Cerebral vasospasm is the leading cause of disability and death in patients with spontaneous subarachnoid hemorrhage,and there is no definitive and effective treatment for it yet.Triple-H therapy is now the first choice in the treatment of cerebral vasospasm,but there are still more controversies about it. In recent years,there have been a number of studies about Triple-H therapy. This article reviews the implementation of Triple-H therapy,fluid selection,and prophylactic application.
2.Basic Quality and Training of Employees in Zhangjiajie Tourists' Drugstores

OBJECTIVE:To provide reference for government in making control policy for tourist' drugstores.METHODS:The basic information,training information and the outcome of the training of 243 employees working in tourist' drugstores in Zhangjiajie were investigated by questionnaire survey.RESULTS & CONCLUSIONS:The basic quality of employees working in tourist drugstores in Zhangjiajie is low,and they have a strong wish for training incited by "base pay plus royalty" and they believe that they can gain a lot benefits from the training.It is advisable to strengthen training among the employees involved in pharmacy,conduct the training after subdividing the employees' needs and work out teaching materials tailored to the characteristics of different people.

4.Clinical Value of Color Doppler Ultrasound in Diagnosis of Lymph Node Diseases

Objective To investigate the clinical value of Color Doppler Ultrasound in diagnosis of lymph node diseases. Methods We observed the ultrasound features of the 93 cases of swelled lymph nodes with Color Doppler ultrasound. The ultrasound features included pseudokidney sign, assessment on blood flow distribution, the Doppler resistive Index (RI), maximal flow rate (Vmax),the longitudinal axis compared to the diameter of a node (L/D ratio) . Results Out of 93 cases of clinically confirmed swelled lymph nodes,the concordance rate of Color Doppler Ultrasound was 86%and 88%in diagnosis of malignant lymph node disease and benign lymph node disease, respectively. 4 cases of lymph tuberculosis were misdiagnosed as lymphoma due to the similar ultrasound characteristics found in malignant lymph group,the rate of misdiagnosis was 8%. In the cases of proliferative lymph node diseases with Pseudokidney sign, 93% of the blood flow distribution was classified as grade 0-I. 90% of lymphadenitis were found with Pseudokidney sign,and 95%of those cases with blood flow distribution was classified as grade II-III. Malignant lymph diseases had no Pseudokidney sign, and 86%of blood flow distribution grade was as III. There was statistically significant difference in the L/D ratio and RI between benign lymph group and malignant lymph group (P<0.01) . There was statistically significant difference in Vmax between lymphoproliferative group and other groups (P<0.01) . Conclusions Pathological characteristics on different lymph node disease determin the ultrasound characteristics. Combined clinical data based on Pseudokidney sign, Blood Flow Distribution, RI value,Vmax value and L/D ratio can enhance the accuracy of various lymph node disease diagnosis.
5.Carrageenan oligosaccharides inhibit growth-factor binding and heparanase activity.

This study is designed to investigate the anti-tumor and anti-angiogenesis mechanism of carrageenan oligosaccharides. The effects of carrageenan oligosaccharides on basic fibroblast growth factor (bFGF) induced cell proliferation, heparanase activity and bFGF binding ability were evaluated in human cervical cancer cells (HeLa) and human umbilical vein endothelial cells (HUVEC). Results indicate that, at rational concentrations, carrageenan oligosaccharides showed low cytotoxic effect. At relatively low concentrations (0.2-200 microg x mL(-1)), these oligosaccharides could competitively bind bFGF and inhibit bFGF induced cell proliferation. In these samples, oligo-lambda-carrageenans (dp2-8) were the most potent bFGF antagonists. At concentration of 20 microg x mL(-1), their inhibitory ratio reached to 30%. The heparanase enzyme assay revealed that three kinds of carrageenan oligosaccharides showed different inhibitory activities to two cell lines. For HeLa cell, oligo-lambda-carrageenans showed highest inhibitory effect, but for HUVEC, oligo-kappa-carrageenans (dp9-17) were the best inhibitors. Current observations demonstrated that the biological activities of carrageenan oligosaccharides are closely related to the molecular weight, carbohydrate structure and the content and linking position of sulfur groups. Carrageenan oligosaccharides with high sulfate fraction, 2-8 units saccharide size and suitable molecular structure are able to achieve potent heparin sulfate-like compounds.
6.Investigation Effect of Naofuqing Capsules on Senile Dementia on 30 Cases

Objective: To investigate the clinical effect of Naofuqing Capsules on senile dementia. Methods: 30 patients with senile dementia were treated with Naofuqing Capsules and the curative results were compared with that of the control group 30 cases. Results: The results showed that the integral titres of MMSE and HDS R of groups increased obviously after treatment( P 0.05). The total effective rates were 83.3% and 80.0%,respectively. No obvious difference was observed in clinical effect between two groups. Conclusion: Naofuqing Capsules possesses a definite therapeutic effect for senile dementia.
7.Clinical study of coronary artery perforation during percutaneous coronary interventions

