Objective To determine the minimum alveolar concentration(MAC) of sevoflurane without body movement during laryngeal mask airway(LMA)intubation in premature infants less than 37 weeks of corrected gestational age undergoing total inhalation general anesthesia induction. Methods Twenty-one ASA Ⅰ or Ⅱ premature infants less than 37 weeks of corrected gestational age undergoing elective inhalation general anesthesia were enrolled in this study. At first, the general anesthesia induction was started by inhaling 6 % sevoflurane. After the premature infant lost consciousness, the end tidal sevoflurane concentration(ET-sev)was adjusted to the predetermined concentration and maintained stable for 15 min. After that,LMA was inserted. The up-anddown sequential allocation was used to determine MAC.The initial ETsev was 2 %,which was increased or decreased by 1 gradient concentration in the next case according to the LMA insertion body movement response. The adjacent concentration gradient was 0.2%.The midpoint from th body movement response to non-body movement response was set as the balance point and the mean value of the concentrations of sevoflurane at all the balance points were calculated as MAC.Results The end tidal sevoflurane con centration without the body movement responses to LMA insertion was 1.71%.Conclusion The MAC of sevoflurane without the body movement responses to LMA insertion in premature infants less than 37 weeks of corrected gestational age is 1.71%,which is lower than that in the normal children and probably because imperfect central nervous system development in premature infants.

Objective (1) To determine the relationship between the MR signal intensity and the actual flow velocity under steady flow condition (2) to analyze the influence produced by the angle (?) between the flow direction and the velocity encoded gradient direction, and flip angle as well as section thickness on the velocity measurement under oblique flow condition (3) to develop a suitable protocol for using this method to measure volumetric flow rate in the transverse sinus system Methods Flow phantom, which simulated blood flow conditions in the transverse sinus system, consisted of a fluid filled cylinder and a bent tube with a 3 4 mm internal diameter A 1 5 T superconductive MR imager and VIGRE sequence were used for these studies A suitable protocol was based on consideration of the effects of (1) the accuracy of velocity and transverse area measurement of flow, and (2) signal to noise ratio (SNR) Results (1) Signal intensity (y) determined by MR and the actual flow velocity (x) showed straight line correlation, y=68 914x+357 206, R 2=0 998 (2) As the angle (?) increased, the transverse area of the signal determined by MR also increased, but the value of flow velocity decreased (3) As the flip angle increased, the SNR varied from 5 7 to 11 2 The maximum SNR was obtained with 30?flip angle (4) As the section thickness increased, the SNR and the transverse area of the signal determined by MR slightly increased Conclusions Phase contrast MR imaging is a practical method for measuring volumetric flow rates The angle (?) influenced the accuracy of flow velocity and the measurement of transverse area of flow whereas the flip angle and the section thickness substantially influenced the signal to noise ratio and the transverse area of flow

The finite element method was used for simulating the mechanical performance and fatigue safety of three different structures of Nitinol stent. According to the actual situation, after proposing reasonable assumptions and simplification, the geometry model and finite element model establishment, material mode selection and boundary condition setting are completed. The strain and fatigue life of different stent edges wide (omega) or strut angle (theta) are computed. The result can provide a valuable reference for the optimal design of stent.

Objective To investigate the effect of sepsis on vecuronium-induced inhibition of acetylcholine release in neuromuscular junction in rats. Methods Thirty-six adult male SPF SpragueDawley rats, aged 2-3 months, weighing 200-220 g,were randomly divided into 3 groups (n=12 each) using a random number table:control group (group C),sham operation group (group S) and sepsis group (group Sep).Sepsis was induced by cecum ligation and puncture (CLP) in rats anesthetized with intraperitoneal chloral hydrate 350 mg/kg.At 12 h after CLP,the sciatic nerve-pretibial muscle was prepared.Vecuronium was added to the culture medium with the final concentration of 0.08 μg/ml,and the sciatic nerve-pretibial muscle was incubated for 15 min. Before and after administration, evoked endplate potentials (EPPs) and miniature endplate potentiais (MEPPs) were recorded by using intracellular microelectrode.EPP/MEPP ratio was calculated. Results Compared to C and S groups,EPPs,MEPPs and EPP/MEPP ratio were significantly increased before and after administration in group Sep.EPPs,MEPPs and EPP/MEPP ratio were significantly lower after administration than before administration in the three groups. Conclusion Sepsis can promote acetylcholine release in neuromuscular junction, thus weakening vecuronium-induced inhibition of acetylcholine release in neuromuscular junction in rats.

Abstract?AlM: To compare the in- the- bag lOL stability of different size of continuous curvilinear capsulorhexis ( CCC) in super high myopic eyes with cataract underwent phacoemulsification.?METHODS: A total of fourteen cataract patients with bilateral super high myopia were included, Phaco+lOL implantation were performed on both eyes, one eye was randomly classified into 5mm diameter CCC observation group, the fellow eye was 6mm diameter CCC observation group. Cataract extraction combined with in-the-bag intraocular lens implantation ( lOL ) with the type of hydrophilic acrylic aspheric intraocular lens ( MCX11 ) by well experienced surgeon. The operation was running smoothly, the next day after operation, all patients were confirmed by lmage-pro plus6. 0 image analysis software for the measurement of main meridian sac diameter with target capsulorhexis diameter no more than ±0. 2mm. Slit lamp examination of lOL shape and position, changes of anterior capculorhexis edge, refraction, anterior chamber depth was measured and observed of all eyes after operation 1wk;1, 3, 6mo.? RESULTS: Compared with postoperation 1wk, the former sac diameter of two groups were slightly smaller at postoperation 1mo, with no statistically significant difference between two groups. 5mm diameter CCC observation group had slightly hyperopic shift in follow-up 1-3mo, 6mm diameter CCC observation group had hyperopic shift in follow-up 1mo, and getting stable after 1mo. Refraction change was related to anterior chamber depth changes. 5mm diameter CCC observation group had 3 minor loop folding in follw-up 3mo.?CONCLUSlON:Relatively smaller continuous curvilinear capsulorhexis in super high myopic eyes underwent cataract surgery may cause a tendency of uneven construction or effective lens position change of in-the-bag lOL. Unusual refraction change or shift after operation 1mo could suggest instability of lOL, early noticing or interruption could prevent further complications.
10.Caveolin-1 is involved in high glucose accelerated human glomerular mesangial cell senescence.
The Korean Journal of Internal Medicine 2017;32(5):883-889
BACKGROUND/AIMS:: We demonstrated the role of caveolin-1 involved in high glucose (HG)-induced glomerular mesangial cells (GMCs) senescence. METHODS:: HG was used to stimulate GMCs. The telomere lengths were analyzed by Southern blot. β-Galactosidase staining was determined. The expressions of caveolin-1 and P53 proteins were determined by Western blot. RESULTS:: Treatment with high concentrations of glucose induced GMC senescence accompanied by shortened telomere length and increase of β-galactosidase staining as well as P53 protein, which was abrogated after application of caveolin-1-siRNA. CONCLUSIONS:: This study proved that HG induced cell senescence in GMCs. The caveolin-1 is involved in HG-induced mesangial cell senescence, and blocking caveolin-1 significantly reduced cell senescence. The effect of caveolin-1 is mediated by P53 pathway.
