1.Application Features of Field Digital Oral Medical Vehicles

Objective To test the performance of the field digital oral medical vehicles(two chairs) developed by School of Stomatology,the Fourth Military Medical University.Methods The field digital oral medical vehicle was developed by using dental health care system,diagnostic imaging systems,oral endoscopic systems,infection control systems,digital management system,the sewage treatment system,temperature conditioning systems,power generation systems,automotive GPS positioning system and other systems.Results The vehicle was characteristized by reliable performance,high degree of functional serialization,complete medical facilities and good operability of the internal layout.Conclusion Its functions fully meet the requirements in field oral disease prevention and control as well as first aid.
2.The epidemiology of multidrug-resistant bacteria colonization and analysis of its risk factors in intensive care unit

ObjectiveTo screen the colonization of multidrug resistant organisms (MDROs) and determine their risk factors in intensive care unit (ICU), so as to provide the basis of prophylaxis and treatment of MDROs colonization.Methods A prospective single-center study was conducted in ICU of China-Japan Friendship Hospital from June 2008 to December 2014. The nostril and anal swabs for each patient who stayed in ICU over 24 hours were collected. Each specimen was cultured and tested for drug sensitivity. Clinical findings and relative risk factors were collected. The risk factors of MDROs colonization were analyzed with univariate analysis. The independent risk factor was selected from the risk factors withP< 0.05 with logistic regression analysis to analyze the related factors of MDROs colonization in ICU.Results 1 672 patients were enrolled. At ICU admission, MDROs colonization was present in 604 cases (36.12%), of whom 62 cases (3.71%) were found to be colonized with methicillin-resistantStaphylococcus aureus (MRSA), 529 (31.64%) were colonized with extended-spectrumβ-lactamase (ESBL) enterobacteria, 7 (0.42%) were colonized with multidrug resistantAcinetobacter baumannii (MDR-AB), and 6 (0.36%) were colonized with multidrug resistantPseudomonas aeruginosa (MDR-PA). ICU acquired MDROs colonization were 197/1 068 (18.45%), among whom 24 patients (1.44%) were colonized with MRSA, 118 (7.06%) were colonized with ESBL enterobacteria, 50 (2.99%) were colonized with MDR-AB, and 5 (0.30%) were colonized with MDR-PA. By multivariable analysis, prior administration of more than two kinds of antibiotics [odds ratio (OR) = 2.352, 95% confidence interval (95%CI)=1.847 - 4.464,P = 0.002], prior use of broad spectrum antibiotics within 3 months (OR = 2.862, 95%CI = 1.458-5.631,P = 0.014), duration of prior antibiotic administration (OR = 1.781, 95%CI = 1.152 - 3.413,P = 0.003) and hospitalization days prior to ICU admission> 9 days (OR = 1.766, 95%CI = 1.235 - 3.986,P = 0.021) were independent risk factors of MDROs colonization on admission to ICU.ConclusionsHigh prevalence of MDROs colonization in ICU patients was found in our hospital, and ESBL enterobacteria was the predominant bacteria. ICU acquired MDROs colonization is also worth considering, especially for MDR-AB. Identification of risk factors for MDROs colonization may help identify and screen patients with high risk, and it is also instructive in prophylaxis of MDROs colonization/infection and restriction of the use of broad spectrum antibiotics.
4.Non-rigid medical image registration based on mutual information and thin-plate spline.

To get precise and complete details, the contrast in different images is needed in medical diagnosis and computer assisted treatment. The image registration is the basis of contrast, but the regular rigid registration does not satisfy the clinic requirements. A non-rigid medical image registration method based on mutual information and thin-plate spline was present. Firstly, registering two images globally based on mutual information; secondly, dividing reference image and global-registered image into blocks and registering them; then getting the thin-plate spline transformation according to the shift of blocks' center; finally, applying the transformation to the global-registered image. The results show that the method is more precise than the global rigid registration based on mutual information and it reduces the complexity of getting control points and satisfy the clinic requirements better by getting control points of the thin-plate transformation automatically.

