Early prognostication of neurological outcome in comatose survives after cardiopulmonary resuscitation,an essential component of post-cardiac arrest care is significantly meaningful in both clinical and economic fields.From the perspectives of predictors based on neurological examination,electroencephalogram,evoked potentials,neuroimaging and blood biomarkers and taking advantage of therapeutic hypothermia into consideration,this article reviews the development in evaluation of neurological outcome in comatose survives after cardiac arrest.
2.An Inquiry into Preparing Diarrhea Model of Mice and Application of Diarrhea Index

In Order to screen antidiarrhea drugs, the standard operation of mice diarrhea model replicated with leave of Cassia angustifolia vahl was introduced. In experiments, diarrhea index expressed with loose stool ineidence rate multiplied by loose stool grade was used as main index. It thoroughly mirrors indi. vidual diarrhea degree and possesses comparability. The result indicates that diarrhea index in mice presented normal distribution. The index between animal model groups kad no difference as well as no differcnce was found in model mice of same group for six consecutive days, with good reproduclbility. It is recommended that the model can be used for the screening of antidiarrhea drugs.
3.The prognostic value of gray-white matter ratio on brain computed tomography in comatose adult survivors from cardiac arrest

Objective To evaluate the correlation between the gray-white matter ratio (GWR) and the outcomes of comatose adult survivors from cardiac arrest (CA) in Chinese.Methods Sixty-one CA patients checked with CT scans within 72 hours of resuscitation from January 2011 to January 2016 were included in this single-center retrospective study.Gray and white matter density (Hounsfield units) were measured,and the GWRs were calculated according to previous studies.The prognostic values of the GWRs in predicting poor outcomes (Cerebral Performance Category 3-5) were analyzed.Results The density values of gray matter were significantly higher in the good outcome group than those in the poor one.All GWRs were significantly higher in the good outcome group (P < 0.05).A GWR (basal ganglia) < 1.18 predicted poor outcomes with a sensitivity and specificity of 50.0% and 88.2%,respectively (P =0.012).Conclusions Low GWRs,determined from brain CT scans in comatose CA patients after resuscitation,were associated with poor neurological outcomes.GWR determination from brain CT can be a useful indicator for outcome prediction aiding in an optimal clinical decision process in comatose survivors from CA.
5.The neuro-prognostic value of the ion shift index in cardiac arrest patients following extracorporeal cardiopulmonary resuscitation

BACKGROUND: The ion shift index (ISI) as a prognostic indicator that can show the severity of hypoxic-ischemic injury. We aimed to evaluate the performance of the ISI in predicting unfavorable neurological outcomes at hospital discharge in cardiac arrest (CA) patients following extracorporeal cardiopulmonary resuscitation (ECPR) and to compare its performance to other prognostic predictors.
METHODS: This was a retrospective observational study including adult CA patients treated with ECPR between January 2018 and December 2022 in a tertiary hospital. Data regarding clinical characteristics and laboratory parameters were collected from medical records. The ISI was determined based on the first available serum electrolyte levels after ECPR. The primary outcome was unfavorable neurological status at hospital discharge, defined as Cerebral Performance Categories 3-5. Comparisons of the characteristics between the two groups were made using the χ2 test for categorical variables and the t-test or non-parametric Mann-Whitney U-test for continuous variables, as appropriate. Correlation analysis was performed using Spearman’s rank correlation coefficient. A two-tailed P-value <0.05 was considered statistically significant.
RESULTS: Among the 122 patients involved, 46 (37.7%) had out-of-hospital CA, and 88 had unfavorable neurological outcomes. The ISI was significantly higher in the unfavorable outcome group than in the favorable outcome group (3.74 [3.15-4.57] vs. 2.69 [2.51-3.07], P<0.001). A higher ISI level was independently related to unfavorable outcome (odds ratio=6.529, 95% confidence interval 2.239-19.044, P=0.001). An ISI level >3.12 predicted unfavorable outcomes with a sensitivity and specificity of 74.6% and 85.2%, respectively (P<0.001). The prognostic performance of ISI (area under the curve [AUC]=0.887) was similar to that of other predictors, such as gray-to-white matter ratio (AUC=0.850, P=0.433) and neuron-specific enolase (AUC=0.925, P=0.394).
CONCLUSION: ISI may be used as a prognostic biomarker to predict neurological outcomes in CA patients following ECPR.
6.Follow up analysis of 6 patients with severe trimethyltin chloride poisoning for 4 years.

