1.Study on histopathological features of chronic gastritis with Helicobacter pylori and the association between them

The study was conducted on 69 patients with Helicobacter pylori-related chronic gastritis that was confirmed by endoscopy, histopathological findings, urease test and culture. The severity of HP-related chronic gastritis and the HP infectivity of body and antrum of stomach followed and the relationship between severity of chronic gastritis and HP infectivity was determined. The results showed that all patients had inflammatous lesions in body and antrum. The HP incidence and infectivity were both higher in body than in antrum. The activity of inflammation and the severity of chronic inflammation in body and antrum increased positively with the HP infectivity.

4.Survey on the Endoscopic Diagnosis of Chronic Gastritis.

BACKGROUND/AIMS: Endoscopic classification of chronic gastritis has not been standardized yet. Patterns of endoscopic classification in the real clinical practice are not defined. MATERIALS AND METHODS: From July 2013 to September 2013, a questionnaire consisting of eight questions on endoscopic gastritis was surveyed. The correct answer for endoscopic diagnosis of chronic gastritis was defined by an advisory group, including professors of gastroenterology. A total of 189 physicians, most of them primary care physicians, participated in the survey. RESULTS: The overall agreement with standard endoscopic diagnoses was 56 percent. The correct answer for each question was 56 percent for erosive gastritis, 58 percent for hypertrophic gastritis, 60 percent for atrophic gastritis, 52 percent for metaplastic gastritis, respectively. In the superficial gastritis case, the ratio of correct answer was 24 percent, which was lowest among all the questions. Forty-four percent of all participants answered superficial gastritis as hemorrhagic gastritis. These results reveal that hemorrhagic gastritis is diagnosed inappropriately and needs further attention to prevent patients from unnecessary worries and misunderstandings. The correct answer for nodular gastritis was 42 percent, which is relatively low as well. Considering the significance of nodular gastritis as a risk factor for gastric cancer, education for endoscopist to detect nodular gastritis is indispensable. CONCLUSIONS: There was significant discrepancy on the endoscopic diagnosis of chronic gastritis. Further studies to develop a new standardized guideline for diagnosis of gastritis should be accompanied.

Gastritis Cystica Profunda is a rare condition showing multiple small cysts in the mucosa and submucosa of the stomach. These lesions have been found not only at the site of a gastroenterostomy but also in tbe stomchs of patients without any previous surgery. Recently, We witnessed a 56-year old e wale gastritis cystica profunda who had not undergone previous gastric surgery. The UGI and EGD revealed a 3.0 * 4.5 cm sized submucosal mass on the posterior wall of the antrum, and endoscopic ultrasonography(EUS) discovered a thickening of the third layer in which well-defined, round and nearly anechoic areas with posterior enhancement were gathered. They were thought to be cystic lesions. We report a case of gastritis cystica profunda without having had any previous surgery, the diagnosis was made based on findings from the EUS and histologic findings through surgery.

6.Three Cases of Gastritis Cystica Polyposa Following Partial Gastric Resection.

Gastritis cystica polyposa was diagnosed in three patients with ages of 49, 56 and 66 years. All patients had been operated on for gastric cancer 1 to 5 years earlier, with Billroth II gastroenteric anastomose being made at that time. The lesions were diagnosed by regular follow-up endoscopic examination without any presenting symptoms. Macroscopically, all lesions were located on the gastric side of the anastomosis, with polypoid growth of 1.5x1.0, 2.0x1.5, and 0.5x0.5 cm in size respectively. The surfaces of the lesions were coarsely nodular and brittle, and one of them protruded into the anastomosis lumen. Histologic examination revealed polypoid mucosal changes associated with functionally active, hyperplastic and cystic dilatation of the glands which had infiltrated to into the underlying submucosa. An endoscopic polypectomy was performed in two patients, and all has gone well, without evidence of a recurrent tumor 6 & 18 months after polypectomy. The other patients refused a polypectomy. Gastritis cystica polyposa should be differentiated from a stump carcinoma to avoid a further unnecessary surgical intervention. Awareness of the entity will lead to better diagnosis of gastritis cystica polyposa.

7.Histopathological Diagnosis System for Gastritis Using Deep Learning Algorithm.

Objective To develope a deep learning algorithm for pathological classification of chronic gastritis and assess its performance using whole-slide images (WSIs). Methods We retrospectively collected 1,250 gastric biopsy specimens (1,128 gastritis, 122 normal mucosa) from PLA General Hospital. The deep learning algorithm based on DeepLab v3 (ResNet-50) architecture was trained and validated using 1,008 WSIs and 100 WSIs, respectively. The diagnostic performance of the algorithm was tested on an independent test set of 142 WSIs, with the pathologists' consensus diagnosis as the gold standard. Results The receiver operating characteristic (ROC) curves were generated for chronic superficial gastritis (CSuG), chronic active gastritis (CAcG), and chronic atrophic gastritis (CAtG) in the test set, respectively.The areas under the ROC curves (AUCs) of the algorithm for CSuG, CAcG, and CAtG were 0.882, 0.905 and 0.910, respectively. The sensitivity and specificity of the deep learning algorithm for the classification of CSuG, CAcG, and CAtG were 0.790 and 1.000 (accuracy 0.880), 0.985 and 0.829 (accuracy 0.901), 0.952 and 0.992 (accuracy 0.986), respectively. The overall predicted accuracy for three different types of gastritis was 0.867. By flagging the suspicious regions identified by the algorithm in WSI, a more transparent and interpretable diagnosis can be generated. Conclusion The deep learning algorithm achieved high accuracy for chronic gastritis classification using WSIs. By pre-highlighting the different gastritis regions, it might be used as an auxiliary diagnostic tool to improve the work efficiency of pathologists.

Emphysematous gastritis is a rare disease characterized by air in the wall of the stomach due to gas-forming microorganisms. This lethal entity was first described by Fraenkel in 1889. Forty-six cases have been reported so far in the literature. The predisposing factors include corrosive ingestion, immunocompromized state, alcohol abuse, cancer, diabetes and so on. Computed tomography is the procedure of choice for early diagnosis. Only prompt diagnosis and treatment can avoid mortality. Initial antibiotic therapy should cover gram-negative organisms and anaerobes. We report a case of emphysematous gastritis in a 41-year-old man with Klatskin's tumor.
