1.Expression of vascular endothelial growth factor in the treatment of diabetic foot using bone marrow mesenchymal stem cell transplantation

BACKGROUND:Bone marrow mesenchymal stem celltransplantation has been confirmed to have excellent effect on the treatment of diabetic foot.
OBJECTIVE:To assess the therapeutic effect of bone marrow mesenchymal stem celltransplantation on diabetic foot ulcers and the expression of vascular endothelial growth factor.
METHODS:Literatures concerning the effect of bone marrow mesenchymal stem celltransplantation on foot diabetic and expression of vascular endothelial stem cells were retrieved. Articles that met the criteria were analyzed in depth. Here, we analyzed the experimental research and clinical application of bone marrow mesenchymal stem cells in the treatment of diabetic foot. In experimental studies, rat models of diabetic foot ulcers were established and subjected to bone marrow mesenchymal stem celltransplantation to observe wound healing of ulcers and analyze the expression of vascular endothelial growth factor. In clinical research, the fol ow-up was conducted in diabetic foot patients undergoing bone marrow mesenchymal stem celltransplantation to observe the wound healing of ulcers and adverse reactions.
RESULTS AND CONCLUSION: Experimental studies have shown that bone marrow mesenchymal stem celltransplantation is better than conventional treatment in the treatment of diabetic foot ulcers, but the healing of diabetic foot ulcers is stil slower than that of normal ulcers. After celltransplantation, the expression of vascular endothelial stem cells is elevated but stil lower than that in the normal ulcer controls. Clinical studies have shown that for patients with diabetic foot ulcers, the ulcer wound can be healed after bone marrow mesenchymal stem celltransplantation, as wel as there are no cardiovascular and cerebrovascular diseases, liver and kidney damage and changes in bleeding and coagulation time.

4.The Research of Cellulose Derivatives as Chiral Stationary Phases and High Performance Liquid Chromatography with Polarimeter as On-line Detector

Three cellulose derivatives were synthesized and used as chiral stationary phases based on silica gel.The effects of adsorbances on the column numbers and stabilities have been investigated.These stationary phases exhibited high chiral recognition for various racemates.At the same time,the on-line curves of polarimeter were obtained by high performance liquid chromatography with polarimeter as on-line detector.
5.Reconstruction of three-dimensional models of knee joint using CT image post-processing technique:evaluation of bone tunnel difference after anterior cruciate ligament reconstruction using three-dimensional computed tomography

BACKGROUND:Studies have shown that factors affecting the outcomes of anterior cruciate ligament reconstruction mainly depend on the position of bone tunnels. However, there stil exists certain controversy over the researches on the position of bone tunnels. OBJECTIVE:To investigate the clinical value of three-dimensional computed tomography on postoperative evaluation of bone tunnel after anterior cruciate ligament reconstruction under arthroscopic assistance. METHODS:Fifty-eight patients with anterior cruciate ligament injury who received the treatment from January 2014 to August 2014 underwent anterior cruciate ligament reconstruction under arthroscopic assistance. The femoral end was fixed using an Endobutton, and tibial end was fixed using absorbable interference screws. 58 knees from patients were scanned respectively by means of a dual-source CT scanner. A three-dimensional model of knee was rebuilt on a CT image post-processing workstation to reproduce the medial wal of the lateral femoral condyle and reconstruct single-bundle anterior cruciate ligament, tibial plateau and bone tunnel. The cases that had a Lysholm score≥ 80 points were included in the excelent and good group and those who had a Lysholm score < 80 points in the poor group. The relative position of the center point of the femoral and tibial bone tunnels were marked and measured and the position relation between the two center points was estimated. RESULTS AND CONCLUSION: Three-dimensional computed tomography reconstruction method clearly reflected the situation of bone tunnel and its entrance, fixtures and grafts after anterior cruciate ligament reconstruction. There was significant difference in the center point of femoral tunnel of the knee on the operated side between excelent and good group and poor group (P < 0.05). There was significant difference in the center point of the tibial tunnel between these two groups (P > 0.05). These results confirm that three-dimensional computed tomography can help to clearly reconstruct the images of bone tunnel and anterior cruciate ligament grafts after operation, which can be used clinicaly to assess the relationship between bone tunnel location and graft misshaping.
6.Correlation between colorectal cancer and Helicobacter pylori infection

