1.Advances in clinical research on autoimmune pancreatitis

In recent years,research on autoimmune pancreatitis (AIP) has gained significant progress.AIP is a special type of chronic pancreatitis which is mediated by an autoimmune mechanism.It is characterized by the infiltration of lymphocytes and plasma cells leading to pancreatic fibrosis and dysfunction.AIP can affect the bile ducts,salivary glands,kidneys and other organs outside the pancreas.The common manifestations include obstructive jaundice,abdominal pain,and weight loss,which are similar to pancreatic cancer.Steroid therapy is of significant effect.There is no consensus nowadays on the diagnostic criteria of AIP across the world.There are many studies on the pathological subtypes,imaging,serological markers and biopsy techniques on AIP,and research focuses on the differential diagnosis from pancreatic cancer.
2.Real-time Three-dimensional Speckle Tracking Imaging in Evaluating Global Left Ventricular Systolic Function in Patients with Type 2 Diabetes and Poor Blood Glucose Control

Purpose To evaluate three-dimensional strain changes of the left ventricle in patients with type 2 diabetes and poor blood glucose control using real-time three-dimensional speckle tracking imaging (RT3D-STI), and to assess the correlation between three-dimensional strains and hemoglobin A1C (HbA1c). Materials and Methods Sixty-nine patients with type 2 diabetes and forty-two control subjects were recruited in this study. The patients were divided into DM1 group (patients with well blood glucose control, HbA1c<6.5%) and DM2 group (patients with poor blood glucose control, HbA1c ≥ 6.5%) according to the glycosylated hemoglobin level. Parameters including global longitudinal strain (GLS), global circumferential strain (GCS), global radial strain (GRS), global area strain (GAS), left ventricular ejection fraction (LVEF), left ventricular end-diastolic volume (LVEDV), left ventricular end-systolic volume (LVESV) and left ventricular mass (LVM) were obtained from cardiac apex full volume images by using RT3D-STI. The parameters of three groups were compared, and the correlation between the cardiac parameters and HbA1c were analyzed. Results The GAS, GLS, GCS, GRS, LVEDV, LVESV, LVM and LVEF showed no statistic difference between DM1 group and control group (P>0.05). LVM of DM2 group was higher than that in control group (P<0.01), but GAS and GLS of DM2 group were lower than those in control group (P<0.05). LVM of DM2 group was higher than that in DM1 group (P<0.01), but GAS and GLS of DM2 group were lower than those in DM1 group (P<0.01). HbA1c was negative correlated with GAS and GLS (r= - 0.773 and 0.465, P<0.01). Conclusion The reduced GAS and GLS of the patients with type 2 diabetes and poor blood glucose control may indicate damaged myocardial function.
3.Far Infrared Thermography Analysis with Acoustic Neuroma

PurposeTo analyze the far infrared thermography characteristics before and after surgery in patients with acoustic neuroma.Materials and Methods Thirty-two patients with acoustic neuroma were included as observation group and 40 normal healthy individuals as control group. Un-cooled thermal imaging system (ATIR-M301B) was used with working temperature of 20-25℃. Craniofacial infrared images were collected to analyze temperature differences among different detection zones.Results Far infrared thermography revealed that there were no obvious temperature differences between both sides of supraorbital region, endocanthion region, frontal region and buccal region in the control group (P>0.05). The temperature differences in bilaterally symmetrical parts of supraorbital region, endocanthion region, frontal region, buccal region were significantly higher in observation group (t=1.557, 1.714, 1.483 and 1.569,P<0.05). The craniofacial temperatures of 32 patients changed after operation, and the differences reduced in supraorbital region, endocanthion region, frontal region, and buccal region (t=2.655, 2.462, 2.897 and 4.465,P<0.05).Conclusion Far-infrared thermography inspection can detect abnormal temperature changes.
4.Study on proliferation capability and human telomerase reverse transcriptase expression of bone marrow-derived mesenchymal stem cells from rheumatoid arthritis patients

