BACKGROUND:At present, the confrontation of CAD/CAM systems and the lost wax casting method for marginal adaptation has been widely concerned, but the results are controversial. OBJECTIVE:To compare the marginal adaptation of single cobalt-chromium PFM crowns produced by 3 Shape CAD/CAM systems and the lost wax casting method. METHODS: A maxilary first premolar was prepared according to the standards. 50 resin-generation models were created by bi-color silicone rubber method and randomly divided into two groups: CAD/CAM group (A1-A25) and cast group (B1-B25). Al the models were cast in plaster. In CAD/CAM group, the digital impressions were obtained by optical scanning, then designed through computer and cut directly by NC miling machine. In cast group, metal ceramic crowns were made by the lost wax casting method. When crowns were al seated on the working dies, the marginal gaps were copied with light silicone rubber and then embedded with occlusal silicone rubber. The thickness of crown marginal gap was measured with a stereo microscope. RESULTS AND CONCLUSION:The average vertical clearance was (34.67±7.48)μm of CAD/CAM group, while (53.70±9.64)μm of the cast group; the difference was statisticaly significant between two groups (P < 0.05). The average level clearance was (37.61±7.63)μm of CAD/CAM group, while (53.02±10.65)μm of the cast group, and the difference was statisticaly significant between two groups (P < 0.05). Experimental findings suggest that, the marginal adaptation of single cobalt-chromium PFM crowns cast produced by 3 Shape CAD/CAM systems is clinical acceptable (< 120μm), and CAD/CAM systems of metal casting is obviously better than the lost wax casting in building metal based crowns with adaptable margin.

Objective To explore the relationship between the apoptosis of cerebellar granule cell and the changes of caspases-3 and Bcl-2 in rats with methylmercury intoxication.Methods We chronologically observed the pathological changes of cerebellum in rats given methylmercury[4 mg/(kg?d)] at day 11, 15, 18 and 19. The apoptosis of cerebellar granule cell was detected by terminal deoxynucleotidyl transferase-mediated dUTP nick-end-labeling (TUNEL) method, and the relationship between the apoptosis and the expression of caspases-3 and Bcl-2 was evaluated.Results At day 18, some sparse TUNEL positive granular cells could be observed, mainly in deep lamina adjacent to the white matter. At day 19, the apoptotic cells markedly increased, while the number of granular cells decreased with a well preservation of Purkinje cells demonstrated by calvindine immunostaining. MRF-1 and glial fibrillary acidic protein(GFAP) dyeing demonstrated severe microglia reaction and astrocytosis. From day 11, caspasese-3 mRNA expression up-regulated, increased gradually and reached peak at day 18, then decreased markedly at day 19. In contrast, Bcl-2 mRNA expression down-regulated from day 11, and decreased obviously at day 19. Zic-1 as a marker of granule cell decreased gradually after methylmercury administration and indicated degeneration of granule cells.Conclusion Cerebellar granular cell degeneration induced by methylmercury is related to apoptosis mediated by caspases-3 and Bcl-2 down regulated.

BACKGROUND:Apolipoprotein E is one of the important plasma apolipoproteins with polymorphism and involved in lipid metabolism and cholesterol balance modulation, playing also a role in the normal growth and repair of the nervous system.OBJECTIVE: To analyze apolipoprotein E gene polymorphism in patients with vascular dementia (VaD) in comparison with patients with cerebral infarction (CI) patients and healthy controls, so as to elicit the relationship between apolipoprotein E gene polymorphism and VaD.DESIGN: Case-control study.SETTING: Department of Neurology, General Hospital of Jinan Military Area Command of Chinese PLA.PARTICIPANTS: The participants were recruited from the inpatients and outpatients with cerebrovascular diseases and healthy subjects for routine physical examination in the General Hospital of Jinan Military Area Command of Chinese PLA between August 2001 and October 2003, including 35 cases of VaD, 36 CI cases and 40 healthy controls.METHODS: Four milliliters of venous blood was collected from the elbow vein form all participants after fasting for 12 hours for detection of apolipoprotein E genotype using polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP). Blood lipids and apolipoprotein contents were also examined.of triglyceride, total cholesterol, high-density lipoprotein cholesterol (HDLC), low-density lipoprotein cholesterol (LDL-C), as well as apolipoprotein A and aoplipoprotein B.RESULTS: Data of 35 VaD 36 CI patients and 40 healthy controls were all VaD group and CI group had higher ε4 allele frequency were higher (22.86% and 22.22% vs 7.5%, respectively, P ＜ 0.05) but lower ε3 allele frequency (70% and 69.45% vs 86.25%, respectively, P ＜ 0.05); the frequencies of all alleles were basically comparable between VaD and CI groups er in E ε4 allele carriers [(5.85±L03), (4.73±0.29), (4.96±0.87) mmol/L, respectively] than in E ε2 [(3.91±0.87), (3.12±0.65), (3.06±0.33) mmol/L] and ε3 [(1.34±0.21), (1.00±0.28), (0.94±0.32) g/L] allele carriers in VaD group (P ＜ 0.05).CONCLUSION: Apolipoprotein E gene polymorphism is closely associated with the onset of VaD, for which ε4 allele might be a risk factor with similar impact in CI.

