1.Serum IGF-1 levels and its clinical significance in patients with unresectable or metastatic gastrointestinal stromal tumors before and after imatinib treatment

ObjectivesTo explore the serum insulin-like growth factor-1 (IGF-1)levels in the patients with unresectable or metastatic gastrointestinal stromal tumors before and after imatinib treatment and to discuss its clinical significance.MethodsThe serum samples of27 patients with unresectable or metastatic gastrointestinal stromal tumors were collected before imatinib treatment(before treatment group) and three months after imatinib treatment(after treatment group).The other serum samples were collected from 20 healthy volunteers who were accepting health examination (control group). The serum IGF-1 levels of the samples were determined by enzyme-linked immunosorbent assay (ELISA).ResultsThe mean serum IGF-1 levels were significantly higher in before treatment group than in control controls[(463.61±120.98)ng/ml vs.(115.75±39.27) ng/ml, t=12.355,P=0.000)]. Three months after imatinib treatment,the mean serum IGF-1 levels were significantly lower in after treatment group than in before treatment group [(244.64±100.11)ng/ml vs.(463.61±120.98) ng/ml, t=7.582,P=0.000].ConclusionsSerum IGF-1 levels may help to judge therapeutic effect,progression,recurrence or metastasis of the gastrointestinal stromal tumor.
5.Changes of serum fibroblast growth factor 2 levels before and after treatment with imatinib mesylate in patients with unresectable or metastatic gastrointestinal stromal tumor

Objective To explore changes of serum fibroblast growth factor 2 (FGF2) levels after treatment with imatinib mesylate in patients with unresectable or metastatic gastrointestinal sromal tumor.Methods Serum FGF2 levels (with ELISA) were determined in 27 patients with unresectable or metastatic gastrointestinal stromal tumor both before and after treatment with imatinib mesylate.Results Before treatment with imatinib mesylate,of 27 cases of unresectable or metastatic gastrointestinal stromal tumor,17 cases (63.0 %) were found positive for serum FGF2,after treatment with imatinib mesylate,9 cases (33.3 %) were found positive for serum FGF2 (P < 0.05).Using imatinib mesylate for 3 months,complete remission rate was 0,partial remission rate was 51.9 % (14/27),stable disease rate was 48.1% (13/27) and progressive rate was 0.Conclusion Patients with unresectable or metastatic gastrointestinal stromal tumor have elevated serum FGF2 levels.Imatinib mesylate treatment can reduce serum FGF2 levels in patients with unresectable or metastatic gastrointestinal stromal tumor.
6.Treatment of recurrent or metastatic gastrointestinal stromal tumors with imatinib mesylate of 35 cases report

Objective To investigate the effects of imatinib mesylate on recurrent or metastatic gastrointestinal stromal tumor.Methods Thirty-five cases of patients with recurrent or metastatic gastrointestinal stromal tumors treated with Imatinib mesylate from January 2007 to February 2010 in our hospital were analyzed retrospectively.Results After treated with Imatinib mesylate for 3 months,complete remission rate was 2.9%( 1/35 ),partial remission rate was 45.7% ( 16/35 ),stable disease rate was 51.4% ( 18/35 ) and progressive rate was 0;clinical benefit rate was 100%.The 2-year overall survival rate was 97.1%.The 2-year progression free survival was 82.9%.Conclusion Imatinib mesylate is effective for recurrent or metastatic gastrointestinal stromal tumors and the treatment is safe and reliable.
7.Expression and significance of hypoxia-inducible factor 1α in vitreous body in eyes with proliferative vitreo-retinal diseases

Background Proliferative vitreo-retinal diseases include all the overly proliferative diseases of eyes.Many kinds of cytokines play important roles in their pathogenesis.Hypoxia-inducible factor 1α(HIF-1α)participates in the pathogenesis of a lot of ischemic diseases.HIF-1α may play an important role in the pathogenesis of proliferative vitreo-retinal diseases. Objective Present study was to observe the expression of HIF-1α in vitreous body in the eyes with proliferative vitreo-retinal diseases.The vitreous samples were collected during the vitrectomy from 71 eyes of 69 patients(experimental group)including 39 eyes with proliferative diabetic retinopathy(PDR)and 32 eyes with proliferative Vitreoretinopathy(PVR).Sixteen eyes of 16 patients as the cases control group,including 14 eyes with macular hole(MH)and 2 eyes with epiretinal macular membrane(ERM).What is more,there were 8 vitreous fluid samples of the normal control group.The concentrations of HIF-1α in vitreous body were detected by enzyme-linked immunosorbent assay(ELISA).The written informed consent was obtained from each patient before any medical procedure. Results The concentrations of HIF-1α in vitreous body in the eyes of experimental group,the cases control group and the normal control group were(294.08±2.97)、(260.41±8.29)、(16.38±3.56)mg/L respectively,showing a significant difference among the three groups(F=248.77,P=0.00).The concentrations of HIF-1α in vitreous body were obviously increased in the eyes of experimental group in comparison with the normal control group and the cases control group respectively(t=22.25,P=0.00;t=2.70,P=0.00 respectively).The concentrations of HIF-1α in vitreous body were increased in the eyes of the cases control group in comparison with the normal control group eyes(t=14.21,P=0.00).There was inferior correlation between the concentrations of HIF-1α in vitreous body in the eyes of experimental group and the course of ophthalmopathy. Conclusion Intravitreous HIF-1α level regulates in the eyes with proliferative vitreo-retinal diseases. Expression of HIF-1α in vitreous had inferior correlation with the course of ophthalmopathy. HIF-1α may play an important role in the pathogenesis of proliferative vitreo-retinal diseases.
8.Clinical Features and Prognosis of Acute Ischemic Stroke Patients with Different Infarction Patterns

