1.Diagnosis and treatment of baseball elbow

Objective To evaluate the methods of early diagnosis and the surgical outcomes of se vere baseball elbow.Methods Six surgically treated cases of base ball elbow were included in this stud y.Two patients were treated with arthrosc opy to remove free loose bodies in elb ow joints and synovectomy.Another t wo patients had the big free loose bodie s removed with both arthroscopy and s urgical interventions through small incisions.The remaining 2underwen t d?bridement,cartilage repair and subchondral bone graft.Careful physical examinations as well as X -r ay,CT,3D -CT and MR were performed an d compared before and after operations.Results All the patients suffered from elbow pain and limitations of elbow extension.Limitations of elbow flexion and forearm supination could be seen in some severe cases.MRI was helpful for detecting early lesions.Compared with plain radiograph,it was more sensitive to conduct tomography or roentgenography at 45degrees flexion of elbow.3D -CT coul d illustrate the conditions of cartilage and free loose bodies vividly.T he patients were followed up with an average range of 8.2months postoperatively.The results were evaluated in 4aspects,which included elbow pain,range of motion,joint snap and image features.Three patients had ex-cellent results,two had good and one fair.Five patients got an average in crease of elbow extension for 10degrees.Two of them were competent f or their original job after operatio ns.One patient had gone back to the baseball fields after one-year rehabilitation.Conclusion Careful physical examination and im aging evalu-ations as MRI,tomography or roentge nography at 45degrees flexion of elb ow were essential for early diagnosis.For the patients suffering from base ball elbow,the best results could be obtained with early diagnosis and proper treatments.The surgical out comes tend to be better in the younger patients with the shorter duration o f the disease and less invasive treatm ents.[
2.Repeated Bronchial Artery Embolization in the Treatment of Recurrent Hemoptysis

This paper reported that bronchial artery embolization(BAE)had been used to treat 35 non tumorous patients who suffered from recurrent hemoptysis or acute severe hemoptysis after the given medical treatment was proved uneffective in 35 patients,7 patients had recurrent hemoptysis during 1.5~19 months postoperatively and were treated by BAE again,and there was no recurrence of hemoptysis during 7~34 months follow-up.Therefore, the authors considered that repeated BAE was still effective in treating recurrent hemoptysis.
3.Relationship of Janus Kinase-Signal Transducers and Activator of Transcription Signal Transduction and Neurogenesis in Neonatal Rats with Periventricular Leukomalacia

Objective To observe the relationship of the Janus kinase-signal transducers and activator of transcription pathway and neurogenesis in the lateral ventricle subventricular zone(SVZ) and choroid plexus of neonatal rats with periventricular leukomalacia(PVL).Methods PVL models were established by right common carotid artery ligation followed by 4 h 60 mL/L oxygen exposure in 2-day-rat;the neonatal rats performed a sham operation,without hypoxia-ischemia were used as control group.The rats were sacrificed at 0 h,3 h,6 h,12 h,1 d,3 d and 7 d after hypoxic-ischemic(HI),and brain tissues were collected,immunohistochemistry was used to detect the expression of P-STAT3 and Nestin in the subventricular zone and choroid plexus.Results Very low expressions of P-STAT3 and Nestin were observed in lateral ventricle SVZ and choroid plexus in control group.The expression levels of P-STAT3 and Nestin increased significantly after HI,peaked at 1 d and 3 d separately,and remained at a higher level at 7 d after HI,demonstrating significant differences at each time point compared with those in control group(Pa
4.Isolated resection of hemangioma in the caudate lobe

Isolated resection of bemangioma in the cau-date lobe is challenging due to the surgical anatomy of caudate lobe. The caudate lobe consists three portions: Spiegel's lobe, paracaval portion and caudate process. Most of the blood supply of caudate lobe is provided by the posterior segmental branches of the portal vein and left hepatic artery. The hepatic venous drainage encompasses a few sizable and several small branches that join the inferior vena cava. Selection of the ideal route for bepatectomy, adequate mobilization of the liver, preparatory placement of band for hepatic vascular occlusion are key factors during the operation.
5.Measurement of specific IgG avidity for verification of recent primary cytomegalovirus infection

