Study was conducted cross on the sections from 3cm and 5cm of tumor upper edge of 125 patients with gastric carcinoma. Results: the percentage of existence of tissue cancer at the section that above the tumors 3cm and 5cm was 20.8% and 10.4%, respectively. The percentage of existence of tissue cancer at the cross section 3cm and 5cm was higher than invasive type in comparison with intestinal type (28.3% vs 16.5% and 15.2% vs 7.6%), in undifferentiated carcinoma, adenocarcinoma and Signet-ring cell carcinoma comparison with duct carcinoma (36.8%, 37.5% and 26.3% vs 9.7% at cross section 3cm and 21.1%, 18.8% and 10.5% vs 3.2% at cross section 5cm), in tumors with diameter >2cm and ≤5cm comparison with tumors ≤2cm (27% vs 7.1% at cross section 3cm and 14.3% vs 0% at cross section 5cm), in invasive carcinoma of sero comparison with shorter invasive carcinoma and at last in carcinoma of gland metastasis comparison with tumors without gland metastasis (4.7% vs 11.4% at cross section 3cm and 16.7% vs 4.5% at cross section 5cm).

Since its initial introduction in clinical practice, endoscopic ultrasonography(EUS) has been considered as a valuable tool for the diagnosis and staging of gastrointestinal cancers. With the improvement of equipments in the past decade, EUS-guided fine needle aspiration (EUS-FNA) techniques has been greatly developed, which opens a new avenue to therapeutic EUS. At present, endoscopic ultrasonography (EUS) has been widely applied in the clinical practice of the diagnosis and management of gastrointestinal cancers. In this paper, we summarize the latest data of the applications of EUS in the diagnosis and management of gastrointestinal cancers.

Today, the comprehensive surgical treatment for gastrointestinal cancer is almost in the perfect stage. It is difficult to further improve the curative effect by surgery only. To improve the overall curative effect of gastrointestinal tumor, translational medicine research should be promoted in the fields of the early diagnosis, etiology and pathogenesis and comprehensive treatment. Researches of discovering the new tumor markers for early diagnosis of tumor, etiology and pathogenesis involve many aspects, including environmental factors, genetic susceptibility, variation and accumulation of genetics, and epigenetic changes, which should be transferred to new methods of treatment. This review summarizes the gastrointestinal tumor-associated hot topics in the field of basic research and its progress, and provides the clinical clues from their conversion.

Multiple primary malignant neoplasms (MPMNs) are rarely reported and it is important to give early diagnosis and proper therapy for these patients. Here reported a case of 62-year-old man with concomitant three early stage cancer lesions in upper gastrointestinal tract, all of which were detected by endoscopy. The first one was an IIc-type lesion at angular part of stomach under endoscopy, which was histologically confirmed to be a mucosal well-differentiated adenocarcinoma. The patient underwent a standard radical gastrectomy for the lesion after the failure of endoscopic treatment. The other two neoplasms were observed during follow-up and were indicated as early stage lesions by synthesizing information from endoscopy, endoscopic ultrasonography, computed tomography and biopsy. One displayed as a hyperemic patch (3 cm×4 cm in size) located at the part of esophagus 27 cm away from the incisor teeth and was proved to be moderately differentiated squamous cancer by histopathological examination. The other was an IIc-type lesion (3.0 cm×3.5 cm in size) located at the part of esophagus 36 cm away from the incisor teeth, and the biopsy result showed a poorly differentiated squamous carcinoma. Both the two lesions were treated with radical radiation because the patient refused surgery management. No recurrence of former lesions or occurrence of novel lesions were observed during post-treatment follow-up, suggesting radical radiation might be effective for this patient.

It's difficult to diagnose precancerous lesion and early cancer for a long time, because both of them haven't typical morphological characteristics. As a novel diagnostic modality, fluorescence endoscopy can accurately reflect minimal changes in human's tissue, thus making a meaningful progress for cancer diagnosing. 200 patients were examined by fluorescence endoscopy to evaluate the diagnostic value. The overall accuracy, sensitivity and specificity for detecting malignant gastrointestinal tumor was 94.0%, 94.6% and 93.5%, respectively. Thus, fluorescence endoscopy can be used to diagnose malignant gastrointestinal tumors with high validity and reliability, and is advantageous over conventional white light endoscopy especially in detecting the atypical and suspicious lesions. Furthermore, fluorescence endoscopy can also guide target biopsy, is significant to improve the early cancer detection rate, has a broad development prospect.

Primary adenocarcinoma of the jejunum which accounts for only approximately 3% of all gastrointestinal tract malignancies, is distinctly unusual. The rarity and the non-specific symptoms of this cancer, which are the major factors contributing to its poor prognosis, make the diagnosis difficult. As the prognosis of primary adenocarcinoma of the jejunum, once metastasized, is poor, a greater awareness of the possibility of a jejunal cancer must accompany aggressive diagnostic and surgical procedure. We report our experience of a child with primary adenocarcinoma of the jejunum with a brief review of the literature.

At present, the level of diagnosis and treatment of early gastrointestinal cancer has a larger gap between China and developed countries. In recent years, the advent of new endoscopic imaging techniques has improved the detection rate of early gastrointestinal cancer. Along with this, endoscopic submucosal dissection(ESD) enables radical resection for early cancer. In this article, we discuss how to improve the level of endoscopic diagnosis and treatment of early gastrointestinal cancer.

The application of artificial neural network algorithm in pathological diagnosis of gastrointestinal malignant tumors has become a research hotspot. In the previous studies, the algorithm research mainly focused on the model development based on convolutional neural networks, while only a few studies used the combination of convolutional neural networks and recurrent neural networks. The research contents included classical histopathological diagnosis and molecular typing of malignant tumors, and the prediction of patient prognosis by utilizing artificial neural networks. This article reviews the research progress on artificial neural network algorithm in the pathological diagnosis and prognosis prediction of digestive tract malignant tumors.
