Background:14 C-urea breath test(14 C-UBT)is widely used for diagnosis of Helicobacter pylori(Hp)infection owing to its noninvasiveness and high sensitivity and specificity. Nevertheless,the presence of false-negative result can influence the diagnostic accuracy. Aims:To investigate the possible causes of false-negative 14 C-UBT for improving the accuracy of diagnosis. Methods:Eight-two cases with confirmed false-negative 14 C-UBT and 813 controls with true-positive 14 C-UBT at the People’s Hospital of Xinjiang Uygur Autonomous Region from Jan. 2014 to Aug. 2014 were enrolled. Patients in both groups were diagnosed as positive for Hp infection by Warthin-Starry silver staining combined with Hp stool antigen test. Univariate and multivariate analysis were performed to screen the factors related with false-negative 14 C-UBT. Results:In univariate analysis,5 variables were significantly different between case group and control group(P ﹤ 0. 05),and were taken into the multivariate analysis. Logistic stepwise regression analysis revealed that bile reflux( OR = 3. 961,P ﹤0. 001),post subtotal gastrectomy(OR = 9. 734,P ﹤ 0. 001),type Ⅱ Hp infection(OR = 1. 892,P = 0. 012)and upper gastrointestinal bleeding( OR = 4. 979,P ﹤ 0. 001 ) were the independent risk factors for false-negative 14 C-UBT. Conclusions:Bile reflux,upper gastrointestinal bleeding,post subtotal gastrectomy and type Ⅱ Hp infection might be the influential factors for false-negative 14 C-UBT. Combined tests for Hp infection should be suggested in patients with negative 14 C-UBT who had undergone subtotal gastrectomy or complicated with bile reflux or upper gastrointestinal bleeding.

Background: Helicobacter pylori (Hp) infection is associated with a variety of gastrointestinal diseases.Studies have shown that Hp eradication rate may be affected by the disease type and stage of disease progression.Aims: To investigate the influence of endoscopic ulcer stage on Hp eradication rate.Methods: A total of 423 peptic ulcer (PU) patients with Hp infection from January 2015 to July 2016 at the People's Hospital of Xinjiang Uygur Autonomous Region were enrolled.Of them,133 were gastric ulcer (GU) and 255 were duodenal ulcer (DU).A 14-day bismuth quadruple therapy was given for Hp eradication,and the eradication rates were analyzed with the endoscopic ulcer stage.Univariate analysis was conducted to identify factors influencing the eradication rate.Results: There was no significant difference in Hp eradication rates between GU and DU groups (82.7％ vs.83.9％,P>0.05).The eradication rates according to endoscopic ulcer stage were significantly different in GU group (active stage vs.inactive stage: 89.0％ vs.75.0％,P＜0.05);but in DU group,the eradication rate in active stage was similar to that in inactive stage (85.6％ vs.83.0％,P>0.05).Univariate analysis revealed that ulcer stage was correlated with the success rate of eradication;the eradication rate in active stage was significantly higher than that in inactive stage (87.8％ vs.80.3％,P＜0.05).Conclusions: Endoscopic ulcer stage is correlated with the Hp eradication rate in PU,especially GU patients.Hp eradication therapy performed in patients with active PU could yield a higher eradication rate.

Background:It has been demonstrated that serum perinuclear antineutrophil cytoplasmic antibody( pANCA)occurred significantly more often in ulcerative colitis( UC ) than in Crohn ’s disease( CD ),so it is of great importance for differential diagnosis of UC and CD. Aims:To study the correlation of serum pANCA with efficacy of aminosalicylic acids ( ASA)on UC. Methods:A retrospective study was conducted in 70 mild to moderate active UC patients admitted between Jan. 2007 and Dec. 2013 at People’s Hospital of Xinjiang Uygur Autonomous Region,all of them received oral mesalazine therapy,35 were positive and 35 were negative for serum pANCA. The efficacy was compared between the two groups. Results:After a four-week oral mesalazine therapy,the remission rate and overall efficacy were significantly higher in pANCA-positive group than in pANCA-negative group( remission rate:80. 0% us. 54. 3%,P﹤0. 05;overall efficacy:94. 3% us. 77. 1%,P﹤0. 05). Conclusions:Mesalazine is more effective in serum pANCA-positive UC patients. Being a specific immunological biomarker of UC,pANCA might be an indicator for predicting the therapeutic efficacy of ASA in UC patients.

Helicobacter pylori(Hp)is one of the most frequently seen pathogenic bacteria in human beings. Hp infection is closely related to the occurrence and development of chronic gastritis,peptic ulcer and gastric cancer. Therefore,eradication of Hp is very important for the prevention and treatment of such diseases. This article reviewed the progress in research on therapeutic regimen for Hp eradication.

