Objective To explore the regulatory mechanism of gastrin and motilin on sling fibers and clasp fibers of human lower esophageal sphincter.Methods Muscle strips of the sling and clasp fibers from human lower esophageal sphincter and the circular layers of the esophagus and gastric fundus were obtained from patients with subtotal esophagectomy.Through the isolated tissue tension detection technique and pharmacological method,the concentration-response curves of the strips responding to gastrin and motilin were constructed,from which E_(max)s were derived.Results All strips contracted to gastrin and motilin with different rules and different intensity in a concentration-dependent fashion.Sling fibers' E_(max)value(4.91?(0.95 mN/mm~(2))) to gastrin was significantly higher than clasp fibers'(0.72?(0.14 mN/mm~(2))).Otherwise,in response to motilin,both sling fibers and clasp fibers contracted to motilin intensively(3.61?(0.65 mN/mm~(2))and 2.64?(0.33 mN/mm~(2))).Conclusions The sling fibers and clasp fibers have different responses to gastrin and motilin.

Traditional Chinese medicine and traditional Chinese medi- cine preparations for the treatment of RA have its unique advantages, show the characteristics of overall adjustment, multi-level and multiple targets, and also can alleviate and against side effects of western medicine. In recent years, more and more get people's atten- tion. This paper reviewed the research progress and treatment features of commonly used therapeutic agents for the treatment of RA in recent years, which provides reference and basis for future medicine anti-RA.

AIM: To investigate the clinical therapeutic effects of human umbilical vein (HUV)implantation and mitomycin C (MMC) in non-penetrating trabecular surgery (NPTS). METHODS:A total of 32 patients (46 eyes) with uncontrolled primary open angle glaucoma (POAG) were divided into two groups: HUV+MMC group (n=25), SKGEL+MMC group (n=21). The procedure commenced with the creation of a limbus based conjunctival flap. After the dissection of a superficial limbus based rectangular scleral flap, MMC(0.4mg/mL) was used superior and inferior surface of the superficial scleral flap for three minutes. A second limbus based scleral flap was carefully dissected beneath the previous one towards the choroid. Schlemm's canal was deroofed during the extension of the deep scleral flap toits limbal edges. HUV or SKGEL fixed on the bed of sclera in experimental group. Postoperative examinations were performed at 1 week,2,4 weeks;2,6,12 months. IOP,best-corrected visual acuity(BCVA), functional blebs and success rate were examined. RESULTS: There were no statistically differences with postoperative IOP in HUV+MMC group and SKGEL+MMC group (P>0.05) during 1 week to 12 months. There was no difference with postoperative function blebs and the change of BCVA during 1 week to 12 months between HUV+MMC group and SKGEL+MMC group (P>0.05).At 12 months after surgery, the success rate was 84% in HUV+MMC group,86% in SKGEL+MMC group. CONCLUSION: The application of HUV in NPTS can prevent the adhesion of filtering channel and it can improve the success rate of NPTS. Compared with SKGEL, HUV has lower price. So it is a better implant.

Objective To evaluate the clinical diagnosis,treatment and prognostic factors of kidney clear cell renal carcinoma(KIRC)in international sample database.Methods From 1998 to 2012,consecutive patients with KIRC who diagnosed and treated in TCGA organizations were enrolled.Clinical characteristics,objective response and survival time were evaluated,and correlation analysis with lncRNA urothelial cancer associated 1 (UCA1)gene was performed.Results 512 patients with KIRC were enrolled.35.4% of patients were female,59.6% of stage Ⅰ -Ⅱ, 90.0% of white and 45.1% of grade 1 -2.Comprehensive treatment was consistent with the clinical guidelines. Significant correlation was found between UCA1 expression and 4 mRNA subtype,30 genes mRNA expression, mir -101 -1 expression and PBRM1 mutation.And patients with UCA1 overexpression could achieve poor prognosis. Conclusion Diagnosis and treatment of the international TCGA -KIRC research meets clinical guidelines.UCA1 overexpression is an important poor prognostic factor.Combined with the clinical relevance of other important driver genes,UCA1 may be significantly valuable for further study.

