2.Clinical observation on acupuncture for treatment of chronic atrophic gastritis.
Chinese Acupuncture & Moxibustion 2009;29(5):361-364
OBJECTIVETo compare the therapeutic effects of acupuncture and western medicine for treatment of chronic atrophic gastritis.
METHODSEighty cases of chronic atrophic gastritis were randomly divided into an observation group and a control group, 40 cases in each group. The observation group was treated with acupuncture and moxibustion on Guanyuan (CV 4), Qihai (CV 6), Zusanli (ST 36), Xuehai (SP 10), Geshu (BL 17); and the control group with oral administration of western medicine, Omeprazole and Amoxicillin, etc. Evaluation of the scores of clinical symptoms, gastroscopic inspection and histopathologic examination of the gastric mucosa were carried out before treatment and after treatment of 4 and 8 weeks in each group to compare the therapeutic effects between the two groups.
RESULTSThe total effective rate of 92.5% in the acupuncture group was better than 75.0% in the con trol group (P < 0.05). For the pathologic changes of gastric mucosa by the gastroscopic inspection, the total effec tive rate of 85.0% in the acupuncture group was superior to 65.0% in the control group (P < 0.05), and for the improvement of histopathologic aspect, the total effective rate of 87.5% in the acupuncture group was better than 65.0% in the control group (P < 0.05).
CONCLUSIONAcupuncture therapy can significantly improve the clinical symptoms, gastroscopic expression and histopathologic situation in the patients of chronic atrophic gastritis, and its therapeutic effect is better than that of the western medicine.

Eosinophilic gastroenteritis (EGE) is a disorder characterized by eosinophilic infiltration of the bowel wall and various gastrointestinal (GI) manifestations. This study aimed to evaluate the characteristics of EGE in infants and children. A total of 22 patients were diagnosed with histologic EGE (hEGE) or possible EGE (pEGE). Serum specific IgE levels, peripheral eosinophil counts, and endoscopic biopsies were carried out. In the hEGE group (n = 13), initial symptoms included hematemesis, abdominal pain, and vomiting. Three of the subjects had normal endoscopic findings. Eight patients were categorized into the infant group and 5 into the child group. All patients in the infant group showed clinical improvement after switching from cow's milk feeding to special formula or breast feeding. The infant group showed a higher eosinophil count in the gastric mucosal biopsy than the child group. In the pEGE group (n = 9) initial symptoms included hematemesis, abdominal pain, and vomiting. Seven patients in this group showed a good response to treatment with restriction of the suspected foods and/or the administration of ketotifen. Both hEGE and pEGE groups showed clinical improvement after restriction of suspected foods in the majority of cases and also showed a similar clinical course. EGE should be considered in the differential diagnosis of patients with chronic abdominal pain, vomiting, and hematemesis of unknown cause. The infant group may have a better prognosis than the child group if treated properly.

OBJECTIVESTo investigate the relationship between H. pylori infection, gastric cancer and other gastric diseases through the changes in gastric mucosa and the status of different gastric diseases within 5 years after H. pylori eradication in H. pylori-positive subjects in a high incidence region of gastric cancer.
METHODSOne thousand and six adults were selected from the general population in Yantai, Shandong province, a high incidence region for gastric cancer in China. Gastroscopy and Campylobacter-like organism (CLO) testing were performed on all subjects. Biopsy samples from the gastric antrum and body were obtained for histology and assessment of H. pylori infection. All H. pylori-positive subjects were then randomly divided into two groups: treatment group receiving Omeprazole Amoxicillin Clarythromycin (OAC) triple therapy and placebo as controls. These subjects were endoscopically followed up in the second and fifth year. We compared the endoscopic appearance and histology of the biopsy specimens from the same site obtained at the first and last visits.
RESULTSAll 552 H. pylori-positive subjects were randomly and evenly divided into treatment group or control group. During the five-year follow-up, the numbers of patients who continued to be negative or positive for H. pylori were 161 and 198, respectively. Statistical analysis revealed that: (1) At the initial visit, there were no significant differences in the severity and activity of inflammation, atrophy and intestinal metaplasia between the biopsy specimens from the antrum and body respectively in both groups. (2) The severity and activity of inflammation in both the antrum and body were markedly reduced after H. pylori eradication (P = 0.000). (3) Within five years after H. pylori eradication, intestinal metaplasia in the antrum either regressed or had no progression, while the proportion of intestinal metaplasia in the H. pylori-positive group increased significantly (P = 0.032). (4) After H. pylori eradication, the atrophy in both the antrum and body had no significant regression. The P value was 0.223 and 0.402, respectively.
CONCLUSIONSH. pylori eradication results in remarkable reduction in the severity and activity of chronic gastritis, marked resolution of intestinal metaplasia in the antrum. On the other hand, continuous H. pylori infection leads to progressive aggravation of atrophy and intestinal metaplasia.