Objective To analyze the medical undergraduates'employment intentions and determinants for policy recommendations on how to solve the contraction between the employment and human resources shortage working in the rural place.Methods Descriptive method was used for the employment recognition and unconditional logistic regression was employed for its factors analysis.Results 74.90% interviewees were not high for policy recognition,and only 21.16% undergraduates were willing to work in rural area.Logistic regression analysis found local culture and life environment (OR=2.668),institute performance(OR=0.630)and work challenge(OR=1.509)were key factors to affect their employment.Conclusion To give reform of primary institute performance mainly on pubic health service,take great importance on the medical service,improve work and life environment of rural workers and highly care health workers'career planning and development.

Objective:To explore the application value of bedside lung ultrasound score(LUS)in evaluating patients with primary graft dysfunction(PGD)after lung transplantation.Methods:A total of 35 PGD patients after lung transplantation in surgical intensive care unit from June 2018 to May 2020 were selected as research objects. After lung transplantation, one physician collected the clinical data, including age, gender, preoperative 24 h APACHE II score, postoperative hemodynamic parameters, lactate and respiratory parameters; another physician was responsible for LUS examination and LUS at 12/24/48/72 h post-operation. The correlation between LUS and oxygenation index was analyzed by bivariate correlation analysis. Receiver operating characteristic curve(ROC)was utilized for calculating the predictive value, sensitivity and specificity of LUS score for severe PGD.Results:A negative correlation existed between LUS and oxygenation index in single lung transplant( r=-0.536, P<0.01)and a negative correlation between LUS and oxygenation index ( r=-0.518, P<0.01)in double lung transplant. The area under ROC curve of LUS showed that the predictive value of LUS of severe PGD patients with single lung transplant was 7.0 with a sensitivity of 86.7% and a specificity of 72.1%, the predicted value of LUS of severe PGD in double lung transplant was 13.0 with a sensitivity of 83.3% and a specificity of 60.0%. Conclusions:Lung ultrasound is simple and easy to operate and LUS can evaluate the severity of PGD patients after lung transplantation. It has a high potency of guiding clinical diagnosis and treatment.

Objective To investigate the basic clinical features in 371 cases of colorectal polyps and its relationship with fecal occult blood and carcinoembryonic antigen(CEA).Methods The retrospective analysis was performed on 371 inpatients with colo-rectal polyps.The relationship among gender,number of polyps and polyps anatomical site in different ages of patients was investi-gated,and the relationship between fecal occult blood and CEA with polyp canceration was analyzed by 1.5?3.0 years follow-up. Results Among 371 cases of colorectal polyps,the female patients were gradually increased and single polyp was gradually de-creased along with the age increase;due to different ages,there was the statistically significant difference in the polyp locations (χ2 =9.759,P=0.045);the distribution difference of the patients with polyp canceration among three age groups was statistically significant(χ2 =5.138,4.107,13.153,P<0.05).The cases of fecal occult blood positive and CEA abnormal increase were gradual-ly increased with age increasing(χ2 =15.544,11.959,P<0.01);with the number of polyps increasing,the cases of fecal occult blood positive showed the increasing trend(χ2 =14.043,P=0.001);the canceration rate in colorectal polyp cases of fecal occult blood positive and CEA abnormal increase was significantly higher than that in the cases of fecal occult blood negative and CEA normal range(χ2 =40.165,43.249,all of P< 0.001).Conclusion The fecal occult blood test and CEA detection results have a certain significance to the follow up for preventing colorectal polyps canceration.

Objective To investigate the clinical and pathological features of necrotizing myopathy with anti-signal recognition particle antibodies and its treatment.Methods Five patients with necrotizing myopathy with anti-signal recognition particle antibodies,admitted to our hospital from March 2011 to July 2012,were chosen in our study; their clinical and pathological manifestations and treatment methods were retrospectively analyzed.Results Five patients showed clinical manifestations of subacute onset,proximal limb muscle weakness.Serum creatine kinase level was significantly elevated.Immunoblotting confirmed positive signal recognition particle antibodies.EMG prompted myogenic damage.Pathology manifested as muscle degeneration,necrosis with regeneration,visible atrophy and hypertrophy of muscle fibers,and connective tissue hyperplasia,and no significant inflammatory cell infiltration was noted.Immunohistochemistry staining showed necrotizing muscle fibers infiltrated with CD4-positive T lymphocytes and CD68-positive macrophages,no CD8-positive T lymphocytes and CD20-positive B lymphocytes.Sarcoglycans staining,dystrophine staining and dysferlin staining showed continuous strong positive expression.Follow-up found patients were poor response to glucocorticoid,and a combination therapy of immunosuppressive agents was better than hormone alone.Conclusion The prominent manifestations of necrotizing myopathy with anti-signal recognition particle antibodies are symmetric proximal muscle weakness,with highly elevated levels of serum creatine kinase; fiber necrosis and regeneration are the main myopathlogical features,without obvious inflammatory cells infiltrates; this disease is poorly responsive to corticosteroids and immunosuppression.