Objective To explore the correlation between Helicobacter pylori (H.pylori) infection and colorectal cancer.Methods From January 2008 to January 2014,263 patients pathologically diagnosed as colorectal cancer were selected as colorectal cancer group,and at the same period 263 patients with normal colon mucosa under colonoscopy were set as control group.The rates of H.pylori infection,the rates of H.pylori infection in patients with different pathological types of colorectal cancer,the rates of H.pylori infection accompanied with atrophic gastritis and the rates of H.pylori infection accompanied with intestinal metaplasia of two groups were retrospectively analyzed.The difference of H.pylori infection between proximal colorectal cancer and distal colorectal cancer was compared.Chi-square test was performed for comparison and odds ratio (OR) value and 95% confidence interval (CI) were calculated.Results Among 263 patients with colorectal cancer,the H.pylori infection rate of colorectal cancer group was 63.50% (167/263),which was higher than that of control group (39.54%,104/263),and the difference was statistically significant (x2 =27.66,OR=2.66,95% CI 1.85 to 3.83,P<0.01).There was no statistical difference in rates of H.pylori infection among different pathological types of colorectal cancer (x2=0.15,P=0.93).The rate of H.pylori infection accompanied with atrophic gastritis in patients with colorectal cancer was 46.39% (122/263) which was higher than that of control group (23.57%,62/263),and the difference was statistically significant (x2=28.53,OR=2.94,95%CI 1.98 to 4.36,P<0.01).The rates of H.pylori infection accompanied with intestinal metaplasia in patients with colorectal cancer was 17.87% (47/263) which was higher than that of control group (4.18%,11/263),and the difference was statistically significant (x2=23.56,OR=5.50,95%CI 2.76 to 10.95,P<0.01).There was no significant difference in the rate of H.pylori infection between the proximal colorectal cancer and the distal colorectal cancer (x2 =1.48,P =0.22).Conclusion H.pylori infection,together with atrophic gastritis and intestinal metaplasia caused by it are correlated with colorectal cancer.
7.Clinical Experiences of Professor Zhang Qi in Treatment of Recurrent Urinary Tract Infection

Recurrent urinary tract infection (RUTI) is a common disease in China. Modern medicine usually treats it with antibiotics. However, the drug resistance and adverse reaction of antibiotics lower the therapeutic effects and the compliance of patients. Based on the overall concept of Chinese medicine and the relationship of pathogenic factors and vital qi, Professor Zhang Qi proposed the general principles of RUTI treatment according to its different phases and patterns. The principles are listed as follows, eliminating method was used in the treatment of excess pattern;invigorating method was used in the treatment of deficiency pattern; and promoting of body resistance and eliminating pathogenic factors method was used in the treatment of the deficiency-excess complex pattern. These principles have instructional significance for clinical application.
8.Therapeutic effect of oral rehabilitation method combined with Shaker exercise on dysphagia in patients with cerebral infarction