Objective To distinguish primary CMV infection from reactivation or reinfection. Methods A urea denaturation test was included in the washstep of the standard IgG ELISA in 12 patients of age<12 whose serum CMV-IgG were positive, avidity indexes of CMV-IgG were measured. Results Among children aged 1~12 years, the varieties of avidity indexes of CMV-IgG in A group(IgM negative and IgG positive) and B group(IgM positive and IgG positive) had the same trend. There was only one case in B group whose avidity index was under 30%, the other were all above 30%, and most were above 50%. But among 40 children aged 1~12 months,the avidity indexes were under 30% in 21 cases,8 cases were 30~50% ,and 11 cases were above 50%. The antibody avidity in 1~6 months children was influenced by their mothers lgG,80% (12/15) of 7 ~12 months children had low-avidity antibodies. Conclusion CMV infection mostly occurred among 1~12 months children. Urea denaturation test was an efficient method to distinguish primary CMV infection from reactivation or reinfection.
6.Treatment of metacarpophalangeal intra-articular fractures assisted with metacarpophalangeal arthroscopy

Objective To explore the clinical results of metacarpophalangeal(MP) intra-articular fractures treatment assisted with MP arthroscopy. Methods Five patients suffered from MP joint fractures were treated with closed reduction and K-wire fixation under the MP arthroscopy. The age of the patients was from 17 to 53 years with an average of 23.5 years. There were four males and one female. All the fractures were caused by direct trauma. The head of metacarpal bone was injured in one case while the bases of proximal phalange were involved in four cases. 2 were of simple fractures and 3 of comminuted fractures. No joint surface defects were found preoperatively. The duration from injury to surgery was from 5 days to 3 weeks. The treatment results were evaluated with respect to MP arthroscopical findings, the fracture union and the postoperative function. Results The fracture lines could be seen in 4 cases under arthroscopy except one located at the palmar aspect of metacarpal head, which was then treated with open reduction and internal fixation. During the examination with MP arthroscopy, one case each of volar plate injury and collateral ligament injury was found. The patients were followed up 3-6 months with an average of 4.8 months postoperatively. All the patients obtained fracture union with a smooth joint surface. The motion of involved MP joints achieved nearly to their normal active range in 3 cases. No pain or snapping was found during the movement of MP joints. There was also no lateral instability. Only in one case, because of the massive and comminuted fracture, the involved finger was immobilized with plaster for five weeks, the ROM of MP joint became 90? for flexion and -56? for extension at 5 months postoperatively. Conclusion It is a less invasive procedure with good results to treat MP joint fractures assisted with MP arthroscopy. It is suitable for some acute MP intra-articular fractures.
7.Effects of cigarrete smoke extract and lipopolysaccharid e on the expression of transforming growth factor-?_ 1 mRNA and protein in human embryonic lung fibroblasts

AIM: To examine the effects of cigarette smo ke extract (CSE) and lipo polysaccharide (LPS) on the production of transforming growth factor-? 1 (TGF- ? 1 ) mRNA and protein in cultured human embryonic lung fibroblasts (HELF). METHODS: The cultured HELF were incubated with CSE, LPS or CSE in com bination with LP S for 24 hours at 37℃, respectively. The total RNA was extracted from the cells . The expression levels of TGF-? 1 mRNA were evaluated by reverse transcription - p olymerase chain reaction (RT-PCR). The mRNA levels of TGF-? 1 were corrected by GAPDH transcripts, and TGF-? 1 protein levels were determined by immunocytoch emistry. RESULTS: CSE stimulated the TGF-? 1 mRNA expression i n HELF at lower concentr ati ons (1∶50 and 1∶25)( P 0.05). 9.Effect of vacuum sealing drainage on bacteria and microcirculation of acute explosion wounds

[Objective]To observe the effect of vacuum sealing drainage(VSD) on bacteria and microcirculation of acute explosion wounds in pigs.[Method]Twelve blast wounds,created by explosion of a specific type of electric detonations which were fixed at 1 cm over the skin of the hips of six Lanzhou healthy pigs(30-40 kg),were divided into two matched groups.Debridement was performed at 6 hours after injury.Group A was treated with conventional dressing change,and group B was treated with VSD under the pressure of-20kPa.Tissue specimens collected from different sites of the wound bed before and at 12,24,48,72 hours after treatment,and experienced bacteria counting.The microcirculation of the blast wounds was examined by color B-mode ultrasonic diagnostic equipment before and 72hours after treatment.[Result]The difference of wound bacteria loads between group A and group B was statistically signficant at different time points.Bacteria loads in group B were increased to 105cfu/g at 48 h after treatment,compared with group A at 24 h.No signal of blood flow in blast wound was found before treatment.In contract,the blast wound had plenty of signals of blood flow in group B.[Conclusion]VSD could reduce the bacteria loads,and delay the time of infection.Meanwhile it improved the microcirculation of the acute blast wound.
