1.Application of CO2 laser and self-made laryngeal dilator in the treatment of the laryngeal stenosis.

OBJECTIVE:
To study the treatment effect of CO2 laser and self-made laryngeal dilator on the laryngeal stenosis.
METHOD:
Twenty-five patients with laryngeal stenosis were treated with CO2 laser and self-made laryngeal dilator. The laryngeal cavity scar was resected as much as possible by using CO2 laser and the laryngeal cavity was exposed by placing laryngeal dilator under general anesthesia.
RESULT:
The 24 cases of all were successfully decanulated 1 to 12 months after operation and could breathe freely. One case had second operation and was successfully decanulated 9 months after operation. The follow-up for 1 to 2 years showed all cases speaking well and 3 cases recurrence.
CONCLUSION
The method for treating laryngeal stenosis using CO2 laser and self-mande laryngeal dilator is effective, fewer complication, less invasive, and faster recovery. The laryngeal dilator is made simply and conveniently.

2.Changes about joint injuries caused by train in recent years

Objective To analyze the recent changes in features of joint injury caused by the fast running railroad train. Methods 1279 joint injuries o f the 10214 cases of train injury after the speeding up of the railway services from 1997 to 2003 were analyzed to find out their particular causal conditions, the severity of the injury, and the classification of the injuries, the correlat ion between which and recovery was also tested by AIS-ISS grading. Results Afte r the speeding up of the railway services, the incidence of joint injuries rose from 33.54% to 34.12%,the mortality rose from 22.88% to 30.33%,the multipl e joint severances rose from 19.84% to 34.19%,the open joint injury rose from 31.71% to 63.15%,and the mortality caused by multiple joint severances rose from 21.19% to 49.07%. Conclusions The incidence of multiple traumas resulted from the train is much higher than those resulted from other causes. Multiple t raumas caused by train are very difficult to deal with. The multiple organ dysfu nction syndrome (MODS) is a main factor to cause the death.
3.Bio-oss combined with fibrin glue in reconstruction of canine mandibular defects

Objective:To evaluate the osteogenic activities of Bio-oss after combining with fibrin glue in reconstruction of canine mandibular defects.Methods:The second and fourth premolar teeth and the second molar teeth were extracted bilaterally, in 9 hibrid canines,resulting in 6 bone defects(1 cm?1 cm)in each canine.Bio-oss,Bio-oss+FG and FG were implanted into the bone defects of the second,fourth premolar teeth and the second molar teeth,respectively.Canines were executed in group of 3 after 4,8,and 12 weeks to observe the healing of soft tissues.The bone density was assessed by X-ray,the property of Bio-oss were observed via gross specimen,and the morphology of the newly-formed bone was observed by tissue sections.The proportion of newly-formed bone was obtained by computer image analysis(SAS software,analysis of variance).Results:StageⅠhealing of soft tissues was achieved in all animals.The bone densities were not significantly different between Bio-oss+FG and Bio-oss groups.The bone in FG group had transparent area.We also found that the bone in Bio-oss+FG group was closely combined and there were sccatered Bio-oss dusts in the soft tissues of the Bio-oss group.The newly-formed bone in the FG group was only found in the border between the defects and FG.The proportion of newly-formed bone was less in the Bio-oss+ FG group than in the Bio-oss group at 4,8,and 12 weeks after extraction(P
6.Curative Effect Observation in the Treatment of Distal Humeral Shaft Fracture with Double Steel Plate Internal Fixation

ObjectiveTo investigate the curative effect of the treatment for the distal humeral shaft fracture with double steel plate internal fixation. Methods A total of 60 individuals who were humeral shaft fractures were included from March 2009 to April 2014 in the second people's hospital of Yunnan province. 28 of them were treated using double plate fixation(group A)and 32 of them were treated using single-plate treatment (group B). Blood loss, hospital stay, postoperative drainage, healing time and Mayo elbow performance score were compared between the two groups. Results(1)Blood loss, hospital stay and postoperative drainage showed no differences between two groups. However, the healing time between these two groups showed difference(P < 0.05).(2)MEPS score results showed a significantly difference in the excellent rate of patients between groups A and B(96.4% and 75%, respectively,P < 0.05). Conclusion The double plate fixation and single plate fixation for treatment of humeral shaft fracture showed no differences in operation time,blood loss,postoperative drainage and hospital stay. However,the double plate fixation showed a shorter healing time and better efficacy. Thus,the double plate fixation of humeral shaft fracture can contribute to obtain the rigid internal fixation,early functional exercise and better clinical efficacy.
7.Analysis of 236 patients with upper limb injuries in "5.12" Wenchuan Earthquake