Objective To evaluate the effect of oral rehabilitation method combined with Shaker exercise on the swallowing function and the quality of life in the patients with dysphagia caused by cerebral infarction.Methods A total of 120 patients with swallowing disorder caused by cerebral infarction were randomly divided into conventional treatment group (n=30),oral rehabilitation method group (n=30),Shaker exercise group (n=30) and comprehensive therapy group (n=30).The patients in the conventional treatment group were treated with conventional treatment,while the patients in oral rehabilitation method group received oral rehabilitation method treatment in addition.The Shaker exercise group accepted Shaker exercise and conventional treatment,while the comprehensive therapy group accepted oral rehabilitation method in addition.All patients were assessed with video fluoroscopic swallowing study (VFSS) and swallowing-related quality of life (SWAL-QOL),before treatment and 4-week after treatment.Results Before treatment there was no significant difference among the four groups in VFSS and SWAL-QOL scores (P>0.05).After 4-week treatment the swallowing function showed different degrees of improvement.The difference of VFSS score in the conventional treatment group was not statistically significant before treatment and 4-week after treatment (P=-0.38),but it was statistically significant among the other three groups (P<0.01).After 4-week treatment there was significant difference among the four groups in VFSS score (P<0.01),and the comprehensive therapy group curative effect was significantly better than that of the other groups.The total effective rate was 96.67% in the comprehensive therapy group,26.67% in the conventional treatment group,66.67% in the oral rehabilitation method group,and 70.00% in the Shaker exercise group.the SWAL-QOL score in the comprehensive therapy group was higher than that of the other three groups.The average SWAL-QOL score was 187.30±10.68 in the comprehensive therapy group,(140.10±16.62) in the conventional treatment group,(161.47 ±24.53) in the oral rehabilitation method group,and (163.97 ± 23.91) in the Shaker exercise group.Conclusion Application of oral rehabilitation method combined with Shaker exercise in the patients with swallowing disorder caused by cerebral infarction could effectively improve the degree of swallowing disorder and the quality of life.

Microsomal triglyceride transfer protein (MTP) is a key transfer protein in lipid metabolism and is mainly expressed in the hepatocytes and enterocytes.It plays an important role in transferring triglyceride as well as in assembly and secretion of very low density lipoprotein.Recent studies have shown that MTP is closely related to the onset and development of non-alcoholic fatty liver disease.
10.Value of low dose multilayer spiral CT three-dimensional reconstruction in diagnosis of children tracheobronchial foreign body

Objective To evaluate the value of low dose multilayer spiral CT (MSCT) threedimensional reconstruction in diagnosis of children tracheobronchial foreign body.Methods Forty-three children with tracheobronchial foreign body which was confirmed by fiberoptic bronchoscopy were enrolled,and all of the children underwent low dose MSCT three-dimensional reconstruction.The volume data were reconstructed on work station (Siemens) with the techniques of multiple plane reformation (MPR),curved planar reconstruction (CPR),minimum intensity projection (MinIP),volume rendering (VR),CT virtual bronchoscopy (CTVB),and the results of MSCT were compared with fiberoptic bronchoscopy.Results The fiberoptic bronchoscopy results were used as the localization and qualitative diagnosis results.Locations of the foreign body were trachea and bifurcation of trachea (6 cases),right bronchus (23 cases) and left bronchus (14 cases),and the coincidence rate of localization diagnosis of MSCT was 100% (43/43).Natures of the foreign body were nuts in 21 cases,beans in 5 cases,corn in 2 cases,fruits and vegetables in 6 cases,and other kinds in 4 cases.Qualitative diagnosis by MSCT was medium density foreign body,and the coincidence rate was 100% (38/38).Five cases had bone mass,qualitative diagnosis by MSCT was high density bony foreign body,and the coincidence rate was 5/5.In 43 children,MPR (CPR) showed 41 cases (95.3%),CTVB showed 33 cases (76.7%),MinIP showed 34 cases (79.1%),VR showed 23 cases (53.5%),and axial scan showed 21 cases (48.8%).MPR (CPR) in showing foreign body rate was higher than other several postprocessing methods,and there were statistical differences (P < 0.05).There was no statistical difference in the showing of foreign body between CTVB and MinIP (P > 0.05),but CTVB and MinIP were higher than VR and axial scan,and there were statistical differences (P < 0.05).Conclusions Low dose MSCT three-dimensional reconstruction is satisfactory in the diagnosis of children tracheobronchial foreign body.It can accurately evaluate the exact size,location,shape and pulmonary complications of foreign body,thus it plays an important role in the diagnosis of suspicious tracheobronchial foreign body.
