Objective To investigate the clinical efficacy of total parathyroidectomy and autologous transplantation(PTX+ AT) for treating secondary hyperparathyroidism(SHPT) in the patients with uremic maintenance hemodialysis and postoperative calcium supplementation. Methods The clinical data in 57 patients with SHPT treated with PTX+ AT in the urology department of the First Affiliated Hospital of Nanjing Medical University from March 2015 to August 2015 were retrospectively analyzed. The changes of serum intact parathyroid hormone(iPTH),calcium and clinical symptoms were observed before and after operation. Results Serum iPTH levels at different time points of immediately after operation, postoperative 20 min,2 d,4 d were(288.57 ±194.71),(185.00±99.28),(83.83 ± 58.26),(17.91± 15.60) pg/mL respectively, which were significantly decreased compared with (2395.80 ± 829.04) pg/mL before operation (P< 0.05);blood calcium at postoperative 4,8,24,72 h was (2.34 ± 0.32),(2.16±0.37),(2.16±0.27),(2.25±0.25) mmol/L respectively, which were significantly decreased compared with(2.61±0.25)mmol/L before operation(P<0.05);the symptoms such as bone pain, skin itching and so on were significantly improved. Conclusion PTX+AT can effectively treat uremic SHPT,and postoperative timely calcium supplementation can reduce the occurrence of hypocalcemia.

Objective To study the method and effect of lateral arm free flap in reconstruction of severe atresic eye socket. Methods Forteen cases of severe atresic eye socket, from June,2011 to June,2013,were repaired by lateral arm free flap. The flaps were designed and harvested as drop shape with size about 6 cm × 10 cm and then were removed epidermis except distal 6 cm × 6 cm area which were transferred to orbit for eye socket reconstruction. The remaining fascia and dermis were filled to augment temporal defect.Superficial temporal artery was anastomosed with posterior branch of radial collateral artery in 14 cases and superficial temporal vein was anstomosed with radial collateral vein in 11 cases, with middle temporal vein in 3 cases. Results All 14 cases lateral arm free flaps survived with no donor site morbidity.Followed up for 1 year to 3 years, artificial eye could be fitted satisfactorily and temporal contour improved. Conclusion Lateral arm free flap is a recommendable option for severe atresic eye socket reconstruction because of concealed donor site scar, proper volume, matched vascular caliber and minor donor site morbidity.

Objective: To evaluate the effect and safety of continuous veno-venous hemofiltration(CVVH) in the treatment of multiple organ failure(MOF) in senile patients. Methods: Sixteen patients with multiple organ failure, aged over 80 years, were divided into a survival group, who lived more than 20 days, and a non-survival group, less than 20 days after CVVH,and observed for such clinical indexes as of renal function,K+and blood gas analysis,APACHEⅡ scores and complications. Results: After CVVH,eleven of the patients survived for over 20 days, with 1 case up to 3 years.CVVH effected a significant improvement in BUN,Scr,K+and blood gas as well as a marked reduction in complications.APACHE Ⅱ scores decreased significantly after CVVH in the survival group though not in the non-survival group,as compared with those before CVVH,which were significantly lower in the former than in the latter. Conclusion: CVVH is a safe, effective and well-tolerated method for the treatment of MOF in senile patients.Patients with higher APACHEⅡscores have a poor prognosis.

Objective To sum up the experience for the treatment of splenic trauma. Methods Data of 129 cases of traumatic splenic rupture from 1984 to 2004 were retrospectively analyzed. Results Conservative management was undertaken in 17 cases, splenorrhaphy in 16 cases, total splenectomy in 86 cases, partial splenectomy in 8 cases, and splenic autotransplantation in 23 cases. The results were all satisfactory. Conclusion In splenic trauma, treatment modality should be on adopted case to case basis according to the types of splenic rupture. Combined splenic salvage can be used on certain conditions.

Objective: To observe the expression of urokinase-type plasminogen activator receptor (uPAR) in human tongue squamous cell carcinoma Tca 8113 cells at different dosages of heat shock. Methods:The expression of uPAR protein in Tca 8113 cells was examined using immunohistochemical technique (IH) and flow cytometry (FCM) after the cells had been treated at 37,40,43 or 45 ℃ for 40 min. Results:The fluorescent intensity of uPAR protein of Tca 8113 cells treated at 37,40,43 and 45 ℃ was 11.36?0.06,9.98?0.12, 10.37?0.05 and 11.31?0.10 respectively. Conclusion:Heat shock can reduce the expression of the uPAR protein of Tca 8113 cells.

Objective To explore the TCM syndrome characteristics of cardiac syndrome X(CSX).Methods The signs and symptoms of 51 patients with CSX were analyzed according to the diagnosis of TCM syndromes to summarize their syndrome character- istics. Results of the 51 CSX cases, the following signs and symptoms took dominance:chest pain, fullness in chest, epigastric and abdominal distention, emotional distress, dark purple tongue with petechia, greasy coating,string-taut pulse. The syndromes were mainly of Biao-Superficial excess, including qi stagnation, phlegm retention and blood stasis, occupying 66. 7%,accompanied with Benroot deficiency, including qi deficiency, yin deficiency,qi and yin both deficiency, occupying 33.3%.Conclusion Qi stagnation, phlegm retention and blood stasis are the primary syndromes of CSX.

ObjectiveTo explore the optimal method of protecting bone marrow in postoperative concurrent chemoradiotherapy of stage Ⅱ - Ⅲ rectal cancer by comparing two techniques of intensitymodulated radiotherapy (IMRT). MethodsFifteen patients with stage Ⅱ - Ⅲ rectal cancer after surgery had CT simulation. Clinical target volume, small bowel, bladder and bone marrow were contoured. Two IMRT treatment plannings with and without bone marrow-sparing (BMS-IMRT and IMRT) were separately designed. The dose distribution was compared based on that 95% of the planning target volume received the prescribed dose. ResultsBMS-IMRT had an advantage over IMRT in terms of conformity indices ( 1. 06∶1. 04, t =- 2. 61, P =0. 023 ), but inferior to I M RT for homogeneity indices ( 0. 81 : 0. 75, t =- 2. 34, P =0.037)).Compared with IMRT, BMS-IMRT reduced the V5, V10, V20, V30, V40 of bone marrow (97.09%∶98.72%, t=-2.34, P=0.037;92.38%∶96.46%, t=-2.41, P=0.033;83.36%∶91.70%, t=-3. 18, P=0.008;51.47%∶69.65%, t=-4.92, P=0.000;36.34%∶49.57%, t=-2.66, P =0. 021 ). The doses received by small bowel and bladder were similar between BMS-IMRT and IMRT, except that the V20 of bladder was lower in BMS-IMRT (77. 32%∶92. 39%, t =-3.52, P=0. 004). Conclusions BMS-IMRT reduces low dose volume of bone marrow without increasing dose to other risk organs.BMS-IMRT might reduce acute hematologic toxicity and increase the feasibility of postoperative concurrent chemoradiotherapy in stage Ⅱ -Ⅲ rectal cancer.