Objective: Objective to investigate the health changes of patients with severe trimethyltin chloride (TMT) poisoning in four years. Methods: Six patients with severe TMT poisoning treated in the First Affiliated Hospital of Gannan Medical College in August 2016 were numbered 1, 2, 3, 4, 5 and 6 respectively. The patients were followed up 0.5, 2 and 4 years after poisoning and compared and analyzed. The follow-up contents include: symptom degree, score of simple mental intelligence examination scale (MMSE) and modified Rankin Scale (MRS) , cranial magnetic resonance imaging (MRI) , EEG, etc. Results: The symptoms of dizziness, headache, chest tightness, palpitation, nausea and vomiting decreased gradually in 6 patients. The symptoms of speech disorder and memory decline in No.1, 2 and 3 patients gradually increased, and the scores of MMSE and Mrs gradually decreased; Patients No.4, 5 and 6 had improved speech disorder, but their memory decreased, MMSE and Mrs scores were still flat, and mild cognitive impairment. The brain atrophy of No.1, 2 and 3 patients was aggravated, which showed obvious atrophy of hippocampus, temporal lobe, insular lobe and cerebellum and enlargement of ventricle; There was no significant change in brain atrophy in No.4, 5 and 6 patients. Conclusion: The neurotoxic symptoms in the later stage of severe TMT poisoning are still serious, and the neurotoxic time is long.

7. The relationship between mindfulness and self-esteem of adolescents: the mediating effect of self-efficacy

Objective:
To explore the mediating effect of self-efficacy between mindfulness and self-esteem.
Methods:
In this study, 400 junior school students from a middle school in Jiangxi Province was recruited and complete mindful self scale, general self-efficacy scale and self-esteem scale.
Results:
The scores of mindfulness, self-efficacy, self-esteem were (63.89±9.12), (22.82±4.83) and (26.14±4.43), respectively. Mindfulness and its four dimensions (self-insight, self-acceptance, self-compassion and non-persistence) were significantly positively correlated with self-esteem (
8. Comparison of pregnancy outcomes after transvaginal cervical cerclage for different surgical indications during pregnancy

OBJECTIVE: To retrospectively analyze and compare the clinical effects of transvaginal cervical cerclage during pregnancy with different surgical indications.METHODS: A total of 39 patients who underwent transvaginal cervical cerclage during pregnancy in the Gynaecology and Obstetrics Department of the First Affiliated Hospital of Gannan Medical University from July 2012 to June 2018 were enrolled.The patients were grouped according to the indications of surgery:those with the history of late abortion or preterm labor being the indication of surgery were the indication of the history of the cerclage group(group H,20),and those with the length of the cervix being shortened or the shape of the cervix being changed in vaginal ultrasound indication cerclage group(group U,20).The patients whose hysical examination revealed dilated cervix with or without amniocentesis were included as emergency cerclage group(group E,7).The clinical effects of vaginal cervical cerclage during pregnancy were compared among the 3 groups.RESULTS: The gestational week of cerclage in the H group[(15.64 ± 2.96)w] was shorter than that in the U group[(22.04±5.01)w]and the E group[(22.08±2.96)w],and the difference was statistically significant(P<0.05),but the gestational age of cerclage was of no significant difference between the E group and the U group(P>0.05).The extended days[(29.43±47.24)d],gestational age of delivery[(26.39±6.52)w],full-term birth rate(14.29%)and live birth rate(28.57%)in group E were all lower than those in group H[(136.45±53.70)d,(35.60±7.07)w,69.57%,86.36%]and group U[(103.40±36.15)d,(36.81±3.45)w,70.00%,100.00% ],with statistically significant differences(P<0.05).No statistically significant differences were observed between group H and group U(P>0.05).The abortion rate(71.42%)in group E was higher than that in group H(13.04%)and group U(00.00%)(P<0.05),but there was no statistically significant difference between group H and group U(P>0.05).There was no significant difference in preterm birth rate among the three groups(P>0.05).CONCLUSION: Cervical cerclage with three different surgical indications can effectively improve the pregnancy outcome of patients with cervical insufficiency.The pregnancy outcomes are similar between the indication of the history of the cerclage and the vaginal ultrasound indication cerclage,but better than the emergency cerclage.
9. Mechanism of GPR119 in regulating lipid metabolism and anti-atherosclerosis by hypoxia-inducible factor-1α/ vascular endothelial growth factor pathway