6.Successful Treatment of Eosinophilic Gastroenteritis with Clarithromycin.
The Korean Journal of Internal Medicine 2012;27(4):451-454
Eosinophilic gastroenteritis (EGE) is an uncommon disease characterized by eosinophilic infiltration of the gastrointestinal tract, which is usually associated with abdominal pain, diarrhea, ascites, and peripheral eosinophilia. Steroids remain the mainstay of treatment for EGE, but symptoms often recur when the dose is reduced. Macrolides have immunomodulatory effects as well as antibacterial effects. The immunomodulatory effect results in inhibition of T-lymphocyte proliferation and triggering of T-lymphocyte and eosinophil apoptosis. Macrolides also have a steroid-sparing effect through their influence on steroid metabolism. We report a rare case of EGE, which relapsed on steroid reduction but improved following clarithromycin treatment.

This report is about the case of gastritis associated with capillariasis. The patient was a 52-yr-old Korean woman who occasionally ate raw fish and chicken. She complained of mild abdominal pain and nausea, but not diarrhea. An endoscopic examination revealed an exudative flat erosive change on the gastric mucosa of the antrum. She was microscopically diagnosed as chronic gastritis with numerous eosinophil infiltrations. The sectioned worms and eggs in mucosa were morphologically regarded as belonging to the genus Capillaria. This is the first case of gastric capillariasis reported in the Republic of Korea.

Annals of the Academy of Medicine, Singapore 2007;36(1):72-73
INTRODUCTIONGas is rarely found within the viscera outside the lumen of the gastrointestinal tract. Emphysematous gastritis is a rare form of infection of the stomach wall by gas producing organisms.
CLINICAL PICTUREA 45-year-old Chinese lady underwent hepatectomy for hepatocellular carcinoma. Postoperatively, she turned septic and encephalopathic with worsening liver function. Computed tomography scan revealed a thickened, oedematous stomach wall with air pockets within.
TREATMENTThe patient was started on a course of broad spectrum antibiotics.
OUTCOMEShe responded and was discharged well.
CONCLUSIONEmphysematous gastritis is a rare condition with high mortality. There is however, still no preferable approach of treatment despite therapeutic advances.

Journal of Biomedical Engineering 2005;22(5):926-929
This study sought to explore the effects of He-Ne laser irradiation on the gastric mucosa of experimental chronic atrophic gastritis (CAG) in rat. Fifty-two male adult Wistar rats were randomly divided into five groups including normal control group, model group and three different doses He-Ne laser groups. The CAG model in rats was made by administering (per gastrogavage) a compositus liquor, including 2% sodium salicylate and 30% alcohol, for 8 weeks to stimulate the rat's gastric mucosa, combined with irregular fasting and compulsive sporting as pathogenic factors. He-Ne laser therapy was used at different doses of He-Ne laser, once a day for twenty days, then the morphological changes and the expression of cyclinD1 were observed. Compared with untreated group, the gastric mucosa of 3.36J x cm(-2) He-Ne laser group was significantly thicker (P < 0.01), the inflammatory cells of gastric mucosa were decreased (P < 0.05), the morphology, structure and volume of the cells were restored or nearly normal and the expressions of cyclinD1 were higher (P < 0.05). In a word, small dose He-Ne laser (3.36 J x cm(-2)) has a good adjuvant therapeutic effect on rat's CAG.