Objective To investigate the clinical features and prognosis of acute ischemic stroke patients with different infarction patterns.Methods87 patients who underwent diffusion weighted imaging (DWI) within 6 h after symptom onset and visualized acute cerebral infarcts within the middle cerebral artery territory were prospectively studied.According to DWI lesion patterns,the patients were divided into 6 groups.Before MR was performed and 24 h after onset,scores of National Institutes of Health Stroke Scale (NIHSS) were recorded.3 months after onset,scores of modified Ranking scale(mRS) were obtained.ResultsThe baseline NIHSS,infarction volume,occlusion of relevant internal carotid artery or middle cerebral artery and clinical outcome were significantly different among 6 groups(P<0.05).ConclusionDifferent infarction patterns of acute ischemic stroke were associated with clinical symptoms,vascular status,and clinical outcome,that may help to guide treatment and judge prognosis.
9.Prognosis of Patients with Acute Internal Carotid Artery or Middle Cerebral Artery Occlusion

Objective To investigate prognosis of acute ischemic patients with internal carotid artery or middle cerebral artery occlusion and define predictors of clinical outcome.MethodsThe clinical and radiological data of 48 stroke patients who underwent magnetic resonance angiography (MRA) within 6 hours of symptom onset and visualized internal carotid artery or middle cerebral artery occlusion on early MRA were analyzed. 90 days after onset, modified Ranking Scale (mRS) scores were obtained and factors related with prognosis were defined by logistic regression analysis.ResultsOf 48 patients, 17 cases (35.4%) got good clinical outcome (mRS 0~2), 31 cases (64.6%) got poor clinical outcome (mRS 3~6) with 7 cases (14.6%) died. In multivariate logistic regression analysis, only large infarction ( OR=21.1, 95%CI 1.4~314.2, P=0.027) and hyperglycaemia ( OR=5.1, 95%CI 1.3~19.8, P=0.019) were independent predictors of poor outcome.ConclusionClinical outcomes of most acute stroke patients with internal carotid artery or middle cerebral artery occlusion are poor. Large infarction and hyperglycaemia are independent predictors of clinical outcome.
10.Effect of Electroacupuncture on Behaviours and Hippocampal Microvascular A? Deposition in 7-month-old APP/PSI Double Transgenic Mice

Objective To explore one mechanism of therapeutic action of electroacupuncture on Alzheimer’s disease (AD), that is, improving the way to eliminate cerebral microvascular Ab by investigating the effect of electroacupuncture on hippocampal microvascular Ab deposition and learning and memory abilities in APP/PSI double transgenic mice. Method Twenty-four 7-month-old male APP/PSI double transgenic mice were randomized into model and electroacupuncture groups, 12 mice each. Same sex transgene-negative littermate mice (12 mice) constituted a blank control group. The electroacupuncture group received electric acupuncture at points Baihui (GV20) and Yongquan (KI1), 15 min once every other day, for a total of six weeks. After treatment, mouse learning and memory abilities were tested using the Morris water maze. The expressions of Ab 1-40 and Ab 1-42 in the hippocampal microvascular wall and senile plaque were determined by immunohistochemical method. Hippocampal microvascular Ab-positive expression was semi-quantitatively analyzed using the Imagine Pro Plus software. Result The Morris water maze test showed that escape latency lengthened (P<0.05), and the number of crossing platform and swimming time in the platform quadrant decreased (P<0.05) in the model group compared with the blank control group. Escape latency shortened (P<0.05), and the number of crossing platform and swimming time in the platform quadrant increased (P<0.05) in the electroacupuncture group compared with the model group. The immunohistochemical results showed that hippocampal microvascular Ab 1-42 and Ab 1-40 integral optical densities were higher in the model group than in the blank control group (P<0.05) and senile plaques appeared in the hippocampus. Hippocampal microvascular Ab 1-42 and Ab 1-40 integral optical densities were lower in the in the electroacupuncture group than in the model group (P<0.05). Conclusion Electroacupuncture reduces mouse learning and memory impairments and hippocampal microvascular Ab deposition. Its mechanism may be that electroacupuncture improves the way of eliminating cerebral microvascular Ab to decrease cerebral Ab deposition.
