1.Evaluation of effectiveness of antigen specific immunotherapy for patients with allergic rhinitis

Objective To evaluate the effectiveness of allergen specific immunotherapy (SIT) in the treatment of allergic rhinitis. Methods A total of 132 patients with seasonal ragweed allergic rhinitis were enrolled in this study. The patients were randomly divided into two groups. SIT group, in which 68 patients were treated with ragweed SIT for six months before July for 3 years. ST (symptomatic therapy) group, in which 64 patients were treated with ST during seasonal symptomatic period. The healthy control group included 40 subjects. The levels of serum total IgE (tIgE), specific IgE (sIgE), ECP, IgG and IL-4, IL-5, IFN-? were measured, and eosinophil numbers were counted. The symptom score, lung-function test and skin prick test were performed before and after the treatment. Results The elevated levels of tIgE, ECP, IL-4, IL-5 in the sera and eosinophil counts in peripheral blood of the patients were significantly reduced, whereas the levels of IgG and IFN-? were increased following SIT. The levels of serum sIgE were not changed in both SIT and ST groups. 30.88% of the patients were clinically cured, the effective rate was 41.18%, the improvement rate was 20.59% after SIT, and only in 7.39% of patients there was no clinical improvement. In 16.18% of patients there were mild side effects to SIT. In the ST group, there was on change in all the laboratory results and clinical symptoms, and 25% of the patients developed asthma. Conclusion SIT was able to improve the symptoms and laboratory results of the patients with allergic rhinitis. The results indicated that SIT may be an effective and safe therapy for patients with allergic rhinitis.
2.The effect of radiation of Nd:YAC laser on sealing ability in retrograde filling

Objective:To discuss the effect of radiation of Nd:YAG laser in apical retrograde filling. Methods: 50 similar roots were randomly divided into 5 groups.The anatomical crowns were removed at the cementoenamel junction and the remaining roots were treated endodontically with conventional methods before the apical 2 mm were cut and a dish-like hole made in the cutting surface by high speed round bur. Except the first group,each hole were radiated by the Nd:YAG laser for different time or with different patterns and after that,the glass ionomer cement were retrofilled into the hole. A microleakage measurements were carried out and the result were measured and recorded. The hole surface was also observed under electron microscopy. Objective To preliminarily study the effect of high concentration prolactin (PRL) on the immune system in schizo-phrenic patients by analyzing the changes of lymphocyte subsets and immunoglobulins (IgG ,IgM ,IgA) levels .Methods The flow cytometry was used to detect the T lymphocytes CD3+ ,CD4+ and CD8+ subsets ,NK cell percentages and CD4+ /CD8+ ratio re-spectively .The three humoral immune indicators(IgG ,IgM ,IgA) in peripheral blood were detected by using the immunoturbidimet-ric turbidimetric method .The data were analyzed by adopting the analysis of variance .Results Compared with the healthy control group and schizophrenia normal PRL group ,T lymphocytes CD4+ subsets percentages and CD4+ /CD8+ ratio as well as IgG were increased in the schizophrenia PRL increase group(P<0 .05) ,while the percentage of NK cells was decreased(P<0 .05) .Conclu-sion High concentration PRL may have a certain effect on the immune system by influencing the balance among lymphocyte sub-sets and the production of immunoglobulins .
4.Association between prodromal headache and propensity of rebleeding after aneurysmal subarachnoid hemorrhage

Objective To investigate the association between prodromal headache and the propensity of rebleeding after aneurysmal subarachnoid hemorrhage.Method Prodromal headache was defined as a sudden attack of severe headache lasting at least one hour with or without accompanying symptoms,not leading to a diagnosis of SAH 4 weeks before the confirmation of subarachnoid hemorrhage.Two hundred and nine patients with aneurymal subarachnoid hemorrhage were enrolled in this research.Age,gender,Hunt-Hess grade,D-dimer, (FBG),site and number of aneurysms and prodromal headache were recorded.Patients were divided into rebleeding group and non-rebleeding group.Comparing the above survey components between two groups was carried out by using case control study method.Age,aneurysm number,D-dimer and fibrinogen of the two groups were analyzed by t test.Prodromal headache,gender,site of aneurysms were analyzed by chi square test.Hunt- Hess grade was analyzed by rank test.Results Twenty-two(10.5%)patients had rebleeding.In the rebleeding group,10 patients(45.5%)had prodromal headache,while in the non-rebleeding group(n=187),21 patients (11.2%)had prodromal headache.Patients with prodromal headache had greater propensity of rebleeding compared with those without prodromal headache(P
5.Role of fibroblast growth factor-2 in neuropathic pain in rats

