Objective To observe the influence of insulin resistance(IR) and isosorbide mononitrate(ISMN) on myocardial cellular apoptosis in spontaneously hypertensive rats (SHR) .Methods Forty male 14‐week old Wistar(W) rats and SHR(S) each were respectively or jointly fed with normal diet (ND) ,high fat and high glucose(HFHG) diet ,normal saline(NS)and ISMN by ga‐vage .Then they were randomly divided into the normal and NS group (normal W and normal S ) ,HFHG and NS group(HFHG W and HFHG S) ,normal and ISMN group(ISMN W and ISMN S) ,HFHG and ISMN group(HI W and HI S) ,with 10 rats in each group .After 12‐week feeding ,carotid arterial blood was collected for detecting blood glucose concentration and insulin level and cal‐culating insulin resistance index (HOMA‐IR);4 myocardial tissue samples were taken for respectively observing the morphology under microscope ,and detecting the NO level ,myocardial Bcl‐2 ,Bax gene and their protein expression levels in myocardial tissue . Results Myocardial NO level ,Bax gene mRNA and related protein levels in the HFHG and ISMN intervention groups were higher than those in the normal group ,while the bcl‐2 gene mRNA and related protein expression were on the contrary ;myocardial tissue NO level ,Bax gene mRNA and related protein expression in the S groups were increased compared with the corresponding W groups ,while the bcl‐2 gene mRNA and related protein expression were on the contrary ;in the HFHG W group ,the myocardial tis‐sue NO level had significantly positive correlation with HOMA‐IR ,and in the ISMN W group ,HOMA‐IR was positively correlated with the NO level in the myocardial tissue .Conclusion Myocardial cellular apoptosis of SHR is increased compared with Wistar rats ;both IR and ISMN can aggravate the apoptosis of SHR myocardial cells ,moreover IR has a mutual induction and reciprocal causation with ISMN .

Objective To study the clinical characteristics and method of diagnosis and treatment of(nonfunctioning) parathyroid cysts(NFPTC).Methods The clinical data of 15 cases of nonfunctioning(parathyroid) cysts admitted in the recent 16 years were retrospectively analyzed.Results None of the cases were diagnosed before operation,and the diagnosis of all of the cases was verified by pathology.All of the(cases) were cured by operation.Follow-up found no case with recurrence.Conclusions Pathologic(examination) is the most reliable diagnostic method for NFPTC.Needle aspiration of fluid to test for PTH or(cytology) is an important method in diagnosis before operation.Surgical resection is the most ideal method of treatment.

Objective To evaluate endoscopic ultrasonography (EUS) in preoperative staging of gastric carcinoma,and to study the molecular biologic basis of its ultrasonic manifestation.Methods Sev-enty-four patients with gastric carcinoma were referred to EUS and staged preoperatively.The staging esults of EUS were compared with the histopathological staging and the expression of PTEN.Results The verall accuracy rate of EUS for determination of T staging was 82.4%(61/74),for T1,T2,T3 and T4 were 100.0%,72.7％,85.O％ and 78.9％,respectively.For N staging,EUS had the overall accuracy rate of 74.3%(55/74),with sensitivity and specificity of 80.8％ and 59.1％,respectively.The preoperative EUS for T and N staging of gastric carcinoma was of inverse correlation with the expression of PTEN.Conclusions EUS is an accurate staging modality in determining carcinoma invasive depth and lymph node involved,with a few exceptions of overstaging and understaging.However,EUS criteria to differentiate benign from malignant nodes still need to be further defined by future studies.The preoperative EUS for T and N staging could well display the molecular pathologic basis of gastric carcinoma.

Objective To investigate the effects of human bone marrow fibroblastoid stromal cell lines HFCL on the proliferation .migrating and homing of multiple myeloma cell lines RPM18226. Methods The co-culture system of RPMI8226 and HFCL was established through cell culture in vitro, growth curves were determined by trypan blue exclusion, cell cycle and expression of adhension molecule CD49d were detected by flow cytometry, and expression of CXCR4 gene was examined by RT-PCR. Results The proliferation of RPMI8226 cells in direct contact with HFCL cells group was inhibited strongerly than that in transwell group. The percentage of G1 phase cells of RPMI8226 cells in direct contact with HFCL group was higher than that of RPM 18226 in transwell group, while the percentage of S phase cells was lower. The expressions of CD49d and CXCR4 in RPMI8226 cells were down-regulated by HFCL cells, and that in transwell group was higher than that in direct contact with HFCL cells group. Conclusion Human bone marrow fibroblastoidss tromal cell HFCL can inhibit the proliferation and the expressions of CD49d and CXCR4 of multiple myeloma cell line RPMI8226, and can prevent multiple myeloma cells from migrating and homing.

