Chinese Journal of Primary Medicine and Pharmacy 2021;28(4):602-605
Objective:To investigate the clinical efficacy of different doses of atorvastatin in the treatment of chronic heart failure. Methods:A total of 100 patients with chronic heart failure who received treatment in the Third People's Hospital of Bengbu Medical College from June 2019 to June 2020 were included in this study. They were randomly divided into a control group (conventional treatment + atorvastatin calcium tablet 10 mg, n = 49) and an observation group (conventional treatment + atorvastatin calcium tablet 20 mg, n = 51). Before and after 12 weeks of treatment, serum levels of N-terminal pro-brain natriuretic peptide (NT-proBNP) and inflammatory cytokines such as high-sensitivity C-reactive protein (hs-CRP), interleukin-6 (IL-6), interleukin-10 (IL-10) as well as ventricular remodeling (echocardiographic index) were compared between the two groups. Results:After treatment, serum level of NT-proBNP was (506.56 ± 62.37) pg/mL and (660.85 ± 74.55) pg/mL, serum level of hs-CRP was (14.74 ± 2.69) mg/L and (23.31 ± 3.45) mg/L, serum level of IL-6 was (36.77 ± 4.78) ng/L and (43.12 ± 5.22) ng/L, serum level of IL-10 was (12.36 ± 2.63) ng /L and (15.39 ± 3.56) ng/L, left ventricular end diastolic diameter was (37.74 ± 6.83) mm and (42.36 ± 7.73) mm, left ventricular ejection fraction was (45.78 ± 3.31)% and (42.63 ± 2.56)%, and the ratio of early (E wave) to late (A wave) diastolic velocities (E/A) was (1.44 ± 0.06) and (1.21 ± 0.03), respectively in the observation and control groups. There were significant differences in serum levels of NT-proBNP, hs-CRP, IL-6 and IL-10 as well as LVEDD and LVEF between the observation and control groups ( t = 3.73, 3.07, 3.58, 3.68, 2.99, 3.12 and 2.98, all P < 0.05). Conclusion:Atorvastatin is beneficial to the prognosis of patients with chronic heart failure. The therapeutic efficacy of 20 mg atorvastatin per day is better than that of 10 mg atorvastatin per day.

Retroperitoneal laparoscopic live donor nephrectomy offers an intrinsic advantage over conventional transperitoneal laparoscopic nephrectomy because of the potentially lower risk for early and late donor intraperitoneal complications. Herein we presented our experience performing retroperitoneal laparoscopic live donor nephrectomy in 105 donors. All donor nephrectomy was successful. There were no donor deaths and no conversion to open surgery. Mean operation time was 112 min (range, 70-200 min). Intraoperative blood loss was 10-150 mL with an average of 30 mL. Warm ischemia time was 1.3 to 6 min with an average of 3.1 min. Postoperative retroperitoneal hematoma occurred in only one case and there were no other surgical complications. Donors were discharged from the hospital 5 to 10 days postoperation. Average postoperative hospital stay was 6.4 days. One graft was removed due to acute rejection. Delayed graft function occurred in two recipients but renal function returned to normal within four weeks. The other recipients had normal renal function in two weeks except three recipients in four weeks. We believe that retroperitoneal laparoscopic live donor nephrectomy is safe, reliable, and less invasive.

Objective To investigate a special optimization technique for computer aided measure, and provide anatomical basis for screw internal fixation in the cavitas glenoidalis through the coracoid process of scapula. Methods Thirty accurate scapula models were reconstructed from CT data sets. First, special optimization objective function was designed for single screw internal fixation configuration, and the optimal placement of screw was found automatically under constraints. Then, the placements of double screws internal fixation configuration were searched taking advantage of principal component analysis. Finally, statistical measure data were provided according to new anatomical reference landmarks for clinical use. Results For single screw internal fixation configuration, the distance from the optimal screw entry point P to the acromion process point X was (39.15±2.28) mm, to the coracoid process point Y was (28.66±2.68) mm, to the angulus superior point Z was (61.13±6.57) mm;The angle was (81.27±7.15)° between PX and PY, and (133.27±6.84)° between PX and PZ. The mean inclination of the lag screw was (104.08±4.41)° for the angle with line PX, (101.29±3.51)° with line PY, and (76.23±5.03)° with line PZ. For double screws configuration, the distance from the original single screw entry point P to the screw entry point E was (5.12±1.37)mm,to the screw entry point F was (3.88±0.94)mm. The angle between the long axis of coracoid process and line EF was (27.41±3.51)°. Conclusion The automatic optimization technique for computer aided measure is very efficient and has many advantages over the conventional manual dissection methods, and is convenient to design new anatomical reference landmark system for clinical use.

OBJECTIVETo evaluate the feasibility and efficacy of unilateral pedicle screw fixation in treating thoracolumbar fractures through paraspinal approach.
METHODSFrom January 2006 to January 2009,21 patients with single level thoracolumbar fracture without neurological symptoms were treated with unilateral pedicle screw fixation through paraspinal approach. There were 14 males and 7 females, aged from 21 to 65 years old with a mean of 36.4 years. The duration from injury to operation ranged from 6 h to 5 d with an average of 3 d. According to the classification of Denis fracture, compression fractures happedned in 12 cases and burst fractures happened in 9 cases, including 1 case with T5 fracture, 2 cases with T7 fracture, 2 cases with T10 fracture, 3 cases with T11 fracture, 8 cases with T12 fracture, and 5 cases with L1 fracture. Based on the Flankel grade, all patients were classified as grade E. Anterior vertebral body height ratio, sagittal Cobb angle, condition of internal fixation failure, visual analogue score (VAS) were evaluated.
RESULTSAll patients were followed up from 12 to 36 months with an average of 20.5 months. No internal fixation failure was found. Anterior vertebral body height ratios at preoperative 3 days after operation and last follow-up were 54.3 +/- 2.8, 92.9 +/- 1.5, 93.8 +/- 1.7, respectively;sagittal Cobb angle at the three timepoints were (27.8 +/- 2.5) degrees, (5.3 +/- 0.8) degrees, (6.3 +/- 1.4) degrees, respectively; the difference was statistical significant (P < 0.05). VAS was (1.2 +/- 0.4) points at last follow-up and had obviously improved (P < 0.05).
CONCLUSIONTreatment of thoracolumbar fractures with unilateral pedicle screw fixation through paraspinal approach is safe with the advantages of micro-trauma and less blood loss, which can not only completely retain the posterior spinal complex structure, reinforce the spinal stability, raise the reductional quality, but also improve the strength of fixation and the distribution of stress force.