Objective To provide a theoretical basis for the pulmonary enteric therapy in treating intestinal dysbacteriosis complicating allergic asthma according to the traditional theory of Lung and Large intestine are interiorly-exteriorly.Methods The experiment included the blank group,model group,positive control group and three kinds of traditional Chinese medicine compound administration group;the intestinal dysbacteriosis in SD rats was duplicated by adopting cefoperazone and candida albicans gavage.On this basis ovalbumin injection sensitization and nebulization were applied to induce asthma.Meanwhile Jia-Wei-ShengJiang Powder(SJS),Guizhi-Jia-Houpu-Xingzi Decoction(GHX) and Zeng-Ye-Cheng-Qi Decoction(ZYCQ) were orally administered for treatment of lung-intestine combine therapy,lung therapy and intestine therapy.The respiratory function was analyzed by using the Buxco whole body plethysmography.The variety of intestinal flora were analyzed by using 16SrDNA.Results Analyzing the specieces composition of intestinal flora in each group by the OUT value found that the recovery of Lactobacillus in each group was close to that in the blank group and better than that in the positive group,meanwhile,3 treatment groups all could promote the recovery of Proteobacteria and bacteroidetes,but the lung-intestine combine therapy had optimal effect,the Alpha diversity analysis found that the intestinal microflora diversities from high to low were in turn the intestinal therapy group,lung therapy group,lungintestine combined therapy group and model group;the respiratory frequency and airway hyperreactivity in the model group was 260.56(1.28),which in the lung-intestine combined therapy group,lung therapy and intestine therapy after treatment were 141.79 (0.93),137.25 (1.30) and 203.16 (0.84) respectively,the rat respiratory frequency in each treatment group was lower than that in the model group,while the airway hyperreactivity only in the lung-intestine combined therapy group and intestinal therapy group was lower than that in the model group,moreover showing statistical significance.Conclusion The lung-intestine combine therapy method for treating rat intestinal dysbacteriosis complicating allergic asthma is superior to simple lung therapy method or intestine therapy method.

Objective To explore the interventional effect of two different methods in the prevention of chemotherapy phlebitis caused by 5-fluorouracil.Methods Eighty cases of patients treated with cisplatin+5-fluorouracil (PF) chemotherapy in the First Affiliated Hospital of Guangxi Medical University from May 2015 to July 2016 were selected and divided into the observation group and control group according to admission time,40 cases in each group.The observation group was smeared with 2% anisodamine along the direction of vein puncture after peripheral venous puncture success,while the control group was smeared with mucopolysaccharide polysulphate emulsion paste.The occurrence of phlebitis and degree of local pain after intervention were compared between the two groups.The blood biochemical indexes and inflammatory factors levels before and after intervention were compared too.Results After the intervention,the distribution of phlebitis level and local pain degree had statistical difference between the two groups (Z=2.57,2.53;P=0.01);the observation group was superior to the control group in the aspects of phlebitis lesion degree and local pain alleviation;WBC,MPV and PDW in the observation group had statistical difference between before and after intervention;WBC,PLT and PCT in the control group had statistical difference between before and after intervention (P<0.05);PLT count was positively correlated with PCT in the two groups (r=0.952,P<0.05);the CRP level and WBC count after intervention had no statistical difference between the two groups(P>0.05).Conclusion 2% anisodamine can effectively prevent and alleviate the occurrence of 5-FU caused chemotherapy phlebitis and local pain.At the same time,PLT can provide a basis for preventing chemotherapy phlebitis.

