From late period of 1980, the treatment of lung volume reduction surgery (LVRS) of emphysema, which is a chronic respiratory disease of intermedicine traditionally, was rerisen. The history, indications, operative technique, postoperative management, present state and some controversies about LVRS were reviewed in this article.

Leukotriene is a crucial inflammation medium in the process of asthma,involved in the inflammation and remodeling of airway.This article summarizes the origin,synthesis and biological activity of leukotriene in asthma.and discuss the implications of leukotriene receptor antagonists(Montelukast)for asthma in children.The randomized.double-blind and muhicenter clinical trials are needed to determine the efficacy of Montelukast in infants less than 2 years old with asthma.

Objective To explore the distribution and genotypes of plasmid-mediated quionlone resistance (PMQR)genes and intI1 integrase genes in Enterobacteriaceae isolates.Methods The PMQR genes and intI1 integrase genes were identified by polymerase chain reaction in the nonduplicate strains of E.coli (80),E.cloacae (18)and K.pneunoniae (27).The positive PCR products were subj ect to DNA sequencing analysis.The gene-positive strains were tested by conj ugation experiment.The minimum inhibitory concentrations (MICs)of donor,recipient strains and transconj ugants were tested by agar dilution method with quinolones and other antimicrobial agents.Results Sixteen (12.8%)of the 125 Enterobacteriaceae isolates were qnr gene positive,including 8 qnrS1 positive and 8 qnrB6 positive.Furthermore,the aac(6′)-Ib-cr gene was identified in 15 (12.0%) strains.Twenty PMQR-positive isolates harbored intI1 integrase gene.The conjugation experiments were successful in 12 of the 26 PMQR-positive isolates and 7 of the 20 intI1-positive isolates.The MICs of quinolones and other antimicrobial agents against the transconj ugants were higher than the MIC values against recipient strains.Conclusions The PMQR genes are prevalent in Enterobacteriaceae isolates.The PMQR-positive isolates can co-harbor integrase genes.These resistance genes have the feature of horizontal transfer,to which close attention should be paid.

Identification and classification of white blood cells are important for clinical diagnosis.Many researchers have been seeking the effective methods for white blood cells' automatic classification based on morphological characters.After cell segmentation,leukocytes' feature acquirement and selection,this paper accomplishes white blood cells' automatic classification using Sugeno-model fuzzy neural network and compares the result with that from classifier of BP network.

Objective To evaluate the clinical value of serum MUC1(sMUC1)in the patients with multiple myeloma.Methods The enzyme linked immunosorbent assay(ELISA)was used to compare the sMUC1 levels in the 12 cases of newly diagnosed multiple myeloma.15 cases of post-chemotherapy and 46 healthy donors.Results The mean concentration of sMUC1 in the newly diagnosed patients with multiple myeloma was 33.44 U/ml(10.86～88.80 U/ml).In the patients of post-chemotherapy the mean concentration was 11.6U/ml(3.92～22.22 U/ml),and in the healthy donors the concentration was 12.81 U/ml(3.84～30.45 U/ml).The level of sMUC1 in the new diagnosed patients was significandy higher than that in the patients of post-chemotherapy(P=0.001)and healthy donors(P=0.000).There was no significant difference between post-chemotherapy and healthy donors(P=0.461).Conclusion sMUC1 may be one of the tumor markers for the diagnosis of multiple myeloma.sMUC1 could also be used as one of the indicators of prognosis and the evaluation of the chemotherapy efficacy.sMUC1 might be a potential target for the immunotherapy to the patients with multiple myeloma.

Objective:To study the clinical efficacy ofYiqi Huoxue Decoction in the treatment of children with spleen and kidney deficiency type nephrotic syndrome.Methods:A total of 80 children with spleen and kidney deficiency type nephrotic syndrome in our hospital from June 2014 to October 2016 were enrolled in this study.The subjects were divided into control group (n=40) and treatment group (n=40) according to the random number table method.The control group was treated with prednisone,and the treatment group was treated with prednisone combined with Yiqi Huoxue Decoction,the two groups were treated for 4 months.The clinical efficacy of the two groups were evaluate.The 24h proteinuria,total cholesterol (TC),plasma albumin (Alb),and renal function parameters including blood urea nitrogen (BUN),serum creatinine (Scr),and serum creatinine clearance (Ccr) of the two groups before and after treatment were compared.Results:The total effective rate of the treatment group was 95.00％,which was significantly higher than 67.50％ of the control group,the difference was statistically significant (P＜0.05).The 24h proteinuria,TC,BUN and Scr of the two groups after treatment were significantly lower than before treatment,the Alb,Ccr of the two groups were significantly higher than before treatment,and above indexes in treatment group were better than the control group,the differences were statistically significant (P＜0.05).Conclusion:Yiqi Huoxue Decoction have good clinical efficacy in the treatment of children with spleen and kidney deficiency type nephrotic syndrome,which can obviously improve children renal function,and is worthy of clinical application.

Objective To investigate the role of fiberoptic bronchoscopy (FOB) in the positioning of ProSeal laryngeal mask airway (PLMA). Methods Sixty ASA Ⅰ or Ⅱ patients undergoing general anesthesia using PLMA were randomly divided into two groups with 30 cases each: inserted PLMA group (group A)and FOB positioning group (group B). After the routine intravenous anesthesia induction,the PLMA inflated to an intracapsular pressure of 50 cm H2O (1 cm H2O =0.098 kPa) with the positive airway pressure by the anesthesia apparatus, the airway seal pressure, airway peak voltage of intermittent positive pressure ventilation and tidal volume were evaluated. Results Patients from two groups achieved adequate lung ventilation (SpO2 ＞0.97,PETCO2 was normal), the operations were all smooth. There were 5 cases adjusted the position two times,and 1 case adjusted 3 times. Patients in group B were all PLMA insertion under FOB and airway seal pressure was higher,but airway peak voltage was lower than in group A (P＜0.05). Conclusion FOB elevates accurate allocation of PLMA,enhances the isolated function from the surrounding tissue of glottis, improves the lung ventilation, and increases the safety of PLMA.