Objective By comparing the proliferation and the human telomerase reverse transcriptase expression of bone marrow mesenchymal stem cells (BMSCs) from rheumatoid arthritis (RA) patients and normal human to explore the existence of BMSCs defects or variation in RA patients,and whether these cells could be used in autologous transplantation.Methods BMSCs were obtained from bone marrow of eight RA patients and healthy volunteers.The cells were isolated andcultured by gradient centrifugation and adherence cultivation,and the surface markers of BMSCs were identitfied by flow cytometertry.Firstly,BMSCs reproductive activity of the two groups were compared:the auxodrome and cell cycle of BMSCs were measured respectively,then the generation doubling time of cell colony and proliferation index were calculated in order to see whether there was a difference of the proliferation capability between the two groups.Furthermore,the hTERT were measured.Results BMSCs were no significant differences between morphology of RA patients and healthy control group.The cells were obtained proof MSCs by flow cytometry.The difference was not statistically significant (P>0.05),RA group and the healthy control group P3-generation cell population doubling time (4.7±0.3) d,(4.8±0.3) d.The difference was statistically significant (P<0.01),P6 generation of healthy control group cell population has doubling time (5.7±0.4) d,RA group (6.3±0.3) d,buut the group RA BMSCs population doubling time is longer than the healthy control group.The difference was not statistically significant (P>0.05),on one hand of the healthy control group P3 BMSCs proliferation index (17.9±2.1)%,on the other hand RA group is (16.9±2.1)%.The difference was statistically significant (P<0.01).P6-generation BMSCs proliferation index in healthy control group (10.7±1.8)%,RA group (6.2±2.2)%,RA group proliferation index than the control group.The difference was also significant (P<0.01).The healthy control group P6 generation of MSCs hTERT expression was (10.6±1.5),RA group is (3.1±1.0),hTERT RA patients expression is lower compared with normal control group.Conclusion There is no significant difference in the proliferative capacity of the two groups at P3,but the BMSCs proliferation at P6 lags behind in the normal group in RA patients,which inferred that the senescences of the BMSCs of RA patients are earlier than healthy donors.The determination results of hTERT have shown that the difference between the two groups is statistically significant,and it proves that the premature aging may be associated with hTERT expression.
5.Changes of MEP and Neurologic Function after Erigeron beviscapus Preconditioning on Spinal Cord Ischemia-reperfusion Injury in Rabbits

Objective To observe the changes of MEP(motor evoked pontential) and neurologic function after Erigeron beviscapus(EBHM) preconditioning on spinal cord ischemia-reperfusion injury in rabbits, and provide substructural experimental theory.Methods Twenty-four domestic rabbits were randomly divided into four groups with six rabbits each. In sham group (group A) only the abdominal aorta was exposed. In the blank group (group B), the abdominal aorta was occluded 40 minute followed by reperfusion. In the methylprednisolone group (group C), it was occluded 30 minute before the abdominal, methylprednisolone (30 mg/kg) was injected into the rabbit by vein, which was used for two days after the surgeon. In the EBHM group (group D), EBHM(50 mg/kg) was injected into the rabbit by vein, the following procedure was the same as group C. MEP was measured in each observed point and neurologic function was observed everyday for 7days.Results At every observed point after SCI, the MEP changed regularly, and compared with group B, there was significant difference in groups C and D(P<0.05). 7 days later, the neurologic function changed significantly in each group(P<0.05).Conclusion MEP monitoring can reflect the early change on spinal cord ischemia-reperfusion injury. EBHM preconditioning can protect the rabbit from spinal cord ischemia reperfusion injury.
6.The contrast research between left ventricular three dimensional strain and coronary angiography for coronary disease patients without segmental ventricular wall motion abnormalities

Objective To evaluate the left ventricular diastolic function in coronary artery disease (CAD)patients without regional wall motion abnormality by real-time three-dimensional specke tracking imaging(RT3D-STI),and to investigate the value of RT3D-STI in detecting CAD.Methods According on coronary angiography,92 patients without regional wall motion abnormality were divided into two groups:control group(n = 21 ),without coronary artery stenosis;case group (n = 50 ),coronary stenosis group. Two dimensional images and the three-dimensional full volume dynamic images of left ventricle were collected.Routine parameters and end-systolic left ventricular globle longitudinal strain (GLS),globle circumferential strain (GCS),globle area strain (GAS),globle radial strain (GRS)and corresponding strain imaging diastolic index (SI-DI)were aquired,respectively.The parameters by RT3D-STI were analyzed to diagnose coronary artery stenosis.Results Compared with control group,there were no significant difference in routine parameters,GLS,GRS,GAS and GCS(P >0.05 ).Longitudinal SI-DI(L-SI-DI),area SI-DI(A-SI-DI),radial SI-DI (R-SI-DI),circumferential SI-DI (C-SI-DI)in case group were significantly reduced compared with control group (P <0.05).The sensitivity of L-SI-DI,A-SI-DI,R-SI-DI and C-SI-DI were 90.5%,81 .0%,85.7%,81 .0%,and the specificity of them were 82.0%,78.0%,68%,56%, respectively,L-SI-DI was the best in sensitivity and specificity.L-SI-DI,A-SI-DI,R-SI-DI were significantly correlated with Gensini score (r = -0.706,-0.547,-0.410 P < 0.05 ).Conclusions RT3D-STI can quantitatively assess the early changes of left ventricular diastolic function,which has important significance for early diagnosis of coronary artery stenosis.
7.Primary Study of Producing UMP by Overexpressing URA 5 and URA 3 Genes in Saccharomyces cerevisiae