Objective To evaluate the role of fibroblast growth faetor-2 (FGF-2) in neuropathic pain (NP) in rats.Methods One hundred male Sprague-Dawley rats,aged 7 weeks,weighing 200-250 g,were randomly divided into 4 groups (n=25 each): sham operation group (group S),group NP,phosphate buffer solution (PBS) group and FGF-2 antibody group (group Ab).FGF-2 antibody 18 μg (40 μl) was injected intrathecally at 1,6,9,13,16 and 20 days after operation in group Ab,while the equal volume of PBS was injected intrathecally in group PBS.Spinal nerve injury was produced by exposing the sciatic nerve and its branches and ligation and transection of tibial nerve and common fibular nerve.Pain behavior was assessed at 1 day before operation and at 1,4,7,14 and 21 days after operation.The paw withdrawal mechanical threshold (PWMT) was measured.The animals were then sacrificed and the lumbar segment (L4-6) of the spinal cord was obtained for determination of the contents of tumor necrosis factor-α (TNF-α) and interleukin-6 (IL-6) in the spinal cord.Results Compared with group S,the PWMT was significantly decreased and the contents of TNF-α and IL-6 in the spinal cord was significantly increased in groups NP,PBS and Ab (P < 0.05).The PWMT was significantly higher and the contents of TNF-α and IL-6 in the spinal cord were significantly lower in group Ab than in groups NP and PBS (P < 0.05).Conclusion FGF-2 is involved in the occurrence and development of NP and induction of the inflammatory response in the rat spinal cord in involved in the mechanism.
7.Fingerprint of X-ray diffraction of Tibetan medicine dairy Nanhanshuishi and its application in processing by microwave.

The characteristic fingerprint of conventional dairy Nanhanshuishi was established by X-ray diffraction (XRD), based on similarity of caculation on public peaks by MATLAB software, and the feasibility of new dairy technology of microwave method was explored between XRD and the dissolution rate in artificial simulation gastric juices. The result showed that similarity of shared peak in XRD of conventional dairy Nanhanshuishi was > 95%, This XRD characteristic fingerprint of conventional dairy Nanhanshuishi had strong specificity, could be used to provide a reference for identification and quality evaluation. This study also showed that the similarity of microware dairy products and conventional dairy products was good, and the sample of microwave 15 min was the best, and new dairy method by the microwave could replace the traditional method.

8.Synergetic use of tripterygium wilfordii and aescine for treatment of posttraumatic acute lung injury

Objective To investigate the effect and mechanism of tripterygium wilfordii combined with aescine in treatment of acute lung injury (ALI) following trauma.Methods A total of 120 patients with posttraumatic ALI were divided into four groups:tripterygium wilfordii + aescine group (combined group),tripterygium wilfordii group,aescine group,and control group according to random number table,with 30 patients per group.Regardless of the same conventional therapy,the patients in combined group were treated by orally or gastrically administered tripterygium wilfordii multiglycoside as well as aescine intravenously,once a day for 12 days; on the contrary,the same method but only with tripterygium wilfordii or aescine was respectively performed in tripterygium wilfordii and aescine groups.Blood gas analysis before and after treatment,length of ICU stay,fatality rate and incidence of ARDS in each group were observed.Changes in levels of inflammatory mediators (TNF-oα,IL-1,IL-6 IL-8,etc)were detected at days 4,8,and 12 after medication.Results Shorter length of ICU stay and lower ARDS incidence as well as 12-day fatality rate were detected in each treatment group as compared with control group,especially in combined group (P < 0.05).Respiratory rate,PaO2 and oxygenation index (PaO2/FiO2) in each treatment group presented better amelioration than those in control group and especially better results were observed in combined group (P < 0.05).Plasma levels of TNF-α,IL-1,IL-6,and IL-8 in each treatment group were lower than those in control group (P < 0.05).Conclusions Tripterygium wilfordii and aescine havetherapeutic effect on posttraumatic ALI.Moreover,synergetic use of the two drugs achieves synergistic effect and better outcome.
9.Application of rapid immunohistochemical staining technique in intraoperative frozen section diagnosis of thyroid neoplasm