Objective To develop a set of indicators for measuring the performance of China's Healthcare Improvement Initiative.Methods Such methods as literature review,expert consultation and onsite preliminary experiment were used,to study and demonstrate the indicators,the evaluation framework,the weight,evaluation indicators,and evaluation standards.Results The final evaluation indicators for medical institutions included nine class-1 indicators,29 class-2 indicators,and 56 class-3 indicators;those for healthcare administrations included six class-1 indicators,nine class-2 indicators,and 13 class-3 indicators.Conclusions The system takes into full account the special nature of healthcare,and mission of the Initiative,as the well as the quality,safety,efficiency and equity of healthcare,to make the system scientific and operational.

Objective To investigate the clinical efficacy of neoadjuvant chemotherapy regimens of TA or AC in the treatment of locally advanced triple negative breast cancer. Methods Data of 99 women with stageⅡ/ Ⅲ locally advanced triple negative breast cancer treated in the Centre Hospital of Cangzhou from Jan. 2006 to Dec. 2011 were retrospectively analyzed. These patients were divided into two groups based on the regimen of the neoadjuvant chemotherapy. Fifty-two cases were received TA regimen(Docetaxel 75 mg/ m2 and pirarubicin 50 mg/ m2 )and 47 cases were received AC( pirarubicin 50 mg/ m2 ,cyclophosphamide 600 mg/ m2 ). IV drip infusion was administered in both groups for 4 cycles before surgery,with 3 weeks for each cycle. The efficacy after treatment,the 2 year recurrence rate and overall survival rate after operation were compared between the two groups. Results The response rates in TA group were 88. 46% ,higher than that of AC group(57. 45% ),and the difference was statistically significant(χ2 = 12. 260,P < 0. 001). Furthermore,the rate of pathological grade 4 and 5 in TA group were 42. 3%(22 / 52)was superior to AC group(23. 4%(11 / 47);P = 0. 046). The 2-year recurrence rate and survival rate in TA group were 23. 08%(12 / 52)and 84. 62% ,as same as that in AC group ((27. 66%(13 / 47)and 80. 85% ;χ2 = 0. 400;P = 0. 53). Conclusion TA and AC are both effective in terms of females with stage Ⅱ/ Ⅲ locally advanced triple negative breast cancer treat with neoadjuvant chemotherapy. Moreover,TA is superior to AC. However,there is no statistical difference of 2-years recurrence rate and survival rate between two groups.

ObjectiveTo determine whether prophylactic cranial irradiation (PCI) has a role in the management of patients with non-small cell lung carcinoma (NSCLC)treated with radical intent.MethodsWe searched The Cochrane Library,MEDLINE,EMbase,CBM,CNKI and VIP.The quality of the includedstudieswascriticallyevaluated.Dataanalyseswereperformed usingtheCochrane Collaboration's RevMan 5.1 software.ResultsFour randomized controlled trials involving 905 patients met the inclusion criteria.The results meta-analyses showed the incidence of brain metastases was lower in PCI group compared with the observation group ( x2 =1.98,P =0.000 ) ; but there is no evidence of 1-year overall survival (OS) benefit ( x2 =1.12,P =0.880).Only RTOG 2009 provides prospective data:There were no significant differences in global cognitive function (P =0.600) or ADL ( P =0.880) after PCI,but there was a significant decline in immediate recall (P=0.030) and delayed recall (P =0.008 ) at 1 year,At 1 year,there was no significant differences in QOL after PCI ( P =0.050).Conclusions This systematic review show significantly decreases the risk of BM without improving 1-year OS in NSCLC patient receiving prophylactic cranial irradiation.There is insufficient evidence to support the use of PCI in clinical practice.Where possible,patients should be offered entry into a clinical trial.