Breast cancer is one of the most serious diseases threatening women's life and health in the world, and the mortality rate is the second in the world. With the progress of nanotechnology and the advantages of nanomaterials in the field of electrochemistry and biosensor, various nanomaterials have been applied in electrochemical biosensors. This makes the electrochemical nano-biosensor in the field of rapid detection of breast cancer has been widely concerned and studied. This paper introduces the important components of electrochemical nano-biosensor for breast cancer detection and the research progress of each component in breast cancer detection, as well as the performance of electrochemical nano biosensor in breast cancer detection and the prospect of its application.

ObjectiveTo investigate current status of hospital infection management in psychiatric hospitals in the Ningxia Hui Autonomous Region， so as to provide references for improving the level of hospital infection management of psychiatric hospitals. MethodsIn December 2020， on-site supervision was conducted on hospital infection management in all 9 psychiatric hospitals in the Ningxia Hui Autonomous Region， meantime， the self-compiled questionnaire on hospital infection management status was used for investigation. ResultsAmong the selected hospitals， nine （100.00%） psychiatric hospitals had the main hospital leaders in charge of hospital infection management， five （55.56%） hospitals had established a hospital infection management committee， six （66.67%） hospitals had established an independent hospital infection management department， and one （11.11%） hospital had developed all 13 systems mentioned in the questionnaire related to hospital infection management and job responsibilities. In terms of hospital infection management staff， there were 23 staff members in the nine psychiatric hospitals， including 3 in the specialty （13.04%） and 20 in the part-time setting （86.96%）. The score of the implementation of the basic system of hospital infection management in nine hospitals was （3.28±2.22）. ConclusionThe system specification related to hospital infection management in the Ningxia Hui Autonomous Region psychiatric hospitals needs to be improved and further strengthened， the professionalism of hospital infection management personnel needs to be improved.

A triple-transgenic (tyrosine hydroxylase/dopamine decarboxylase/GTP cyclohydrolase 1, TH/DDC/GCH1) bone marrow mesenchymal stem cell line (BMSCs) capable of stably synthesizing dopamine (DA) transmitters were established to provide experimental evidence for the clinical treatment of Parkinson's disease (PD) by using this cell line. The DA-BMSCs cell line that could stably synthesize and secrete DA transmitters was established by using the triple transgenic recombinant lentivirus. The triple transgenes (TH/DDC/GCH1) expression in DA-BMSCs was detected using reverse transcription-polymerase chain reaction (RT-PCR), Western blotting, and immunofluorescence. Moreover, the secretion of DA was tested by enzyme-linked immunosorbent assay (ELISA) and high-performance liquid chromatography (HPLC). Chromosome G-banding analysis was used to detect the genetic stability of DA-BMSCs. Subsequently, the DA-BMSCs were stereotactically transplanted into the right medial forebrain bundle (MFB) of Parkinson's rat models to detect their survival and differentiation in the intracerebral microenvironment of PD rats. Apomorphine (APO)-induced rotation test was used to detect the improvement of motor dysfunction in PD rat models with cell transplantation. The TH, DDC and GCH1 were expressed stably and efficiently in the DA-BMSCs cell line, but not expressed in the normal rat BMSCs. The concentration of DA in the cell culture supernatant of the triple transgenic group (DA-BMSCs) and the LV-TH group was extremely significantly higher than that of the standard BMSCs control group (P < 0.000 1). After passage, DA-BMSCs stably produced DA. Karyotype G-banding analysis showed that the vast majority of DA-BMSCs maintained normal diploid karyotypes (94.5%). Moreover, after 4 weeks of transplantation into the brain of PD rats, DA-BMSCs significantly improved the movement disorder of PD rat models, survived in a large amount in the brain microenvironment, differentiated into TH-positive and GFAP-positive cells, and upregulated the DA level in the injured area of the brain. The triple-transgenic DA-BMSCs cell line that stably produced DA, survived in large numbers, and differentiated in the rat brain was successfully established, laying a foundation for the treatment of PD using engineered culture and transplantation of DA-BMSCs.
