1.Effect of Air-Craft Noise on Gastric Function.

Thirty three healthy young men and six dogs equipped with gastric fistulae were stimulated by noise of 100 to 120 phons emanating from a F-86F jet engine. The basal secretion of gastric juice in the dogs was little changed, but in human subjects the secretion of gastric juice was altered as follows; 30.3% of 33 subjects showed an increased acid output, 63.6% showed a decrease and the remaining showed no change. Furthermore, the basal resting secretion of those showing decreased acidity from noise exposure was higher than that of those showing increased acidity. Gastric motility was greatly inhibited by exposure to noise in both dogs and humans but the inhibition was more sensitive and more prolonged in humans. Rats fed synthetic diet were placed under conditions of repeated noise for either short or long periods and the occurrence of gastric ulcers by the procedure described by Shay et al was observed. The prevalence of ulcer lesions was increased and the severity of the lesion was enhanced in rats exposed to noise for either short or long periods. The above results indicate that the influences of air-craft noise were not remarkable, but the noise to a considerable degree, is responsible for the occurrence of gastric disorders in man. Sound fields surrounding air-craft engines are thought to be the cause of physical or mental disturbances experienced by persons at close range. Laird (1932) reported that 60 decibels of noise decreased the normal secretion of gastric juice in four of five human subjects who previously had been given an Ewald meal and also caused a decrease in the normal secretion of saliva by about 40 per cent. Previously Laird and Smith (1930) had observed that 80 to 90 decibels of noise caused a decrease in gastric motility in human beings. Vaughan and Van Liere (1940) reported a significant reduction in acid secretion in dogs with Pavlov pouches from a noise of l00 decibels and 2,000 frequency. However, noise frequency of 600 was ineffective. The present study was undertaken to determine in humans and animals whether digestive function or other disorders occurred readily with either single or repeated exposure to air-craft noise.

2.Capsaicin-sensitive afferent fibers and endogenous NO mediate the gastric acid secretion and gastric mucosal blood flow in intragastric distention in rats.

AIM AND METHODSBy hydrogen gas clearance technique to measure gastric mucosal blood flow (GMBF) and a high dose of capsaicin to ablate the capsaicin-sensitive afferent fibers, the roles of capsaicin-sensitive afferent fibers and endogenous NO in the gastric acid secretion and hyperemic response to intragastric distention were studied in rats.
RESULTS(1) There was an increase in acid secretion associated with the increase in GMBF to intragastric distention. (2) Pretreatment with a high dose of capsaicin to ablate afferent fibers completely abolished the GMBF and partially inhibited the acid secretion during the intragastric distention. (3) The increase in GMBF to intragastric distention was completely blocked by pretreatment with L-NAME, whereas the acid secretion was significantly attenuated.
CONCLUSIONCapsaicin-sensitive afferent fibers and endogenous NO are involved in the increases of gastric acid secretion and GMBF.

3.Gastric antisecretory and cytoprotective effects of hydroalcoholic extracts of Plumeria alba Linn. leaves in rats.

OBJECTIVEThis study was conducted to evaluate the antiulcerogenic property of hydroalcoholic extract obtained from the leaves of Plumeria alba Linn.
METHODSAntiulcer assays were performed using the protocols of ulcer induced by non-steroidal anti-inflammatory drugs, ethanol and pylorus ligation. The hydroalcoholic extract (HAPA), and various fractions of HAPA like, n-hexane extract (HPA), ethyl acetate extract (EAPA) and n-butanol extract (BPA) were administered at doses of 200 and 400 mg/kg for HAPA and 100 and 200 mg/kg for fractions of extracts. Parameters of gastric secretion (volume, pH, total protein, and free and total acidity) were determined by the pylorus ligation model. Parameters like aspartate aminotransferase and alanine aminotransferase were also determined in ethanol-induced ulcer model. To determine the mechanism of action, role of nitric oxide was also evaluated.
RESULTSEAPA and BPA (100 and 200 mg/kg, p.o.) showed gastric ulcer-healing effect in indomethacin-induced ulcer model, while HAPA (200 mg/kg) and HPA showed no significant antiulcer effect. Both EAPA and BPA showed gastric cytoprotective effect in ethanol-induced gastric ulcer and inhibited gastric secretion in pylorus-ligated rats.
CONCLUSIONThe results of the present study show that some hydroalcoholic extract of Plumeria alba L. displays antiulcer activity, as demonstrated by the significant inhibition of ulcer formation induced by different models, which is consistent with the literature report in folk medicine.