Objective To summarize clinical characteristics of coronary artery perforation during percutaneous coronary interventions.Methods Retrospective analysis of consecutive 32 patients who had coronary artery perforation in Fu Wai Hospital from April,2004 to August,2006 was carried out.Results Thirty-two cases of coronary perforation occurred during 7102 PCI procedures performed within this period(incidence:0.5%).These cases are comparatively complicated lesions including 81.3% of type B2+C and 62.5% of chronic total occlusion(CTO)lesions.Ellis classification:typeⅠ:14(43.8%),type Ⅱ:10(31.2%),type Ⅲ:8(25.0%),type Ⅳ:0.Mechanism of coronary artery perforation:guiding wire:21(65.6%),predilatation:6(18.8%),postdilatation:3(9.4%),stent implantation:2(6.2%).Clinical consequence:death:3(9.4%),cardiac temponade:7(21.9%),acute myocardial infarction(AMI):9(28.1%).Treatment:reversal of heparin-induced anticoagulation by application of protamine:14(43.8%),prolonged balloon inflation:9(28.1%),percardiocentesis:7(21.9%),implantation of membrane covered stent:4(12.5%),bail-out surgical repair:2(6.2%).Conclusion Coronary perforation during PCI is a rare complication;type Ⅲ perforation is associated with significant morbidity and mortality,which needs urgent and intensive treatment.
9.Analysis of mechanism in acute myocardial infarction with previous percutaneous coronary intervention

Objective To observe the characteristics of culprit lesions in AMI with previous PCI.Methods Retrospective analysis was carried out in 61 consecutive AMI patients with previous PCI treated with primary PCI from April, 2004 to April, 2006 in Fuwai hospital.Results Location of culprit lesions in the 61 patients (62.1?10.0 yrs; male 88.5%) were: LAD 47.5%, RCA 39.5%, LCX 13.0%. Comparing the medication of the patients during the first year after the former PCI, besides the continous use of aspirin (93.8% vs 100%,P=0.113), all patients had stopped using clopidogrel, and the use of ?-blockers, ACEIs and statins also dropped significantly after 1 year (46.9% vs 75.0%, P= 0.001; 34.4% vs 70.8%, P= 0.001; and 28.1% vs 77.1%, P= 0.000, respectively). The characteristics of the culprit lesions included: acute and sub-acute in-stent thrombosis in 12 cases (19.7%), late and very late in-stent thrombosis in 6 cases (9.8%), plaque rapture in 41 cases (70.5%), but no restenosis was involved. The time of the recurrant AMI after the former PCI were: 13 cases (21.3%) within 1 month including 12 cases of acute or subacute in-stent thrombosis and 1 case of plaque rupture in anther coronary artery 3 days after primary PCI; 16 cases (26.2%) after the first month to 1 year including 12 cases of plaque rupture and 4 cases of late in-stent thrombosis; 32 cases (52.5%) 1 year including 30 cases of plaque rupture and 2 cases of very late in-stent thrombosis. Conclusion The major mechanism of recurrant AMI after preoious PCI is plaque rapture.
10.Suppression of breast cancer proliferation and induction of apoptosis via AKT and ERK1/2 signal transduction pathways by synthetic polypeptide derived from viral macrophage inflammatory protein II.

SDF-1α, a ligand for the chemokine receptor CXCR4, is well known for mediating the migration of breast cancer cells. In a previous study we demonstrated that a synthetic 21-mer peptide antagonist of CXCR4 (NT21MP) derived from the viral macrophage inflammatory protein II could antagonize tumor growth in vivo by inhibiting cellular proliferation and inducing apoptosis in breast cancer cells. However, the role of SDF-1α in the signaling pathways underlying the proliferation of human breast cancer cells and associated signaling pathways and inhibiting signal pathways of NT21MP remained unclear. The present study investigated the mechanism of NT21MP on anti-tumor in breast cancer in vitro. The effect of NT21MP on the viability of cells was determined by the MTT assay. Annexin V-FITC and PI staining was performed to detect early stage apoptosis in SKBR3 cells treated with SDF-1α and AMD3100 or NT21MP. Western blotting techniques were used to assay the composition of phosphoproteomics and total proteins present in the SKBR3 breast cancer cells. RT-PCR and Western blotting technique were used to detect the effect of NT21MP and AMD3100 on Bcl-2 and Bax expression. The results indicated that SDF-1α prevented apoptosis and promoted the proliferation of SKBR3 human breast cancer cells. As compared with untreated SKBR3 cells, Treatment with SDF-1α significantly increased cell viability, and NT21MP abolished the protective effects of SDF-1α dose-dependently (P<0.05). There was a significant decrease in the percentage of apoptotic cells after SDF-1α treatment as compared with control group (2.7%±0.2% vs. 5.7%±0.4%, P<0.05). But pretreatment of SKBR3 cells with NT21MP significantly attenuated the antiapoptotic effects of SDF-1α as compared with SKBR3 cells without NT21MP pretreatment. The proliferative and anti-apoptotic effects of SDF-1α in SKBR3 cells were associated with an increase in AKT and ERK1/2 phosphorylation as well as a decrease in Bax expression and an increase in Bcl-2 expression. These changes in intracellular processes were blocked by NT21MP in a dose-dependent manner(P<0.05). In conclusion, NT21MP efficiently inhibits SDF-1α-induced proliferation and antiapoptosis in SKBR3 cells by reducing the levels of phosphorylated AKT and ERK1/2, as well as decreasing the ratio of expression of Bcl-2 relative to Bax.