To increase the biotransfomation efficiency from the orotic acid to the uridine 5'-monophosphate(UMP),URA5 gene encoding orotate phosphoribosytransferase was amplified from Saccharomyces cerevisiae BY4742 by PCR,then it was inserted into the expression vector pYX212(contained orotidine monophosphate decarboxylase gene URA3)and the pYX212-URA5 was transformed into Saccharomyces cerevisiae BJX12 by electroporation.The recombinant strain was elementarily used to convert orotic acid to UMP.The results showed that pYX212-URA5/BJX12 could accumulate 7mmol/L UMP from 32mmol/L orotic acid in 26h,significantly higher than both control groups pYX212/BJX12(2.7mmol/L) and BJX12(2.4 mmol/L).
8.Clinical efficacy of apatinib combined with S-1 in the treatment of advanced esophageal cancer

Objective:To explore the efficacy of apatinib combined with S-1 capsule in the treatment of patients with advanced recurrent and metastatic esophageal cancer.Methods:A total of 140 patients with advanced esophageal cancer were selected as test subjects from January 2017 to January 2019 in Shandong Tai′an Cancer Prophylaction-Therapeutic Hospital. These patients were randomly divided into observation group (72 cases) and control group (68 cases) using random number table method. The patients in the observation group were treated with oral apatinib combined with S-1 chemotherapy, and the patients in the control group was only given S-1 chemotherapy. The short-term and long-term efficacy and adverse reactions of the two groups were observed.Results:The objective remission rates of the observation group was 38.9% (28/72), higher than that in the control group (22.1%, 15/68), with a statistically significant difference ( χ2=4.655, P=0.031). The disease control rate of the observation group was 88.9% (64/72), higher than that in the control group (61.8%, 42/68), and there was a significant difference between the two groups ( χ2=13.993, P<0.001). The median progression-free survival of the observation group and the control group was 5.9 months and 2.7 months respectively, the median overall survival was 14.8 months and 7.9 months respectively, and there were significant differences between the two groups ( χ2=5.477, P=0.026; χ2=6.083, P=0.014). The adverse reactions of the two groups were mild, grade 1-2, mainly including fatigue, leukopenia, hand-foot syndrome, hypertension and proteinuria, with incidences of 59.7% (43/72), 50.0% (36/72), 8.3% (6/72), 12.5% (9/72), 9.7% (7/72) in the observation group, and 51.5% (35/68), 57.4% (39/68), 17.6% (12/68), 4.4% (3/68), 4.4% (3/68) in the control group, there were no significant differences between the two groups ( χ2=0.965, P=0.326; χ2=0.760, P=0.383; χ2=2.708, P=0.100; χ2=2.919, P=0.088; χ2=0.794, P=0.373). Conclusion:Apatinib combined with S-1 is effective, safe and tolerable in the treatment of recurrent and metastatic esophageal cancer.
9.Centerline analysis for the measurement of aortic diameter at proximal landing zone in type B aortic dissection:a preliminary study

Objective To investigate the feasibility of centerline measurement method in estimating aortic diameter at the proximal landing zone in Stanford B type aortic dissection. Methods CT angiography materials of 30 patients with type B aortic dissection were randomly selected from the hospital database (24 males with a median age of 49.5 years), which were retrospectively analyzed with multiplanar reformation (MPR) and centerline technique by two experts in vascular radiology. Difference between two measurement techniques was analyzed by using mixed linear model, and the agreement of measurements between two readers as well as between two techniques were evaluated by Bland-Altman plots. Results The diameters measured with MPR method by two experts were (29.73±2.99) mm and (29.86±2.95) mm respectively, while the diameters measured with centerline measurement method by two experts were (29.66 ±2.81) mm and (29.71 ±2.91) mm respectively. No statistically significant differences in the diameter value existed between the two measurement methods, although the results determined by centerline measurement method were more stable. Conclusion In determining aortic diameter at the proximal landing zone in Stanford B type aortic dissection, the centerline analysis provides a checking method for MPR measurement.
10.Sequence Analysis and Genotypes of Glutamate Rich Protein of Plasmodium falciparum Isolates from Different Malaria Endemic Areas in China

To sequence the gene encoding glutamate rich protein (GLURP) and identify the genotypes of geographically different Plasmodium falciparum (P. f ) isolates from China. Methods The gene of R2 repeat region of GLURP was amplified by nested polymerase chain reaction and cloned into T-vector. The nucleotide sequence of GLURP gene was determined by automatic sequencer (Dideoxy termination method) and analyzed by DNA Star software. Results At least 7different GLURPgenotypesranging from 600 bp to 1 500 bp were found in Yunnan and Hainan provinces. R2 region of GLURP gene consisted of several repeat units. Each repeat unit was composed of 19-20 residues which were shown to be highly conserved. GLURP gene was also size polymorphic due to differences in the number of repeat units, whereas the repeat sequence was conserved. Sequence analysis showed that DNA sequences and deduced amino acid sequences were highly homologous among the geographically dispersed isolates or various isolates from the same geographical region. No obvious differences were found in the GLURP gene sequences among geographically different isolates. Conclusion GLURP gene is highly structure conserved and size polymorphic, and so is useful in searching for malaria vaccine candidate antigen and developing a genotyping method for malaria research.