4.Experimental study on primary pharmacodynamics of Niuhuang Qingwei wan.

OBJECTIVETo study the primary effects of Niuhuang Qingwei wan on the gastrointestinal function in aninmal for justifying its efficacies in clinic.
METHODMice were twice administered with Niuhuang Qingwei wan (0.83, 1.67, 3.33 g x kg(-1), ig) and rats were twice administered with Niuhuang Qingwei wan (0.59, 1.18, 2.36 g x kg(-1), ig). The effects on the stomach function were evaluated by the gastric emptying test in mice and the gastric analysis in rats. The effect on the intestinal function were evaluated by the propulsive motility of the total gastrointestinal tract test in mice by recording the time and frequency of excreting carbo medicinalis. Its analgesia was explored by using the abdominal constriction test induced by acetic acid.
RESULTNiuhuang Qingwei wan decreased the activity and secretion of pepsin in a dose-dependent manner (P < 0.01, P < 0.05), the gastric juice volume at middle and high doses (P <0.01, P <0.05), and the gastric acid volume at high dose (P <0.05); However, it had no significant effects on the gastric emptying in normal mice and the acidity in gastric juice. It shortened the excreting time of feces and increased the frequency of defecation (P < 0.01, P < 0.05). It also inhibited abdominal constriction responses at high dose, and the inhibition rate was 40.0% (P <0.01).
CONCLUSIONNiuhuang Qingwei wan can promote gastrointestinal motility, decrease gastric acid volume and activity of pepsin and show certain analgesia effect. Those findings are consistent with its treating stomach heat in clinic.

5.Study on the association of gastric conduit width and postoperative early delayed gastric emptying in middle-lower esophageal cancer patients undergoing Ivor-Lewis procedure.

OBJECTIVETo examine the relationship between gastric conduit width and postoperative early delayed gastric emptying (DGE) in patients with middle-lower esophageal carcinoma who underwent Ivor-Lewis operation.
METHODSClinical data of 282 consecutive patients with middle-lower esophageal cancer who underwent the Ivor-Lewis operation by same surgical team in our department from January 2013 to June 2015 were retrospectively analyzed. Patients were divided into three groups according to the width of gastric conduit: width > 5.0 cm as broad group (n=93); width 3.0-5.0 cm as moderate group (n=70); width < 3.0 cm as narrow group (n=119). The gastric conduits of patients in narrow group were completely positioned the esophageal bed and fixed to the pericardium posterior wall. None of patients received pyloroplasty or pylorotomy. Perioperative data, operation-associated complications, and postoperative upper gastrointestinal radiographic results(1 week and 4 weeks after operation) were compared among groups.
RESULTSThe baseline data among these groups were comparable in terms of age, gender, tumor TNM staging, pathological types, serum albumin level, and the rate of receiving neoadjuvant therapy(all P>0.05). There were no significant differences in operative time, blood loss, and postoperative hospital stay among groups(all P>0.05). No patients died during perioperative peried. Anastomotic leakage occurred in 2 cases, one from broad group and another from narrow group. The incidences of arrhythmia and postoperative pulmonary complications, including infection, atelectasis, pneumothorax, and pleural effusion were similar among groups (all P>0.05). The average amount of gastric juice drainage in narrow group was (98±57) ml/day, which was markedly lower than that in broad group [(157±62) ml/day, P=0.000] and in moderate group [(123±68) ml/day, P=0.008]. One week after operation, the overall incidence of DGE was 10.6%(30/282), the incidence of DGE in broad, moderate, narrow groups was 17.2%(16/93), 14.3%(10/70), and 3.4%(4/119) respectively, and broad and moderate groups had higher incidence as compared to narrow group (P=0.001 and P=0.006).
CONCLUSIONDuring the Ivor-Lewis operation, application of a narrow gastric conduit (width < 3.0 cm), which completely position the esophageal bed with fixation to the pericardium posterior wall, can significantly reduce the incidence of postoperative early DGE.
