OBJECTIVE To investigate the distribution of familial hepatitis B virus (HBV) genotypes and analyze the relationship between genotypes and clinical outcome. METHODS A total of 136 cases of familial hepatitis B were involved in this study, including 25 cases of asymptomatiec HBV carriers (ASC),85 cases of chronic hepatits B (CHB),24 cases of liver cirrhosis (LC) and 4 cases of hepatocellular carcinoma (HCC). Nested polymerase chain reaction-restriction fragment length polymorphism (ntPCR-RFLP)was used to identify the genotypes of HBV. RESULTS There were 124 cases with hepatitis genotype C,7 cases with genotypes B+C mixed,3 cases with genotype B and 2 cases that did not conformed to be any of genotypes from A to H. Genotype C in the HCC was predominant genotype (100%),next in the LC(96%),CHB (95.3%) and ASC(66.7%).Compared with genotype C,genotypes B and B+C only existed in ASC and CHB,the proportion was 9.5% vs 23.8% and 1.2% vs 2.3%,respectively. There were no other HBV genotypes from A to H seen in familial hepatitis B in Shanxi province. CONCLUSIONS HBV genotype C is the major one in familial hepatitis B in Shanxi. There are a few genotypes B and B+C existed. Genotype C induces a relatively bad prognosis.

Objective To observe the change of ?-smooth muscle actin (?-SMA) expression in carbon tetrachloride (CCl4)-induced liver fibrosis in rats after treatment with Ganle keli. Methods Liver fibrosis rat model was induced by CCl4 twice a week for 12 weeks. Ganle keli was used daily intragastric administration from the 7th week. Contents of NO, NOS and iNOS were determined by absorption spectrometry. Liver samples were collected after experiments and stained by immunohistochemistry of ?-SMA. Moreover ?-SMAmRNA expression in liver tissue was detected by RT-PCR technology. Results Ganle keli could significantly reduce NO, NOS and iNOS contents, effectively prevent the expression of ?-SMA, and significantly reduce ?-SMA mRNA in liver fibrosis rats. Conclusion Ganle keli can effectively inhibit the expression of ?-SMA in CCl4-induced fibrotic liver in rats.

BACKGROUND:Percutaneous vertebroplasty for treatment of osteoporotic vertebral compression fractures has achieved very good results.
OBJECTIVE:To discuss and resolve some problems related to thoracic and lumbar vertebroplasty.
METHODS:225 patients (78 males and 147 females) aged 53 to 92 years old were included in this study. They al accepted percutaneous vertebrolplasty and we observed and made a record about some questions related this surgery during perioperative period.
RESULTS AND CONCLUSION:Six cases (2.7%) missed diagnosis. More than two-third of the compression degree were found in sixteen cases (7.1%). Forty-five (12.8%) vertebrae suffered from bone cement leakage in twenty-nine cases (12.9%). Recurrent fractures appeared in ten cases (4.4%). Multiple vertebrae fractures appeared in seventy-nine cases (35.1%). (1) Strategies for missed diagnosis:conduct preoperative physical examination careful y;avoid missing the point of pain;increase MRI scanning when necessary. (2) Coping strategies for severe vertebral fractures:place the needle into the paral el endplates as far as possibly;puncture along the lumbar spine pedicle base and the lateral thoracic pedicle. (3) Coping strategies for cement leakage:when cement leakage over the lower edge of the end plate occurred in operation, we should immediately put the C-arm X-ray machine into anteroposterior position to determine the orientation of the leakage and stop injecting;we should continue to inject the bone cement into the no leak-side to ensure the fil ing amount of vertebral bone cement. (4) Coping strategies for recurrent fractures:reoperation. (5) Coping strategies for multiple fractures:patients with multiple fractures often have a feature of poor constitution and more complications. So, it is necessary to shorten the operation time, and determine unilateral or bilateral puncture ways depending on the degree of vertebral compression to save operation time.

OBJECTIVE:
To investigate effect and safty evaluation of stellate ganglion catheter retention with discontinuous block on sudden deafness.
METHOD:
One hundred and twenty-six patiens with sudden monaural deafness were randomly divided into Catheterp and block and control groups with 42 cases in each group. All patients' throats were given conventional blood activating drugs, hormone and hyperbaric oxygen therapy. stellate ganglion puncture retained catheter were administrated to the patients in catheter group followed by ropivacaine block 1 times/day, block group stellate ganglion puncture and ropivacaine block 1 times/day. The patients in control group were only received routine comprehensive treatment. Patients in both catheter group and block groups were treated by hyperbaric oxygen therapy after the block treatment. Curative effects of three groups were observed. The patients' satisfaction, heart rate, the chages of blood pressure before and after the block, detachment of tubes, and adverse drug reaction were recorded.
RESULT:
The effect of the treatment in both catheter group, block group was better than in control group (85.7%, 37 cases); 83.3%, 35 cases) vs 64.3%, 27 cases, P < 0.05). The satisfactory rate in the patients in catheter group was significantly higher than block group (83.3%, 35 cases vs 61.9%, 26 cases, P < 0.05). The heart rate and the blood pressure before and 5 minutes after catheterization in catheter group and block groupwere changed obviously. Moreover, no adverse drug reaction and detachment of tubes were observed.
CONCLUSION
It is a safe and effective administration of stellate ganglion catheter retention with interrupted ropivacaine block.

Chinese Journal of Clinical Oncology 2016;43(19):855-859
Objective:To retrospectively investigate the regulation of cervical and posterior to level V (PLV) lymph node metastasis on clinical target delineation in radiotherapy for nasopharyngeal carcinoma (NPC). Methods:A total of 428 NPC cases from February 2013 to April 2016 were subjected to enhanced CT scan from the base of the skull to the clavicle for pathological diagnosis. A deputy chief physician and an attending physician assessed the nodal distribution in each level in accordance with the RTOG guidelines proposed in 2013. The central point of the metastatic lymph nodes of PLV in the patients were recreated proportionally on the CT images of a stan-dard patient with N0 NPC in reference to the normal anatomy of the PLV area. SPSS 19.0 was used to analyze the correlation between PLV and the other levels. Moreover, the nodal location and characteristics of PLV were analyzed. Results:Among the 428 patients, 381 (89.0%) showed nodal involvement. The top four metastatic probabilities were presented as follows:Ⅱb (75.2%),Ⅶa (60.3%),Ⅱa (59.6%), andⅢ(42.0%). Up to 21 (4.9%) patients exhibited nodal involvement of PLV with 32 nodes. The mean vertical distance of all central points of PLV from the anterior border of the trapezius was 16 mm. Correlation analysis indicated the nodal involvement of PLV with the ipsilateral level Va (P=0.001). Conclusion:NPC showed a high probability of nodal metastasis. Nodes were mostly metasta-sized from the upper to the lower level, as well as from the proximal to the distal area. The leap metastasis rate was very low. The nod-al involvement of PLV correlated with the ipsilateral metastasis of level Va. Thus, the ipsilateral delineation of the posterior border of level V should be contoured to 25 mm far from the anterior surface of the trapezius during the nodal involvement of level Va.