[Objective]To discuss the injury severity and treatment of 236 patients with upper limb injuries in Wenchuan Earthquake.[Method]The basic informations of patients with upper limb injures in Wenchuan Earthquake,and the injury mechanisms,clinical symptoms and treatment were collected and analyzed.[Result]In 236 cases of patients with upper limb injuries,there were 208 patients,378 cases with fracture,and 64 cases with open fracture.There were 35 cases of clavicle fracture,31 cases of scapula fracture,113 cases of humerus fracture,46 cases of ulna fracture,87 cases of radial fracture and 66 cases of carpometacarpai phalangeal fracture.Fifty cases received debridement and suture,60 cases received fracture reduction and internal fixation and 7 cases received external fixation.Totally,19 limbs were carried on amputation,and only one patient died of hyperkalemia and combined injury.[Conclusion]Analysis of patients with injury mechanisms,clinical symptoms and treatment can provide advices for rescuing and treating of patients with upper limb injuries in earthquake or other disaster cases.
8.Effect of Mutouhui Extracts on Hemorheology of U14 Cervical Cancer Mice

Objective To study the influence of Mutouhui extracts on hemorheology of U14 cervical cancer mice.Methods The U14 cervical cancer mice were divided into Mutouhui extracts high,middle and low dose group,Cisplatin Injection group,model group and blank comparing group.The influence on hemorheology change was observed after being given the medicine.Results The high,middle dose group of Mutouhui extracts and Cisplatin injection group could reduce the blood viscosity of U14 cervical cancer mouse in different shear,and hemotcrit of the low did not obviously change,groups of Mutouhui extracts plasma viscosity changed slightly.Cisplatin Injection group could reduce the blood viscosity,but did not obviously affect to the plasma viscosity.Conclusion Mutouhui extracts can reduce the U14 cervical cancer mouse blood viscosity,and improve the indexes of hemorheology.
9.Value of radiotherapy following breast-conserving surgery in early stage breast cancer

Purpose:To validate further the importance of r adiotherapy following breast-conserving surgery in early breast cancer. Methods:From 1994.12 to 2001.12,237patients with early stage breast can cer were treated Of these patients,eighty-three received breast-conserving surgery followed by radiotherapy,and one hundred and fifty-four received modified radical mastectom y.All patients with breast-conserving surgery underwent postoperative radiother apy.The range of radiation dose depends on the size,location of tumor,and involv ement of axillaty nodes.Tangential chest wall fields delivered 5000cGy with 8MV -X ray beam in 5 weeks .An additional 1500cGy was delivered in 8 days to tumor bed with electron beam.Among the patients with modified radical mastectomy.only 3 received postoperative radiotherapy.Results:In both the breast-conserving surgery group and modified radical mastectomy gro up,one patient recurred regionally,and one died respectively .In the modified ra dical mastectomy group,there were no regional lymph nodes recurrence,but two of them had distant metastasis.The 5year survival of the two groups were 96.49%and 98.61%,respectively.The breasts are soft and resilient in the breast-conserving surgery group,and breast shape whole after surgery.Conclusions :Breast-conserving surgery plus postoperative radiotherapy in early breast can cer can obtain treatment results similar to modified radical mastectomy,with goo d cosmetic

Objective To study the expression of hMLH1, hMSH2 and their association with clinico-pathological characteristics in hepatocellular carcinoma(HCC). Methods The expression of hMLH1 and hMSH2 in 37 HCC tissues was detected by SABC immunohistochemistry.