Objective To generate an autoimmune cholangitis animal model and investigate whether CXCR3 and its ligand were involved in the pathogenesis of primary biliary cirrhosis (PBC). Methods Female C57BL/6 wild-type (WT) mice and CXCR3 knockout (CXCR3-/-) mice were injected with 5 mg/kg of poly I:C intra-peritoneally twice a week for 24 consecutive weeks. Liver specimens were collected to evaluate the degree of cell infiltration. AMA was assayed by ELISA. Differences in pathology were compared between CXCR3-/- mice and wild-type. Student's test was used to assess the differences. Results Anti-mitochon-drial antibody (AMA) and alkaline phosphatase (ALP) level were elevated significantly in the sera of all poly Ⅰ:C injected mice compared with control mice. AMA titers in serum were increased in the poly I:C injected WT mice compared with that of the control mice at week 8, 16, and 24 respectively (0.70±0.41 vs 0.17±0.04,0.48±0.35 vs 0.19±0.07, 0.69±0.44 w 0.20±0.06, P＜0.01). There was no significant difference between AMA titers in the serum of WT PBC mice and CXCR3-/- PBC mice (0.70±0.41 vs 0.56±0.25, 0.48±0.35 vs 0.46±0.35, 0.69±0.44 vs 0.85±0.34). Serum alkaline phosphatase (ALP) levels were raised among the poly I:Cinjected WT mice compared with WT controls (115±33) vs (65±7) U/ml; (119±32) vs (70±9) U/ml; (133±52) vs (77±10) U/ml. The serum ALP levels in CXCR3-/- PBC mice were (106±29), (112±29)and (122±60) U/ml at week 8, 16 and 24 respectively. There was no significant difference in ALP level between WT and CXCR3-/- mice PBC model. Considerable numbers of infiltrating cells were detected at the portal areas 8weeks after poly I:C injection, which progressed up to 24 weeks. At week 24, the interlobular bile ducts were lost and bile-plug were evident. Compared to WT mice model, this results revealed that CXCR3-/- mice, had fewer foci of inflammation and significant reduction in total inflammatory cells in their livers than those of the WT mice at 8, 16 and 24 weeks after injection of poly I:C. At week 24, there were no cholestasis orgranulomas in CXCR3-/- mice of PBC models. Compared to the control model, CXCL10 serum level was increased in PBC model. With the disease progression, CXCL10 serum level was elevated gradually. In comparison with wild mice, CXCR3-/- mice model had a higher serum level of CXCL10. At week 24, there was significant difference of CXCL10 serum level between wild-type mice and CXCR3-/- mice model. Conclusion In conclusion, these findings indicate that, CXCR3-/- mice might have a delayed onset and ultimately could not recruit sufficient effector cells to the liver for inflammation development.

Objective To determine the effects and significance of endotoxin preconditioning on iNOS activity,nNOS activity in rat cerebral cortex,hippocampus and cerebella region after global brain ischemia/reperfusion.Methods Global ischemia for 20 min was made by four-vessel occlusion model(4-vo) in 60 Wistar rats,among which 30 were injected of 20 ?g/kg MPL through caudal vein 24 h before model establishment.Another 8 rats undergoing sham operation served as controls.The dynamic change of iNOS activity,nNOS activity and neuron density of the cerebral cortex,hippocampus and cerebella region were observed at 1,4,8 h after reperfusion. Results The activity of iNOS and nNOS decreased significantly after endotoxin preconditioning in the regions mentioned above,as compared with that of rats only undergoing the ischemia/reperfusion.The neuron number in rat hippocampus decreased after ischemia/reperfusion,but no significant difference was found between control and endotoxin preconditioning groups.Conclusion The activity of iNOS and nNOS changed significantly after global brain ischemia/reperfusion.That endotoxin preconditioning decreased iNOS and nNOS activities may be the protective mechanism.

Objective To investigate the expression of survivin (an apoptosis inhibitor) in thyroid carcinoma and its relationship with vascular endothelial growth factor (VEGF) expression. Methods Sixty-eight cases of thyroid carcinoma, 12 cases of thyroid adenoma and 10 cases of normal thyroid tissue were involved. Immunohistochemistry (SABC method) was used to detect the expression of survivin, caspase-3 and VEGF, and then their relationship with the major clinicalpathologicalparametersofthyroidcarcinoma was analysed. Results No survivin was expressedinnormalthyroidtissue,butsurvivinwasweakly expressed in thyroid adenoma (16.7%) and strongly expressed in thyroid carcinoma (57.4%); caspase-3 was obviously expressed in three kinds of tissue (70.0%, 75.0%, 69.1%), and VEGF was expressed in 75.0% of thyroid carcinoma and 41.7% of thyroid adenoma.