ObjectiveTo investigate the application of rapid immunohistochemical staining technique in intraoperative frozen section diagnosis of thyroid neoplasm.Methods MaxVision one-step rapid immunohistochemical staining technique was used to detect the expression of CK19,HBME-1,and Gal-3 in frozen section of papillary thyroid carcinoma(PTC)andthyroid benign lesions.MaxVision conventional immunohistochemistry of frozen remaining tissue was served as control.ResultsMaxVision one-step rapid immunohistochemical staining technique could be completed in 20 minutes.The positive localizations of three markers detected by rapid immunohistochemistry were similar to conventional immunohistochemistry, in general.The expression of CK19 was located in cytoplasm and cellular membrane.Gal-3 and HBME-1 were mainly detected in follicular luminal border and/or surface of papilla. The staining intensity in rapid immunohistochemistry was stronger than that in conventional immunohistochemistry. The positive rates of CK19,HBME-1,and Gal-3 by rapid immunohistochemistry in frozen sections were: 0 (0/28),10.7 % (3/28),0 (0/28),respectively,for benign lesions (nodular goiter,Hashimoto thyroiditis,thyroid adenoma); and 94.9 %(37/39),92.3 % (36/39),92.3 % (36/39),respectively,for PTC.The expression of three markers between thyroid benign lesions and PTC had a significant difference (x2 =59.326,55.861,44.605,all P < 0.001).In benign lesions,the rate of same case with two and more positive markers was 0,while in PTC it was 100 % and significantly different (x2 =67.000,P < 0.05).ConclusionMaxVision one-step rapid immunohistochemical staining technique could be applied in intraoperative frozen section diagnosis.Detecting CK19,HBME-1,and Gal-3 expression in intraoperative frozen section has an auxiliary value for diagnosis of PTC.
10.Diagnosis and treatment of traumatic diaphragmatic rupture

Objective To probe timely diagnosis and surgical intervention of traumatic diaphragmatic rupture(TDR). Methods The clinical data of 161 patients with TDR treated surgically in our department during the past 17 years were analyzed retrospectively in respects of diagnostic methods,accuracy of preoperative judgment of TDR,incidence of diaphragmatic hernia,surgical procedures and outcome,etc. Results There were 139 males and 22 females at a mean age of 32.4 years(9-84 years),with average ISS of 27.8 points(13-66 points).Of all patients,65.2%had shock at admission.For these 161 patients,36 suffered from blunt injuries and 125 from penetrating injuries.For diaphragmatic injury.preoperative diagnostic rate was 88.9%for blunt injuries and 78.4%for penetrating injuries (P>0.01).The incidence of diaphragmatic hernia was 94.4%in blunt injuries and 14.4%in penetrating injuries(P<0.05).In this series,thoracotomy was performed in 30 patients,laparotomy in 106,thoracotomy plus laparotomy in 18 and combined thoraco-laparotomy in 7,with overall fatality rate of 10.6%and a mean ISS of 41.6 points.The mortality rate was 22.2%in blunt injuries and 7.2%in penetrating injuries(P<0.01).The main causes for death were hemorrhagic shock and septic complications; Conclusions Blunt diaphragmatic injury can be diagnosed by radiographic signs of diaphragmatic hernia.According to"offside sign",which implies a thoracic wound with positive physical or radiological signs in the abdomen or in the thorax,penetrating diaphragmatic injury can be recognized.To deal with diaphragmatic hernia,it is important to judge the vitality of viscera.Penetrating injury has a relatively good prognosis.