Objective To evaluate the value of color ultrasound-guided biopsy in diagnosing breast masses. Methods The breast masses of 20 patients were punctured with automatic biopsy gun guided by color ultrasound and analysed by pathological examination. Results The mass tissues of all 20 patients were acquired successfully and diagnosed by pathology. Compared with the results of postoperative pathology,the accuracy rate of preoperative pathological diagnosis was 95%. Conclusions The automatic biospy guided by color ultrasound in diagnosis of breast masses has many advantages,such as simple technique,accurate diagnosis and few complication.

AIM: To analyzed the effect of keratoconjunctivitis sicca treated by different medicines after phacoemulsification in patients with diabetes.METHODS: Totally 78 diabetic patients 78 eyes with keratoconjunctivitis sicca (KCS) after phacoemulsification from March 2015 to March 2016 were selected and divided into two groups using random number table.In Group A, 39 eyes were treated with pranoprofen eye drops + artificial tears + Qiming granules.In Group B, 39 eyes were treated with pranoprofen eye drops + artificial tears.the two groups were observed for clinical symptoms, tear film break up time(BUT), corneal fluorescein staining and adverse reaction and so on before and after treatment.RESULTS: Total effective rate of Group A was 97%, significantly higher than that of Group B (64%)(P<0.05).After treatment, symptoms and signs scores of the two groups were significantly lower, BUT were significantly prolonged.At 1, 2wk and 1mo after treatment, symptoms and signs scores of Group A were significantly lower the BUT was longer than those of Group B (P<0.05).Corneal fluorescein staining of Group A at 2wk was lower than that of Group B (P<0.05).CONCLUSION: The combined therapy, pranoprofen eye drops + artificial tears + Qiming granules, is able to quickly and effectively improve the clinical symptoms and signs, with no serious adverse reactions for keratoconjunctivitis sicca after phacoemulsification in patients with diabetes.

Objective To compare effects of irbesartan and captopril on left ventricular function in elderly patients with cardiovascular disease and cardiac functional reserve .Methods 100 cases of elderly patients with car-diovascular disease were chosen ,according to a list of numbers randomly divided into the two groups ,50 cases in each group,a group with irbesartan treatment ,as the irbesartan group and the other group received captopril ,as the capto-pril group compared the two groups of patients left ventricular function and cardiac functional reserve .Results (1) Left ventricular function after the irbesartan group therapy LVDd indicators ,IVSd,LVPWd,LVEF,VE/VA,LVM was significantly better than before treatment ,the difference was statistically significant (t=31.23,18.36,28.12,46.36, 51.13,16.03,all P<0.05),the captopril group before and after treatment was significantly better than the treatment of various indicators,the difference was statistically significant (t=24.53,21.36,31.12,14.33,18.19,16.45,all P<0.05);(2) The irbesartan group of patients after treatment resting heart rate ,exercise heart rate,6min walking distance improved significantly over the treatment of former CCRI and CRI ,the difference was statistically significant (t=39.09,23.77,39.01,23.18,22.45,all P<0.05),after treatment with captopril treatment was significantly better than the indexes,the difference was statistically significant (t=91.21,37.16,26.74,46.33,36.74,all P<0.05),6min walking distance from the irbesartan group therapy ,CRI better than the captopril group ,the difference was statistically significant(t=39.02,42.14,all P<0.05).Conclusion Irbesartan and captopril on improving left ventricular function in elderly patients with cardiovascular disease and cardiac reserve function has a significant effect,irbesartan 6min walking distance test and cardiac reserve indices better .

Objective To evaluate the outcomes of pregnancy of unknown location (PUL) with serial of serum β-human chorionic gonadotropin (β-hCG) and transvaginal sonography.Methods A retrospective study was performed to analyze the relationship between serial serum β-hCG and transvaginal sonography among 315 women who were diagnosed with nonvisible intrauterine pregnancy during Jan 2015 to Dec 2015.Results The pregnancy outcomes of 315 women included 254 intrauterine pregnancies,43 ectopic pregnancies,and 18 spontaneously resolving miscarriages or spontaneously resolving ectopic pregnancies.The serial serum β-hCG curve was classified as rising hCG levels,declining or plateauing hCG levels.Different pregnancy outcomes could exhibit as rising or declining hCG curves.Conclusions It is critical to perform two or more serial serum β-hCG examinations followed by transvaginal sonography to establish the correct diagnosis for PUL.
