcinoma with USG was 69.9%,CT of that was 80.8%.There were no significant difference between them (P =0.077).The diagnostic rate of metastatic lymph node in the central compartment with USG was 61.6%,positive predictive value was 64.0%,negative predictive value was 60.4%.The diagnostic rate of metastatic lymph node in the central compartment with CT was 67.1%,positive predictive value was 61.7%,negative predictive value was 76.9%.There were not significant difference between them(P =0.848,P =0.152,P =0.489).Conclusion There was no significant difference between USG and CT in The final diagnostic rate of thyroid carcinoma and its lymph node metastasis in the central compartment.
