Professor SUN Jian-hua proposes that the disease location of functional gastrointestinal disorders is brain, spleen and intestines; the liver-depression and spleen-deficiency is the basis of pathogenesis; the core pathogenesis is the dysfunction of qi and disability of conducting; the key to pathogenesis is the imbalance of heart, brain and mind. The "regulating-mind and strengthening-spleen" acupuncture therapy could treat functional gastrointestinal disorders. The first essence of treatment is regulating the mind, and the treatment principles are soothing the liver and strengthening the spleen, improving the mind and regulating the intestines. In addition, the moxibustion therapy and auricular points therapy are suggested to use together. Moreover, psychological counseling and health education are important, especially attention should be paid to the treatment of the mind and body, so as to synergize the treatment effect.

Relative literatures on the two-way regulatory eltect ot acupuncture on gastrointestInal muiiiity 01 inc recent 20 years were retrieVed in CNKI. Researches on effects of acupuncture on electrical activity of the gastric muscle, and mechanism of nerve and humor were summarized. And the result shows that the two-way regulatory effect of acupuncture on gastrointestinal motility function which eventually causes the recovery of gastrointestinal function, is a benign regulation reached by relative antagonism and inter-coordination of the neural, endocrine and immunity in all aspects. Meanwhile, the detailed conditions of the experiments are not strictly controlled, the intervention methods are not unanimous, the indices of observation are not unified, and the results are lack of scientificalness, regularity and corporeality. It is proposed that the future research should be carried out in the following 3 aspects, namely whether single points have the two-way regulatory effect or not; the application of single points in point prescriptions and the variety of quantity; and differences on the two-way regulatory effect and working processes between acupuncture and medication.

Small bowel disease is one of the difficulties in the diagnosis and treatment of digestive system diseases, and limited examination techniques seriously restrict the diagnosis and treatment level of digestive tract diseases in children. With the wide clinical application of enteroscopy in pediatrics and the optimization of enteroscopy equipment and accessories, enteroscopy technique provides a new method for the diagnosis and treatment of pediatric digestive tract diseases, but there are still many issues and challenges in the standardization of clinical operation and endoscopic treatment. In order to standardize the diagnosis and treatment techniques for enteroscopy in children and improve the diagnosis and treatment level of small bowel disease, the Subspecialty Group of Gastroenterology, the Society of Pediatrics, Chinese Medical Association organized experts to fully discuss and formulate the expert consensus on the clinical application management of enteroscopy in children, with reference to the latest advances in the application of enteroscopy in children.

From the perspective of autonomic nervous system, this paper analyzes the mechanism, current western medicine treatment methods and acupuncture-moxibustion treatment mechanism of gastroesophageal reflux disease (GERD). It is believed that the main cause of GERD is that the gastric acid goes to the wrong place due to gastrointestinal motility disorder, which belongs to "acid dislocation". At present, western medical treatment cannot effectively target the pathogenesis of the disease, and its effect is limited. Acupuncture-moxibustion could regulate the neuroendocrine immune network to regulate the function of autonomic nerve, restore the power of digestive tract to treat GERD, which is worthy of further research.

Gastrointestinal (GI) cancers cause obstruction of the GI tract including biliary tree. In advanced GI cancer, endoscopic stent insertion is the treatment of choice. However, the current stent allows only mechanical palliation of obstructed GI tract and has no anti-tumor effect. The primary role of a drug eluting stent (DES) in gastrointestinal malignancy is that it decreases the tumor re-growth and sustains the stent patency. This might not be as impressive as the effect of a vascular DES which decreases the incidence of restenosis and thus increases the survival rate of the patient. However, in terms of improving the quality of life of the patient, maintaining the stent patency is one of the most important task of GI doctors. With an advanced technology in stent production, Korea would be able to play an active role in the field of DES especially with more advanced anti-tumor effect. Moreover, advances in this type of drug delivery system will enable the development of local treatment of GI malignancy using endoscopy.

Gastrointestinal tract is an organ for digestion, absorption and utilization of nutrients. Also it functions as an immunological organ in the human body. Patients with gastrointestinal disease are at increased risk for nutritional problem due to dietary restriction during the treatment or diagnostic examinations, anorexia or altered nutritional requirement. Clinically, it is important for gastroenterologists to be aware of the principles of nutritional therapy and the relationship between gastrointestinal diseases and the combined nutritional abnormalities. Removal of enteral feeding causes mucosal atrophy and leads to increased mucosal permeability to bacteria and endotoxin. The intestinal endotoxemia results from the translocation of bacteria and endotoxin to systemic circulation, may triggers off systemic inflammatory response syndrome. Therefore, it is crucial in critically ill patients to maintain the gastrointestinal mucosal integrity along with the intestinal flora that enables the host immunity to be maintained or enhanced. Immunonutrition is a therapeutic approach to enhance the gastrointestinal mucosal barrier with various specific nutrients. The intestinal endotoxemia and immunonutrition will also be reviewed briefly.

This is probably due to the large functional reserve capacity, such as redundancy in the structure of the organ system and excessive production of hormones and enzymes. Most gastroenterologic disorders developed in younger persons may also develop in the elderly. However, the presentation, treatment and prognosis may be different between elderly patients and younger patients. One important thing is the high prevalence of certain disorders in the elderly. For example, the incidence of peptic ulcer, ischemic complications of vascular abnormalities, drug-induced disorders, malignancies, and some other disorders significantly increases with age. These disorders results either from age-determined changes in the gastroenterologic organs or from extragastroenterologic disorders, such as diabetes mellitus, neurologic diseases, and vascular changes. In this paper, an important consideration of common gastroenterologic disorders frequently developed in the Korean elderly is described. In addition, physiologic and pathologic changes of the gastroenterologic organs associated with aging are also discussed.

The fundamental mechanism of gastrointestinal diseases is the imbalance between the attack factor and the protective factor of stomach mucous membrane. It can not outbreak if the protective factor is stronger than the attack factor; otherwise, it will. Attack and protective function inside the stomach are mediated by various factors. These related factors interaction constitutes a complicated network system. This kind of outbreak theories of imbalance in Western medicine is same as that, "The sense of right saves inside, the evil can't do" and "Evil is competing with right, all diseases occur", in Chinese medicine. So, it is the key which points to futher study the interaction between gastrointestinal attack factors and protective factors by traditional Chinese medicine, thus improving the clinical effect.