Objective:
To investigate the effect and mechanism of G protein coupled receptor 119 (GPR119) in regulating lipid metabolism.
Methods:
(1) Macrophage THP-1 was induced by oxidized low density lipoprotein (oxLDL) to the formation of lipid foam cells, protein expression of GPR119, hypoxia-inducible factor-1α (HIF-1α), vascular endothelial growth factor (VEGF) were detected by Western blotting. (2) Constructing GPR119 over-expressed and low-expressed plasmids, the plasmids were transfected into THP-1 cells which induced by oxLDL. The lipid content in macrophages was observed by oil red O staining. Cholesterol efflux was detected by liquid scintillation counter. The mRNA and protein expressions of HIF-1α, VEGF were detected by reverse transcription PCR and Western blotting. (3) Constructing GPR119, HIF-1α, and VEGF over-expressed plasmids, then co-transfection of GPR119 and HIF-1α/VEGF plasmids. The lipid content in macrophages was observed by oil red O staining. Cholesterol efflux was detected by liquid scintillation counter.
Results:
Compared with the control group, the lipid droplets were densely distributed in macrophages, with a large number and volume. The protein expression of GPR119 was significantly decreased and HIF-1α, VEGF were significantly increased in macrophages induced by oxLDL (
10.Effect of violence exposure and emotional balance on depression among adolescents

Objective:
To investigate the relationship between violence exposure and depression in adolescents, and to explore possible moderating effects of emotional balance, so as to provide the evidence reference for understanding the status and characteristics of adolescents violence exposure.
Methods:
A total of 2 506 middle school students from three middle schools in Ganzhou selected by convenient cluster random sampling. Violence Exposure Scale(VES), Center for Epidemiological Studies Depression Scale(CES-D) and Emotional Balance Scale were used for questionnaire survey.
Results:
There were statistically significant differences in the scores of violence exposure among middle school students of different genders, grades, whether the only child or not, father educational level,mother s educational level and family types ( t/F =3.85, 21.14, 2.90, 7.62, 8.13 ,8.12, P < 0.01 ). There were statistically significant differences in the scores of depression among middle school students of different genders, grades and family types ( t/F =-7.05, 18.61,7.42, P <0.01 ). There were statistically significant differences in the scores of emotional balance among middle school students of different genders, grades, father s educational level and mother s educational level ( t/F =7.93, 9.22, 7.71 ,6.86, P <0.01 ) . Emotional balance was positively related to the depressive mood of middle school students ( r =0.34, P < 0.01 ),and emotional balance was negatively related to depression( r =-0.20, P <0.01). Emotional balance had the moderating effect on violence exposure and depression among middle school students ( t =-3.35, β =-0.02, P <0.01).
Conclusion
Violence exposure is related to depression of middle school students. Improving the emotional balance ability of adolescents can effectively reduce the negative effects of violent exposure on early adolescent depression, and reduce the incidence of adolescent depression.