5.Cloning and functional characterization of pathogenesis-related PR10-1 gene in Panax notoginseng.

With homology cloning approaches coupling with RACE (rapid-amplification of cDNA ends) techniques, the full-length coding sequence of pathogenesis-related protein PR10-1 with differential expression was cloned from the total RNA of the root of Panax notoginseng, and its function was explored furtherly. As a result, the longest 465 bp ORF (named as PnPR10-1 with the Accession No. KJ741402 in GenBank) was detected from the cloned sequence with full-length of cDNA of 863 bp. The corresponding peptide encoded consisted of 155 amino acids, contained some domains such as Bet-v-I, and showed high similarity with that from Panax ginseng by analysis of phylogenetic trees created from the alignments. Real-time quantitative PCR showed that the expression of PnPR10-1 gene was constitutive in different tissues of 1-3 year old plant, suggesting that it might be involved in growth, development, and secondary metabolism; yet it was up-regulated significantly with the infection of Fusarium oxysporum in root, suggesting that it might be involved in defense against many diseases including root rot in P. notoginseng.

8.The Important Category of the Modern Medical Ethics Theory -Honest-trust

9.Clinical Study:Concurrent Interventional Cisplatin Ieucovorin Fluorouracil Hydroxy Camptothecin Chemotherapy Plus Radiotherapy in Patients with Ⅳ Stage Middle-lower Section Esophageal Cancer

Objective To investigate the efficacy of concurrent interventional cisplatin,leucovorin,fluorouracil,hydroxy camptothecin chemotherapy plus radiotherapy in patients with Ⅳ stage middle-lower section esophageal cancer.Methods The subjects were all the patients with Ⅳ stage middle-lower section esophageal cancer.Among of them,42 patients,via artery,had been enrolled for concurrent interventional cisplatin,leucovorin,fluorouracil,hydroxy camptothecin chemotherapy plus radiotherapy.40 patients were given irradiation.Results The incidence rate of Ⅰ grade esophageal X-ray in the former group was 57.1%,and that in the later group was 25.0%,and the parameters had significant difference(?0.05).Conclusion Concurrent interventional cisplatin,leucovorin,fluorouracil,hydroxy camptothecin chemotherapy plus radiotherapy may be attributed to progression of Ⅳ stage middle-lower section esophageal cancer.
10.Effect of Endoscopic Sinus Surgery in the Treatment of Sinusitis and Nasal Polyps and Its Effect on Nasal Ventilation and Olfactory Function

Objective:To investigate the clinical effect of endoscopic sinus surgery in the treatment of sinusitis and nasal polyps and its effect on nasal ventilation and olfactory function.Methods:80 patients with sinusitis and nasal polyps were selected in our hospital from January 2014 to June 2016,who were randomly divided into observation group and control group,each group of 40 cases.The control group was treated with traditional enucleation,while the observation group was treated with endoscopic sinus surgery.The clinical efficacy,the symptom scores,nasal airway resistance and olfactory function score before treatment and 3 months after treatment were compared between the two groups.Results:The total effective rate of the observation group was 95.00%,which was significantly higher than that of the control group 77.50% (P<0.05).There was no significant difference in nasal congestion,purulent nasal discharge,olfactory dysfunction,pain and total symptom score between the two groups before treatment (P>0.05),nasal congestion,purulent nasal discharge,olfactory dysfunction,pain and total symptom score between the two groups at 3 months after treatment were lower than before treatment,and nasal pus,smell disorder,pain and total symptom scores of the patients in the observation group were lower than the control group (P<0.05).There was no significant difference in nasal airway resistance and olfactory function score between the two groups before treatment and 3 months after treatment (P>0.05),nasal airway resistance,olfactory function score of patients in two groups at 3 months after treatment were lower than before treatment,and the observation group was lower than that ofthe control group (P<0.05).Conclusion:Endoscopic sinus surgery for sinusitis and nasal polyps is helpful to improve the clinical symptoms,promote the olfactory function and recovery of nasal ventilation,which is an effective method for treatment of sinusitis and nasal polyps.
